    Social Marketing: Planning Before Conceiving Preconception Care

    Social marketing approaches can help to shape the formation of and to create demand for preconception care services. This article describes four components of social marketing, often referred to as the 4 P’s, that should be carefully researched and set in place before a national effort to launch and sustain preconception care services is pursued. First, the product or package of services must be defined and adapted using the latest in scientific and health care standards and must be based on consumer needs and desires. Second, the pricing of the services in financial or opportunity costs must be acceptable to the consumer, insurers, and health care service providers. Third, the promotion of benefits must be carefully crafted to reach and appeal to both consumers and providers. Fourth, the placement and availability of services in the marketplace must be researched and planned. With the application of market research practices that incorporate health behavior theories in their exploration of each component, consumer demand for preconception care can be generated, and providers can take preconception care to the market with confidence

    Serum screening with Down's syndrome markers to predict pre-eclampsia and small for gestational age: Systematic review and meta-analysis

    <p>Abstract</p> <p>Background</p> <p>Reliable antenatal identification of pre-eclampsia and small for gestational age is crucial to judicious allocation of monitoring resources and use of preventative treatment with the prospect of improving maternal/perinatal outcome. The purpose of this systematic review was to determine the accuracy of five serum analytes used in Down's serum screening for prediction of pre-eclampsia and/or small for gestational age.</p> <p>Methods</p> <p>The data sources included Medline, Embase, Cochrane library, Medion (inception to February 2007), hand searching of relevant journals, reference list checking of included articles, contact with experts. Two reviewers independently selected the articles in which the accuracy of an analyte used in Downs's serum screening before the 25<sup>th </sup>gestational week was associated with the occurrence of pre-eclampsia and/or small for gestational age without language restrictions. Two authors independently extracted data on study characteristics, quality and results.</p> <p>Results</p> <p>Five serum screening markers were evaluated. 44 studies, testing 169,637 pregnant women (4376 pre-eclampsia cases) and 86 studies, testing 382,005 women (20,339 fetal growth restriction cases) met the selection criteria. The results showed low predictive accuracy overall. For pre-eclampsia the best predictor was inhibin A>2.79MoM positive likelihood ratio 19.52 (8.33,45.79) and negative likelihood ratio 0.30 (0.13,0.68) (single study). For small for gestational age it was AFP>2.0MoM to predict birth weight < 10<sup>th </sup>centile with birth < 37 weeks positive likelihood ratio 27.96 (8.02,97.48) and negative likelihood ratio 0.78 (0.55,1.11) (single study). A potential clinical application using aspirin as a treatment is given as an example.</p> <p>There were methodological and reporting limitations in the included studies thus studies were heterogeneous giving pooled results with wide confidence intervals.</p> <p>Conclusion</p> <p>Down's serum screening analytes have low predictive accuracy for pre-eclampsia and small for gestational age. They may be a useful means of risk assessment or of use in prediction when combined with other tests.</p

    Caracterização de crianças e adolescentes atendidos por maus tratos em um hospital de emergência no município de Fortaleza-CE Characterization of maltreated children and adolescents taken into a paediatric emergency health care service in Fortaleza-CE-Brazil

    O estudo teve como objetivo caracterizar o perfil da criança e adolescente maltratados e atendidos em um hospital de emergência, no Município de Fortaleza, Ceará. Utilizou-se a pesquisa documental retrospectiva com análise dos formulários da Comissão de Maus Tratos à Infância e Adolescência. Os resultados revelaram que o sexo masculino e a faixa etária de 1 a 5 anos foram os mais atingidos; o principal agressor é o pai biológico e os tipos de violência mais freqüentes foram o hematoma, o abuso sexual e as lesões por objetos cortantes. Conclui-se que o tema requer estudos aprofundados que modifiquem o ambiente em que a criança, adolescente e famílias estão inseridos.<br>This study aimed to characterise the profile of the child and the adolescent who were maltreated and taken into paediatric emergency care in a hospital, in Fortaleza, Ceará. It was used a documented retrospective evaluation. The results showed that the children in the age group of 1-5 years were the most victimised as well as the children of the male sex. The main perpetrator was a biological father and the most frequent kind of violence were hematomas, sexual abuse and wounds by slashing objects. In conclusion, this subject needs to be more investigated for providing changes within the' environment where a child, adolescent and families were inserted
