    Normal limit for serum alanine aminotransferase level and distribution of metabolic factors in old population of Kalaleh, Iran

    Backgrounds: Normal or elevated values of serum alanine aminotransferase level (ALT) vary in different studies mostly related to characteristics of reference population including age, gender, body mass index, nonalcoholic fatty liver disease (NAFLD), and metabolic syndrome prevalence. Objectives: To measure upper normal limit (UNL) for serum ALT in an apparently healthy Iranian old population (which we had not sufficient data before this study), and its modulating factors. Patients and Methods: All inhabitants (> 50 years old) of Kalaleh, Golestan, Iran (N = 1986) were invited to the study. ALT measurements were performed for all subjects using the same laboratory method. Upper limit of normal (ULN) ALT was calculated based on its 95th percentile in normal weight subjects. Modulating factors of ALT were determined by multivariate analysis. Results: A total of 1309 subjects, with the mean age of 61.5 ± 7.5 years were included. UNL of ALT was 18.8 U/L and 21.4 U/L in women and men, respectively. Based on univariate analysis, waist circumference (r = 0.124, P = 0.01), body mass index (r = 0.118, P = 0.01), triglyceride (r = 0.143, P = 0.01), and having metabolic syndrome (OR = 2.04) modulate ALT levels in men. Also triglyceride (r = 0.119, P = 0.01) modulates ALT levels in women. Conclusions: The calculated level for UNL of ALT is considerably far lower than previous accepted value. Age, gender, ethnicity, and metabolic factors should be accounted in future studies to determine normal ALT level. © 2013, Kowsar Corp.; Published by Kowsar Corp

    Wireless Health Monitoring using Passive WiFi Sensing

    This paper presents a two-dimensional phase extraction system using passive WiFi sensing to monitor three basic elderly care activities including breathing rate, essential tremor and falls. Specifically, a WiFi signal is acquired through two channels where the first channel is the reference one, whereas the other signal is acquired by a passive receiver after reflection from the human target. Using signal processing of cross-ambiguity function, various features in the signal are extracted. The entire implementations are performed using software defined radios having directional antennas. We report the accuracy of our system in different conditions and environments and show that breathing rate can be measured with an accuracy of 87% when there are no obstacles. We also show a 98% accuracy in detecting falls and 93% accuracy in classifying tremor. The results indicate that passive WiFi systems show great promise in replacing typical invasive health devices as standard tools for health care.Comment: 6 pages, 8 figures, conference pape

    Hierarchical Design Based Intrusion Detection System For Wireless Ad hoc Network

    In recent years, wireless ad hoc sensor network becomes popular both in civil and military jobs. However, security is one of the significant challenges for sensor network because of their deployment in open and unprotected environment. As cryptographic mechanism is not enough to protect sensor network from external attacks, intrusion detection system needs to be introduced. Though intrusion prevention mechanism is one of the major and efficient methods against attacks, but there might be some attacks for which prevention method is not known. Besides preventing the system from some known attacks, intrusion detection system gather necessary information related to attack technique and help in the development of intrusion prevention system. In addition to reviewing the present attacks available in wireless sensor network this paper examines the current efforts to intrusion detection system against wireless sensor network. In this paper we propose a hierarchical architectural design based intrusion detection system that fits the current demands and restrictions of wireless ad hoc sensor network. In this proposed intrusion detection system architecture we followed clustering mechanism to build a four level hierarchical network which enhances network scalability to large geographical area and use both anomaly and misuse detection techniques for intrusion detection.     In this paper, (3+1)-dimensional Jimbo-Miwa and (3+1)-dimensional potential-YTSF equations are considered and the Exp-Function method is employed to compute the exact solutions. The solutions obtained by this method are compared with the exact solutions obtained through other methods. These equations play a very important role in mathematical physics and engineering sciences.     Concept and challenges of delivering preventive and care services in prehospital emergency medical service: A qualitative study

