    Early and late complications of bariatric operation.

    Weight loss surgery is one of the fastest growing segments of the surgical discipline. As with all medical procedures, postoperative complications will occur. Acute care surgeons need to be familiar with the common problems and their management. Although general surgical principles generally apply, diagnoses specific to the various bariatric operations must be considered. There are anatomic considerations which alter management priorities and options for these patients in many instances. These problems present both early or late in the postoperative course. Bariatric operations, in many instances, result in permanent alteration of a patient\u27s anatomy, which can lead to complications at any time during the course of a patient\u27s life. Acute care surgeons diagnosing surgical emergencies in postbariatric operation patients must be familiar with the type of surgery performed, as well as the common postbariatric surgical emergencies. In addition, surgeons must not overlook the common causes of an acute surgical abdomen-acute appendicitis, acute diverticulitis, acute pancreatitis, and gallstone disease-for these are still among the most common etiologies of abdominal pathology in these patients

    Cancer-associated TERT promoter mutations abrogate telomerase silencing.

    Mutations in the human telomerase reverse transcriptase (TERT) promoter are the most frequent non-coding mutations in cancer, but their molecular mechanism in tumorigenesis has not been established. We used genome editing of human pluripotent stem cells with physiological telomerase expression to elucidate the mechanism by which these mutations contribute to human disease. Surprisingly, telomerase-expressing embryonic stem cells engineered to carry any of the three most frequent TERT promoter mutations showed only a modest increase in TERT transcription with no impact on telomerase activity. However, upon differentiation into somatic cells, which normally silence telomerase, cells with TERT promoter mutations failed to silence TERT expression, resulting in increased telomerase activity and aberrantly long telomeres. Thus, TERT promoter mutations are sufficient to overcome the proliferative barrier imposed by telomere shortening without additional tumor-selected mutations. These data establish that TERT promoter mutations can promote immortalization and tumorigenesis of incipient cancer cells

    Robust High-Dynamic-Range Vector Magnetometry via Nitrogen-Vacancy Centers in Diamond

    We demonstrate a robust, scale-factor-free vector magnetometer, which uses a closed-loop frequency-locking scheme to simultaneously track Zeeman-split resonance pairs of nitrogen-vacancy (NV) centers in diamond. Compared with open-loop methodologies, this technique is robust against fluctuations in temperature, resonance linewidth, and contrast; offers a three-order-of-magnitude increase in dynamic range; and allows for simultaneous interrogation of multiple transition frequencies. By directly detecting the resonance frequencies of NV centers aligned along each of the diamond's four tetrahedral crystallographic axes, we perform full vector reconstruction of an applied magnetic field

    Melanoma-associated adhesion molecule MUC18/MCAM (CD146) and transcriptional regulator Mader in normal human CNS

    The proteins MUC18 and Mader have been identified as markers of tumor progression in melanoma cells, MUC18, also known as MCAM (melanoma cell adhesion molecule) and as CD146 (endothelial antigen), is a cell adhesion molecule belonging to the immunoglobulin superfamily, Mader is a transcriptional regulator shown to negatively regulate EGR-1. As it is known that neoplastic cells of neuroectodermal origin frequently express neuron-specific molecules, we studied whether these melanoma-associated antigens are found in normal CNS tissue. We investigated the expression of MUC18/MCAM and Mader in adult human post mortem CNS tissue by immunohistochemistry, immunoblot and two-dimensional gel electrophoresis. Our results show that Mader is preferentially expressed on neurons and glial cells and that the adhesion protein MUC18/MCAM is mainly expressed on vasculature within the CNS. These observations may have important implications for further studies investigating their possible roles in cell adhesion and proliferation control within the CNS

    Simulating Dynamical Features of Escape Panic

    One of the most disastrous forms of collective human behaviour is the kind of crowd stampede induced by panic, often leading to fatalities as people are crushed or trampled. Sometimes this behaviour is triggered in life-threatening situations such as fires in crowded buildings; at other times, stampedes can arise from the rush for seats or seemingly without causes. Tragic examples within recent months include the panics in Harare, Zimbabwe, and at the Roskilde rock concert in Denmark. Although engineers are finding ways to alleviate the scale of such disasters, their frequency seems to be increasing with the number and size of mass events. Yet, systematic studies of panic behaviour, and quantitative theories capable of predicting such crowd dynamics, are rare. Here we show that simulations based on a model of pedestrian behaviour can provide valuable insights into the mechanisms of and preconditions for panic and jamming by incoordination.     C.H.A.M.P. Families : Description and Theoretical Foundations of a Paediatric Overweight and Obesity Intervention Targeting Parents-A Single-Centre Non-Randomised Feasibility Study.

    Childhood obesity represents a significant global health challenge, and treatment interventions are needed. The purpose of this paper is to describe the components and theoretical model that was used in the development and implementation of a unique parent-focussed paediatric overweight/obesity intervention. C.H.A.M.P. Families was a single-centre, prospective intervention offered to parents of children aged between 6⁻14 years with a body mass index (BMI) ≥85th percentile for age and sex. The intervention included: (1) eight group-based (parent-only) education sessions over 13-weeks; (2) eight home-based activities; and (3) two group-based (family) follow-up support sessions. The first section of the manuscript contains a detailed description of each intervention component, as well as an overview of ongoing feasibility analyses. The theoretical portion details the use of evidence-based group dynamics principles and motivational interviewing techniques within the context of a broader social cognitive theory foundation. This paper provides researchers with practical examples of how theoretical constructs and evidence-based strategies can be applied in the development and implementation of parent-focussed paediatric obesity interventions. Given the need for transparent reporting of intervention designs and theoretical foundations, this paper also adds to the areas of implementation science and knowledge translation research

    Effects of increasing soybean hulls in finishing diets with wet or modified distillers grains plus solubles on performance and carcass characteristics of beef steers

    Two experiments evaluated feeding soybean hulls (SBH) in finishing diets that contain distillers grains plus solubles on performance and carcass characteristics. Dietary concentrations of SBH were 0, 12.5, 25, and 37.5% of diet DM. In Exp. 1, 167 crossbred yearling steers (395 ± 22 kg of BW) were fed for 117 d in a randomized block design in which pelleted SBH replaced dry-rolled corn. All diets contained 25% modified distillers grains plus solubles, 15% corn silage, and 5% liquid supplement. As SBH concentration increased, DMI decreased linearly (P = 0.04). Gain and G:F decreased linearly (P \u3c 0.01) in response to increasing concentrations of SBH, which decreased relative energy value from 91 to 79% of corn. Hot carcass weight linearly decreased (P \u3c 0.01) by 24 kg as SBH increased. In Exp. 2, a randomized block design used 160 backgrounded steer calves (363 ± 16 kg of BW) in a 138-d finishing study with 0, 12.5, 25, or 37.5% SBH in the meal form. Basal ingredients consisted of a 1:1 ratio of high-moisture corn and dry-rolled corn, 40% wet distillers grains plus solubles, 8% sorghum silage, and 4% dry meal supplement. There was a tendency (P = 0.12) for a quadratic increase in ADG and G:F as dietary SBH increased, with numerically greatest ADG and G:F with 12.5% SBH. Feeding 12.5 to 25% SBH with 40% wet distillers grains plus solubles (Exp. 2) had little effect on performance but decreased ADG and G:F in diets with 25% modified distillers grains plus solubles (Exp. 1)
