    Evolution of 2D Truss Structures using Topology Optimization Technique with Meshless Method

    p. 1058-1065Particle Swarm Optimization (PSO) is a new paradigm of Swarm Intelligence which is inspired by concepts from 'Social Psychology' and 'Artificial Life'. Essentially, PSO proposes that the co-operation of individuals promotes the evolution of the swarm. In terms of optimization, the hope would be to enhance the swarm's ability to search on a global scale so as to determine the global optimum in a fitness landscape. It has been empirically shown to perform well with regard to many different kinds of optimization problems. PSO is particularly a preferable candidate to solve highly nonlinear, non-convex and even discontinuous problems. In this paper, one enhanced version of PSO: Modified Lbest based PSO (LPSO) is proposed and applied to one of the most challenging fields of optimization -- truss topological optimization.     Dissolved Organic Matter Fractions and Their Removal in Full-Scale Drinking Water Treatment Process

    Objectives The objective of this study is to investigate characteristics about removal of dissolved organic matter (DOM) and its fraction for improving operation efficiency in an advanced water treatment plant. Methods The monitoring of water quality was conducted at five processes such as raw water, pre-oxidation, after sedimentation, post-ozonation, after biological activated carbon (BAC) from July 2020 to August 2021 in advanced water treatment process (AWTP) supplying 180,000m3/day. The concentration of DOC (dissolved organic carbon) and number of algae were monitored and LC-OCD was used to fractionate DOC to four species, biopolymer (BP), humic substance (HS), building blocks (BB), and low molecular weights (LMWs). Results and Discussion The characteristics of raw water showed that the concentration of DOC and the number of algae increased with an increase in water temperature. The portion of BP and HS significantly increased at low and high water temperature, respectively, while BB and LMWs maintained the similar portion. The removal efficiency of DOC in the AWTP was achieved at 59% with each species of BP, HS, BB, and LMWs obtaining removal efficiency of 87%, 65%, 26%, and 52%, respectively. Coagulation/sedimentation/filtration showed removal efficiency of 84%, 56%, 20%, and 18% for BP, HS, BB, and LMWs, respectively, corresponding to their molecular weight. The effect of post-ozonation would be negligible except for BP. In BAC, removal efficiency of 49% and 12% were obtained for LMWs and BB (low molecular weight, respectively. As a result, the BP portion significantly decreased due to high removal efficiency, while BB portion was increased in the final treated water compared to raw water. Conclusion Most of DOM was removed in coagulation/sedimentation/filtration and BAC, whereas oxidation process such as pre-chlorination, pre-ozonation and post-oxidation did not have an effect on DOM removal. In coagulation/sedimentation/filtration, 84% and 54% of the BP and HS were removed, while BAC removed 49% and 12% of LMWs and BB, respectively. It has been recommended to improve the removal efficiency of BB, which obtained the lowest removal efficiency in AWTP, and to enhance the removal efficiency of LMWs in BAC to inhibit microbial regrowth in the distribution system

    Evaluation of protective effect of cyclodextrin glucanotransferase-treated Gastrodia elata Blume extract on ultraviolet B-induced premature skin aging

    Purpose: To investigate the protective effect of Gastrodia elata Blume (G. elata, GE) and cyclodextrin glucanotransferase (CGTase) enzyme-treated G. elata extract (EGE) against premature skin aging using ultraviolet B (UVB)-exposed normal human dermal fibroblasts (NHDFs).Methods: The extract was characterized by liquid chromatography with tandem mass spectrometry (LC-MS/MS), ultra-high performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC–QToF–MS) and nuclear magnetic resonance spectroscopy (NMR). The expression of matrix  metalloproteinases (MMP-1,3), interleukin-6 (IL-6), transforming growth factor (TGF-β1) and procollagen type I was assayed using ELISA kits. Safety evaluation of EGE’s dietary administration and topical application was performed by in vivo acute oral toxicity and local lymph node tests.Results: Lower MMP-1 and IL-6 and higher procollagen type I and TGF-β1 levels were observed after treatment with EGE than with GE, indicating that EGE was more effective than GE in treating UVBinduced photoaging. With respect to phenolic composition, EGE had lower 4-hydroxybenzaldehyde (4- HBA) level and higher α-gastrodin level than GE. In UVB-irradiated NHDFs, α-gastrodin exhibited higher anti-aging activity than 4-HBA and β-gastrodin based on the expression of MMP-1, MMP-3, and procollagen type I. The in vivo data indicate that EGE was safe at concentrations of up to 2000 mg/kg for dietary administration and 0.1 % for topical application.Conclusion: EGE protects UVB-induced photoaged human skin better than GE owing to its higher α- gastrodin content.     Intravenous Vitamin C administration reduces fatigue in office workers: a double-blind randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Studies of the efficacy of vitamin C treatment for fatigue have yielded inconsistent results. One of the reasons for this inconsistency could be the difference in delivery routes. Therefore, we planned a clinical trial with intravenous vitamin C administration.</p> <p>Methods</p> <p>We evaluated the effect of intravenous vitamin C on fatigue in office workers. A group of 141 healthy volunteers, aged 20 to 49 years participated in this randomized, double-blind, controlled clinical trial. The trial group received 10 grams of vitamin C with normal saline intravenously, while the placebo group received normal saline only. Since vitamin C is a well-known antioxidant, oxidative stress was measured. Fatigue score, oxidative stress, and plasma vitamin C levels were measured before intervention, and again two hours and one day after intervention. Adverse events were monitored.</p> <p>Results</p> <p>The fatigue scores measured at two hours after intervention and one day after intervention were significantly different between the two groups (p = 0.004); fatigue scores decreased in the vitamin C group after two hours and remained lower for one day. Trial also led to higher plasma vitamin C levels and lower oxidative stress compared to the placebo group (p < 0.001, p < 0.001, respectively). When data analysis was refined by dividing each group into high-baseline and low-baseline subgroups, it was observed that fatigue was reduced in the lower baseline vitamin C level group after two hours and after one day (p = 0.004). The same did not hold for the higher baseline group (p = 0.206).</p> <p>Conclusion</p> <p>Thus, intravenous vitamin C reduced fatigue at two hours, and the effect persisted for one day. There were no significant differences in adverse events between two groups.     Infantile Marfan syndrome in a Korean tertiary referral center

