    Get PDF
We came up with the idea of using a cost analysis to clearly demonstrate the advantage of home care vs. hospitalization. John Hopkins Bay view Medical Center conducted a study within their geriatrics unit to test their Hospital At Home program against typical on-site care at the hospital. The study was held over the course of 30 days involving hundreds of patients across three cities and the results were staggering. Not only was at home care 32% cheaper (5,081versus5,081 versus 7,480) but also overall customer satisfaction was significantly higher

Fourteen motets from the Florilegium Portense of Erhard Bodenschatz: Critical editions and comparative analysis with the original sources

The Florilegium Portense encompasses three anthology publications in Leipzig by Erhard Bodenschatz from 1603 to 1621. Born around 1576, Bodenschatz was a composer and pastor who was active in Saxony and Thuringia. While serving as the cantor at the prominent Schulpforta near Naumburg, Saxony from 1600 to 1603, he compiled an anthology of eighty-nine motets called the Florilegium Selectissimarum Cantionum, which was published by Abraham Lamberg in 1603. Nearly all of the works contained in the anthology were in Latin and most were for eight voices. An expanded and revised volume of 115 motets was published in 1618 and was the only publication to truly bear the name of Florilegium Portense. The anthology was designed for use at the school, primarily to provide music for daily functions such as singing at meals. Three years later, Bodenschatz published another volume of 150 motets, entitled Florilegii Musici Portensis. The last was intended more for liturgical use and consists mostly of music by Italian composers. Eighty-three of the pieces in this latter volume also occur in the Promptuarium Musicum, a contemporary four-volume collection edited by Abraham Schadaeus and Caspar Vincentius. The anthologies have become known primarily because of their use by Johann Sebastian Bach in Leipzig, but they were widely used in schools and churches throughout Germany. Research on the Florilegium has slowly progressed, but much of the music remains unavailable in score. The primary purpose of this project is to present editions of fourteen motets from the 1618 volume: three by Andreas Berger, one by Bodenschatz, five by Albinus Fabricius, two each by Andrea Gabrieli and Jacob Handl (known as Gallus), and one motet by Vincentius. These were chosen because they represent some of the more prolific composers found in the anthology, its editor, and possible contributor of the continuo part. The motets by Gabrieli and one by Handl were chosen because they are eight-voice arrangements of works originally scored for ten or twelve voices. During the transcription and editing process, it was discovered that the motets in the anthology differ, often in significant ways, from the composers’ original publications. The changes, although inconsistent, appear to be driven by a desire to make the music more accessible, most likely for the students at Schulpforta. There are also many errors in the Florilegium. The text of this study will explore each of the motets that were transcribed and examine the differences between the composers’ original publications and the versions in the Bodenschatz anthology.Ope

OBJECTIVES: The aim of this study was to gain an insight into the molecular mechanisms of the evolution of rifampicin resistance in response to controlled changes in the environment. METHODS: We determined the proportion of rpoB mutants in the chemostat culture and characterized the sequence of mutations found in the rifampicin resistance-determining region of rpoB in a steady-state chemostat at pH 7.0 and 6.2. RESULTS: The overall proportion of rpoB mutants of strain H37Rv remained constant for 37 days at pH 7.0, ranging between 3.6 x 10(-8) and 8.9 x 10(-8); however, the spectrum of mutations varied. The most commonly detected mutation, serine to leucine mutation at codon 531 (S531L), increased from 40% to 89%, while other mutations (S531W, H526Y, H526D, H526R, S522L and D516V) decreased over the 37 day sampling period. Changing the pH from 7.0 to 6.2 did not significantly alter the overall proportion of mutants, but resulted in a decrease in the percentage of strains harbouring S531L (from 89% to 50%) accompanied by an increase in the range of different mutations from 4 to 12. CONCLUSIONS: The data confirm that the fitness of strains with the S531L mutation is greater than that of strains containing other mutations. We also conclude that at low pH the environment is permissive for a wider spectrum of mutations, which may provide opportunities for a successful mutant to survive

An exploration of the organisational stressors encountered by international disability footballers

    Get PDF
Presently, disability athletes remain under-represented in organisational stressor research. Our study sought to bring novel insights to this area by determining the organisational stressors experienced by international disability footballers. Twelve current international disability footballers (10 male, 2 female) from a range of UK impairment squads took part in the study. Semi-structured interviews were completed with each participant, and data were analysed by content analysis procedures. Organisational stressors data were abstracted into Arnold, Wagstaff, Steadman, and Pratt’s (2017) concepts, and Arnold and Fletcher’s (2012) four general dimensions: leadership and personnel issues, cultural and team issues, logistical and environmental issues, and performance and personal issues, revealing a series of football specific nuances. Our study is the first exploration of the prevalence of organisational stressors within international disability football. Our study also provides practitioners with an understanding of the common and unique organisational stressors faced by international disability footballers. Finally, we suggest a series of practical recommendations for policy development within disability football organisations to aid athletes to effective manage organisational stressors.</div

The marketplace is going to be one of the highlights on Thames Street, serving as a destination for leisure, shopping and dining. The concept of the project is to have indoor space continue out to the water, providing an outdoor space for dining and leisure, but also giving the boardwalk a resting point. The building is planned as two floors, with the fish market and multipurpose area on the first and an eatery, sitting area, balcony and facilities on the second. The building will be made of a light metal frame with panels to enclose the space and is designed with a folded glass panel that can be opened up to the outside, yet decrease wind pressure in a storm

Objectives: The objectives of this study were to (1) describe asthma knowledge, control, and administration techniques in Hispanic caregivers of pediatric asthma patients in primary care clinics and (2) determine the association between asthma knowledge and control. Methods: A pilot, cross-sectional study was conducted by administering a survey to Spanish-speaking primary caregivers of children under 12 years old with a current diagnosis of asthma. The survey contained: demographic questions (8 items), a modified version of the Asthma Control Test (ACT, 5 self-assessment items, 5-point, Likert-type scale), knowledge (21 items, correct/incorrect response), and inhaler use (1 item, rank ordering steps for using inhaler). Descriptive statistics were performed for all items, and a Pearson correlation analysis was utilized to assess the association between asthma knowledge and control in SPSS v. 22.0 (Armonk, NY), with α =0.05. Results: Of the 12 caregivers, who completed the survey, all were parents of the patients. Seven patients (58%) had controlled asthma. On the asthma knowledge portion of the survey, three caregivers of these patients (25%) scored over 75%, three (25%) scored under 50%, and six (50%) scored between 50-75%. There was a statistically-significant, positive correlation between asthma knowledge and asthma control (r=0.668, p=0.017). No caregivers were able to correctly order the eight step sequence of using an inhaler. Conclusion: A positive correlation between asthma knowledge and asthma control was found in caregivers of asthmatic pediatric patients. Inhaler technique, recognition of asthma triggers, and understanding situations necessitating a physician visit were areas in which participants showed a knowledge deficiency

In the fall of 1992, the TAC Team was presented with a Request for Proposal (PFP) for a mid-size (250-350 passenger) commercial transport. The aircraft was to be extremely competitive in the areas of passenger comfort, performance, and economic aspects. Through the use of supercritical airfoils, a technologically advanced Very High By-pass Ratio (VHBR) turbofan engine, a low overall drag configuration, a comparable interior layout, and mild use of composites, the JB-300 offers an economically viable choice to the airlines. The cents per passenger mile of the JB-300 is 1.76, which is considerably lower than current aircraft in the same range. Overall, the JB-300 is a technologically advanced aircraft, which will meet the demands of the 21st century
