Selected cardiovascular risk factors in primary care patients
Wstęp Choroby układu krążenia są odpowiedzialne za blisko połowę zgonów w Polsce. Jest to niewątpliwie efekt znacznego rozpowszechnienia i kumulacji czynników ryzyka sercowo-naczyniowego. W pracy zaprezentowano wyniki ogólnopolskiej akcji „Zmierz ciśnienie — oceń ryzyko”, dotyczące częstości ważnych czynników ryzyka chorób układu krążenia wśród osób zgłaszających się do lekarza pierwszego kontaktu. Materiał i metody Badania prowadzono w 256 ośrodkach podstawowej opieki zdrowotnej na terenie całego kraju, w latach 2004–2005. Objęto wszystkie osoby zgłaszające się do lekarza pierwszego kontaktu, niezależnie od przyczyny. Od każdego pacjenta uzyskano informacje dotyczące wieku, zachowań zdrowotnych, schorzeń współistniejących i występowania chorób układu krążenia w rodzinie. Wykonano pomiary antropometryczne, pomiary ciśnienia tętniczego i akcji serca, oznaczono metodą paskową poziom glikemii. Wyniki Przebadano 61 641 osób w wieku 14–102 lat. Średni wiek badanych wyniósł 57,2 (± 14,9) lat, przy czym 42% badanych stanowili mężczyźni. Nadciśnienie tętnicze występowało u około 74% pacjentów. Optymalne ciśnienie ( 125 mg/dl) wynosił od: 7,5% u młodych osób do 27% wśród najstarszych. Częstość palenia tytoniu kształtowała się od 35% w najmłodszej grupie do 13% wśród osób w wieku podeszłym. Wnioski Rozpowszechnienie modyfikowalnych czynników ryzyka w populacji osób zgłaszających się do lekarza pierwszego kontaktu jest większa niż w populacji generalnej, co rodzi konieczność wprowadzenia programów edukacyjnych dla osób korzystających z podstawowej opieki zdrowotnej.Background Cardiovascular diseases remain the main cause of mortality in Poland due to the high prevalence of cardiovascular risk factors in population. Low-grade chronic inflammation and immune alterations in childhood and adolescent cancer survivors : a contribution to accelerated aging?
Abstract Background The long‐term consequences of chemotherapy and radiotherapy result in a high prevalence and early onset of age‐related chronic diseases in survivors. We aimed to examine whether childhood and adolescent cancer survivors (CS) demonstrate biomarkers of accelerated aging. Methods We evaluated 50 young adult CS at 11 [8–15] years after cancer diagnosis, and 30 healthy, age and sex‐matched controls, who were unexposed to cancer therapy. Using a machine‐learning approach, we assessed factors discriminating CS from controls and compared selected biomarkers and lymphocyte subpopulations with data from the Framingham Heart Study (FHS) cohort and the Genotype Tissue Expression (GTEx) project. Results Survivors compared with controls had higher levels of C‐reactive protein and fibrinogen. The surface expression of CD38 on T cells was increased, and there was an increase in the percentage of memory T cells in survivors, compared with the unexposed group. The relationships between above cell subpopulations and age were consistent in CS, FHS, and GTEx cohorts, but not in controls. Conclusions Young pediatric cancer survivors differ from age‐related controls in terms of activation of the adaptive immune system and chronic, low‐grade inflammation. These changes resemble aging phenotype observed in older population. Further research in biomarkers of aging in young, adult childhood cancer survivors is warranted, as it may facilitate screening and prevention of comorbidities in this population

Optymalne ciśnienie (< 120/80 mm Hg) stwierdzono
jedynie u 5,6% badanych, a "stan przednadciśnieniowy"
u 20,4% pacjentów. Blisko połowa badanych
miała nadwagę, dalsze 25% pacjentów charakteryzowała
otyłość. Otyłość brzuszną rozpoznano
u 33,4% badanych, a przy zastosowaniu nowych
kryteriów z 2005 roku aż u 59% badanych. Podwyższony
poziom glikemii przygodnej (> 125 mg/dl)
wynosił od: 7,5% u młodych osób do 27% wśród
najstarszych. Częstość palenia tytoniu kształtowała
się od 35% w najmłodszej grupie do 13% wśród osób
w wieku podeszłym.
