    Introduction: Relapse is not rare, and its frequency and speed of onset often originate in defective drug compliance. Facilitating compliance is therefore an important issue; this requires a special approach, centered on well-defined protocols. In recent years, programs of adherence combined with those of therapeutic education, implement services to increase the capacity of patients to support themselves. The objective of our research work was to set up a psychoeducation program with neuroleptic treatment (Lieberman's program) and to evaluate its impact on the rate of relapse at two years. Materials and methods: This is a prospective, randomized, comparative study that involved 300 man and woman patients aged 16 to 60 and older who met the DSM-5 criteria for schizophrenia, recruited in hospitalization services and consultations with clinical evaluation using the PANSS scale and an evaluation adherence treatment using the MARS scale. The patients were randomized into two groups of one hundred and fifty, depending on the type of care: psychoeducational group versus control group. Surveillance parameters were statistically comparable in both inclusion groups (relapse / re-hospitalization and clinic). The results did not show any significant differences between the two groups. Faciliter l’observance est donc un enjeu important ; cela nécessite une approche particulière, centrée sur des protocoles bien définis. Depuis quelques années, des programmes d’observance alliés à ceux d’éducation thérapeutique, mettent en œuvre des prestations visant à accroitre les capacités des patients à se prendre en charge. L’objectif de notre travail de recherche, a été de mettre en place un programme de psychoéducation au traitement neuroleptique (programme de Lieberman) et d’évaluer son impact sur le taux de rechutes à deux ans. Matériels et méthodes : Il s’agit d’une étude prospective comparative randomisée, qui a concerné 300 patients des deux sexes âgés entre 16 et 60 et plus répondants aux critères de DSM-5 de schizophrénie, recrutés au sein des services d’hospitalisation et des consultations avec une évaluation clinique à l’aide de l’échelle PANSS et l’évaluation de l’observance au traitement au moyen de l’échelle MARS. Les patients été randomisés en deux groupes de cent cinquante, selon le type de prise en charge : groupe psychoéducatif versus groupe témoin. Les paramètres de surveillances étaient statistiquement comparables dans les deux groupes à l’inclusion (rechute / ré-hospitalisation et clinique). Les résultats n’ont pas montré de différences significatives entre les deux groupes. De notre travail.     Effects after use of laser therapy for vulvovaginal atrophy in a patient with lichen sclerosus: a case report

    Objective: Use of laser therapy has most recently been introduced as a non-invasive option for the treatment of genitourinary syndrome of menopause (GSM). Recent literature has shown promise in providing benefit for patients with symptoms of vulvovaginal atrophy (VVA) and stress urinary incontinence with minimal adverse effects. Despite this, the United States Food and Drug Administration has not cleared laser therapy for these specific indications given the lack of sufficient evidence to support safety and efficacy. A case is presented of a patient with GSM in the setting of lichen sclerosus who was referred to a tertiary vulvovaginal disease clinic after worsening of symptoms after three laser therapy treatments. Patient data is protected by the Health Insurance Portability and Accountability Act of 1996. The case presented demonstrates initial worsening of symptoms after treatment with laser therapy, with initial exam findings showing atrophic vaginitis, marked introital narrowing, partial phimosis of the clitoral hood, and fusion of the labia minor and majora. The patient ultimately had complete resolution of post-laser therapy symptoms by one year after initial presentation. The case findings and follow-up are presented. Conclusions: Despite case series and studies in the literature showing promise of the use of laser therapy for GSM and urinary incontinence, laser therapy is not currently FDA-approved for these indications. Large scale, long-term prospective randomized controlled data is necessary to provide data on the safety, efficacy, indications, and appropriate candidates for laser therapy

