    Blockade of cannabinoid 1 receptor improves glucose responsiveness in pancreatic beta cells

    Cannabinoid 1 receptors (CB1Rs) are expressed in peripheral tissues, including islets of Langerhans, where their function(s) is under scrutiny. Using mouse beta-cell lines, human islets and CB1R-null (CB1R(-/-)) mice, we have now investigated the role of CB1Rs in modulating beta-cell function and glucose responsiveness. Synthetic CB1R agonists diminished GLP-1-mediated cAMP accumulation and insulin secretion as well as glucose-stimulated insulin secretion in mouse beta-cell lines and human islets. In addition, silencing CB1R in mouse cells resulted in an increased expression of pro-insulin, glucokinase (GCK) and glucose transporter 2 (GLUT2), but this increase was lost in cells lacking insulin receptor. Furthermore, CB1R(-/-) mice had increased pro-insulin, GCK and GLUT2 expression in cells. Our results suggest that CB1R signalling in pancreatic islets may be harnessed to improve beta-cell glucose responsiveness and preserve their function. Thus, our findings further support that blocking peripheral CB1Rs would be beneficial to beta-cell function in type 2 diabetes

    A second hit somatic (p.R905W) and a novel germline intron-mutation of TSC2 gene is found in intestinal lymphangioleiomyomatosis: a case report with literature review

    Background Tuberous sclerosis complex (TSC) is an autosomal dominant disorder characterized by hamartomas in multiple organs associated with germline mutations in TSC1 and TSC2, including exonic, intronic, or mosaic mutations. Gastrointestinal (GI) tract Lymphangioleiomyomatosis (LAM) is an extremely rare manifestation of TSC, with few reported cases. Herein, we aimed to determine the driver mutation, pathogenesis, and relationship of germline and somatic mutations of LAM through whole-genome sequencing (WGS) of the tumor and blood samples and whole transcriptome sequencing (WTS) analysis. Case presentation A nine-year-old girl with a full-blown TSC presented with abdominal masses detected during a routine check-up. Resected intestinal masses were diagnosed as LAM by thorough pathological examination. Interestingly, the LAM presented a somatic TSC2 gene mutation in exon 24 (p.R905W, c.C2713T), and the patient had intron retention by a novel germline mutation in the intron region of TSC2 (chr16:2126489, Cā€‰>ā€‰G). Conclusion Our case suggests that intron retention by a single nucleotide intronic mutation of TSC2 is sufficient to develop severe manifestations of TSC, but the development of LAM requires an additional somatic oncogenic mutationĀ of TSC2.This work was supported by IITP grant funded by the Korean government (MSIP) (No.2019-0567)

    Verification and Application of Sequence Stratigraphy to Reservoir Characterization of Horn River Basin, Canada

    Shale reservoirs, the most important unconventional resource, are difficult to characterize. Shale formations require detailed interpretation of geological, petrophysical, and geochemical analyses, and an integration of these disciplines. In terms of geological interpretation, the commonly used sequence stratigraphy analysis includes a lithofacies analysis. The application of sequence stratigraphy to shales facilitates the ability to relate between lithofacies and mineral composition, petrophysical parameters, and kerogen contents, which are affected by depositional setting. The classification of lithofacies is indispensable for reservoir quality prediction. In this study, porosity, permeability, and TOC content largely depend on lithofacies, and their correlation coefficient is relatively high. The sequence stratigraphic interpretation shows that organic carbon content usually increases within the maximum flooding surfaces and decreases stepwise. However, the relationship between total organic carbon contents and systems tract is less direct and redox dependent

