    Efficacy of intraarticular steroid injection in knee osteoarthritis using knee society score and visual analog score

    Background: Osteoarthritis of knee is a chronic degenerative joint disease which leads to pain, swelling, stiffness and decreased quality of life. The purpose of this study was to assess the effect of intra-articular steroid injection for osteoarthritis knee by using knee society score (KSS) and visual analog scale (VAS).Methods: A prospective study of 38 patients (44 knees) diagnosed with osteoarthritis of knee were given 80mg of intra-articular methylprednisolone injection. The outcome was assessed using KSS and VAS before the injection and 3 months post injection.Results: The mean VAS improved 3 months post injection whereas there was no improvement in the mean KSS. The scores of VAS 3 months post injection were statistically significant whereas the KSS scores were statistically insignificant compared with the pre injection values.Conclusions: Intra-articular steroid injection for osteoarthritis of knee produces significant pain relief for most patients even in severe cases but this not translate to better functional outcomes

    An illustrative case report emphasizing the biological plating of a comminuted intra-articular fracture

    Comminuted intra-articular fractures are very common in high energy traumas, conventional open reduction and plate fixation has been associated with many complications as they disturb the fracture biology and hence minimally invasive plating came into existence. We are reporting a 17 year old boy who met with road traffic accident sustained an AO type 33C2 injury to left femur with severe metaphyseal comminution treated by minimal invasive plate osteosynthesis technique who achieved full radiological union at the end of 4 months and full functional recovery at 8 months follow-up. This case emphasizes the need for preservation of fracture biology which is of paramount importance in healing of comminuted fractures

    Tibial deformity correction by Ilizarov method

    The case series illustrates the correction of bony deformities of the tibia through a percutaneous osteotomy and gradual distraction with the Ilizarov apparatus in order to restore shape and function of the lower limb. A total of 13 cases of tibial deformity which were gradually corrected by the Ilizarov method were included in the study. The plane and degree of deformity was calculated by Drorr Paley’s method. The tibial deformity was gradually corrected in all patients by the Ilizarov method. Hinges were appropriately placed usually at the level of deformity (CORA). In most of the cases percutaneous osteotomy was done at the level of CORA. In juxta-articular deformities, hinges were placed at the level of CORA but the osteotomy was done at different levels. The mean tibial varus in 12 patients was 24 degrees (range of 18 to 34 degrees) and one patient had a tibial valgus of 22 degrees which was corrected to restore a 90 degree medial proximal tibial angle. No healing problems in the regenerate except for one probable hypertrophic non-union. None of the patients with a tibial varus developed any neurological deficit or compartment syndrome following correction

    A displaced T type intercondylar distal humerus fracture with radial nerve palsy in a 10-year-old treated by closed reduction and K wire fixation

    T type intercondylar fractures of the distal hummers in the pediatric age group is an extremely rare injury. These fractures are difficult to diagnosed as they can be mistaken for extension type injuries. The few clinal trials on the management of the fracture, it is widely regarded that the accepted method of treatment is open reduction and rigid internal fixation. It is easier to ensure stable articular reconstruction of the intra articular fragments by this method. Open reduction and internal fixation of there fractures are associated with high complication rate, which include wound site problems, non-union due to periosteal stripping, heterotrophic ossification and iatrogenic nerve injury. Large soft tissue dissection can also lead to vascular compromise and significant scar tissue which is associated with a stiff elbow. We present a case of a 10-year-old boy who sustained an intercondylar fracture with radial nerve palsy which was treated by closed reduction and Kirshner wire fixation with excellent results at 1 year follow up

    Aggressive giant cell tumour of the talus: a rare case report and challenges faced during its management

    Giant cell tumour (GCT) of the bone is a locally aggressive benign tumour that commonly occurs in the epiphyseal ends of long bones. Its occurrence in the small bones of the foot like the talus is rare. In this case report we described a case of aggressive GCT of the talus (Campanacci grade 3) in a twenty-one years old male diagnosed by the presence of a well-defined osteolytic lesion seen involving the talar body with cortical breach and soft tissue extension confirmed to be a GCT by closed needle biopsy. The patient was managed surgically by excision of the tumour mass in-toto along with the talus followed by tibio-calcaneal fusion using free fibular strut graft and Ilizarov frame. At 3 months follow up no evidence of fusion was seen and therefore secondary augmentation with a cortico-cancellous graft from the iliac crest was done to aid fusion and Ilizarov fixator was removed 6 months after the surgery. The patient at one year follow up had shown no signs of recurrence of the tumour and has a pain free, stable fused tibiocalcaneal joint with a bipedal unassisted gait. Our case report found excision of talus combined with tibio-calcaneal fusion using free fibular strut graft along with cortico cancellous iliac bone graft to have a good outcome in terms of aiding tibio-calcaneal arthrodesis and thereby providing pain relief and providing a stable foot to the patient

