    Foreign bodies in the torax

    Background. Metallic foreign bodies in the lung could be recognized using radiography. Non-metallic foreign bodies make difficulties because they are not as dense as metals. The aim of this report was to present the case of nonmetallic foreign bodies in the lung. Case report. A soldier of the Federation Army of Bosnia and Hertzegovina, injured in the explosion in 1998 was presented. The soldier was subjected to thoracic drainage and surgery tretment of the wound. After one year, the pulmologist treated him with tuberculostatics because of hemoptysis and pulmonary infiltration shown by a radiogram. This therapy had no effects, and the pulmologist presented this case to the thoracic surgeon who made thoracotomy removing a piece of wood (13 × 2 × 0.7 cm), 20 different-size pieces of wood, and a piece of textile from the lung. Conclusion. Patients with penetrating explosive lung injuries without metallic foreign bodies shown by a radiogram, with complications as hemoptisis, lung abscess, bronchiectasis, obstruction of the bronchus, chronic pneumonia should be subjected to thoracotomy for removing suspected foreign bodies without resecting the lung

    Acute descending necrotizing mediastinitis as a complication of the retropharyngeal abscess caused by anaerobes

    Descending necrotizing mediastinitis (DNM) is a rare, life-threatening form of mediastinitis caused by odontogenic, pharyngeal, or cervical infections. The retropharyngeal space is the most common primary site of infection. Given the fulminant course and high mortality rate, early diagnosis and prompt treatment are important predictors of survival in patients with DNM. Appropriate empirical antibiotic treatment, prompt surgical intervention, and proper management of patients in the intensive care unit can be of vital importance. We present the case of a previously healthy 20-year-old male patient who was successfully cured and discharged from the Clinical Center University of Sarajevo after suffering from a severe form of mediastinitis as a complication of the retropharyngeal abscess caused by anaerobes

    Thoracosurgical treatment of pleural complications in patients with coronavirus disease 2019 (COVID-19): A cross-sectional study

    Introduction: Pleural complications in coronavirus disease 2019 (COVID-19) are relatively uncommon findings. Pleural involvement in these patients may directly correlate to disease severity and overall prognosis. We aimed to review clinical features and treatment approaches for pleural complications (accumulation of fluid/air inside the pleural cavity) in 45 patients with COVID-19, who were treated at our institution between April 2020 and October 2021. Methods: Our study was designed as single-center, observational, cross-sectional study of 45 patients with COVID-19 and at least one radiologically verified pleural complication. Demographic data, radiological findings, as well as type and number of thoracosurgical intervention(s) were recorded for every patient. We included patients of both genders and various age groups, with positive RT-PCR assay for COVID-19 and radiologic features of pleural complications, which required single or multiple thoracosurgical interventions. Results: Unilateral pleural complications were more common, right-sided pleural complications were found in 44.4% of patients. Right-sided pneumothorax was reported in 26.7% of patients. Almost one-fourth of our patients required invasive mechanical ventilation. Tube thoracostomy was performed in 84.4% of patients with unilateral pleural complications. A fatal outcome was most common in patients over 60 years old. More than half of patients with bilateral pleural complications died in our study. Conclusions: Pleural complications are a rare finding in patients with COVID-19. Tube thoracostomy is the mainstay of treatment for most symptomatic patients with pleural complications. Future research should be directed toward investigation of long-term pulmonary consequences in patients with COVID-19

    Krizni menadžment je ključan za uspješan izlazak iz bilo koje krizne situacije u organizaciji. Svaka kriza ima svoje uzroke koje je potrebno identificirati i riješiti. Isto tako, svaka kriza ima veliki potencijal za novi uzlet organizacije, što treba prepoznati i iskoristiti. I pojedinci i organizacije imaju na raspolaganju i vrijeme i resurse potrebne za upravljanje krizom, no često im nedostaju vještine i tehnike da bi se s teškoćama mogli uspješno nositi. Pojam razvoj poslovnih aplikacija direktno je povezan s potrebom rješavanja poslovnih problema. Naime, kada se u poslovanju tvrtke pojavi neki problem javlja se potreba da se taj problem riješi. Pri rješavanju problema najvažnije je razumjeti samu prirodu problema, otkriti bit problema. Informacijska tehnologija ma koliko savršena bila samo je sredstvo u rješavanju problema. Pri rješavanju problema koriste se različitim metodama i tehnikama, alatima, procedurama i paradigmama sve u cilju generiranja efikasnog rješavanja problema. Danas se kao mogućnost traženja rješenja problema obično spominje primjena informacijskih tehnologija i odgovarajućih softverskih rješenja. Prevencija je bolja od rješenja koja dolaze za vrijeme krize ili nakon krize kada se mogu jedino zbrajati posljedice i formirati statistika nesretnih događaja. Najbolje su preventivne situacije u kojima se može potpuno otkloniti ili skrenuti prijetnju, ali treba biti spreman i na alternativna rješenja za bilo koji oblik krizne situacije na koju se ne može preventivno djelovati

    A model-based kinetic modelling approach for assessment of low-rank coal and biomass co-combustion characteristics

    Low-rank coal, characterized by its high moisture, ash, and volatile matter content, besides the significant contribution to GHG emission, can be challenging to combust efficiently. Biomass, on the other hand, is a renewable energy source that can enhance combustion performance and reduce emissions. The transition from fossil fuels to renewables should involve the co-combustion of coal and biomass which requires a detailed understanding of their individual and combined combustion kinetics. In this paper, the model-based kinetic modelling approach is applied to studying the thermal decomposition of coal and biomass using a combination of experimental data and mathematical models. By quantifying the rates at which various components of these fuels degraded and release heat, the optimization of process parameters, enhanced energy efficiency, and minimized environmental impacts could be achieved. The obtained results presented in the manuscript offers several advantages in comparison with the standard fuel characterization techniques. For example, the non-additive issues for different fuel characteristics could be overcome. Furthermore, the ability to predict combustion behaviour under different operating conditions, optimize burner designs, and develop cleaner combustion technologies could be enabled. In that sense, performed research in this paper could further supports the transition towards sustainable and carbon-neutral energy systems by harnessing the energy potential of low-rank coal while utilizing biomass as a clean and renewable co-fuel.Power Plants 2023 : Elektrane 2023; November 8-10, 2023, Zlatibor, Serbi
