The role of low-frequency intraseasonal oscillations in the anomalous Indian summer monsoon rainfall of 2002
We analyze the dynamical features and responsible factors of the low-frequency intraseasonal time scales which influenced the nature of onset, intensity and duration of active/break phases and withdrawal of the monsoon during the anomalous Indian summer monsoon of 2002 - the most severe drought recorded in recent times. During that season, persistent warm sea surface temperature anomalies over the equatorial Indian Ocean played a significant role in modulating the strength of the monsoon Hadley circulation. This in turn affected the onset and intense break spells especially the long break during the peak monsoon month of July. Strong low-frequency intraseasonal modulations with significant impact on the onset and active/break phases occurred in 2002 which were manifested as a good association between low-frequency intraseasonal oscillations and the onset and active/break spells. Further, SST anomalies over the equatorial Indo-Pacific region on low-frequency intraseasonal time scales were found to affect the equatorial eastward and thereby off-equatorial northward propagations of enhanced convection over the Indian region. These propagations in turn modulated the active/break cycle deciding the consequent severity of the 2002 drought
Careseeking for illness in young infants in an urban slum in India
Illness in infants in the first two months of life can take a precipitous life-threatening course, and requires timely and appropriate medical assessment and management. We conducted a focused ethnographic study of illness in young infants and associated careseeking practices in an urban slum in New Delhi, India, in order to identify the constraints in securing effective care for severe illness in this age group. The findings suggest that maternal recognition of illness is not a limiting factor in the use of health care services for sick young infants in this setting. Mothers respond to a number of important signs of illness, including changes in the young infant's sleeping or feeding behavior, and they are usually prompt in seeking care outside the home. They are not able, however, to discriminate among the many sources of health care available in this setting, and give preference to local unqualified private practitioners. Most practitioners, including qualified medical practitioners, display critical failures in the assessment and management of sick young infants. The continuity and effectiveness of care is further compromised by the caretakers' expectations of rapid cure, which result in discontinued treatment courses and frequent changes in practitioners, and by their reluctance to seek hospital care. The implications of these findings for the design of programs to reduce young infant mortality are discussed. In particular, the feasibility and acceptability of hospital referrals according to current program guidelines are called into question
Insight into the molecular pathophysiology of myelodysplastic syndromes: targets for novel therapy
Myelodysplastic syndromes (MDS) are clonal hematopoietic stem cell disorders characterized by abnormal cellular differentiation and maturation with variable progression to acute leukemia. Over the last decade, scientific discoveries have unraveled specific pathways involved in the complex pathophysiology of MDS. Prominent examples include aberrations in cytokines and their signaling pathways (such as tumor necrosis factor-alpha, interferon-gamma, SMAD proteins), mutations in genes encoding the RNA splicing machinery (SF3B1, SRSF2, ZRSR2, and U2AF1 genes), mutations in genes disrupting the epigenetic machinery (TET2, DNMT3A, DNMT3B, EZH2, ASXL1). In addition, abnormalities in regulatory T-cell dynamics and atypical interactions between the bone marrow microenvironment, stroma and progenitor cells, and abnormal maintenance of telomeres are also notable contributors to the complex pathogenesis of MDS. These pathways represent potential targets for novel therapies. Specific therapies include drugs targeting aberrant DNA methylation and chromatin remodeling, modulating/activating the immune system to enhance tumor-specific cellular immune responses and reduce anomalous cytokine signaling, and blocking abnormal interaction between hematopoietic progenitors and stromal cells
Psychosocial and behavioural interventions towards HIV risk reduction for serodiscordant couples in Africa: A systematic review
Background: Sexual transmission of HIV frequently occurs in the context of a primary relationship between two partners; however, HIV prevention interventions generally focus on individuals at risk, rather than specifying couples as a unit of change and analysis, neglecting the crucial role that partners may play in sexual behaviour. This article reviews published scientific literature addressing couple-oriented HIV counselling and testing and other behavioural interventions using an online search for peer-reviewed papers.
Methods: A systematic review was conducted to evaluate what has been published on psychosocial interventions in HIV serodiscordant couples in Africa. Electronic databases were searched from January 1990 to December 2015. Quality assessment of included studies was conducted using the Systematic Appraisal of Quality in Observational Research tool.
Results: The electronic database searches initially retrieved 493 records; after cross-referencing, removing duplicates and applying strict inclusion and exclusion criteria, only eight papers were included in this review. All the studies under review showed that couples-focused counselling and educational programmes were associated with positive outcomes including reduced HIV transmission, reduced unprotected sex, increased rates of status disclosure and high levels of treatment adherence.
