Two-dimensional Nanolithography Using Atom Interferometry
We propose a novel scheme for the lithography of arbitrary, two-dimensional
nanostructures via matter-wave interference. The required quantum control is
provided by a pi/2-pi-pi/2 atom interferometer with an integrated atom lens
system. The lens system is developed such that it allows simultaneous control
over atomic wave-packet spatial extent, trajectory, and phase signature. We
demonstrate arbitrary pattern formations with two-dimensional 87Rb wavepackets
through numerical simulations of the scheme in a practical parameter space.
Prospects for experimental realizations of the lithography scheme are also

The impact of using the Web in a mixed-mode follow-up of a longitudinal birth cohort study: Evidence from the National Child Development Study
A sequential mixed-mode data collection, online-to-telephone, was introduced into the National Child Development Study for the first time at the study's age 55 sweep in 2013. The study included a small experiment, whereby a randomised subset of study members was allocated to a single mode, telephone-only interview, in order to test for the presence of mode effects on participation and measurement. Relative to telephone-only, the offer of the Web increased overall participation rates by 5.0 percentage points (82.8% vs. 77.8%; 95% confidence interval for difference: 2.7% to 7.3%). Differences attributable to mode of interview were detected in levels of item non-response and response values for a limited number of questions. Most notably, response by Web (relative to telephone) was found to have increased the likelihood of non-response to questions relating to pay and other financial matters, and increased the likelihood of ‘less desirable’ responses. For example, response by Web resulted in the reporting of more units of alcohol consumed, and more negative responses to subjective questions such as self-rated health, self-rated financial status and well-being. Multi-frequency VLBA Observations of the Compact Double B2 2050+36: Constraints on Interstellar Scattering Revisited
We present multi-frequency observations with the Very Long Baseline Array of
the compact double radio source B2 2050+36. Our observations are at 0.33, 0.61,
1.67, 2.3, and 8.4 GHz, with the 0.61 GHz observations forming the third epoch
of observation of this source at that frequency. At 0.61 GHz, the structure of
B2 2050+36 is dominated by two components 56 mas apart. Within the
uncertainties of the various measurements, this separation has remained
unchanged for the past 16 years. Any differential image wander caused by
refractive interstellar scattering is less than 4 mas. Both the lack of
differential image wander and the frequency dependence of the angular diameter
of B2 2050+36 below 1 GHz indicate that the electron density power spectrum
along this line of sight has a spectral index near the Kolmogorov value, with a
value of 4 being highly unlikely. We conclude that diffractive scattering
dominates along this line of sight; results in the literature indicate that
this conclusion also holds true for the line of sight to the pulsar PSR
B2020+28 (8.7 deg. from B2 2050+36). Comparison of our 1.67 GHz observations
with those obtained 21 years previously place a limit on the projected linear

In vitro assembly of a prohead-like structure of the Rhodobacter capsulatus gene transfer agent
AbstractThe gene transfer agent (GTA) is a phage-like particle capable of exchanging double-stranded DNA fragments between cells of the photosynthetic bacterium Rhodobacter capsulatus. Here we show that the major capsid protein of GTA, expressed in E. coli, can be assembled into prohead-like structures in the presence of calcium ions in vitro. Transmission electron microscopy (TEM) of uranyl acetate staining material and thin sections of glutaraldehyde-fixed material demonstrates that these associates have spherical structures with diameters in the range of 27–35 nm. The analysis of scanning TEM images revealed particles of mass ∼4.3 MDa, representing 101±11 copies of the monomeric subunit. The establishment of this simple and rapid method to form prohead-like particles permits the GTA system to be used for genome manipulation within the photosynthetic bacterium, for specific targeted drug delivery, and for the construction of biologically based distributed autonomous sensors for environmental monitoring
On the formation of a Hawking-radiation photosphere around microscopic black holes
We show that once a black hole surpasses some critical temperature
, the emitted Hawking radiation interacts with itself and forms a
nearly thermal photosphere. Using QED, we show that the dominant interactions
are bremsstrahlung and electron-photon pair production, and we estimate
, which when calculated more precisely is
found to be 45 GeV. The formation of the photosphere is
purely a particle physics effect, and not a general relativistic effect, since
the the photosphere forms roughly Schwarzschild radii away from
the black hole. The temperature of the photosphere decreases with distance
from the black hole, and the outer surface is determined by the constraint
(for the QED case), since this is the point at which electrons
and positrons annihilate, and the remaining photons free stream to infinity.
