Patophysiology of atherosclerosis based on research on apoE-knockout mice and their usefulness in checking new antiatherosclerotic agents
Obecnie wiadomo, że miażdżyca jest chorobą o podłożu zapalnym. Długo brakowało jednoznacznego
dowodu na istotny wpływ zapalenia na rozwój miażdżycy. Uzyskano go dzięki nowemu zwierzęcemu modelowi
miażdżycy - myszy, u której zastosowano technikę celowania genowego (apoE-knockout mice).
Kluczowym etapem rozwoju miażdżycy jest prezentacja limfocytom T antygenu przez komórki dendrytyczne.
Antygenem tym może być fragment "strawionej" przez makrofag, utlenionej lipoproteiny o małej dogęstości
(LDL), białko szoku cieplnego 60 (HSP60), β2 glikoproteina I lub fragmenty antygenów bakteryjnych.
W wyniku oddziaływania tych komórek następuje odpowiedź immunologiczna typu T helper 1
(komórkowa) lub typu T helper 2 (humoralna). Obecnie uważa się, że odpowiedź typu Th1 i jej mediatory:
interferon γ, czynnik martwicy nowotworów α, interleukina 1, interleukina 12 oraz interleukina 18
działają przyspieszająco na rozwój miażdżycy, podczas gdy odpowiedź typu Th2 i jej mediatory: interleukina
4, interleukina 5, interleukina 10 oraz interleukina 13 hamują rozwój miażdżycy.
Koncepcja miażdżycy jako zapalenia ukształtowała się dopiero w ostatnich latach, lecz obecnie jej wartość
jest niekwestionowana, co wiąże się z określonymi konsekwencjami terapeutycznymi. W ostatnich
latach oczywiste stało się więc, że szlak produktów 5-lipooksygenazy może odgrywać istotną rolę patogenezie
miażdżycy. Zaczęto rozważać leki przeciwleukotrienowe jako potencjalną terapię miażdżycy.
W niniejszej pracy omówiono wyniki badań własnych przeprowadzonych na eksperymentalnym modelu
miażdżycy: myszach z podwójnie wyłączonymi genami dla apolipoproteiny E oraz dla receptora LDL.
Wykazano, że inhibitory białka aktywującego 5-lipooksygenazę (FLAP) oraz bloker receptora dla leukotrienów
cysteinylowych zmniejszają miażdżycę u zmienionych genetycznie myszy.Although atherosclerosis was previously thought to be primarily a degenerative disease, it is now well ascertained
that its pathogenesis is inflammatory. This review describes the history of the new atherogenetic concept, including
the pivotal role of apoE-knockout mice in understanding the inflammatory background of atherosclerosis.
The pivotal stage of atherogenesis is antigen presentation by macrophages to T lymphocytes. This antigen
could be a fragment of oxidized LDL"digested" by macrophage, heat shock protein 60, β2 glycoprotein I,
or fragments of bacterial antigens. During interaction between these cells, an immunological response of
type T helper 1 (cellular) or T helper 2 (humoral) arises. Th1 response and its mediators: (IFN-γ, TNF-α,
interleukin 1, interleukin 12, and interleukin 18) increase atherogenesis, whereas Th2 response and its
mediators: (interleukin 4, interleukin 5, and interleukin 10) decrease the development of atherosclerosis.
The concept of atherosclerosis as an inflammatory disease is quite fresh; however, it is already considered
an undisputable achievement of science, bringing particular therapeutic consequences.
Since inflammation plays an important role in atherogenesis, during recent years it has become apparent
that the 5-lipoxygenase (5-LO) pathway may play an important role in modifying the pathogenesis of
atherosclerosis. These data raised the possibility that antileukotriene drugs may be an effective treatment
regimen in atherosclerosis. In fact, we have found that among apoE and LDLR-double knockout mice the
inhibition of FLAP, as well as cysteinyl leukotriene receptor blockade, was able to significantly prevent the
development of atherosclerosis in gene-targeted mice
Skin microcirculation in the diabetic type 2 patients
Wstęp. Zmiany w obrębie mikrokrążenia leżą u podstaw powikłań spotykanych
u osób z cukrzycą. Celem pracy była ocena mikrokrążenia skórnego u chorych na
cukrzycę typu 2 za pomocą dopplerowskiego przepływomierza laserowego (LDF,laser
doppler flowmeter).