Wnioski Rozpowszechnienie modyfikowalnych
czynników ryzyka w populacji osób zgłaszających się
do lekarza pierwszego kontaktu jest większa niż
w populacji generalnej, co rodzi konieczność wprowadzenia

25% of them were obese, 33,4% had visceral obesity. When
referring to the latest definition of the metabolic syndrome
and abdominal obesity the number increased to 59%. Glucose
>125 mg/dl has been measured in 7.5% of younger
and 27% of older subjects. About 35% of younger and 13%
of older patients were smokers. Conclusions The prevalence of cardiovascular risk factors
among the patients visiting general practitioners is much
higher than in general population in Poland, which proves
the importance of education programs in primary care
units
Decreased number of regulatory T lymphocytes is related to inflammationand number of CD8+ T cells expressing programmed cell death protein-1 in common variable immunodeficiency
Common variable immunodeficiency (CVID) is a primary immunodeficiency disorder related to recurrent infections, as well as a range of non-infectious manifestations including autoimmune and inflammatory disorders. We hypothesized that patients with CVID and different clinical phenotypes would demonstrate alterations in lymphocyte T subsets, including T lymphocytes expressing programmed cell death protein 1 (PD-1), and regulatory T lymphocytes. We performed flow cytometry in two CVID groups: group 1 with infections only, and group 2 with infections and concomitant noninfectious manifestations. Patients were 18–59 years old (mean 35.8 years of age). Increased proportions of CD8+PD-1+ T cells and reduced regulatory T cells were associated with lymphadenopathy. Amount of regulatory T cells correlated with CD8+PD-1+ T lymphocytes (r = 0.54; p = 0.013), and with CRP (r = –0.64; p = 0.004). Forty percent of patients expressed manifestations in addition to infections (group 2), and they had reduction in number of regulatory T cells [8 (3-12) vs. 24 (11-26)/μl; p = 0.034), naive CD4+ T lymphocytes [36 (27-106) vs. 149 (81-283)/μl; p = 0.034], and elevated C-reactive protein (CRP) [5.33 (3.15-8.82) vs. 1 (1-2.16) mg/l; p = 0.003] in comparison to group 1. In conclusion, the amount of CD8+ T cells expressing PD-1 is associated with lymphadenopathy and number of regulatory T cells in patients with CVID. Patients with CVID and non-infectious complications have increased level of inflammation and alterations in regulatory T cells
Cranial irradiation in childhood acute lymphoblastic leukemia Is related to subclinical left ventricular dysfunction and reduced large artery compliance in cancer survivors
Long-term survivors of acute lymphoblastic leukemia (ALL), the most common childhood
malignancy, are at remarkably increased risk of heart failure (HF) in middle age, most likely due
anthracycline cardiotoxicity. The role of cranial radiation therapy (CRT) in the development of left
ventricular (LV) dysfunction, a predecessor of overt HF, remains unclear. Our aim was to compare
LV function and systemic arterial properties according to past CRT in young adult survivors of
anthracycline-treated ALL.We studied young adult survivors of childhood ALL at a median of 16
years from diagnosis treated with anthracycline-based chemotherapy, with (n = 12) or without (n = 30)
CRT. In addition to fractional shortening (FS) and ejection fraction (EF), LV function was quantified
by tissue Doppler imaging of the mitral annulus. Aortic strain/distensibility and arterial compliance
were derived from echocardiography and simultaneously recorded pulse pressure. Despite similar
FS and EF, peak mitral annular systolic velocity (median (interquartile range): 9.0 (7.5–10.0) vs. 10.0
(8.8–11.5) cm/s, p = 0.05), and early diastolic velocity (13.8 (13.0–14.8) vs. 15.5 (14.0–17.3), p = 0.01) were
decreased after chemotherapy combined with CRT compared to chemotherapy without CRT. Systemic
arterial compliance was lower in post-CRT subjects (1.0 (0.8–1.2 vs. 1.4 (1.1–1.7) mL/mmHg, p = 0.002).