Observational consequences are discussed, and it is found that, although the
QED photosphere will not affect the Page-Hawking limits on primordial black
holes, which is most important for 100MeV black holes, the inclusion of QCD
interactions may significantly effect this limit, since for QCD we estimate
. The photosphere greatly reduces possibility of
observing individual black holes with temperatures greater than ,
since the high energy particles emitted from the black hole are processed
through the photosphere to a lower energy, where the gamma ray background is
much higher. The temperature of the plasma in the photosphere can be extremely
high, and this offers interesting possibilities for processes such as symmetry

Assessment of Disparities Associated with a Crisis Standards of Care Resource Allocation Algorithm for Patients in 2 US Hospitals during the COVID-19 Pandemic
Importance: Significant concern has been raised that crisis standards of care policies aimed at guiding resource allocation may be biased against people based on race/ethnicity. Objective: To evaluate whether unanticipated disparities by race or ethnicity arise from a single institution\u27s resource allocation policy. Design, Setting, and Participants: This cohort study included adults (aged ≥18 years) who were cared for on a coronavirus disease 2019 (COVID-19) ward or in a monitored unit requiring invasive or noninvasive ventilation or high-flow nasal cannula between May 26 and July 14, 2020, at 2 academic hospitals in Miami, Florida. Exposures: Race (ie, White, Black, Asian, multiracial) and ethnicity (ie, non-Hispanic, Hispanic). Main Outcomes and Measures: The primary outcome was based on a resource allocation priority score (range, 1-8, with 1 indicating highest and 8 indicating lowest priority) that was assigned daily based on both estimated short-term (using Sequential Organ Failure Assessment score) and longer-term (using comorbidities) mortality. There were 2 coprimary outcomes: maximum and minimum score for each patient over all eligible patient-days. Standard summary statistics were used to describe the cohort, and multivariable Poisson regression was used to identify associations of race and ethnicity with each outcome. Results: The cohort consisted of 5613 patient-days of data from 1127 patients (median [interquartile range {IQR}] age, 62.7 [51.7-73.7]; 607 [53.9%] men). Of these, 711 (63.1%) were White patients, 323 (28.7%) were Black patients, 8 (0.7%) were Asian patients, and 31 (2.8%) were multiracial patients; 480 (42.6%) were non-Hispanic patients, and 611 (54.2%) were Hispanic patients. The median (IQR) maximum priority score for the cohort was 3 (1-4); the median (IQR) minimum score was 2 (1-3). After adjustment, there was no association of race with maximum priority score using White patients as the reference group (Black patients: incidence rate ratio [IRR], 1.00; 95% CI, 0.89-1.12; Asian patients: IRR, 0.95; 95% CI. 0.62-1.45; multiracial patients: IRR, 0.93; 95% CI, 0.72-1.19) or of ethnicity using non-Hispanic patients as the reference group (Hispanic patients: IRR, 0.98; 95% CI, 0.88-1.10); similarly, no association was found with minimum score for race, again with White patients as the reference group (Black patients: IRR, 1.01; 95% CI, 0.90-1.14; Asian patients: IRR, 0.96; 95% CI, 0.62-1.49; multiracial patients: IRR, 0.81; 95% CI, 0.61-1.07) or ethnicity, again with non-Hispanic patients as the reference group (Hispanic patients: IRR, 1.00; 95% CI, 0.89-1.13). Conclusions and Relevance: In this cohort study of adult patients admitted to a COVID-19 unit at 2 US hospitals, there was no association of race or ethnicity with the priority score underpinning the resource allocation policy. Despite this finding, any policy to guide altered standards of care during a crisis should be monitored to ensure equitable distribution of resources

Pharmacological risk factors associated with hospital readmission rates in a psychiatric cohort identified using prescriptome data mining
Background
Worldwide, over 14% of individuals hospitalized for psychiatric reasons have readmissions to hospitals within 30 days after discharge. Predicting patients at risk and leveraging accelerated interventions can reduce the rates of early readmission, a negative clinical outcome (i.e., a treatment failure) that affects the quality of life of patient. To implement individualized interventions, it is necessary to predict those individuals at highest risk for 30-day readmission. In this study, our aim was to conduct a data-driven investigation to find the pharmacological factors influencing 30-day all-cause, intra- and interdepartmental readmissions after an index psychiatric admission, using the compendium of prescription data (prescriptome) from electronic medical records (EMR).