Materiał i metody. Zbadano 60 osób (109 kończyn). Grupę chorych stanowiło
35 osób (65 przebadanych kończyn) w wieku 30–92 lat, ze średnią wieku 62,3 ±
12,6 lat, leczonych z powodu cukrzycy typu 2 (DM, diabetes mellitus) za pomocą
insuliny lub leków doustnych. Grupę kontrolną tworzyło 25 osób zdrowych w wieku
38–76 lat, ze średnią wieku 59,4 ± 10,5 lat (49 przebadanych kończyn). Z badań
wykluczono osoby przyjmujące leki wpływające na mikrokrążenie skórne oraz z
innymi chorobami naczyń kończyn dolnych. Grupę chorych podzielono na podstawie
czasu trwania cukrzycy na 3 grupy (mniej niż 5 lat, 5–10 lat, więcej niż 10
lat). Badania przeprowadzono przy użyciu dopplerowskiego przepływomierza laserowego
Periflux 4001 (Perirmed, Szwecja), umieszczając sondę na grzbietowej stronie
stopy badanej kończyny. Po przeprowadzeniu pomiaru przepływu spoczynkowego (RF,
rest flow), wykonywano kolejno testy prowokacyjne przepływu: test przekrwienia
reaktywnego (RH, reactive hyperemia test), odruchu żylno-tętniczkowego (VAR, veno-arteriolar responde),
test stymulacji termicznej (TSR, thermal stimulation test) w temperaturze 44°C.
Za uzyskane z pomiarów parametry przyznawano określoną liczbę punktów, których
suma stanowiła scoring. Osobnej analizie poddano grupy chorych podzielone na
podstawie czasu trwania cukrzycy. Analizę statystyczną przeprowadzono za pomocą
testu t-Studenta, natomiast zachodzące zależności analizowano, używając testu
korelacji liniowej.
Wyniki. Wielkość perfuzji skóry, mierzona w spoczynku u wszystkich chorych
na cukrzycę, była wyższa (16,9 PU) niż u osób zdrowych (14,9 PU). Nie stwierdzono
znamiennej różnicy w wartościach indeksu mikrokrążenia i RH. Obie grupy różniła
natomiast wartość VAR (średnio 33,6% u chorych na cukrzycę i 54,7% u osób zdrowych)
oraz TSR (średnio 491% u chorych na cukrzycę i 1463% u osób zdrowych). Obliczony
scoring w grupie chorych wyniósł średnio 7,5, a u zdrowych 1,0 (p < 0,001).
Badanie związku pomiędzy czasem trwania cukrzycy a scoringiem wykazało, że istnieje
dodatnia liniowa korelacja między tymi parametrami (r = 0,77).
Wnioski. Przeprowadzone badania wykazały przydatność LDF w ocenie stopnia
mikroangiopatii u chorych we wczesnym okresie choroby. Natomiast perfuzję skóry
u chorych na cukrzycę należałoby oceniać na podstawie kilku parametrów uzyskanych
z testów prowokujących zachowanie się mikrokrążenia.Introduction. Late compilcations of diabetes mellitus are caused by
microangiopathy.The issue of this study was to evaluate skin microcirculation
by means of doppler flowmeter in type 2 diabetic patients.
Material and methods. We examined 60 subjects (109 limbs). The diabetic
group was composed of 35 patients (65 limbs) suffering from type 2 diabetes
mellitus treated by insulin or oral therapy, aged 30 to 92 years. Mean age in
this group was 62.3 ± 12.6 years. Control group was composed of 25 subjects
(49 limbs), aged 38 to 76 years. Mean age was 59.4 ± 10.5 years. Subjects receiving
drugs affecting skin microcirculation or with symptoms of other vascular abnormalities
of lower limbs were excluded. The diabetic group was divided into 3 subgroups
according to duration of diabetes (below 5 years, 5–10 years, above 10 years).
Laser doppler flowmeter Periflux 4001 (Perimed, Sweden) was used in the study.
The probe of LDF was placed on dorsal aspect of foot. Tests affecting microcirculation
were performed after measurement of rest flow (RF) — it means reactive hyperemia
(RH), veno-arteriolar response (VAR) and thermal hyperemia test (TSR) in 44°C.
This parameters were evaluated in points, sum of it formed scoring. Diabetic
group was compared with normals. Previously separated subgroups of diabetics
were analysed additionaly. Statystical analysis was performed with aid of t-Student
and linear correlation tests.
Results. Skin perfusion at rest was higer in diabetic group than in normals
(16.9 vs.14.9 PU). No Significant differences were found in microangiopathy
index and reactive hyperemia test. Diabetics presented significantlydiminished
veno-arteriolar response (33.6% vs. 4.7%) and thermal hyperemia (491 vs. 1463%).
Scoring calculated for diabetics was 7.5 1.0 of normals (p < 0.001). Positive
linear orrelation was found betwween duration of diabetes and mentioned above
parameters (r = 0.77).
Conclusions. Usefulness of laser doppler flowmetry for evaluation of
diabetic microangiopathy in early stage of disease was proved. To improve reliability
of this method test stimulating microcirculation should be used

Wstęp. Modyfikacja stylu życia i ograniczenie czynników ryzyka rozwoju
miażdżycy poprawiają dystans marszu u chorych z chromaniem przestankowym. W leczeniu
tych dolegliwości stosuje się jednocześnie wiele preparatów farmakologicznych.
