    Parental presence within households and the impact of antiretroviral therapy in Khayelitsha, Cape Town

    Background. While household support is an important component of effective care and treatment in HIV/AIDS, there are few insights from Southern Africa into how household support arrangements change over time for patients starting antiretroviral therapy (ART).Objective. We hypothesised that patients initiating ART are more likely to be living with family, especially their mothers, compared with the general population, but that over time these differences disappear.Methods. A panel survey of ART patients was matched by age, gender and education to a comparison sample drawn from adults in Khayelitsha, Cape Town.Results. The results show that there is a substantial potential burden of care on the families of patients starting ART, particularly mothers, and that the use of ART appears to reduce this burden over time. But, even after their health is restored, ART patients are significantly less likely to have a resident sexual partner and more likely to be living in single-person households than their counterparts in the general population

    “What’s his is his and what’s mine is his”: Financial power and economic abuse of women in Aotearoa

    AIM: This study aimed to understand the experiences and effects of economic abuse for women in Aotearoa New Zealand, particularly in relation to methods of coercive control, with the intention of developing risk matrices to be used by practitioners.METHODS: We conducted a survey with 448 respondents—with 398 the focus of analysis for this article. The survey contained a combination of scaling and open-ended questions.FINDINGS: Abusers employed a range of abusive methods to restrict victims’ freedom and exercise domination. These abusive behaviours seemed to follow traditional hegemonic construction of masculinity as synonymous with “provider” in that many of these methods relied on the reproduction of gendered stereotypes which subjugate women to a subordinate position in the household. Women experienced a range of adverse emotional impacts as a result of this abuse

Para identificar los aspectos abordados en la investigaci?n se tom? la muestra total de los ni?os del grado primero de la sede, a quienes se les realiz? una entrevista semiestructurada con preguntas abiertas que posteriormente fueron analizadas desde la perspectiva del enfoque de la investigaci?n cualitativa para realizar un proceso descriptivo y comprensivo de los fen?menos sociales observados. Con respecto a la comprensi?n de los datos se realiz? inicialmente el proceso de transcripci?n, sistematizaci?n y codificaci?n en esquemas que permitieron categorizar, ordenar y llegar a conclusiones generales a partir de las respuestas de los ni?os, posteriormente se dio el proceso de construcci?n de tres redes sist?micas que permitieron hacer el levantamiento de tres categor?as de an?lisis. Basados en los resultados y como producto de la confrontaci?n de la teor?a y la pr?ctica se cre? un conjunto de nueve estrategias did?cticas para mejorar el comportamiento lector de los ni?os.The research ?Strategies to improve reading behavior of children in the first year of elementary school Camilo Torres headquarters of the school Policarpa Salavarrieta of Girardot city? analyzes the initial state of the reading behavior of children Camilo Torres headquarters and designs an action plan that includes various didactic strategies to improve it. In order to identify the issues raised in the investigation the total sample of children enrolled in the first grade of headquarters, who underwent a semistructured interview with open questions which were subsequently analyzed from the perspective of the approach to be taken qualitative research to carry out a descriptive and understanding of social phenomena observed process. With regard to the understanding of the transcription process data, systematization and coding schemes that enabled categorize, organize, and to draw general conclusions from the responses of children, then the process of construction of three systemic networks were initially performed that allowed you lift three analysis categories. Similarly a theoretical review of the key elements in the development of the reading behavior through the history of reading, concepts of didactic language and motivation in reading was performed. Based on the results and as a result of the confrontation of theory and practice a set of nine didactic strategies was established to improve the reading behavior of children.     Exploring BAFF: its expression, receptors and contribution to the immunopathogenesis of Sjogren's syndrome

    SS is an autoimmune condition characterized by exocrine gland destruction, autoantibody production, immune complex deposition and systemic complications associated with lymphocytic infiltration of many organs. Genetic, environmental and viral factors play a role in disease aetiology, however, the exact mechanisms driving the immunopathogenesis of SS remain uncertain. Here we discuss a role for B cell activating factor (BAFF), whereby B cell hyperactivity and increased BAFF secretion observed in patients and animal models of the disease can be explained by the altered expression of cell-specific BAFF/BAFF receptor (BAFF-R) variants in several immune cell types. Understanding the role of BAFF/BAFF-R heterogeneity in SS pathogenesis could help to facilitate new treatment strategies for patients

    Treatment strategies for Sjögren’s syndrome with childhood onset: a systematic review of the literature