Methods
The data scientists in the project received a deidentified database from the Mount Sinai Data Warehouse, which was used to perform all analyses. Data was stored in a secured MySQL database, normalized and indexed using a unique hexadecimal identifier associated with the data for psychiatric illness visits. We used Bayesian logistic regression models to evaluate the association of prescription data with 30-day readmission risk. We constructed individual models and compiled results after adjusting for covariates, including drug exposure, age, and gender. We also performed digital comorbidity survey using EMR data combined with the estimation of shared genetic architecture using genomic annotations to disease phenotypes.
Results
Using an automated, data-driven approach, we identified prescription medications, side effects (primary side effects), and drug-drug interaction-induced side effects (secondary side effects) associated with readmission risk in a cohort of 1275 patients using prescriptome analytics. In our study, we identified 28 drugs associated with risk for readmission among psychiatric patients. Based on prescription data, Pravastatin had the highest risk of readmission (OR = 13.10; 95% CI (2.82, 60.8)). We also identified enrichment of primary side effects (n = 4006) and secondary side effects (n = 36) induced by prescription drugs in the subset of readmitted patients (n = 89) compared to the non-readmitted subgroup (n = 1186). Digital comorbidity analyses and shared genetic analyses further reveals that cardiovascular disease and psychiatric conditions are comorbid and share functional gene modules (cardiomyopathy and anxiety disorder: shared genes (n = 37; P = 1.06815E-06)).
Conclusions
Large scale prescriptome data is now available from EMRs and accessible for analytics that could improve healthcare outcomes. Such analyses could also drive hypothesis and data-driven research. In this study, we explored the utility of prescriptome data to identify factors driving readmission in a psychiatric cohort. Converging digital health data from EMRs and systems biology investigations reveal a subset of patient populations that have significant comorbidities with cardiovascular diseases are more likely to be readmitted. Further, the genetic architecture of psychiatric illness also suggests overlap with cardiovascular diseases. In summary, assessment of medications, side effects, and drug-drug interactions in a clinical setting as well as genomic information using a data mining approach could help to find factors that could help to lower readmission rates in patients with mental illness
D-cycloserine augmentation of exposure-based cognitive behavior therapy for anxiety, obsessive-compulsive, and posttraumatic stress disorders: a systematic review and meta-analysis of individual participant data
Importance: Whether and under which conditions D-cycloserine (DCS) augments the effects of exposure-based cognitive behavior therapy for anxiety, obsessive-compulsive, and posttraumatic stress disorders is unclear. Objective: To clarify whether DCS is superior to placebo in augmenting the effects of cognitive behavior therapy for anxiety, obsessive-compulsive, and posttraumatic stress disorders and to evaluate whether antidepressants interact with DCS and the effect of potential moderating variables. Data Sources: PubMed, EMBASE, and PsycINFO were searched from inception to February 10, 2016. Reference lists of previous reviews and meta-analyses and reports of randomized clinical trials were also checked. Study Selection: Studies were eligible for inclusion if they were (1) double-blind randomized clinical trials of DCS as an augmentation strategy for exposure-based cognitive behavior therapy and (2) conducted in humans diagnosed as having specific phobia, social anxiety disorder, panic disorder with or without agoraphobia, obsessive-compulsive disorder, or posttraumatic stress disorder. Data Extraction and Synthesis: Raw data were obtained from the authors and quality controlled. Data were ranked to ensure a consistent metric across studies (score range, 0-100). We used a 3-level multilevel model nesting repeated measures of outcomes within participants, who were nested within studies. Results: Individual participant data were obtained for 21 of 22 eligible trials, representing 1047 of 1073 eligible participants. When controlling for antidepressant use, participants receiving DCS showed greater improvement from pretreatment to posttreatment (mean difference, -3.62; 95% CI, -0.81 to -6.43; P = .01; d = -0.25) but not from pretreatment to midtreatment (mean difference, -1.66; 95% CI, -4.92 to 1.60; P = .32; d = -0.14) or from pretreatment to follow-up (mean difference, -2.98, 95% CI, -5.99 to 0.03; P = .05; d = -0.19). Additional analyses showed that participants assigned to DCS were associated with lower symptom severity than those assigned to placebo at posttreatment and at follow-up. Antidepressants did not moderate the effects of DCS. None of the prespecified patient-level or study-level moderators was associated with outcomes. Conclusions and Relevance: D-cycloserine is associated with a small augmentation effect on exposure-based therapy. This effect is not moderated by the concurrent use of antidepressants. Further research is needed to identify patient and/or therapy characteristics associated with DCS response.2018-05-0