Aortic strain and distensibility were similar regardless of prior CRT. In conclusion, lower arterial
compliance and subclinical LV dysfunction may be possible late consequences of past CRT in adult
survivors of childhood ALL. Whether arterial stiffening is associated with future HF development in
CRT-exposed ALL survivors remains to be investigated
Cardiovascular risk in patients with uncontrolled hypertension and its estimation in outpatient setting in Poland. Results of nationwide RAPORT NT programme
Wstęp Celem badania było sprawdzenie, jak lekarze
w opiece ambulatoryjnej w Polsce oceniają ryzyko
sercowo-naczyniowe (CV) u chorych z niekontrolowanym
nadciśnieniem tętniczym.
Materiał i metody Badanie przeprowadzono w ramach
grantu edukacyjnego firmy Servier Polska.
U chorych z niekontrolowanym nadciśnieniem tętniczym,
którzy zgłaszali się do lekarzy rodzinnych
oraz poradni specjalistycznych, zbierano wywiad
dotyczący danych demograficznych, obecności cukrzycy
i innych towarzyszących sytuacji klinicznych,
nałogu palenia tytoniu oraz obciążeń rodzinnych.
Na podstawie danych z dokumentacji medycznej
uzyskiwano wyniki badań i potwierdzenie uszkodzeń
narządowych. W trakcie wizyty wykonywano
także pomiar ciśnienia oraz obwodu pasa. Wartości
ciśnienia oraz ryzyko klasyfikowano zgodnie z wytycznymi
Polskiego Towarzystwa Nadciśnienia
Tętniczego z 2003 roku. Ryzyko CV oszacowane
przez lekarza porównano z uzyskanym na podstawie
wytycznych w trzech grupach niekontrolowanego
nadciśnienia: stopień 1-3 (Bowker’s
Test of Symmetry). Podobną analizę wykonano
w podgrupie pacjentów z cukrzycą i/lub niewydolnością
nerek.
Wyniki Przebadano 11 745 chorych w średnim wieku
59,0 ± 11,3 roku (50,1% mężczyzn). Ryzyko CV
oszacowane przez lekarzy było podwyższone bardzo
znacznie u 38,2%, znacznie u 30,4%, umiarkowanie
u 27,9% i nieznacznie u 3,6% chorych, ale wyliczone
według wytycznych wynosiło odpowiednio 49,6%,
25,1%, 24,2%, 1,1% (p < 0,001). Lekarze zawyżają
nieznacznie, umiarkowanie i znacznie podwyższone
ryzyko, a niedoszacowują bardzo znacznie podwyższone.
Ta sama tendencja występowała u chorych na
cukrzycę i/lub niewydolność nerek, ale nieznacznie i umiarkowanie podwyższone ryzyko lekarze stwierdzali
tylko sporadycznie.
Wnioski Lekarze dokonują nieprecyzyjnej oceny ryzyka
CV według aktualnych wytycznych, co może
wpływać na intensywność stosowanego leczenia. Dla
poprawy sytuacji konieczne jest wdrożenie działań

Material and methods The study was a part of educational
grant of Servier-Poland. Demographic data, history of diabetes,
associated clinical conditions, smoking habit and
family history were obtained from patients with uncontrolled
hypertension, who visited general practitioners and
specialists. Laboratory measurements and occurrence of
target organ damage (TOD) were checked in medical
records. Blood pressure (BP) and waist circumference,
were measured. BP levels were classified and CV risk was
estimated according to the Polish Society of Hypertension
guidelines 2003. CV risk estimated by physicians and real
risk were compared in three groups of uncontrolled hypertension:
grade 1-3 (Bowker’s Test of Symmetry). Similar
analysis was performed in subgroup of patients with diabetes
and/or renal insufficiency.
