    The response of the growing pig to changes in energy intake achieved through changes in dietary energy concentration versus restriction of feed intake

    This study was undertaken to compare the effects of changing energy intake, either by restricting feed intake or by altering dietary energy concentration, on the growth performance of pigs. The relative abilities of the DE, ME and NE systems to predict pig growth performance were also evaluated. A total of 72 barrows, having initial body-weights of 30 Ā± 2 kg, were randomly assigned to one of nine dietary treatments. These treatments were organized in a 3 x 3 factorial arrangement consisting of three dietary energy concentrations (2.18, 2.29 and 2.40 Mcal NE/kg) and three levels of feed allowance (80, 90 and 100% of ad libitum). Actual feed allowances of 79, 91 and 100% of ad libitum were achieved. No interactive effects of feeding level and dietary energy concentration were observed (P > 0.10). Increases in feed allowance from 79 to 91 to 100% of ad libitum resulted in increases in daily intakes of feed (P 0.10) basis. The efficiency with which dietary energy was used for weight gain was unaffected by feeding level (P > 0.10). Increases in dietary energy concentration were accompanied by decreases in daily feed intakes (P = 0.0016); however, dietary energy concentration did not affect daily energy intakes (P > 0.10). Neither average daily gains nor feed conversion efficiencies were affected by changes in dietary energy concentration (P > 0.10). Additionally, there were no differences among energy concentration treatments in terms of the amount of dietary energy available for body-weight gain (P > 0.10) or the efficiency with which it was used (P > 0.10). No differences were observed between the DE, ME and NE systems in terms of their abilities to predict the growth performance of pigs. The present study demonstrates that the energy intake of pigs can be effectively manipulated via adjustments in feeding level and that changes in dietary energy density, over the range studied here, are unable to affect changes in energy intake. This finding indicates that extreme caution should be used when extrapolating data obtained from studies in which feed allowance was manipulated to scenarios (e.g. commercial practice) in which energy density is to be altered, and vice versa

    What makes continuing education effective : perspectives of community pharmacists

    Objective To explore how different aspects of the professional environment for Australian community pharmacists are perceived to be influencing the effectiveness of continuing education models in improving practice.Setting Australian community pharmacy.Methods A convenience sample of practising community pharmacists (n = 15) was recruited using the \u27snowballing\u27 technique to participate in one of four focus group teleconferences. Each focus group examined continuing education experiences from different professional perspectives and training needs (recent graduates, experienced practitioners, specialist practitioners and rural/remote practitioners).Key findings Facilitation of professional development by accreditation bodies, and new challenges resulting from the introduction of cognitive services were seen to promote a favourable environment for continuing education engagement. Complex continuing education delivery models combined with high costs and excessive workloads made it more difficult to engage with continuing education systems or try to apply knowledge to the workplace.Conclusion Results support findings from previous research that practice development requires a multifaceted approach with continuing education as just one component. Affordable and integrated models of continuing education are required in order to optimise efficacy for participants.<br /

    Pharmacists and prescribing rights: review of international developments

    Purpose. Continuity of care, equitable access, and quality and safety are major foci in health services management. The introduction of limited prescribing rights to pharmacists has the potential to reduce fragmentation within the health system, optimise medication management, improve continuity of patient care and improve patient access to medication. Results. Eight models for pharmacists' prescribing have been implemented internationally, varying in their dependency on protocols, formularies and collaboration with physicians. These have also been described using terms such as Supplementary Prescribing and Patient Group Directions. Conclusion. Issues relating to practical implementation of pharmacists' prescribing include negotiation of national health policy, pharmacists' training and accreditation, liability, reimbursement and documentation

    Breastfeeding, the use of docosahexaenoic acid-fortified formulas in infancy and neuropsychological function in childhood

    OBJECTIVE: To investigate the relation between breastfeeding, use of docosahexaenoic acid (DHA)-fortified formula and neuropsychological function in children. DESIGN: Prospective cohort study. SETTING: Southampton, UK. SUBJECTS: 241 children aged 4 years followed up from birth. MAIN OUTCOME MEASURES: IQ measured by the Wechsler Pre-School and Primary Scale of Intelligence (3rd edn), visual attention, visuomotor precision, sentence repetition and verbal fluency measured by the NEPSY, and visual form-constancy measured by the Test of Visual-Perceptual Skills (Non-Motor). RESULTS: In unadjusted analyses, children for whom breast milk or DHA-fortified formula was the main method of feeding throughout the first 6 months of life had higher mean full-scale and verbal IQ scores at age 4 years than those fed mainly unfortified formula. After adjustment for potential confounding factors, particularly maternal IQ and educational attainment, the differences in IQ between children in the breast milk and unfortified formula groups were severely attenuated, but children who were fed DHA-fortified formula had full-scale and verbal IQ scores that were respectively 5.62 (0.98 to 10.2) and 7.02 (1.56 to 12.4) points higher than children fed unfortified formula. However, estimated total intake of DHA in milk up to age 6 months was not associated with subsequent IQ or with score on any other test. CONCLUSIONS: Differences in children's intelligence according to type of milk fed in infancy may be due more to confounding by maternal or family characteristics than to the amount of long-chain polyunsaturated fatty acids they receive in milk

