Tengsl D-vítamíns og áhættuþátta hjartaog æðasjúkdóma meðal íslenskra barna

Tilgangur: Að kanna tengsl á milli D-vítamíngilda í blóði og þekktra áhættu­þátta fyrir hjarta- og æðasjúkdómum meðal heilbrigðra íslenskra barna og jafnframt að kanna þessi tengsl óháð líkamsþyngdarstuðli (BMI). Efniviður/aðferðir: Metin voru tengsl milli styrks 25-hydroxyvítamín D í blóði, líkamsþyngdarstuðuls og sjö áhættuþátta hjarta- og æðasjúkdóma (háþrýstings, heildarkólesteróls, HDL, LDL, þríglýceríðs, blóðsykurs og styrks insulíns í blóði). Þátttakendur voru 7 ára skólabörn í 6 grunnskólum Reykjavíkur, haustið 2006. Niðurstöður: D-vítamín var mælt hjá 159 börnum. 35 þeirra (22%) voru undir 37,5 nmól/L, 70 (44%) á milli 37,5 og 50,0 nmól/L og 55 (34%) yfir 50 nmól/L. D vítamínskortur var skilgreindur sem gildi undir 37,5 nmól/L. Ekki reyndist vera marktækur munur á milli kynja, stelpur (n:85 = 44,2 nmól/L), strákar (n:74 = 46,9 nmól/L), p-gildi 0,52 milli hópa. Börn með D-vítamínskort höfðu tilhneigingu til að hafa hærri líkamsþyngdarstuðul (p=0,052), lægra HDL (p=0,044) og hærra HbA1C (p=0,015) og serum insúlín (p=0,014) samanborið við börn með eðlileg D-vítamíngildi, það er yfir 50 nmól/L. Marktæk fylgni var á milli lágs D-vítamíns og hárra gilda insúlíns í blóði (p=0,014) og hárra gilda HbA1c (p=0,015), óháð líkamsþyngdarstuðli. Ályktanir: D-vítamínskortur hefur verið tengdur við þróun hjarta-og æðasjúkdóma. Mikilvægt er að kanna tengsl milli þekktra áhættuþátta hjarta- og æðasjúkdóma og D-vítamíns, sérstaklega hjá börnum og ungu fólki. Hugsanlegt er að D-vítamínskortur auki áhættuna á að þróa með sér hjartasjúkdóm snemma í lífinu gegnum insúlínviðnám og breytta blóðsykurstjórnun. Mikilvægt er að fylgja opinberum ráðleggingum varðandi D-vítamíngjöf fyrir alla aldurshópa, en rannsóknin sýndi að 2/3 barnanna voru undir þeim kjörgildum sem Embætti landlæknis mælir með

D-vítamínbúskapur íslenskra barna og ungmenna Langtímarannsókn

TILGANGUR D-vítamín er mikilvægt fyrir vöxt og líkamlegan þroska barna, ekki eingöngu til að bæta beinheilsu heldur einnig vegna áhrifa þess á aðra starfsemi líkamans. Embætti landlæknis ráðleggur að D-vítamínþéttni í blóði sé minnst 50 nmól/l. Tilgangur rannsóknarinnar var að kanna hve stór hluti íslenskra barna og ungmenna næðu ráðlagðri D-vítamínþéttni við 7, 9, 15 og 17 ára aldur, ásamt því að kanna breytingar á D-vítamínþéttni yfir tíma og tengsl við kalkvaka (parathyroid hormone status (S-PTH)). EFNIVIÐUR OG AÐFERÐIR Rannsóknarhópurinn samanstóð af nemendum 6 grunnskóla í Reykjavík, fæddum árið 1999. Blóðprufur voru teknar fjórum sinnum árin 2006, 2008, 2015 og 2017. Að hluta til var um sömu börn að ræða en fleiri bættust í hópinn árið 2015 og 2017. NIÐURSTÖÐUR Í öllum mælingum voru um eða yfir 60% barna með lægri þéttni D-vítamíns í blóði en Embætti landlæknis ráðleggur. Einungis 13% náðu viðmiðum um þéttni yfir 50 nmól/l í endurteknum mælingum og 38,9% einstaklinganna voru með lægri en ráðlagða þéttni í minnst tveimur blóðprufum. Ekki var marktækur munur milli kynja nema hvað 17 ára stelpur höfðu marktækt hærra D-vítamínþéttni en strákar (p=0,04). S-PTH hafði neikvæða fylgni við D-vítamín við 7, 15 og 17 ára aldur en náði ekki marktækni við 9 ára aldur. ­Meðaltalsgildi S-PTH var lægst við 7 ára aldur en hækkaði síðan með aldri. ÁLYKTUN Þéttni D-vítamíns í blóði hjá meirihluta barna og ungmenna er undir ráðlögðum gildum Embættis landlæknis. Hjá stórum hluta er þéttnin endurtekið of lág. Ljóst er að auka þarf D-vítamíninntöku hjá þessum hópi ef markmið um æskilega þéttni á að nást. Áhrif D-vítamínskorts á lýðheilsu eru þó ekki að fullu þekkt

Serum 25-hydroxyvitamin D concentrations in 16-year-old Icelandic adolescent and its association with bone mineral density

Objective: The aim of the study was to assess the potential association between serum 25-hydroxyvitamin D (25(OH)D) and whole-body bone mineral density (BMD) among 16-year-old adolescents and to study the prevalence of 25(OH)D insufficiency, defined as concentration under 50 nmol/l. Design: A cross-sectional study. Setting: Reykjavik, Iceland, latitude 64°08′N. Measurements took place in the Icelandic Heart Association's research lab during April–June 2015. Participants: In total, 411 students in Reykjavik, Iceland, were invited to participate, 315 accepted the invitation (76·6 %) and 289 had valid data (mainly Caucasian). Results: 25(OH)D < 50 nmol/l was observed in 70 % of girls and 66·7 % of boys. 25(OH)D ≥ 50 nmol/l was significantly associated with higher whole-body BMD after adjusting for the influence of sex, height, fat mass and lean mass. A linear relationship between 25(OH)D and whole-body BMD was significant for 25(OH)D < 50 nmol/l (n 199, P < 0·05) but NS for 25(OH)D ≥ 50 nmol/l (n 86, P = 0·48). Conclusions: Our results are in line with some but not all previous studies on the relationship between BMD and 25(OH)D in adolescents. The observed difference in BMD between those with above v. below a 25(OH)D concentration of 50 nmol/l was of about a fifth of one SD, which may have a clinical relevance as one SD decrease in volumetric BMD has been associated with a 89 % increase in 2 years risk of fracture. Icelandic adolescents should be encouraged to increase their vitamin D intake as it is possible that their current intake is insufficient to achieve optimal peak bone mass