Results 11 745 patients (mean age - 59.0 ± 11.3, 50.1%
men) were examined. CV added risk estimated by physicians
was very high in 38.2%, high in 30.4%, moderate in
27.9%, and low in 3.6%, but risk calculated according to the guidelines was 49.6%, 25.1%, 24.2%, 1.1% (respectively),
(p < 0.001). Physicians overestimated low, moderate
and high risk and underestimated very high added
risk. High BP values were strong predictor of underestimation.
The same tendency was observed in diabetic patients,
but low and moderate risks were incidentally estimated
by physicians.
Conclusions Estimation of CV risk according to current
guidelines by physicians is imprecise what may influence
the intensity of treatment. Educational programs are needed
to improve the situation
Combined use of third generation anti-cyclic citrullinated peptide antibodies (aCCP3 IgG/IgA), rheumatoid factors (IgM, IgG and IgA isotypes), joints magnetic resonance imaging and ultrasonography in diagnosis of juvenile idiopathic arthritis
Cel pracy: Ustalenie przydatności oznaczania w surowicy aCCP3
(tzw. III generacji) w klasie IgG i IgA w skojarzeniu z oznaczaniem
czynników reumatoidalnych (RF) klasy IgG, IgA i IgM w rozpoznawaniu
młodzieńczego idiopatycznego zapalenia stawów (MIZS)
u dzieci z niezróżnicowanym zapaleniem stawów.
Materiał i metody: Ustalono następujące kryteria zakwalifikowania
do badania: wiek < 18. roku życia, wykrycie choroby < 16. roku
życia, zapalenie jednego lub więcej stawów, początek zapalenia
stawów > 6 tyg., wyłączenie chorób z listy wykluczeń. U 11 pacjentów
oznaczono w surowicy czynniki reumatoidalne w klasie IgM,
IgG i IgA oraz przeciwciała antycytrulinowe w klasach IgG (aCCP
II generacji) oraz IgG/IgA (aCCP III generacji). U 6 pacjentów wykonano
USG zajętych stawów; w 4 najbardziej wątpliwych diagnostycznie
przypadkach wykonano badanie MR. Wyniki: Dodatniego wyniku aCCP2 IgG (> 20 j./ml) nie stwierdzono
u żadnego z badanych dzieci; średnie stężenie wynosiło
4,85 j./ml; u żadnego dziecka nie otrzymano również dodatniego
wyniku aCCP3 IgG/IgA (> 20 j./ml), a średnie stężenie wyniosło
1,91 j./ml. W badanej grupie uzyskano natomiast dodatnie wartości
RF (> 6 j./ml) w różnych klasach. W klasie IgM wynik powyżej
6 j./ml miało 8 pacjentów; średnie stężenie wynosiło 20,97 j./ml.
Dla RF IgA średnie stężenie wynosiło 22,85 j./ml, a wartości dodatnie
(> 6 j./ml) obserwowano u 6 pacjentów. Dla RF w klasie IgG,
obecnego u 4 pacjentów z badanej grupy, średnie stężenie wyniosło
6,48 j./ml. U 6 dzieci, u których wykonano badanie USG zajętych
stawów, nadżerki wykazano u 2 w stawach rąk. Badanie MR
rąk wykonano w 4 najbardziej wątpliwych diagnostycznie przypadkach,
a zmiany o typie nadżerek wykazano tylko u 1 dziecka.
Wnioski: 1. Przeciwciała antycytrulinowe III generacji (IgG/IgA) -
o wyższej swoistości i czułości niż przeciwciała II generacji (IgG) -
nie znajdują zastosowania w algorytmie diagnostycznym MIZS.