    Generation of reactive oxygen species in 1-methyl-4-phenylpyridinium (MPP+) treated dopaminergic neurons occurs as an NADPH oxidase-dependent two-wave cascade

    <p>Abstract</p> <p>Background</p> <p>Reactive oxygen species (ROS), superoxide and hydrogen peroxide (H<sub>2</sub>O<sub>2</sub>), are necessary for appropriate responses to immune challenges. In the brain, excess superoxide production predicts neuronal cell loss, suggesting that Parkinson's disease (PD) with its wholesale death of dopaminergic neurons in substantia nigra pars compacta (nigra) may be a case in point. Although microglial NADPH oxidase-produced superoxide contributes to dopaminergic neuron death in an MPTP mouse model of PD, this is secondary to an initial die off of such neurons, suggesting that the initial MPTP-induced death of neurons may be via activation of NADPH oxidase in neurons themselves, thus providing an early therapeutic target.</p> <p>Methods</p> <p>NADPH oxidase subunits were visualized in adult mouse nigra neurons and in N27 rat dopaminergic cells by immunofluorescence. NADPH oxidase subunits in N27 cell cultures were detected by immunoblots and RT-PCR. Superoxide was measured by flow cytometric detection of H<sub>2</sub>O<sub>2</sub>-induced carboxy-H<sub>2</sub>-DCFDA fluorescence. Cells were treated with MPP+ (MPTP metabolite) following siRNA silencing of the Nox2-stabilizing subunit p22<sup>phox</sup>, or simultaneously with NADPH oxidase pharmacological inhibitors or with losartan to antagonize angiotensin II type 1 receptor-induced NADPH oxidase activation.</p> <p>Results</p> <p>Nigral dopaminergic neurons <it>in situ</it> expressed three subunits necessary for NADPH oxidase activation, and these as well as several other NADPH oxidase subunits and their encoding mRNAs were detected in unstimulated N27 cells. Overnight MPP+ treatment of N27 cells induced Nox2 protein and superoxide generation, which was counteracted by NADPH oxidase inhibitors, by siRNA silencing of p22<sup>phox</sup>, or losartan. A two-wave ROS cascade was identified: 1) as a first wave, mitochondrial H<sub>2</sub>O<sub>2 </sub>production was first noted at three hours of MPP+ treatment; and 2) as a second wave, H<sub>2</sub>O<sub>2 </sub>levels were further increased by 24 hours. This second wave was eliminated by pharmacological inhibitors and a blocker of protein synthesis.</p> <p>Conclusions</p> <p>A two-wave cascade of ROS production is active in nigral dopaminergic neurons in response to neurotoxicity-induced superoxide. Our findings allow us to conclude that superoxide generated by NADPH oxidase present in nigral neurons contributes to the loss of such neurons in PD. Losartan suppression of nigral-cell superoxide production suggests that angiotensin receptor blockers have potential as PD preventatives.</p

    Management of childrenā€™s fever by parents and caregivers: practical measurement of functional health literacy

    Functional health literacy is founded on general and numerical literacy and practical skills and is required for the appropriate and effective management of health symptoms in children. This study aimed to assess the health literacy skills of parents and caregivers of preschool-aged children, using a progressive scenario describing a child with fever and presenting tasks relating to selection of a medicine and hypothetical dosing of their child. Participants (n Ā¼ 417) from 33 childcare- and health-related sites in Sydney, Brisbane, Melbourne and Auckland completed the study. Participantsā€™ responses were largely appropriate regarding actions in response to worsening symptoms, selection of an appropriate product (from a limited range), whereby 84.5% of responses were for a single-ingredient paracetamol product and use of the package directions to state the frequency of dosing (93.1% of frequencies appropriate for paracetamol and 66.7% for ibuprofen). However, in only 50.8% of cases was an appropriate weight-based dose calculated, and doses were not measured to within 10% of the stated dose in 16.7% of cases. Future studies should focus on skill development via educational campaigns for parents and caregivers

    Stable isotope signatures reveal small-scale spatial separation in populations of European sea bass

    Scientific information about European sea bass Dicentrarchus labrax stocks in the NE Atlantic is limited and a more accurate definition of the stock boundaries in the area is required to improve assessment and management advice. We investigated the connectivity and movement patterns of D. labrax in Wales (UK) using the stable isotope (Ī“13C and Ī“15N) composition of their scales. Analysis of Ī“13C and Ī“15N values in the last growing season was performed on 189 adult sea bass caught at 9 coastal feeding grounds. Fish >50 cm total length (TL) caught in estuaries had very low Ī“13C, which is characteristic of freshwater (organic/soil) input, indicating the primary use of estuaries as feeding areas. A random forest classification model was used to test for any differences in Ī“15N and Ī“13C values between north, mid and south Wales and whether it was possible to correctly assign a fish to the area where it was caught. This analysis was restricted to fish of a similar size (40-50 cm TL) caught in open coastal areas (n = 156). The classification model showed that about 75% of the fish could be correctly assigned to their collection region based on their isotope composition. The majority of the misclassifications of fish were of fish from north Wales classifying to mid Wales and vice versa, while the majority of fish from south Wales were correctly assigned (80%). Our findings suggest that 2 sub-populations of sea bass in Welsh waters use separate feeding grounds (south vs. mid/north Wales), and may need separate management