    Background and purpose: Prehospital emergency medical service (EMS) is an important part of the health care system. Today, the need for integrated emergency care services and delivering qualified preventive and care services felt more than before in accidents and emergencies. This survey aimed to investigate the views of emergency medical personnel on the concept and nature of preventive and care services and current challenges in delivering these services in prehospital EMS centers in Golestan province, Iran. Materials and methods: A qualitative study was done with 16 emergency medical personnel working in EMS sites in Golestan province using purposeful sampling. Data was collected by semistructured interview guide and framework analysis was implemented to analyze the data. Results: Two general themes were identified including 1) the concept of preventive and care services in the EMS and 2) the challenges in providing qualified services in the EMS sites in Golestan province. Also, five sub-themes and 12 subclasses were determined. Conclusion: To enhance and promote services, the EMS system of Golestan province need reforms in organizational structure, laws and administrative regulations, training programs, the system of monitoring and evaluation of personnel, also provision of equipment and manpower and provision of personnel amenities to improve the staff performance. Furthermore, it is necessary to boost emergency services at the community level through training people and enhancing collaboration and participation with other organizations. © 2015, Mazandaran University of Medical Sciences.     On the cytotoxicity of carbon nanotubes

    The cytotoxicity of carbon nanotubes (CNTs) is a major concern today well before its unusual physicochemical, mechanical, and electrical properties are fully exploited for commercial interests and subsequent mass production leading to greater possibilities for its exposure to humans and the environment. Contradictory reports on cytotoxicity of CNTs often appear in the literature and a mechanistic explanation of the reported toxicity remains obscure. We review here the conflicting results to focus categorically on an array of issues in CNT cytotoxicity. They include dispersion, aggregation status, coating or functionalization and immobilization, cellular uptake or internalization, purity in terms of metal catalyst contaminants, size and size distribution, surface area, surface chemistry and surface reactivity, cell types selected for experimentation as well as bioassay of nanotoxicity itself attesting as an issue in cytotoxicity. Recently a general agreement has emerged towards the potential toxicity of CNTs, although various paradigms explaining the mechanisms of CNT cytotoxicity continue to be elusive in the literature. A lack of synergy among various issues while studying cytotoxicity and most developed paradigms for the mechanism of CNT toxicity is highlighted

    The effect of adjuvant vitamin C after varicocele surgery on sperm quality and quantity in infertile men: A double blind placebo controlled clinical trial

    Varicocele is one of the most common causes of male infertility and spontaneous pregnancy rate after varicocelectomy is only about 30. The most important seminal antioxidant is vitamin C but recent studies about the effects of vitamin C on spermatogenesis are controversial; therefore, we decided to evaluate its role after varicocelectomy. In a double blind randomized controlled clinical trial, 115 men with infertility and clinical varicocele with abnormal semen analyses were recruited. After surgery, the intervention group received vitamin C (250 mg bid) and the control group received placebo for three months. Mean sperm count, motility, and morphology index of two semen analyses (before and after surgery) were compared between the two groups. Univariate general linear model and stepwise linear regression were used in analysis. The mean age (±SD) of participants was 27.6±5.3 years. Vitamin C group had statistically significant better normal motility (20.8 vs. 12.6, P=0.041) and morphology (23.2 vs. 10.5, P<0.001) than placebo group. Considering the values prior to surgery as covariate, vitamin C was not effective on sperm count (P=0.091); but it improved sperm motility (P=0.016) and morphology (P<0.001) even after excluding the confounding effect of age (P=0.044 and P=0.001, respectively). Vitamin C was also an independent factor in predicting motility and normal morphology after surgery. Ascorbic acid can play a role as adjuvant treatment after varicocelectomy in infertile men

This study was aimed at determine appropriate pattern for Iran using comparative survey of Health benefit package in various countries. A review exploration was done, scholars was selected population of both developed and developing countries, required information was also extracted by articles, searches and reports of reliable sources and date were analyzed by SPSS, in brief. The vast majority frequencies was respectively allocated to accessibility (40.7%), cost- effectiveness (29.6%), prioritize, efficacy and cost (22.2%). most countries located in WHO African region were selected cost-effectiveness and accessibility, WHO southeast Asia region were selected, coverage, prioritize, efficacy and quality and finally most WHO Europeans region were elected effectiveness and services costs for including services in Health benefit package. According to most Health benefit package designer emphasis on criteria including accessibility and costeffectiveness, to design Health benefit package for Iran, these criteria must be noticed
