    Ludwig’s angina in children

    AbstractLudwig’s angina is a potentially life-threatening, rapidly spreading, bilateral cellulitis of the submandibular spaces. It uncommonly occurs in adults and children and its early recognition is paramount. With early diagnosis, airway observation and management, aggressive intravenous antibiotic therapy, and judicious surgical intervention, this disease should resolve without any complications. Here, we report a case of Ludwig’s angina in a 14-year-old boy. We also review the relevant anatomy and discuss the clinical presentation and current management of this disease

    The incidence of symptomatic venous thromboembolism following hip fractures with or without surgery in Taiwan

    AbstractBackgroundInformation on the incidence of venous thromboembolism (VTE) following hip fractures in Asia is rare. This study will investigate the epidemiology of symptomatic VTE in Taiwanese patients experiencing hip fractures.Methods and resultsWe used Taiwan's National Health Insurance Research Database to retrospectively identify patients (≧45years) who experienced hip fractures from 1998 to 2007 and were followed up for 3months after the discharge. Logistic regression analysis determined the independent risk factors of symptomatic VTE after the fractures. We identified 134,034 patients (mean age: 76.2±9.7years; female: 57.8%) who experienced hip fractures, 83.2% of whom underwent hip surgery. The overall pharmacological thromboprophylaxis rate was 2.7%. The mean length of stay was 11.3±7.9days. The 3-month cumulative incidence of symptomatic VTE was 77 events per 10,000 persons. Multivariate analysis showed that previous DVT, previous PE, varicose veins, cancer, heart failure, renal insufficiency, and older age were independent risk factors of developing VTE.ConclusionsThe incidence of symptomatic VTE after hip fractures is low in Taiwan. Patients rarely received pharmacological thromboprophylaxis following hip fractures. Universal thromboprophylaxis for patients experiencing hip fractures was not necessary in Taiwan, but it should be considered in high-risk populations

    Prediction and validation of the unexplored RNA-binding protein atlas of the human proteome

    Detecting protein-RNA interactions is challenging both experimentally and computationally because RNAs are large in number, diverse in cellular location and function, and flexible in structure. As a result, many RNA-binding proteins (RBPs) remain to be identified. Here, a template-based, function-prediction technique SPOT-Seq for RBPs is applied to human proteome and its result is validated by a recent proteomic experimental discovery of 860 mRNA-binding proteins (mRBPs). The coverage (or sensitivity) is 42.6% for 1217 known RBPs annotated in the Gene Ontology and 43.6% for 860 newly discovered human mRBPs. Consistent sensitivity indicates the robust performance of SPOT-Seq for predicting RBPs. More importantly, SPOT-Seq detects 2418 novel RBPs in human proteome, 291 of which were validated by the newly discovered mRBP set. Among 291 validated novel RBPs, 61 are not homologous to any known RBPs. Successful validation of predicted novel RBPs permits us to further analysis of their phenotypic roles in disease pathways.     Correlation of the composite equilibrium score of computerized dynamic posturography and clinical balance tests

    AbstractBackgroundThe computerized dynamic posturography has been widely used to access balance control in patients with balance dysfunction. A composite-equilibrium score (CS) can be calculated from the sensory organization test using the computerized dynamic posturography. However, the correlation between the composite equilibrium score and clinical tests and its ability to predict falls has rarely been explored in the past.MethodsA total of 60 patients with chief complaint of dizziness were enrolled in our study, and clinical assessments were done including the sensory organization test (SOT), Timed Up and Go test (TUG), Tinetti Performance-Oriented Mobility Assessment (POMA), and the dynamic gait index (DGI). The age and the subjective feeling of the severity of dizziness quantified by the visual analog scale (VAS) of each patient were also recorded.ResultsStatistical analysis revealed significant correlation between the composite equilibrium score and the TUG, POMA (gait, balance and total scores), and the DGI. However, there is statistically significant correlation between neither the CS and the age nor the VAS of dizziness. When grouping the DGI, POMA (total score), and the TUG cutoff to predict fall risks, the correlations to the CS can still be established except the TUG.ConclusionFrom the results of our study, the validity of the clinical tests was established in assessment of balance function, and clinicians can utilize these tools for preliminary evaluation of patient balance when computerized dynamic posturography is not available. In addition, CS can be used to predict the risk of falls

    The Liquid Sensor Using Thin Film Bulk Acoustic Resonator with C-Axis Tilted AlN Films

    Dual-mode thin film bulk acoustic resonator (TFBAR) devices are fabricated with c-axis tilted AlN films. To fabricate dual-mode TFBAR devices, the off-axis RF magnetron sputtering method for the growth of tilted piezoelectric AlN thin films is adopted. In this report, the AlN thin films are deposited with tilting angles of 15° and 23°. The frequency response of the TFBAR device with 23° tilted AlN thin film is measured to reveal its ability to provide dual-mode resonance. The sensitivities of the longitudinal and shear modes to mass loading are calculated to be 2295 Hz cm2/ng and 1363 Hz cm2/ng with the mechanical quality factors of 480 and 287, respectively. The sensitivities of the longitudinal and shear modes are calculated to be 0 and 15 Hz cm2/ÎŒg for liquid loading