    PurposeInfantile Marfan syndrome (MFS) is a rare congenital inheritable connective tissue disorder with poor prognosis. This study aimed to evaluate the cardiovascular manifestations and overall prognosis of infantile MFS diagnosed in a tertiary referral center in Korea.MethodsEight patients diagnosed with infantile MFS between 2004 and 2014 were retrospectively evaluated.ResultsTheir median age at the time of diagnosis was 2.5 months (range, 0–20 months). The median follow-up period was 25.5 months (range, 0–94 months). The median length at birth was 50.0 cm (range, 48–53 cm); however, height became more prominent over time, and the patients were taller than the 97th percentile at the time of the study. None of the patients had any relevant family history. Four of the 5 patients who underwent DNA sequencing had a fibrillin 1 gene mutation. All the patients with echocardiographic data of the aortic root had a z score of >2. All had mitral and tricuspid valve prolapse, and various degrees of mitral and tricuspid regurgitation. Five patients underwent open-heart surgery, including mitral valve replacement, of whom two required multiple operations. The median age at mitral valve replacement was 28.5 months (range, 5–69 months). Seven patients showed congestive heart failure before surgery or during follow-up, and required multiple anti-heart failure medications. Four patients died of heart failure at a median age of 12 months.ConclusionThe prognosis of infantile MFS is poor; thus, early diagnosis and timely cautious treatment are essential to prevent further morbidity and mortality

    Effects of transcutaneous electrical nerve stimulation on myocardial protection in patients undergoing aortic valve replacement: a randomized clinical trial

    Abstract Background Cardiopulmonary bypass-related myocardial ischemia-reperfusion injury is a major contributor to postoperative morbidity. Although transcutaneous electrical nerve stimulation (TENS) has been found to have cardioprotective effects in animal studies and healthy volunteers, its effects on cardiac surgery under cardiopulmonary bypass patients have not been evaluated. We investigated the effects of TENS on myocardial protection in patients undergoing aortic valve replacement surgery using cardiopulmonary bypass. Methods Thirty patients were randomized to receive TENS or sham in three different anesthetic states – pre-anesthesia, sevoflurane, or propofol (each n = 5). TENS was applied with a pulse width of 385 μs and a frequency of 10 Hz using two surface electrodes at the upper arm for 30 min. Sham treatment was provided without stimulation. The primary outcome was the difference in myocardial infarct size following ischemia-reperfusion injury in rat hearts perfused with pre- and post-TENS plasma dialysate obtained from the patients using Langendorff perfusion system. The cardioprotective effects of TENS were determined by assessing reduction in infarct size following treatment. Results There were no differences in myocardial infarct size between pre- and post-treatment in any group (41.4 ± 4.3% vs. 36.7 ± 5.3%, 39.8 ± 7.3% vs. 27.8 ± 12.0%, and 41.6 ± 2.2% vs. 37.8 ± 7.6%; p = 0.080, 0.152, and 0.353 in the pre-anesthesia, sevoflurane, and propofol groups, respectively). Conclusions In our study, TENS did not show a cardioprotective effect in patients undergoing aortic valve replacement surgery.     Falciform Ligament Abscess after Omphalitis: Report of a Case

    A falciform ligament abscess is a rare type of intra-abdominal abscess. A 2-yr-old male, who had omphalitis two months previously, presented with a fever and right upper quadrant abdominal pain. The ultrasound and CT scan showed an abdominal wall abscess located anterior to the liver, which was refractory to conservative management with percutaneous draninage and antibiotics. On the third recurrence, surgical exploration was performed and revealed an abscess arising from the falciform ligament; the falciform ligament was excised. A follow up ultrasound confirmed complete resolution of the abscess with no further recurrence

    A qualitative study on the internal response of medical students during the transgender healthcare education: a focus on professional identity

    Purpose This qualitative study examined the inner response of medical students who participated in a transgender healthcare education program. The factors that effected the inner response were investigated in order to suggest strategies for improving the effectiveness of the transgender healthcare education program. Methods The transgender healthcare education program consisted of 2 hours of lectures and 2 hours of clinical role-play over 3 weeks. Eight 4-year medical students in Seoul, South Korea, were selected considering gender, the route of admission to medical school, and religion. Each student was interviewed individually for approximately 30 minutes 3 times before, during, and after the educational program, and interviews were analyzed using thematic analysis. Results By attending the transgender healthcare education program, students have shown three types of inner response—confusion, acceptance, and negotiation. The students’ personal identities and professional identities influenced these responses. In particular, students’ existing professional identities motivated them to learn about transgender healthcare and played a key role in resolving the discomfort that occurred during the educational program. Through the transgender healthcare education, students were able to reduce prejudice against transgender people, understand the unique medical needs of the transgender population and increase their self-efficacy related to transgender health care. Conclusion The transgender healthcare education program allowed medical school students to acquire medical knowledge related to transgender patients and increase their cultural competence as future medical professionals. In addition, as the professional identity is critical in educating transgender healthcare, it should be considered thoroughly in order to effectively educate the medical students
