Danas se laser koristi u brojnim podruÄjima, neka od kojih su preraÄivaÄka industrija, telekomunikacije, graÄevina i vojna industrija. Ubrzo nakon otkriÄa, laserska se tehnologija poÄela primjenjivati u medicini.
Laserske metode lijeÄenja hemoroidalne bolesti obuhvaÄaju lasersku hemoroidektomiju, lasersku hemoroidoplastiku te HeLP. Laserska hemoroidektomija najÄeÅ”Äe se izvodi pomoÄu CO2 lasera, a tehnika odgovara otvorenoj hemoroidektomiji. Diodnim se laserom izvodi hemoroidoplastika. Laser se primjenjuje submukozno kako bi se postigla destrukcija submukoznih krvnih žila i stvaranje fibroznog tkiva. HeLP je dearterijalizacijska metoda koja se temelji na okludiranju ogranaka a. rectalis superior laserskom fotokoagulacijom. U lijeÄenju perianalnih fistula koristi se laserska ablacija kanala fistule diodnim laserom. Radi se o tehnici koja poÅ”teÄuje analni sfinkter te deepitelizacijom i obliteracijom kanala fistule omoguÄuje cijeljenje kroniÄne fistule. Lasersko lijeÄenje analnih fisura ukljuÄuje primjenu CO2 lasera kojim se izvodi modificirani oblik parcijalne sfinkterotomije. TakoÄer se primjenjuje i lasersko skeniranje kojim se stimulira regeneracija i angiogeneza submukoze i kože.
U kontekstu dugoroÄnih rezultata i manje postoperativne boli, nema joÅ” dovoljno dokaza da laserska hemoroidektomija postiže bolje rezultate od klasiÄne. Laserska hemoroidoplastika pouzdana je metoda za lijeÄenje hemoroida II. i III. stupnja kojom se postižu rezultati usporedivi s klasiÄnim kirurÅ”kim metodama, a uz znaÄajno smanjenje postoperativne boli. HeLP je prikladna metoda za ograniÄen broj bolesnika s hemoroidima II. i III. stupnja bez prolapsa, meÄutim nedostaje istraživanja koji bi potvrdili prednost naspram povoljnijim tehnikama. Laserska ablacija fistule omoguÄuje cijeljenje perianalne fistule uz oÄuvanje sfinktera, nisku stopu komplikacija i inkontinencije. In terms of long-term results and reduced postoperative pain, there is still not enough sufficient evidence to suggest that laser hemorrhoidectomy achieves better outcomes than the conventional method. Laser hemorrhoidoplasty is a reliable method for treating second and third-degree hemorrhoids, with results comparable to traditional surgical approaches and a significant reduction in postoperative pain. HeLP is a suitable method for a limited number of patients with second and third-degree hemorrhoids without prolapse; however, further research is needed to confirm its advantages over other, more affordable techniques. Laser ablation of the fistula allows for the healing of perianal fistulas while preserving the sphincter, with a low rate of complications and incontinence. Laser treatment of anal fissures is currently not widely used, but the results of initial studies show satisfactory outcomes