Postanowiono zbadać, czy skojarzone leczenie farmakologiczne będzie wpływało na
zwiększenie dystansu marszu u chorych z chromaniem przestankowym.
Materiał i metody. W prospektywnym, randomizowanym badaniu uczestniczyło
79 chorych chorych z przewlekłym niedokrwieniem kończyn dolnych (II stopień według
klasyfikacji Fontaine). Chorych leczono przez 12 tygodni skojarzoną terapią buflomedilem
(600 mg/d) i pentoksyfiliną (1200 mg/d), albo monoterapią za pomocą wyłącznie
pentoksyfiliny (1200 mg/d). U wszystkich pacjentów na początku i na końcu badania
przeprowadzono próbę na bieżni ruchomej (tempo marszu 3,2 km/h, nachylenie 12°),
mierząc dystans marszu wolny od bólu i całkowity dystans marszu.
Wyniki. Dystans marszu wolny od bólu wydłużył się istotnie statystycznie
w obu grupach badanych (74,4% - w grupie terapii skojarzonej oraz 27,3% w grupie
leczonej pentoksyfiliną). Natomiast całkowity dystans marszu istotnie statystycznie
wydłużył się tylko w grupie terapii skojarzonej (41,8%). Odsetek działań niepożądanych
był podobny w obu grupach badanych.
Wniosek. Skojarzone stosowanie pentoksyfiliny i buflomedilu wydaje się
efektywnym i bezpiecznym leczeniem u chorych z chromaniem przestankowym u chorych.Background. It has been known that modification of risk factors for atherosclerosis
improves the walking distance of patients with intermittent claudication. On the
other hand there are variety of different drugs widely used by physicians to meet
the patients expectations. We decided to study whether a combine medical therapy
has an impact on walking distance among the patients with intermittant claudication.
Material and methods. In a randomised, prospective study, 79 patients with
chronic arterial occlusive disease (Fontaine stage II) were treated either by
buflomedil (600 mg daily) and pentoxifylline (1200 mg daily) or by pentoksyfilline
(1200 mg daily) alone over 12 weeks. All patients underwent treadmill testing
at the beginning and at the end of the study (treadmill speed 3,2 km/h and of
12° incline). The pain-free and maximal walking distance were measured at both
time points.
Results. The pain-free distance was significantly improved in both groups
(74,4% - buflomedil and pentoxifylline group and 27,3% - pentoxifylline group)
whereas the total walking distances was significantly higher only in the group
of treated by buflomedil and pentoxifylline (41,8%). The percentage of side-effects
and adverse events were equal among the studied groups.
Conclusion. The combine treatment of buflomedil with pentoxifylline turned
out to be effective and safe for the patients with intermittent claudication
Differences in sex hormone levels in the menstrual cycle due to tobacco smoking — myth or reality?
Introduction: Tobacco smoke contains, among others, polycyclic aromatic hydrocarbons (PAHs), heterocyclic analogues, aromatic amines, N-nitrosamines, volatile hydrocarbons, aldehydes, phenols, miscellaneous organic compounds, metals, and inorganic compounds. Tobacco smoking can harm women’s reproductive system and may reduce fertility. The objective of the study was to explore the effect of tobacco smoke on the menstrual cycle due to smoking and second-hand smoke-exposure.
Material and methods: The study was performed on 153 women of reproductive age, who received care at the Gynaecological-Obstetric Clinical Hospital of the Poznan University of Medical Sciences. They were divided into three treatment groups: non-smokers, secondhand smokers, and smokers. Comprehensive assessment of all hormone levels: follicle-stimulating hormone (FSH), luteinizing hormone (LH), 17β-oestradiol (E2), and progesterone (P), in the various phases of the menstrual cycle and with concomitant determinations of serum cotinine concentrations was performed. The menstrual cycle was observed with ultrasonography.
Results: Cigarette smoking may be an important factor in disrupting reproduction: 1. The increase in the oestradiol E2 level was accompanied by significantly lowered serum cotinine concentrations in tobacco smokers; 2. In smoking patients, the serum level of LH significantly increased on the first days of the menstrual cycle; 3. The higher levels of P (in the 14th and 21st days) were assumed to be the result of a longer menstrual cycle.
Conclusions: Active and passive smoking may be an important contributor to reproductive health issues and deserves greater focus in health education programs directed towards women of reproductive age
The value of different forms of compression therapy in the treatment of iatrogenic femoral pseudoaneurysms
Wstęp. Tętniaki rzekome (PA) należą do najczęstszych powikłań towarzyszących przeprowadzaniu zabiegów
endowaskularnych. Podstawową formą terapii zachowawczej jest leczenie za pomocą ucisku, w postaci
opatrunku uciskowego, celowanego ucisku głowicą ultrasonograficzną lub ucisku ręcznego. Celem niniejszej
pracy było określenie skuteczności poszczególnych form leczenia uciskowego PA w pachwinie.
