    Management of Khalitya through Pracchana followed by Vanadhanyakadi Lepa - A Case Report

    Alopecia is caused by androgen and genetic factors. Androgens cause hair follicles to shrink and eventually disappear if they are not addressed. It is inherited as an autosomal dominant trait. A 26-year-old male presented to the Panchakarma OPD Sri Dharmasthala Manjunatheshwara College of Ayurveda and Hospital, Hassan, with complaints of gradual hair loss and scalp itching over a two-year period. It was diagnosed as a case of Khalitya and was treated with Udwartana to Shira Pradesha, Pracchana, and Lepa application. The Norman Hamilton scale, hair pull test and symptoms before and after treatment were used to evaluate the case. Following treatment, there was a significant reduction in symptoms and significant hair growth

    Cortical suspensory endobutton versus aperture interference screw fixation for arthroscopic anterior cruciate ligament reconstruction using hamstring graft: a prospective study

    Background: Anterior cruciate ligament injury is a common injury which occurs in sports with an annual incidence of 68.6% per 100,000 population. Anatomic arthroscopic reconstruction remains the main stay treatment of ACL injury. Fixation of graft is classified as direct and indirect. To compare the stability of knee joint, complications in arthroscopic anterior cruciate ligament reconstruction by aperture interference screw fixation versus suspensory device fixation in femur. Methods: The randomized control study was conducted in department of orthopedics, The Oxford Medical College Hospital and RC from 2020-2021. 30 knees were operated, Ethical clearance taken and patient is followed up for six months. Results: The preoperative Tegner Lysholm knee score was poor in both groups. Post-op in interference screw group, 12 patients 80% had excellent functional outcome, 3 patients 20% had good outcome. In the Endbutton group, 11 patients (73%) had excellent outcome while 4 patients (27%) had good outcome. Conclusions: Suspensory device fixation gives equivalent results when compared to aperture screw fixation. complications of screw fixation were graft rupture cyst formation; complications of suspensory devices were loosening of graft and bungee cord effect. Hence, Both the techniques showed no statistical difference in post operative outcome

    Impact evaluation of marine fisheries interventions among Tribal Fisher Commune of Car Nicobar Island, India

    Fisheries and tourism are two important sectors that provide livelihood and employment in the tropical Islands. Andaman and Nicobar group of Islands is a biodiversity hotspot in the Bay of Bengal inhabited by settler population along with indigenous tribal communities. The Nicobar tribal community is one among occupying the Nicobar group of Islands in the majority and are quite socialized tribal communities of the Islands. Agriculture, animal husbandry and fisheries are the major sectors that contribute to the livelihood and nutritional security of the Nicobar tribes. Traditional fishing activities have been a part of their culture over the years however, tribal youths were reluctant in taking up fishing as a profession considering the changing scenario and alternative employment options. To reinstate fisheries as a profession and to improve their livelihood and employment opportunities, interventions were carried out in Car Nicobar Island. The interventions were mainly aimed towards increasing fish catches, to expand their fishing grounds and to sensitize safe fishing and navigation practices. Fishing inputs such as modern fishing boats, GPS and outboard engines were provided as inputs to tribal fishers based on the outcome of an expert consultation meeting. Structured questionnaires were prepared to analyze the impact of the interventions. The results indicated that the interventions led to a significant improvement (p <0.001) in the expansion of their fishing grounds (3.47 ± 0.2 km), reduced time spent to reach the fishing grounds (2.3 ± 0.25 hour) and increased catch per unit effort (7.93 ± 1.23 kg/boat). This study shows that the targeted interventions coupled with awareness and sensitization programmes could make fishing activities profitable, attractive and can motivate the unemployed youths to opt for fishing as a profession

    Quantification and visualization of the variability of phytoplankton assemblage in a semi-lotic seasonal Canal in Sundarbans, India

    Canals of Sundarbans in India are highly vulnerable to anthropogenic pressures which directly or indirectly impact the biological components of such ecosystems. Being at the base of the trophic pyramid, phytoplankton is exposed to environmental stressors and the impact will be reflected in the upper strata of the aquatic food chain. Hence, phytoplankton communities along with their interrelationship with environmental parameters were assessed in Bhetkimari canal, a semi-lotic system in Indian Sundarbans during June 2017 to September 2018. Seventy-seven species under 66 genera were recorded with Cyanophyceae (34%) as a major contributor, followed by Bacillariophyceae (31%) and Chlorophyceae (22%). Diatoms dominated round the year in terms of diversity (25 species) with maximum contribution from Pennales. ANOVA (post-hoc test) showed significant temporal heterogeneity (p≤0.05) in phytoplankton distribution in the canal. The Margalef richness index (d) and Shannon-Weiner diversity index (H′) were 2.59±0.43 and 2.26±0.59, respectively indicating that the environment is good and phytoplankton diversity in the system is moderate. Water temperature, dissolved oxygen, total alkalinity and nutrients (nitrate, silicate and phosphate) were the explanatory variables in shaping the phytoplankton assemblages in the canal, which was evident from Canonical correspondence analysis.     Impact of Second Primary Malignancy Post–Autologous Transplantation on Outcomes of Multiple Myeloma: A CIBMTR Analysis

