Older adults experiences of rehabilitation in acute health care
Rehabilitation is a key component of nursing and allied healthcare professionals’ roles in most health and social care settings. This paper reports on stage 2 of an action research project to ascertain older adult's experience of rehabilitation. Twenty postdischarge interviews were conducted and the interview transcripts were analysed using thematic content analysis. All older adults discharged from an acute older acute rehabilitation ward to their own homes in the community were eligible to participate. The only exclusion criterion was older adults who were thought to be unable to give consent to participate by the nurse in charge and the researcher. Whilst 92 older adults were eligible to participate in this research study, only 20 were interviewed. The findings from this study suggest that older adults valued communication with health professionals but were aware of their time constraints that hindered communication. This study suggests that both nurses and allied health professionals are not actively providing rehabilitative services to promote health and well-being, which contradicts the focus of active ageing. Furthermore, there was evidence of unmet needs on discharge, and older adults unable to recall the professions that were involved in their interventions and the rationale for therapy input. It is suggested that further research is needed to explore the effectiveness of allied health rehabilitation in the acute setting. Infant cortex responds to other humans from shortly after birth
A significant feature of the adult human brain is its ability to selectively process information about conspecifics. Much debate has centred on whether this specialization is primarily a result of phylogenetic adaptation, or whether the brain acquires expertise in processing social stimuli as a result of its being born into an intensely social environment. Here we study the haemodynamic response in cortical areas of newborns (1–5 days old) while they passively viewed dynamic human or mechanical action videos. We observed activation selective to a dynamic face stimulus over bilateral posterior temporal cortex, but no activation in response to a moving human arm. This selective activation to the social stimulus correlated with age in hours over the first few days post partum. Thus, even very limited experience of face-to-face interaction with other humans may be sufficient to elicit social stimulus activation of relevant cortical regions
Linking dwarf galaxies to halo building blocks with the most metal-poor star in Sculptor
Current cosmological models indicate that the Milky Way's stellar halo was
assembled from many smaller systems. Based on the apparent absence of the most
metal-poor stars in present-day dwarf galaxies, recent studies claimed that the
true Galactic building blocks must have been vastly different from the
surviving dwarfs. The discovery of an extremely iron-poor star (S1020549) in
the Sculptor dwarf galaxy based on a medium-resolution spectrum cast some doubt
on this conclusion. However, verification of the iron-deficiency and
measurements of additional elements, such as the alpha-element Mg, are
mandatory for demonstrating that the same type of stars produced the metals
found in dwarf galaxies and the Galactic halo. Only then can dwarf galaxy stars
be conclusively linked to early stellar halo assembly. Here we report
high-resolution spectroscopic abundances for 11 elements in S1020549,
confirming the iron abundance of less than 1/4000th that of the Sun, and
showing that the overall abundance pattern mirrors that seen in low-metallicity
halo stars, including the alpha-elements. Such chemical similarity indicates
that the systems destroyed to form the halo billions of years ago were not
fundamentally different from the progenitors of present-day dwarfs, and
suggests that the early chemical enrichment of all galaxies may be nearly

Minimally invasive treatment of patients with bronchobiliary fistula: a case series
<p>Abstract</p> <p>Introduction</p> <p>Bronchobiliary fistula is an uncommon complication secondary to hepatobiliary surgery. Bilioptysis is a pathognomic finding for bronchobiliary fistulas. Diagnosis may be easily established in the light of clinical history, which can be aided by imaging studies to pinpoint the exact location. Some diagnostic procedures such as endoscopic retrograde cholangiopancreatectomy are also useful for treatment.</p> <p>Case presentation</p> <p>We present three Turkish patients with bronchobiliary fistula secondary to previous hepatic surgery due to hydatid cyst in two, a 19-year-old and a 47-year-old man, and iatrogenic trauma of the common bile duct by endoscopy in a 35-year-old man. All of the patients were successfully treated by minimally invasive methods including percutaneous drainage and endoscopic retrograde cholangiopancreatography.</p> <p>Conclusion</p> <p>We suggest that bronchobiliary fistula could be managed through conservative treatment methods which do not require in-hospital follow-up, particularly in uncomplicated cases. Otherwise, surgical management can be unavoidable.</p
Dynamics of direct inter-pack encounters in endangered African wild dogs
Aggressive encounters may have important life history consequences due to the potential for injury and death, disease transmission, dispersal opportunities or exclusion from key areas of the home range. Despite this, little is known of their detailed dynamics, mainly due to the difficulties of directly observing encounters in detail. Here, we describe detailed spatial dynamics of inter-pack encounters in African wild dogs (Lycaon pictus), using data from custom-built high-resolution GPS collars in 11 free-ranging packs. On average, each pack encountered another pack approximately every 7 weeks and met each neighbour twice each year. Surprisingly, intruders were more likely to win encounters (winning 78.6% of encounters by remaining closer to the site in the short term). However, intruders did tend to move farther than residents toward their own range core in the short-term (1 h) post-encounter, and if this were used to indicate losing an encounter, then the majority (73.3%) of encounters were won by residents. Surprisingly, relative pack size had little effect on encounter outcome, and injuries were rare (<15% of encounters). These results highlight the difficulty of remotely scoring encounters involving mobile participants away from static defendable food resources. Although inter-pack range overlap was reduced following an encounter, encounter outcome did not seem to drive this, as both packs shifted their ranges post-encounter. Our results indicate that inter-pack encounters may be lower risk than previously suggested and do not appear to influence long-term movement and ranging
F-Theorem without Supersymmetry
The conjectured F-theorem for three-dimensional field theories states that
the finite part of the free energy on S^3 decreases along RG trajectories and
is stationary at the fixed points. In previous work various successful tests of
this proposal were carried out for theories with {\cal N}=2 supersymmetry. In
this paper we perform more general tests that do not rely on supersymmetry. We
study perturbatively the RG flows produced by weakly relevant operators and
show that the free energy decreases monotonically. We also consider large N
field theories perturbed by relevant double trace operators, free massive field
theories, and some Chern-Simons gauge theories. In all cases the free energy in
the IR is smaller than in the UV, consistent with the F-theorem. We discuss
other odd-dimensional Euclidean theories on S^d and provide evidence that
(-1)^{(d-1)/2} \log |Z| decreases along RG flow; in the particular case d=1

Algorithms for flows over time with scheduling costs
Flows over time have received substantial attention from both an optimization and (more recently) a game-theoretic perspective. In this model, each arc has an associated delay for traversing the arc, and a bound on the rate of flow entering the arc; flows are time-varying. We consider a setting which is very standard within the transportation economic literature, but has received little attention from an algorithmic perspective. The flow consists of users who are able to choose their route but also their departure time, and who desire to arrive at their destination at a particular time, incurring a scheduling cost if they arrive earlier or later. The total cost of a user is then a combination of the time they spend commuting, and the scheduling cost they incur. We present a combinatorial algorithm for the natural optimization problem, that of minimizing the average total cost of all users (i.e., maximizing the social welfare). Based on this, we also show how to set tolls so that this optimal flow is induced as an equilibrium of the underlying game