    Geographical variations and influential factors of disease prevalence are crucial information enabling optimal allocation of limited medical resources and prioritization of appropriate treatments for each regional unit. The purpose of this study was to explore the geographical variations and influential factors of cardiometabolic disease prevalence with respect to 230 administrative districts in South Korea. Global Moran's I was calculated to determine whether the standardized prevalences of cardiometabolic diseases (hypertension, stroke, and diabetes mellitus) were spatially clustered. The CART algorithm was then applied to generate decision tree models that could extract the diseases' regional influential factors from among 101 demographic, economic, and public health data variables. Finally, the accuracies of the resulting model-hypertension (67.4%), stroke (62.2%), and diabetes mellitus (56.5%)-were assessed by ten-fold cross-validation. Marriage rate was the main determinant of geographic variation in hypertension and stroke prevalence, which has the possibility that married life could have positive effects in lowering disease risks. Additionally, stress-related variables were extracted as factors positively associated with hypertension and stroke. In the opposite way, the wealth status of a region was found to have an influence on the prevalences of stroke and diabetes mellitus. This study suggested a framework for provision of novel insights into the regional characteristics of diseases and the corresponding influential factors. The results of the study are anticipated to provide valuable information for public health practitioners' cost-effective disease management and to facilitate primary intervention and mitigation efforts in response to regional disease outbreaks

    Effect of Reciprocating Gait Orthosis with Hip Actuation on Upper Extremity Loading during Ambulation in Patient with Spinal Cord Injury: A Single Case Study

    Reciprocating gait orthosis (RGO) is a traditional passive orthosis that provides postural stability and allows for independent upright ambulation with the assistance of walking aids, such as crutches, canes, and walkers. Previous follow-up studies of patients with RGOs have indicated a high frequency of nonusage. One of the main reasons for avoiding the use of RGOs is the excessive upper extremity loading induced by walking aids. The purpose of this study was to investigate the effect of hip actuation on the upper extremity loading induced by crutches when ambulating with an RGO. One female individual with a chronic complete spinal cord injury classified as ASIA A participated in this study. We compared the upper extremity loading during ambulation when individualized hip assistive forces were applied on the RGO (POWERED condition) and when wearing the RGO without actuation (RGO condition). Upper extremity loading was assessed by measuring the forces acting on the crutches. Compared with the RGO condition, the average upper extremity loading per unit distance and per unit time were lower for the POWERED condition by 15.21% (RGO: 0.307 Ā± 0.056 and POWERED: 0.260 Ā± 0.034 %bwĀ·māˆ’1) and by 21.19% (RGO: 0.120 Ā± 0.020 and POWERED: 0.094 Ā± 0.011 %bwĀ·sāˆ’1), respectively. We believe that a substantial reduction in upper extremity loading during ambulation provided by hip actuation holds promise to promote long-term RGO use and enable patients with paraplegia to perform frequent and intensive rehabilitation training. As this is a single case study, subsequent studies should aim to verify this effect through a higher number of patients and to different injury levels

    Short-term Effects of Ambient Air Pollution on Emergency Department Visits for Asthma: An Assessment of Effect Modification by Prior Allergic Disease History

    Objectives The goal of this study was to investigate the short-term effect of ambient air pollution on emergency department (ED) visits in Seoul for asthma according to patientsā€™ prior history of allergic diseases. Methods Data on ED visits from 2005 to 2009 were obtained from the Health Insurance Review and Assessment Service. To evaluate the risk of ED visits for asthma related to ambient air pollutants (carbon monoxide [CO], nitrogen dioxide [NO2], ozone [O3], sulfur dioxide [SO2], and particulate matter with an aerodynamic diameter <10 Ī¼m [PM10]), a generalized additive model with a Poisson distribution was used; a single-lag model and a cumulative-effect model (average concentration over the previous 1-7 days) were also explored. The percent increase and 95% confidence interval (CI) were calculated for each interquartile range (IQR) increment in the concentration of each air pollutant. Subgroup analyses were done by age, gender, the presence of allergic disease, and season. Results A total of 33 751 asthma attack cases were observed during the study period. The strongest association was a 9.6% increase (95% CI, 6.9% to 12.3%) in the risk of ED visits for asthma per IQR increase in O3 concentration. IQR changes in NO2 and PM10 concentrations were also significantly associated with ED visits in the cumulative lag 7 model. Among patients with a prior history of allergic rhinitis or atopic dermatitis, the risk of ED visits for asthma per IQR increase in PM10 concentration was higher (3.9%; 95% CI, 1.2% to 6.7%) than in patients with no such history. Conclusions Ambient air pollutants were positively associated with ED visits for asthma, especially among subjects with a prior history of allergic rhinitis or atopic dermatitis