    The overall survival (OS) has improved significantly in multiple myeloma (MM) over the last decade with the use of proteasome inhibitor and immunomodulatory drug-based combinations, followed by high-dose melphalan and autologous hematopoietic stem cell transplantation (auto-HSCT) and subsequent maintenance therapies in eligible newly diagnosed patients. However, clinical trials using auto-HSCT followed by lenalidomide maintenance have shown an increased risk of second primary malignancies (SPM), including second hematological malignancies (SHM). We evaluated the impact of SPM and SHM on progression-free survival (PFS) and OS in patients with MM after auto-HSCT using CIBMTR registry data. Adult patients with MM who underwent first auto-HSCT in the United States with melphalan conditioning regimen from 2011 to 2018 and received maintenance therapy were included (n = 3948). At a median follow-up of 37 months, 175 (4%) patients developed SPM, including 112 (64%) solid, 36 (20%) myeloid, 24 (14%) SHM, not otherwise specified, and 3 (2%) lymphoid malignancies. Multivariate analysis demonstrated that SPM and SHM were associated with an inferior PFS (hazard ratio [HR] 2.62, P \u3c .001 and HR 5.01, P \u3c .001, respectively) and OS (HR 3.85, P \u3c .001 and HR 8.13, P \u3c .001, respectively). In patients who developed SPM and SHM, MM remained the most frequent primary cause of death (42% vs 30% and 53% vs 18%, respectively). We conclude the development of SPM and SHM leads to a poor survival in patients with MM and is an important survivorship challenge. Given the median survival for MM continues to improve, continued vigilance is needed to assess the risks of SPM and SHM with maintenance therapy post-auto-HSCT

    Not AvailableTropical marine Islands are home to rich and highly endemic biodiversity because of their geographical seclusion. The geographical isolation supports the biodiversity against anthropogenic activities despite being vulnerable to natural disasters. Marine Islands hosts invaluable marine resources such as coral reefs, seagrass beds, mangroves and its associated fisheries which play an important role in livelihood of the coastal communities through tourism and fishing. It is important to judiciously protect and manage these islands to conserve the rich terrestrial and marine biodiversity it hosts. Andaman and Nicobar Islands (ANI), India is a strategically located linear chain of 572 tropical Islands located in Bay of Bengal in close proximity to South East Asian (SEA) countries such as Myanmar, Thailand and Indonesia. This proximity attracts poachers from SEA countries for harvesting living marine resources in the Exclusive Economic Zone (EEZ) of ANI posing significant threat to marine biodiversity and coastal security. Prudent approaches are required to address the issue of poaching by involving various stakeholders and a science-policy interface at regional level. Implementation of efficient and reliable monitoring-reporting systems could be worthy to address the issue of poaching. Mobile apps help lines, fishermen groups, aerial vehicles, Monitoring, Control and Surveillance (MCS) system and other satellite based tools can effectively deter poaching and illegal fishing. The importance of community participation and the ways through which the modern tools and community participation could compliment in curbing the incidences of poaching were discussed. Necessary precautionary approaches are essential at this point in ANI as it was understood through the review that poachers have gone down in recent years and this period could be effectively utilized towards planning and efficient management of such incidences in future. The status of poaching and its associated activities over these years and the possibilities on implementation of suitable monitoring and reporting systems in collaboration with stakeholders were discussed.Not Availabl

Conservationists need ecological indicators showing the level of alterations or impairment for decision making in managing these important resources. Fish are widely used as sentinel species to evaluate ecological health and are excellent indicators of overall health of aquatic systems they inhabit. This article focuses on briefing some reliable and useful approaches in assessing the ecosystem health in which fishes are used as indicator organism.     The Outcomes of Selective Nerve Root Block for Disc Induced Lumbar Radiculopathy

    Introduction : Use of Selective Nerve Root Block (SNRB) in lumbar region to relieve radicular pain along the course of a particular nerve is being practiced in the recent past. We observed and tabulated therapeutic outcomes of this procedure in management of radiculopathy induced by a prolapsed disc affecting a particular lumbar nerve root. Methods : 40 patients with various grades of disc prolapse affecting a particular lumbar nerve root presenting with chronic radicular pain were identified irrespective of age and sex. All were injected with a combination of 40 mg of Methylprednisolone based suspension with local anaesthetic over the affected nerve root and results were analyzed. Results : Those graded mild had 4.3 months relief and those graded moderate had 2.5 months relief. Those with severe disc prolapse had no relief except for the immediate postprocedural relief. Only 20% patients had relief up to 6 months. Conclusion : Effect of SNRB is typically short acting in majority of patients and recurrence is expected. It creates a window period with reduced pain but of varied intervals depending on the pathology. It did not alter the prognosis in those with severe disease where surgery is well indicated.
