A perianalis fisztulázĂł Crohn-betegsĂ©g konzervatĂv kezelĂ©si lehetĹ‘sĂ©gei = Conservative Treatment Modalities of Perianal Fistulizing Crohn's Disease
A perianalis fisztulák a Crohn-betegség ismert szövődményei, jelentős morbiditással és életminőségromlással járnak. Különböző osztályozások léteznek, amelyek alapvetően a külső és belső anális sphincterhez való viszonyon alapulnak. A perianalis fisztulák optimális kezeléséhez a gasztroenterológus és sebész együttműködése szükséges. A gyógyszeres kezelés előterében az antibiotikumok, a biológiai szerek és az immunmodulátorok állnak. A hagyományos, biológiai gyógyszeres és sebészi kezelések ellenére a legjobb eredmények 50% körüli fisztulagyógyulást mutatnak. Az új sejtterápiás kezelési lehetőségek reménykeltőek a kedvezőbb kimenetel eléréséhez

Background and aims: Previous studies have suggested an increasing use of complementary and alternative medicine (CAM) in patients with inflammatory bowel disease (IBD). Furthermore, a significant number of IBD patients fail to comply with treatment. The aim of our study was to

Subjective expectations regarding longevity and future health:a cross-sectional survey among patients with Crohn’s disease
AimThe aim was to explore the subjective healthexpectations (sHE) of patients with Crohn’s disease(CD) for both the near future and the elderly.MethodA cross-sectional survey was performed in fourgastroenterology centres in Hungary. Consecutive outpa-tients with CD with age≥18 were recruited. Socio-demo-graphic and disease characteristics were recorded and theCrohn’s Disease Activity Index (CDAI), Perianal DiseaseActivity Index, Patients’ Global Assessment (PGA) andcurrent pain visual analogue scale (VAS) were assessed.Subjective life expectancy (sLE) was explored and com-pared to statistical life expectancy. Current health and sHEfor 1 year ahead and for ages 60/70/80/90 were assessedusing the descriptive system of the EQ-5D-3L.ResultsIn all, 206 patients (54.9% men) with a meanage of 34.7 (SD 10.5 years) and disease duration of10.5 (SD 6.3) years were studied. The CDAI score was110.5 (SD 77.0) and 66% were treated by biologicdrugs. Mean current EQ-5D-3L score was 0.80 (SD0.17) and patients expected a 0.05 (SD 0.15) improve-ment within a year (P<0.05). For ages 60/70/80/90,a mean EQ-5D-3L score of 0.59, 0.38, 0.10 and 0.12 respectively was provisioned. Age, current healthstatus, sLE, PGA and pain VAS showed significant cor-relation with both 1-year and older age sHE(P<0.05). Long-term sHE and sLE were negativelyaffected by the presence of extraintestinal manifestationsbut not by previous CD-related surgery.ConclusionPatients with CD expect severe deteriora-tion in health in later life. Given that unrealistic sHEmay affect patients’ current quality of life and healthbehaviour, we encourage physicians to explore and con-sider CD patients’ sHE in clinical care
A hereditaer nonpolyposus colorectalis carcinoma szindrómás betegek szűrésének és szoros utánkövetésének fontossága egy családfa bemutatása kapcsán | Significance of the monitoring and screening for hereditary nonpolyposis colorectal carcinoma syndrome patients by presenting a case of a family tree
Absztrakt:
Bevezetés: A hereditaer nonpolyposus colorectalis carcinomára
jellemző mutációk (HNPCC) autoszomális domináns öröklődésmenetet mutatnak.
Leginkább vastagbéldaganatok kialakulásáért felelősek.
Célkitűzés: A HNPCC-szindrómás betegek szűrésének és
követésének fontosságát szeretnénk hangsúlyozni egy igazolt MMR-gén-mutációt
hordozó betegünk jelenlegi és 10 évvel korábbi családfájának
összehasonlĂtásával. Betegek Ă©s mĂłdszer: Hazánkban elĹ‘fordulĂł,
HNPCC-re gyanús családok kiszűrése érdekében alapos családi anamnézist veszünk
fel. Amennyiben az immunhisztokémiai és mikroszatellitainstabilitás-vizsgálatok
HNPCC-szindrómára utalnak, elvégezzük az MMR-gének szekvenálását.
Eredmények: Betegünknél egy, a hMSH2-gén 6. exon két
bázispárt Ă©rintĹ‘ deletiĂłja (c.969–970delTC) igazolĂłdott. TĂzĂ©ves utánkövetĂ©s
során betegünknél és rokonainál újabb, a HNPCC-re jellemző tumorok jelentek meg.
Következtetés: A veszélyeztetett családtagok követése során
a szekunder prevenció a jól együttműködő betegeknél hatékony volt. Orv Hetil.

Abstract:
Introduction: Hereditary nonpolyposis colorectal carcinoma
(HNPCC) is an autosomal dominant disease, which shows familial clustering.
Aim: We would like to emphasize the importance of
monitoring the HNPCC syndrome patients by presenting a case of a proven MMR gene
mutation carrier and her family tree encompassing 10 years. Materials
and method: To screen a suspected HNPCC Hungarian family member we
are taking thorough family histories. If the diagnosis of HNPCC was further
supported by immunohistology and the microsatellite status, sequencing of the
MMR genes was carried out. Results: A novel mutation in exon 6
of the hMSH2 gene leading to the deletion of two nucleotide pairs
[c.969-970delTC] was detected in our patient. During the 10-year follow-up
period of our patient new HNPCC-associated tumors have developed in several
family members. Conslusion: Close surveillance of the patient
and its family members at risk was effective, although it requires compliance

MindkĂ©t kĂłrkĂ©p esetĂ©n jelentĹ‘s sejtkárosodás jön lĂ©tre, ezen felĂĽl a gyakran társulĂł infekciĂł is tĂşlzott immunválaszt, „citokinvihart” eredmĂ©nyezhet, amelynek kontrollálására a gyĂłgyszeres prĂłbálkozások nem jelentettek kellĹ‘ hatĂ©konyságĂş megoldást, ezĂ©rt az extracorporalis technikák kerĂĽltek elĹ‘tĂ©rbe. A citokin-adszorberek közĂĽl a CytoSorb alkalmazásával van jelenleg világszerte a legtöbb tapasztalat, amely kombinálhatĂł egyĂ©b extracorporalis szervpĂłtlĂł kezelĂ©sekkel is.