    New complexes of the type [ML2(H2O)2] ,[FeL2(H2O)Cl] and [VOL2] were M=Co(II),Ni(II) and Cu(II) ,L=4-(2-methyl-4-oxoquinazoline-3(4H)-yl) benzoic acid were synthesized and characterized by element analysis, magnetic susceptibility ,molar conductance ,FT-IR and UV-visible. The studies indicate that the L acts as doubly monodentate bridge for metal ions and form mononuclear complexes. The complexes are found to be octahedral except V(IV) complex is square pyrimde shape . The structural geometries of compounds were also suggested in gas phase by theoretical treatments, using Hyper chem-6 program for the molecular mechanics and semi-empirical calculations, addition heat of formation(?Hf ?) and binding energy (?Eb)for the free ligands and it’s metal complexes were calculated by using PM3 method .PM3 was used to evaluate the vibration spectra of ligand and compare the theoretically calculated wave numbers with experimental values ,the theoretically obtained frequencies agreed calculation helped to assign unambiguously the most diagnostic bands

    Which politicians receive abuse? Four factors illuminated in the UK general election 2019

    The 2019 UK general election took place against a background of rising online hostility levels toward politicians, and concerns about the impact of this on democracy, as a record number of politicians cited the abuse they had been receiving as a reason for not standing for re-election. We present a four-factor framework in understanding who receives online abuse and why. The four factors are prominence, events, online engagement and personal characteristics. We collected 4.2 million tweets sent to or from election candidates in the six week period spanning from the start of November until shortly after the December 12th election. We found abuse in 4.46% of replies received by candidates, up from 3.27% in the matching period for the 2017 UK general election. Abuse levels have also been climbing month on month throughout 2019. Abuse also escalated throughout the campaign period. Abuse focused mainly on a small number of high profile politicians, with the most prominent individuals receiving not only more abuse by volume, but also as a percentage of replies. Abuse is ``spiky'', triggered by external events such as debates, or certain tweets. Some tweets may become viral targets for personal abuse. On average, men received more general and political abuse; women received more sexist abuse. Conservative candidates received more political and general abuse. We find that individuals choosing not to stand for re-election had received more abuse across the preceding year

    Mainstreaming of genomic medicine in gastroenterology, present and future: a Nationwide Survey of UK Gastroenterology Trainees

    Objective: Genomics and personalised medicine are increasingly relevant for patients with gastroenterological conditions. We aim to capture the current state of genomics training in gastroenterology to review current understanding, clinical experience and long-term educational needs of UK trainees.Design & Setting: A web-based nationwide survey of all UK gastroenterology specialty trainees was conducted in 2017.Results: 100 trainees (14% of UK gastroenterology trainees) completed this survey. Only 9% and 16% of respondents believe that their local training programme adequately prepares them for future clinical practice utilising genomic medicine and personalised medicine respectively. Barriers identified include the need for greater trainee education (95%), inadequate clinical guidance to base interventions on the results of genomic testing (53%), concerns over misinterpretation by patients (43%) and overuse/misuse of testing by clinicians (34%).Survey respondents felt prepared to perform HFE genotyping (98%), assess TPMT status (97%), and interpret HLA-subtyping for suspected coeliac disease (85%). However, only a minority felt prepared to perform the following investigations: polyposis screening (34%), hereditary pancreatitis screening (30%), testing for Lynch Syndrome (33%), and KRAS testing for colorectal cancer (20%).Most respondents would support holding dedicated training days on genomic medicine (83%), formal training provisions for the mainstreaming of genomic testing (64%), an update to the UK gastroenterology specialty training curriculum and examinations (57%), and better-defined referral pathways for local genomic services (91%).Conclusion: Most gastroenterology trainees in this survey feel ill-equipped to practice genomic and personalised medicine as consultants. We propose specific revisions to the UK gastroenterology specialty curriculum that address trainees needs

    Production and Characterization of an Alkaline Lipase from Thermophilic Anoxybacillus sp. HBB16