Conclusions: The literature on interventions for HIV serodiscordant couples is sparse. However, most interventions indicate that couples-focused interventions are effective in HIV risk reduction. In spite of the limited available data and repeated recommendations by different health authorities, couple-centred approaches to HIV prevention have not been implemented on a large scale
Characteristics of spectral aerosol optical depths over India during ICARB
Spectral aerosol optical depth (AOD) measurements, carried out regularly from a network of observatories spread over the Indian mainland and adjoining islands in the Bay of Bengal and Arabian Sea, are used to examine the spatio-temporal and spectral variations during the period of ICARB (March to May 2006). The AODs and the derived Angstrom parameters showed considerable variations across India during the above period. While at the southern peninsular stations the AODs decreased towards May after a peak in April, in the north Indian regions they increased continuously from March to May. The Angstrom coefficients suggested enhanced coarse mode loading in the north Indian regions, compared to southern India. Nevertheless, as months progressed from March to May, the dominance of coarse mode aerosols increased in the columnar aerosol size spectrum over the entire Indian mainland, maintaining the regional distinctiveness. Compared to the above, the island stations showed considerably low AODs, so too the northeastern station Dibrugarh, indicating the prevalence of cleaner environment. Long-range transport of aerosols from the adjoining regions leads to remarkable changes in the magnitude of the AODs and their wavelength dependencies during March to May. HYSPLIT back-trajectory analysis shows that enhanced long-range transport of aerosols, particularly from the west Asia and northwest coastal India, contributed significantly to the enhancement of AOD and in the flattening of the spectra over entire regions; if it is the peninsular regions and the island Minicoy are more impacted in April, the north Indian regions including the Indo Gangetic Plain get affected the most during May, with the AODs soaring as high as 1.0 at 500 nm. Over the islands, the Angstrom exponent (α) remained significantly lower (~1) over the Arabian Sea compared to Bay of Bengal (BoB) (~1.4) as revealed by the data respectively from Minicoy and Port Blair. Occurrences of higher values of α, showing dominance of accumulation mode aerosols, over BoB are associated well with the advection, above the boundary layer, of fine particles from the east Asian region during March and April. The change in the airmass to marine in May results in a rapid decrease in α over the BoB

Experiments on degradation of the fungicide mancozeb on cardamom (Elettaria cardamomum) capsules by processing techniques indicated that thorough washing had a profound influence on the removal of surface residues. A waiting period of 8 to 14 days in the case of fresh capsules could be brought down to 3 to 4 days by washing.

Novel VPS13B Mutations in Three Large Pakistani Cohen Syndrome Families Suggests a Baloch Variant with Autistic-Like Features.
BackgroundCohen Syndrome (COH1) is a rare autosomal recessive disorder, principally identified by ocular, neural and muscular deficits. We identified three large consanguineous Pakistani families with intellectual disability and in some cases with autistic traits.MethodsClinical assessments were performed in order to allow comparison of clinical features with other VPS13B mutations. Homozygosity mapping followed by whole exome sequencing and Sanger sequencing strategies were used to identify disease-related mutations.ResultsWe identified two novel homozygous deletion mutations in VPS13B, firstly a 1 bp deletion, NM_017890.4:c.6879delT; p.Phe2293Leufs*24, and secondly a deletion of exons 37-40, which co-segregate with affected status. In addition to COH1-related traits, autistic features were reported in a number of family members, contrasting with the "friendly" demeanour often associated with COH1. The c.6879delT mutation is present in two families from different regions of the country, but both from the Baloch sub-ethnic group, and with a shared haplotype, indicating a founder effect among the Baloch population.ConclusionWe suspect that the c.6879delT mutation may be a common cause of COH1 and similar phenotypes among the Baloch population. Additionally, most of the individuals with the c.6879delT mutation in these two families also present with autistic like traits, and suggests that this variant may lead to a distinct autistic-like COH1 subgroup
Effects of betel nut on cardiovascular risk factors in a rat model
Background: Areca nut (commonly known as betel nut) chewing has been shown to be associated with metabolic syndrome and cardiovascular disease (CVD). The mechanism by which betel nut ingestion could lead to development of CVD is not precisely known; however, dyslipidemia, hyperhomocysteinemia, hypertriglyceridemia and inflammation could be some of the potential risk factors. This study was undertaken to investigate the effects of two dosages of betel nut on homocysteinemia, inflammation and some of the components of metabolic syndrome, such as hypertriglyceridemia, low HDL-cholesterol, obesity and fasting hyperglycemia in a rat model.Methods: Thirty-six adult female Sprague Dawley rats, aged 10–12 weeks were divided into three equal groups. Group-1 served as the control group (n = 12) and received water, whereas groups 2 and 3 were given water suspension of betel nut orally in two dosages, 30 mg and 60 mg, respectively for a period of 5 weeks. At the end of the fifth week, the animals were weighed and sacrificed, blood was collected and liver, kidney, spleen and stomach were removed for histological examination. Plasma/serum was analyzed for glucose, total cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides, homocysteine, folate, vitamin B12 and N-acetyl-β-D-glucosaminidase (NAG) – a marker of inflammation.Results: When the mean concentration values of 3 groups were compared using one way ANOVA followed by Tukey’s HSD-test, there was a significant increase in the concentration of total cholesterol (p = 0.04) in the group receiving 30 mg/day betel nut compared to the control group. However, administration of a higher dose of betel nut (60 mg/day) had no significant effect on the serum concentrations of glucose, total cholesterol, HDL-cholesterol, LDL-cholesterol, and NAG. Histological examination of spleen revealed a dose-dependent extramedullary hematopoiesis. No other remarkable change in the tissues (liver, kidney and stomach) was observed. Mean serum/plasma levels of folate, vitamin B12 and homocysteine were not found to be significantly different in all the groups. Betel nut ingestion had no effect on the mean body weights of rats.Conclusions: Low dosage of betel nut is found to be associated with hypercholesterolemia. However, betel nut ingestion is not associated with hyperhomocysteinemia, hypertriglyceridemia, hyperglycemia, inflammation and increase in body weight in a rat model