    Charting the Unexplored RNA-binding Protein Atlas of the Human Genome

    poster abstractDetecting protein-RNA interactions is challenging–both experimentally and computationally– because RNAs are large in number, diverse in cellular location and function, and flexible in structure. As a result, many RNA-binding proteins (RBPs) remain to be identified and characterized. Recently, we developed a bioinformatics tool called SPOT-Seq that integrates template-based structure prediction with RNA-binding affinity prediction to predict RBPs. Application of SPOT-Seq to human genome leads to doubling of RBPs from 2115 to 4296. Half of novel (>2000) RBPs are poorly or not annotated. The other half possesses Gene Ontology leaf IDs that are associated with known RBPs. In particular, we identified 36 novel RBPs in cancer, cardiovascular, diabetes and neurodegenerative pathways and 26 novel RBPs associated with disease-causing SNPs. Half of these disease-associating, predicted novel RBPs are annotated to interact with known RBPs. Accuracy of predicted novel RBPs is further validated by same confirmation rate of novel and annotated RBPs in human proteome microarrays experiments. The large number of predicted novel RBPs and their abundance in disease pathways and disease-causing SNPs are useful for hypothesis generation.     Manufacture of Micromirror Arrays Using a CMOS-MEMS Technique

    In this study we used the commercial 0.35 Όm CMOS (complementary metal oxide semiconductor) process and simple maskless post-processing to fabricate an array of micromirrors exhibiting high natural frequency. The micromirrors were manufactured from aluminum; the sacrificial layer was silicon dioxide. Because we fabricated the micromirror arrays using the standard CMOS process, they have the potential to be integrated with circuitry on a chip. For post-processing we used an etchant to remove the sacrificial layer and thereby suspend the micromirrors. The micromirror array contained a circular membrane and four fixed beams set symmetrically around and below the circular mirror; these four fan-shaped electrodes controlled the tilting of the micromirror. A MEMS (microelectromechanical system) motion analysis system and a confocal 3D-surface topography were used to characterize the properties and configuration of the micromirror array. Each micromirror could be rotated in four independent directions. Experimentally, we found that the micromirror had a tilting angle of about 2.55° when applying a driving voltage of 40 V.     Incidence of type 2 diabetes mellitus in men receiving steroid 5α-reductase inhibitors:population based cohort study

    OBJECTIVE: To investigate the incidence of new onset type 2 diabetes mellitus in men receiving steroid 5α-reductase inhibitors (dutasteride or finasteride) for long term treatment of benign prostatic hyperplasia. DESIGN: Population based cohort study. SETTING: UK Clinical Practice Research Datalink (CPRD; 2003-14) and Taiwanese National Health Insurance Research Database (NHIRD; 2002-12). PARTICIPANTS: Men in the CPRD who received dutasteride (n=8231), finasteride (n=30 774), or tamsulosin (n=16 270) were evaluated. Propensity score matching (2:1; dutasteride to finasteride or tamsulosin) produced cohorts of 2090, 3445, and 4018, respectively. In the NHIRD, initial numbers were 1251 (dutasteride), 4194 (finasteride), and 86 263 (tamsulosin), reducing to 1251, 2445, and 2502, respectively, after propensity score matching. MAIN OUTCOMES MEASURE: Incident type 2 diabetes using a Cox proportional hazard model. RESULTS: In the CPRD, 2081 new onset type 2 diabetes events (368 dutasteride, 1207 finasteride, and 506 tamsulosin) were recorded during a mean follow-up time of 5.2 years (SD 3.1 years). The event rate per 10 000 person years was 76.2 (95% confidence interval 68.4 to 84.0) for dutasteride, 76.6 (72.3 to 80.9) for finasteride, and 60.3 (55.1 to 65.5) for tamsulosin. There was a modest increased risk of type 2 diabetes for dutasteride (adjusted hazard ratio 1.32, 95% confidence interval 1.08 to 1.61) and finasteride (1.26, 1.10 to 1.45) compared with tamsulosin. Results for the NHIRD were consistent with the findings for the CPRD (adjusted hazard ratio 1.34, 95% confidence interval 1.17 to 1.54 for dutasteride, and 1.49, 1.38 to 1.61 for finasteride compared with tamsulosin). Propensity score matched analyses showed similar results. CONCLUSIONS: The risk of developing new onset type 2 diabetes appears to be higher in men with benign prostatic hyperplasia exposed to 5α-reductase inhibitors than in men receiving tamsulosin, but did not differ between men receiving dutasteride and those receiving finasteride. Additional monitoring might be required for men starting these drugs, particularly in those with other risk factors for type 2 diabetes
