    Association of Socio-demographic Attributes with Mothers Knowledge regarding Childhood Diarrhea

    Background: Childhood diarrheal diseases have a major impact on morbidity and mortality and these deaths are due to dehydration and mismanagement or delayed management of the disease. The mothers’ knowledge in management of diarrhea is likely related to its mortality and morbidity. The study aimed to determine the association between socio demographic attributes and mother’s knowledge on childhood diarrhea. Materials and Methods:In this cross sectional analytic study, 170 mothers who had at least one child aged below five years old were selected purposively from the out-patient department of ICDDR,B, Dhaka. Data were collected using a structured questionnaire by face to face interview. The level of knowledge was categorized as poor, average and good. Univariate and bivariate analysis were done with level of significance P<0.05. Results:The mean age of the respondents was 27 (SD=±5.6) years. Among them 46.5% were educated up to primary level and 47% had average monthly income between 5001 and 10000 taka. In the case of accessibility to mass media, 20% were found who never watch TV, 75.9% participants were found who never listen to radio, and 87.6% were found who never read newspaper. Despite the level of average knowledge was 59.5% but the proportion of the level of good knowledge was 17% among the respondents. Socio-demographic characteristics such as age, education and income (p<0.001) were significantly associated with mothers knowledge. Conclusion:The mothers had inadequate knowledge about diarrhea and their socio-demographic parameters are strongly associated with mother’s knowledge regarding diarrhea

    Predictors of Relapse in Idiopathic Nephrotic Syndrome in Children Presenting to a Tertiary Center

    Background and Aim: Idiopathic nephrotic syndrome (INS) is a disease of relapse and remission in children in which about 20-30% of the patients may experience a single episode in their lives while the rest suffer from relapse. Some factors contribute to relapse. The objective of this study was to explore the predictors of relapse in INS.Methods: A prospective longitudinal study was conducted in the Department of Pediatric Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU) from September 2015 to August 2016. One hundred and ten patients with the first episode of INS were uninterruptedly enrolled in this study. After screening and subsequent confirmatory diagnosis, the patients were treated with a single morning dose of oral prednisolone 60 mg/m2/day for 6 weeks followed by oral prednisolone 40 mg/m2 every alternate day for the subsequent 6 weeks. The total number of relapses within six months after the initial episode was collected in a data collection sheet. Every relapse was correlated with age of onset, sex, initial serum albumin level, fasting serum cholesterol level, and time to response in the initial attack. The association between relapse and its potential risk factors was investigated using logistic regression analysis. Results: The study found that INS was significantly associated with sex and age (p< 0.05). Male children were 3.04 times (95% CI= 1.09- 8.45, P=0.03) more likely to experience relapse. Furthermore, children with respiratory tract infection  had a higher chance of relapse (OR= 4.43, 95% CI= 1.26-15.53, P=0.02). Lower cholesterol levels (≤500 mg/dl) were found to be a protective factor (OR= 0.13, 95% CI= 0.04-0.54, P=0.005) in this study. The chance of relapse was higher in children who responded after 2-4 weeks of starting treatment compared to those who responded in less than one week (p<0.005).Conclusion: The study found that the first episode INS had a higher risk of relapse in boys with respiratory tract infection and in patients who needed more than two weeks of prednisolone treatment to achieve remission.Keywords: Children; Frequently Relapsing; Predictors; Idiopathic Nephrotic Syndrome

    How Do Telomere Abnormalities Regulate the Biology of Neuroblastoma?

    Telomere maintenance plays important roles in genome stability and cell proliferation. Tumor cells acquire replicative immortality by activating a telomere-maintenance mechanism (TMM), either telomerase, a reverse transcriptase, or the alternative lengthening of telomeres (ALT) mechanism. Recent advances in the genetic and molecular characterization of TMM revealed that telomerase activation and ALT define distinct neuroblastoma (NB) subgroups with adverse outcomes, and represent promising therapeutic targets in high-risk neuroblastoma (HRNB), an aggressive childhood solid tumor that accounts for 15% of all pediatric-cancer deaths. Patients with HRNB frequently present with widely metastatic disease, with tumors harboring recurrent genetic aberrations (MYCN amplification, TERT rearrangements, and ATRX mutations), which are mutually exclusive and capable of promoting TMM. This review provides recent insights into our understanding of TMM in NB tumors, and highlights emerging therapeutic strategies as potential treatments for telomerase- and ALT-positive tumors