    Effect of Spacing and Crop Duration on Growth, Flowering and Bulb Production in Tuberose (Polianthes tuberosa L.) Cv. Double

    Field experiments were conducted at Junagadh during 2002-05 to study the response of spacing (45 x 45, 45 x 30, 45 x 15, 30 x 30 and 30 x 15 cm) and crop duration (first year crop, first ratoon and second ratoon) on growth, flowering, cut flower yield and bulb production in tuberose cv. Double. The widest spacing (45 cm x 45 cm) registered the highest values for plant height (46.18 cm), number of leaves per clump (67.25), spike length (89.64 cm), spike diameter (0.95 cm), diameter of open flower (4.6 cm), rachis length (34.8 cm), number of spikes per clump (4.1), number of florets per spike (48.2), number of bulbs per clump (18.40) and number of bulblets per clump (31.60). It also induced early spike emergence and flowering. A planting distance of 30 x 30 cm realized the highest cut flower yield (2.72 lakh ha-1) and that of 30 cm x 15 cm recorded the highest bulb production (22 lakh ha-1). Ratoon crops showed higher plant height, number of leaves, bulbs, bulblets and spikes per clump and cut flower yield as well as bulb production over the first year crop. Early spike emergence and flowering was also noted in ratoon crops compared to the first year crop. However, spike and flower quality was inferior to that of first year crop with regard to spike length and diameter, number of florets per spike, diameter of open flower and rachis length

    Osteofibrous dysplasia of the proximal tibia: an illustrative case report

    A 5 years old girl presented 3 years back with pain and swelling over the upper right leg for 2 years duration. X-ray, CT and MRI revealed osteolytic well defined lesion 2.5 × 2 cm in the meta-diaphyseal region of the proximal tibia with pathological fracture of anterolateral cortex. Child underwent thorough curettage of the lesion and the defect was filled with allograft (iliac crest graft) from mother which was harvested in an adjacent operation theatre. The limb was protected with a plaster splint for a period of 3 months. The graft gradually consolidated and new bone formation was apparent by 6 months. Remodelling of the medullary canal occurred at 1 year follow up. Child has been followed up to 3 years (till date) and there is no evidence of recurrence. The case illustrates that osteofibrous dyplasia can be effectively treated by curettage and replacement of defect by allograft from parents.

    Diagnostic spectrum of CSF involvement by malignant cells: three-year experience of tertiary care hospital in CSF cytology

    Background: Cerebrospinal fluid (CSF) cytology is the diagnostic gold standard test for primary or metastatic involvement of central nervous system (CNS) by malignancy. It is a very important and routinely done test in the laboratory but its sensitivity is low due to many confounding factors and varies amongst type of malignancies. We did retrospective analysis of CSF cytology carried out at our center from January 2013 to December 2015 to find out about spectrum of malignancies being detected on CSF cytology.Methods: From January 2013 to December 2015, three hundred and seventy-five CSF samples were received and processed for presence of malignant cells at our center. The cytospin preparations stained with LG and PAP stains of these CSF samples were analyzed along with any special stain that was available in the records. Clinical details of the patients with positive cytology were correlated from records.Results: The diagnostic spectrum obtained through study of positive CSF cytology included cases of metastatic solid malignancies and hematolymphoid malignancies. Four cases out of ten were of Metastatic Adenocarcinoma with primary origin being Breast, Lung and Stomach. Out of six cases of hematolymphoid malignancies; five were known cases of lymphoma/leukemia and one was primarily diagnosed to have Non-Hodgkin’s lymphoma (NHL) on CSF cytology.Conclusions: Both Epithelial and Hematolymphoid malignancies can be diagnosed on CSF cytology and these are predominantly metastatic in origin; Hematolymphoid malignancies are more common in CSF than solid malignancies.