2. W wątpliwych przypadkach MIZS wykonanie badania MR lub
USG stawów może potwierdzić obecność objawów niemożliwych
do wykrycia w klasycznym badaniu radiologicznym.Objective: The purpose of the study was to assess the utility of
aCCP3 IgG and aCCP IgG/IgA (third generation anti-cyclic citrullinated
peptide antibodies) and rheumatoid factors isotypes (IgG, IgA,
IgM) in the diagnosis of undifferentiated juvenile idiopathic arthritis.
Material and methods: The following inclusion criteria were used:
age < 18 years, disease onset < 16 years, inflammation of at least
one joint, disease/arthritis duration > 6 weeks, other diseases
have been excluded during differential diagnosis. Rheumatoid factors
isotypes IgM, IgG, IgA and aCCP3 IgG and aCCP IgG/IgA have
been evaluated in sera of 11 patients. Ultrasound imaging of
affected joints has been done in 6 cases. MRI was carried out only
in 4 most uncertain cases of patients.
Results: There were no positive results of aCCP2 IgG (> 20 IU/ml) in
examined group; the mean blood level was 4.85 j/ml. There were also no positive results of aCCP3 IgG/IgA (> 20 IU/ml) in tested
cohort; the mean blood level was 1.91 IU/ml. Elevated levels of RF
isotypes (> 6 IU/ml) have been found in studied group. Increased
level (> 6 IU/ml) of IgM RF was found in 8 patients, mean level was
20.97 IU/ml; IgA RF mean level was 22.85 IU/ml, and positive
results (> 6 IU/ml) were found in 6 patients; RF IgG isotype was
present in 4 patients from tested group, and mean concentration
amounted to 6.48 IU/ml. In 6 cases ultrasound of affected hands'
joints was performed; erosions have been demonstrated in 2 children.
MR examination was carried out in 4 most uncertain cases
and erosions have been demonstrated only in 1 child.
Conclusions: 1. Third generation anti-cyclic citrullinated peptide
antibodies (aCCP3 IgG/IgA) assessment – with higher specificity
and sensitivity than second generation antibodies (aCCP IgG) -
has no utility in JIA diagnostic algorithm. 2. MRI and ultrasound of
affected joints are helpful in the diagnosis of undifferentiated JIA
due to ability to confirm signs impossible to demonstrate in standard
X-ray imaging

Differential associations of inflammatory and endothelial biomarkers with disease activity in rheumatoid arthritis of short duration
Objectives. To estimate endothelial dysfunction in patients with rheumatoid arthritis (RA) of short duration in relation to disease activity based on the assessment of 28 joints (DAS28). Methods. We studied 29 patients (22 women, mean age 41 (SD, 9) years) with RA of short duration and 29 healthy controls. The RA subjects were divided into those with low (DAS28: 2.6–5.1, n=18) or high (DAS28>5.1, n=11) disease activity. Exclusion criteria included clinically overt atherosclerosis and other coexistent diseases. Biochemical markers of inflammatory activation and endothelial dysfunction were measured. Results. There were no significant intergroup differences in the majority of classical cardiovascular risk factors. High-sensitivity C-reactive protein, tumor necrosis factor-α, and interleukin-6 were increased in RA subjects. Compared to the controls, levels of soluble vascular cell adhesion molecule-1, von Willebrand factor, and pentraxin-3 were significantly elevated in RA subjects with low disease activity, exhibiting no further significant rises in those with high disease activity. Asymmetric dimethyl-L-arginine, soluble E-selectin, monocyte chemotactic protein-1, and osteoprotegerin were increased only in RA patients with high disease activity. Conclusions. Our findings might suggest a dissociation of pathways governing generalized and joint-specific inflammatory reactions from those involved in endothelial activation and inflammation within the vascular wall
