    A New Era in Mental Health Care in Vanuatu

    Inequity in health-care delivery for those with mental illness is widespread throughout low- and middle-income countries. In the Pacific Island countries there are many barriers to addressing the growing mental health burden. In an effort to address this problem, the WHO is coordinating the Pacific Islands Mental Health Network involving 18 countries in the Pacific region with the financial support of New Zealand Aid (NZAid). JB and DP have developed and presented mental health training to health professionals, community leaders, and social service personnel in an environment in Vanuatu that is very different from that of their usual Australian-based general practices. They discuss evidence for their work, an outline of the programme, some difficulties working across different cultures, and the enthusiasm with which the training has been greeted. Vanuatu is now well on its way to addressing the inequity of access to mental health care with a culturally appropriate and self-sustaining mental health workforce

    Conservation of the Illinois flora: A climate change vulnerability assessment of 73 plant species

    It will be important for land managers, ecological researchers and policymakers to understand how predicted climate changes may affect the flora of Illinois. A climate change vulnerability assessment was completed in 2011 for the162 Animal Species in Greatest Need of Conservation using NatureServeā€™s Climate Change Vulnerability Index (CCVI) tool. Here we selected 73plant species found in Illinois and calculated their relative vulnerabilities to predicted climate changes, also using the NatureServe CCVI tool. We selected species from several groups that we felt would be broadly representative of the Illinois flora. These groups included: rare plants, invasive plants, important prairie species, important woodland/savanna species, important forest species, and plants important to society. We compiled and entered data regarding both the speciesā€™ exposures to predicted climate changes and their sensitivities to those changes. Exposures were determined by overlapping species range maps for Illinois with maps of temperature and moisture (AET:PET) predictions for the middle of this century. Species sensitivities were determined by interviewing between 4 and 12 experts for each plant species. Experts answered questions found in the CCVI tool regarding the speciesā€™ biologies, ecologies and behaviors. Results for each individual survey were averaged for each species. Results fell into one of five vulnerability categories: Extremely Vulnerable, Highly Vulnerable, Moderately Vulnerable, Not Vulnerable/Presumed Stable, and Not Vulnerable/Increase Likely. Results for these 73species in Illinois fell into all 5 vulnerability categories, with the majority (67%) falling into the Presumed Stable category. The species most vulnerable to predicted climate changes were all of conservation concern; most were federal or state listed species. Native species tended to be more vulnerable than non-natives, and plants important to prairies, savannas and forests were equally vulnerable to predicted changes. The four species were ranked as likely to increase in population size or range extent due to predicted climate changes were: Ailanthus altissima(tree of heaven), Ambrosia artemisiifolia(ragweed), Microstegium vimineum(Japanese stiltgrass) and Toxicodendron radicans(poison ivy). We advocate for 8 important next-steps to ensure adequate conservation of Illinois plants in a future with climate change, based on the findings of this report: 1) Investigate the climate change vulnerabilities of all rare plants in Illinois. Rare plants are the most vulnerable group of species in this report. 2) Prioritize research on plantsā€™ abilities to phenologically track changes in seasonality, population genetics, species interactions, dispersal distances, thermal and hydrological tolerancesand soil preferences/tolerances. 3) Continue to monitor population trends. Increase capacity to monitor species with the most uncertain responses to climate changes, the most vulnerable species, and all rare species. 4) Monitor invasive speciesfor changes in populations and behavior. Assess all invasive species using NatureServeā€™s CCVI tool or another tool. 5) Increase connectivity between natural areas. Increase acreage of natural areas. 6) Managers, policymakers, researchers and the public shouldwork together to fully consider the role that assisted migration should or should not play in Illinois plant conservation. 7) Compile work done by various agencies and NGOs on the climate change vulnerabilities of Illinois species and ecosystems to detect trends, and to identify appropriate research, management and policy priorities. 8) Use adaptive management approaches to care for natural areas in Illinois in order to best achieve land management goals in an uncertain future.Illinois Natural History SurveyIllinois Department of Natural ResourcesIllinois Department of Transportationunpublishednot peer reviewedOpe

    The Ile-84-->Ser amino acid substitution in transthyretin interferes with the interaction with plasma retinol-binding protein.

    In plasma the thyroid hormone-binding protein transthyretin (TTR) forms a tight complex with the specific retinol carrier retinol-binding protein (RBP). The Ile-84ā€“>Ser mutation and several other point mutations in TTR are associated with familial amyloidotic polyneuropathy, which is characterized by extracellular depositions of amyloid fibrils mainly consisting of mutated TTRs. The interactions with human RBP of recombinant human normal and Ser-84 TTRs were investigated by monitoring the fluorescence anisotropy of RBP-bound retinol. A nearly negligible affinity of the recombinant Ser-84 TTR for RBP was found. This result indicates the participation of a region on the outer surface of TTR that comprises Ile-84 in the recognition of RBP. In preliminary studies the Ser-84 TTR was the only one among several amyloidogenic variant TTRs to display negligible interaction with RBP. Therefore, in general a substantially altered binding of TTR to RBP is not associated with familial amyloidotic polyneuropathy. Instead, the altered binding of Ser-84 TTR to RBP appears to be responsible for an abnormal plasma transport of RBP. The recombinant normal TTR exhibits binding properties, in its interaction with human RBP, approximately similar to those of TTR purified from human plasma. Two independent and equivalent RBP binding sites on recombinant normal TTR are characterized by a dissociation constant of about 0.4 microM

    The Burlington Probation and Parole population confronts numerous social, economic, and healthcare challenges upon their return to the community. While health and healthcare issues of inmates have been studied extensively, the health status and medical issues of the reentry offenders, particularly in rural areas have not been previously assessed. Data about health risks, major medical issues, and lifestyle choices among offenders on parole in the rural setting may prove helpful in the identification of preventative measures and development of strategies to promote positive health behaviors among the target population. The aim of this study is to evaluate the health risks among offenders on parole in the Burlington area and guide recommendations towards improving their health outcomes through community and educational initiatives.     Long Term Therapy with Lenalidomide in a patient with POEMS Syndrome

    Lenalidomide is an effective therapy against malignant plasma cells and a potent agent against proinflammatory and proangiogenic cytokines. The use of lenalidomide in POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal protein with plasma cells, skin changes) has been reported, but its benefit in long-term use is not well established. A 55-year-old man with POEMS and debilitating polyneuropathy was treated with lenalidomide and dexamethasone followed by maintenance lenalidomide. He remains in haematologic remission and in complete recovery of functional status 3.5 years after diagnosis. This case supports the long-term use of lenalidomide in patients with POEMS syndrome
