    On the perils of categorizing responses

    The assumptions underlying the categorization of numeric measurements are examined and it is concluded that some numeric data that are measured by categories might better be obtained by direct estimates. Statistical tests are performed on artificially generated data of normal, triangular and empirically measured distributions, and on various categorizations of these data. It is shown that categorization can markedly affect the outcome of significance tests, in some cases leading to both Type I and Type II errors. When high local densities of values are numerically separated by categorization, test statistics can be substantially inflated from the uncategorized values. It is recommended that response categorization be subjected to the same critical analysis as data transformation techniques like arbitrary dichotomization

    IT-assisted comprehensive geriatric assessment for residents in care homes: Quasiexperimental longitudinal study

    Background: Frailty interventions such as Comprehensive Geriatric Assessment (CGA) can provide significant benefits for older adults living with frailty. However, incorporating such proactive interventions into primary care remains a challenge. We developed an IT-assisted CGA (i-CGA) process, which includes advance care planning (ACP). We assessed if, in older care home residents, particularly those with severe frailty, i-CGA could improve access to advance care planning discussions and reduce unplanned hospitalisations. Method: As a quality improvement project we progressively incorporated our i-CGA process into routine primary care for older care home residents, and used a quasi-experimental approach to assess its interim impact. Residents were assessed for frailty by General Practitioners. Proactive i-CGAs were completed, including consideration of traditional CGA domains, deprescribing and ACP discussions. Interim analysis was conducted at 1 year: documented completion, preferences and adherence to ACPs, unplanned hospital admissions, and mortality rates were compared for i-CGA and control (usual care) groups, 1-year post-i-CGA or post-frailty diagnosis respectively. Documented ACP preferences and place of death were compared using the Chi-Square Test. Unplanned hospital admissions and bed days were analysed using the Mann-Whitney U test. Survival was estimated using Kaplan-Meier survival curves. Results: At one year, the i-CGA group comprised 196 residents (severe frailty 111, 57%); the control group 100 (severe frailty 56, 56%). ACP was documented in 100% of the i-CGA group, vs. 72% of control group, p < 0.0001. 85% (94/111) of severely frail i-CGA residents preferred not to be hospitalised if they became acutely unwell. For those with severe frailty, mean unplanned admissions in the control (usual care) group increased from 0.87 (95% confidence interval ± 0.25) per person year alive to 2.05 ± 1.37, while in the i-CGA group they fell from 0.86 ± 0.24 to 0.68 ± 0.37, p = 0.22. Preferred place of death was largely adhered to in both groups, where documented. Of those with severe frailty, 55% (62/111) of the i-CGA group died, vs. 77% (43/56) of the control group, p = 0.0013. Conclusions: Proactive, community-based i-CGA can improve documentation of care home residents’ ACP preferences, and may reduce unplanned hospital admissions.     Palatability, familiarity, and underage, immoderate drinking

    Data gathered in a study of palatability (“liking”) and familiarity ratings of alcoholic and nonalcoholic beverages by 350 subjects from 12 to 30 years of age included the usual number of drinks consumed. Blind ratings of palatability and familiarity for the beverages were tested for association with immoderate drinking (more than four for males, two for females). Palatability ratings were combined in a principal components analysis to determine whether any component scores were related to such drinking. Liking for the spirits used in the ready-to-drink beverages was related to immoderate drinking in participants under 18 years of age, while liking for the ready-to-drink beverages themselves was not. Liking for beer was related to immoderate drinking in males under 18. The principal components analysis suggested that males who drank immoderately preferred pungent (bitter or acrid) beverages, while females who drank immoderately did not. The associations of palatability ratings with immoderate drinking were substantial and consistent with familiarity ratings, but were found among the least palatable beverages. The results are related to previous studies of beverage preference in underage, heavy drinkers. It is concluded that relative palatability is not an overriding factor in the choice of beverages among underage drinkers who drink beyond the recommended limits.13 page(s

    Empathy as a function of clinical exposure--reading emotion in the eyes.

    BACKGROUND: Evidence based largely on self-report data suggests that factors associated with medical education erode the critical human quality of empathy. These reports have caused serious concern among medical educators and clinicians and have led to changes in medical curricula around the world. This study aims to provide a more objective index of possible changes in empathy across the spectrum of clinical exposure, by using a behavioural test of empathic accuracy in addition to self-report questionnaires. Moreover, non-medical groups were used to control for maturation effects. METHODS: Three medical groups (N = 3×20) representing a spectrum of clinical exposure, and two non-medical groups (N = 2×20) matched for age, sex and educational achievements completed self-report measures of empathy, and tests of empathic accuracy and interoceptive sensitivity. RESULTS: Between-group differences in reported empathy related to maturation rather than clinical training/exposure. Conversely, analyses of the "eyes" test results specifically identified clinical practice, but not medical education, as the key influence on performance. The data from the interoception task did not support a link between visceral feedback and empathic processes. CONCLUSIONS: Clinical practice, but not medical education, impacts on empathy development and seems instrumental in maintaining empathetic skills against the general trend of declining empathic accuracy with age