    OBJECTIVES: SS with childhood onset is a rare autoimmune disease characterized by heterogeneous presentation. The lack of validated classification criteria makes it challenging to diagnose. Evidence-based guidelines for treatment of juvenile SS are not available due to the rarity of disease and the paucity of research in this patient population. This systematic review aims to summarize and appraise the current literature focused on pharmacological strategies for management of SS with childhood onset. METHODS: PubMed and MEDLINE/Scopus databases up to December 2020 were screened for suitable reports highlighting pharmacological treatment of SS with childhood onset using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2009 reporting checklist. Animal studies were excluded. RESULTS: A total of 43 studies (34 case reports, 8 mini case series and 1 pilot study) were eligible for analysis. The studies retrieved included girls in 88% (120/137) of cases and had very low confidence levels. HCQ was prescribed for parotid swelling, as well as in association with MTX and NSAIDs in patients with arthritis and arthralgia. Corticosteroids such as long courses of oral prednisone and i.v. methylprednisolone were commonly prescribed for children with severe disease presentations. Rituximab was mainly indicated for mucosa-associated lymphoid tissue lymphoma and renal and nervous system complications. Other conventional DMARDs were prescribed in selected cases with extraglandular manifestations. CONCLUSION: Various therapies are used for the management of juvenile SS and are prescribed based on expert clinician’s opinion. There are currently no good-quality studies that allow clinical recommendations for treatment of SS with childhood onset

    Systematic review and meta-analysis of the epidemiology of polyautoimmunity in Sjögren’s syndrome (secondary Sjögren’s syndrome) focusing on autoimmune rheumatic diseases

    Healthcare on license from Scandinavian Rheumatology Research Foundation Objective: The epidemiology of polyautoimmunity in Sjögren’s syndrome (secondary Sjögren’s syndrome – sSS) is not well defined and has not been investigated before using a systematic approach. We conducted a systematic review of the epidemiology of sSS associated with rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), scleroderma, and myositis, assessing the prevalence rates (PRs) and clinical and serological features of sSS. Method: A systematic literature search of PubMed and Embase databases (updated to March 2016) was performed to identify all published data on PR, demographic proïŹle, clinical manifestations, laboratory features, and causes of death associated with sSS. The PR’s of sSS were summarized with PRs and 95% confidence intervals (CIs). Results: The literature search identified 1639 citations, of which 42 fulfilled the inclusion criteria. Only 19 studies were of moderate to good quality and were selected for the meta-analysis. According to a random-effects model, the pooled PR for sSS associated with RA was 19.5% (95% CI 11.2 to 27.8) and the pooled PR for sSS associated with SLE was 13.96% (95% CI 8.88 to 19.04). The female/male ratio of sSS in the RA population was 14.7 (95% CI 7.09 to 256) and in the SLE population was 16.82 (95% CI 1.22 to 32.4). Conclusion: Prevalence rates of sSS vary widely in different populations. Both meta-analyses conducted in the RA and SLE populations were characterized by a high degree of study heterogeneity. The results of this meta-analysis highlight the need for better quality population studies

    Hierarchical Multimodel Ensemble Estimates of Soil Water Retention with Global Coverage

    A correct quantification of mass and energy exchange processes among land surface and atmosphere requires an accurate description of unsaturated soil hydraulic properties. Soil pedotransfer functions (PTFs) have been widely used to predict soil hydraulic parameters. Here, 13 PTFs were grouped according to input data requirements and evaluated against a well-documented soil database with global coverage. Weighted ensembles (calibrated by four groups and the full 13-member set of PTFs) were shown to have improved performance over individual PTFs in terms of root mean square error and other model selection criteria. Global maps of soil water retention data from the ensemble models as well as their uncertainty were provided. These maps demonstrate that five PTF ensembles tend to have different estimates, especially in middle and high latitudes in the Northern Hemisphere. Our full 13-member ensemble model provides more accurate estimates than PTFs that are currently being used in earth system models

    Excitations in the Halo Nucleus He-6 Following The Li-7(gamma,p)He-6 Reaction

    A broad excited state was observed in 6-He with energy E_x = 5 +/- 1 MeV and width Gamma = 3 +/- 1 MeV, following the reaction Li-7(gamma,p)He-6. The state is consistent with a number of broad resonances predicted by recent cluster model calculations. The well-established reaction mechanism, combined with a simple and transparent analysis procedure confers considerable validity to this observation.Comment: 3 pages of LaTeX, 3 figures in PostScript, approved for publication in Phys.