    A thermophilic lipase-producing bacterium (Anoxybacillus sp. HBB16) was analyzed using 16S rRNA. The maximum growth rate and intracellular lipase production occurred at 50 °C and pH 6.5. Among the various nitrogen and carbon sources tested, meat extract, olive oil and olive mill wastewater (OMW) were the best sources for lipase production. Enzyme production increased when the strain HBB16 was grown at a 180 rpm shaking speed. The maximum activity of the lipase occurred at 55 °C and pH 9.5. The presence of phenylmethylsulfonyl fluoride (PMSF), N-cyclohexyl-N′-(2-morpholinoethyl) carbodiimidemetho-p-toluenesulfonate (CMC), N-bromosuccinimide (NBS) and sodium dodecyl sulfate (SDS) inhibited enzyme activity. Bivalent metal ions caused a significant inhibition in enzyme activity, whereas univalent metal ions displayed no negative effects.     Planned use of long acting reversible postpartum contraception in low-risk women in CenteringPregnancy® group versus individual physician prenatal care

    Introduction: Education on effective contraceptive methods is necessary during the prenatal period to help women achieve optimal birth spacing. This study identified rates of long-acting reversible contraception (LARC) uptake in women who attended CenteringPregnancy® (CP) group prenatal care versus individual physician care (IP). Methods: Charts for low-risk women who participated in group CP or IP prenatal care between March 2012 and May 2016 were reviewed. Charts of IP subjects were randomly selected in each year to achieve a CP:IP ratio of at least 1:3. The primary outcome was rate of LARC use at discharge and within 8 weeks postpartum. Pearson chi-squared test and Wilcoxon rank-sum tests were performed, and a p-value <0.05 was considered significant. Results: 129 women participated in CP care and 412 in IP care. CP women were more likely nulliparous (91, or 70.5% vs 212, or 51.5%, p=0.0001) and more likely to attend at least 15 prenatal visits (54, or 41.9% vs 62, or 15.1%, p<0.0001). LARC use rates at discharge and at the postpartum visit were similar (36, or 27.9% vs 89, or 21.6%, p=0.142; 39, or 32.2% vs 110, or 29.4%, p=0.557). Rates of women using effective contraception (LARC and other hormonal options, including oral contraceptives and Depo Provera) at discharge and at the postpartum visit were similar (59, or 45.7% vs 206, or 50.0%, p=0.177; 72, or 59.5% vs 229, or 61.2%, p=0.157). IUD use was greater than subdermal implant use in both groups (31, or 24.0% vs 5, or 3.9%; 72, or 17.5% vs 17, or 4.1%; p=0.081). Rates of routine postpartum visit attendance at 6-8 weeks postpartum were similar and high in both groups (121, or 93.8% vs 374, or 90.8%; adjusted p-value=0.164). Conclusion: Although CP subjects had more prenatal visits and spent more time with providers, there was no difference on uptake of LARC or effective contraception at discharge or at the postpartum visit when compared to IP subjects

    Hong-Ou-Mandel interference between independent III-V on silicon waveguide integrated lasers

    The versatility of silicon photonic integrated circuits has led to a widespread usage of this platform for quantum information based applications, including Quantum Key Distribution (QKD). However, the integration of simple high repetition rate photon sources is yet to be achieved. The use of weak-coherent pulses (WCPs) could represent a viable solution. For example, Measurement Device Independent QKD (MDI-QKD) envisions the use of WCPs to distill a secret key immune to detector side channel attacks at large distances. Thus, the integration of III-V lasers on silicon waveguides is an interesting prospect for quantum photonics. Here, we report the experimental observation of Hong-Ou-Mandel interference with 46\pm 2% visibility between WCPs generated by two independent III-V on silicon waveguide integrated lasers. This quantum interference effect is at the heart of many applications, including MDI-QKD. Our work represents a substantial first step towards an implementation of MDI-QKD fully integrated in silicon, and could be beneficial for other applications such as standard QKD and novel quantum communication protocols.Comment: 5 pages, 3 figure