    Exact solutions to the Benney–Luke equation and the Phi-4 equations by using modified simple equation method

    The modified simple equation (MSE) method is a competent and highly effective mathematical tool for extracting exact traveling wave solutions to nonlinear evolution equations (NLEEs) arising in science, engineering and mathematical physics. In this article, we implement the MSE method to find the exact solutions involving parameters to NLEEs via the Benney–Luke equation and the Phi-4 equations. The solitary wave solutions are derived from the exact traveling wave solutions when the parameters receive their special values

    Formulation and In-Vitro Evaluation of Orally Disintegrating Tablets (ODTs) of Tramadol Hydrochloride

    Since orally disintegrating tablets (ODTs) of tramadol hydrochloride are not available in the market, so an attempt has been taken to formulate and evaluate ODT preparation of tramadol hydrochloride. In this present work, direct compression was the technique used for preparing ODT using superdisintegrants like croscarmellose sodium, sodium starch glycolate and crospovidone at different concentrations. Prepared formulations were evaluated for various quality parameters- angle of repose, Carr’s index, Hausner ratio, weight variation, friability, hardness, drug content, dispersion time, wetting time and in-vitro dissolution. The angle of repose data indicated that the flow property of all the formulations was good to excellent. Comparing with the specifications, the results of Carr's index (%) and Hausner’s ratio indicated that the flowability of all the formulations blend was significantly good. Prepared formulations showed average wetting time ranging from 40-45 seconds, average dispersion time with 3-6 minutes. In-vitro dissolution profile indicated the cumulative % drug release between 30-80% for most of the cases.     Changes in Organics and Nitrogen during Ozonation of Anaerobic Digester Effluent

    The objective of this study is to investigate the consequence of ozone dosage rate on the qualitative change in organic compounds and nitrogen in anaerobic digester effluent during the ozone process. Therefore, ozonation improves the biodegradability of recalcitrant organic compounds, quickly oxidizes the unsaturated bond, and forms radicals that continue to deteriorate other organic matter. In this study, ozonation was performed in a microbubble column reactor; the use of microbubble ozone improves the status of chemical oxygen demand (COD) and changes of organic nitrogen to inorganic compounds. The ozone injection rates were 1.0, 3.2, and 6.2 mg/L/min. The samples obtained during the ozone treatments were monitored for CODMn, CODCr, TOC, NO2−-N, NO3−-N, NH4+-N, T-N, and Org-N. The ozone dose increased 1.0 to 6.2 mg/L and it increased the degradation ratio 40% and the total organic carbon 20% during 20 min of reaction time. During the ozonation, the CODCr and CODMn values were increased per unit of ozone consumption. The ozone treatment showed organic nitrogen mineralization and degradation of organic compounds with the contribution of the microbubble ozone oxidation process and is a good option for removing non-biodegradable organic compounds.     Feeding Practices and Care-Seeking Behaviours among Mothers of Under-Two Children with Diarrhoeal Diseases in Bangladesh

    In Bangladesh, the prevalence of diarrhoea is the highest among children aged 6-23 months and the outbreaks are common among slum-dwellers of Dhaka city, Bangladesh. A qualitative explorative research was undertaken among slum-dwellers to explore the feeding practices and care-seeking behaviours of mothers with children with diarrhoeal diseases aged less than two years. Data were collected through in-depth interviews, focus-group discussions and observation checklist with mothers and elder family members. Breastmilk was blamed for causing diarrhoea among predominantly or exclusively breastfed children. The blameworthiness leads to withholding of breastfeeding during diarrhoeal episodes especially with recurrent and persistent diarrhoea. Teething, eating protein diet by children and eating leafy vegetables by mothers were believed to be responsible for diarrhoea among the older children. Hand-washing before preparing foods and before feeding children was virtually not practised. Usual complementary foods were not offered to the children with diarrhoea. Almost all of the children were offered oral rehydration salt solution and other fluids at home. A common healthcare-seeking behaviour of the mothers was to give medicines from local drug stores. Some mothers performed some rituals for the purification of their breastmilk. As the mother’s diet was believed to be responsible for the child’s diarrhoea, some foods, especially leafy-vegetables, some types of fish, and meat, were restricted to the mothers. The study concludes that perceptions of mothers regarding the causes of children’s diarrhoea direct to inappropriate feeding practices and care-seeking behaviours of under-two children with diarrhoeal diseases