Materiał i metody. Ocenie poddano grupę 273 chorych, u których w następstwie przeprowadzonych zabiegów
endowskularnych powstały PA tętnic udowych. W zależności od sytuacji klinicznej oraz od ośrodka, w którym leczono chorych wdrożono jedną z trzech form terapii: opatrunek uciskowy u 142 chorych (grupa I),
celowany ucisk ręczny u 94 pacjentów (grupa II) oraz celowany ucisk głowicą ultrasonograficzną - u 23
badanych (grupa III).
Wyniki. Wykrzepnięcie PA obserwowano u 23,2% chorych z grupy I, u 75,5% pacjentów z grupy II oraz
u 8,7% z grupy III. Różnice skuteczności terapii występujące pomiędzy poszczególnymi grupami były znamienne
statystycznie.
Wnioski. Zgodnie z obserwacjami autorów niniejszej pracy najlepszą skuteczność w leczeniu PA odnotowano
po zastosowaniu celowanego ucisku ręcznego. Obok dużej skuteczności metoda ta charakteryzowała się
łatwością wykonania oraz ograniczoną częstością występowania zdarzeń niepożądanych. Ta forma leczenia
powinna być metodą pierwszego wyboru w leczeniu jatrogennych PA w pachwinie.Background. Pseudoaneurysms (PA) belong to the group of most frequent complications following percutaneous
endovascular procedures. The main stem of their conservative management is compression therapy with
compression dressing or ultrasound-guided compression either manually or with ultrasonographic transducer.
The purpose of this study was to assess the value of different forms of compression therapy in the treatment
of femoral pseudoaneurysms.
Material and methods. A group of 273 patients with femoral pseudoaneurysm following percutaneous
endovascular intervention was analyzed. Depending on the clinical situation and the centre where the patients
were treated, three different forms of therapy were administered: compression dressing in 142 patients
(group I), ultrasound-guided manual compression in 94 patients (group II), or ultrasound-guided transducer
compression in 23 patients (group III).
Results. Thrombosis of femoral pseudoaneurysms occurred in 23.2%, 75.5% and 8.7% of patients from
group I, II and III, respectively. The differences between the groups were statistically significant.
Conclusions. According to our observations, ultrasound guided manual compression was the best method of
treatment of femoral pseudoaneurysm. As well as showing high efficacy it was easy to perform and free from
complications. Ultrasound guided manual compression should be the method of choice in the treatment of
iatrogenic, femoral pseudoaneurysm
Does Diabetes Accelerate the Progression of Aortic Stenosis through Enhanced Inflammatory Response within Aortic valves?
Diabetes predisposes to aortic stenosis (AS). We aimed to investigate if diabetes affects the expression of selected coagulation proteins and inflammatory markers in AS valves. Twenty patients with severe AS and concomitant type 2 diabetes mellitus (DM) and 40 well-matched patients without DM scheduled for valve replacement were recruited. Valvular tissue factor (TF), TF pathway inhibitor (TFPI), prothrombin, C-reactive protein (CRP) expression were evaluated by immunostaining and TF, prothrombin, and CRP transcripts were analyzed by real-time PCR. DM patients had elevated plasma CRP (9.2 [0.74–51.9] mg/l vs. 4.7 [0.59–23.14] mg/l, p = 0.009) and TF (293.06 [192.32–386.12] pg/ml vs. 140 [104.17–177.76] pg/ml, p = 0.003) compared to non-DM patients. In DM group, TF−, TFPI−, and prothrombin expression within valves was not related to demographics, body mass index, and concomitant diseases, whereas increased expression related to DM was found for CRP on both protein (2.87 [0.5–9]% vs. 0.94 [0–4]%, p = 0.01) and transcript levels (1.3 ± 0.61 vs. 0.22 ± 0.43, p = 0.009). CRP-positive areas were positively correlated with mRNA TF (r = 0.84, p = 0.036). Diabetes mellitus is associated with enhanced inflammation within AS valves, measured by CRP expression, which may contribute to faster AS progression
Background level of 8-oxo-2'-deoxyguanosine in lymphocyte DNA does not correlate with the concentration of antioxidant vitamins in blood plasma.
Antioxidant vitamins, being effective free radical scavengers, can protect cellular DNA from oxidative damage. Therefore, in the present study we report on the relationship between basal level of 8-oxo-2'-deoxyguanosine in human lymphocyte DNA and the concentration of antioxidant vitamins (A, C and E). The average level of 8-oxo-2'-deoxyguanosine in lymphocytes of the studied group (15 males and 20 females) was 9.57 per 106 dG molecules. The endogenous level of ascorbic acid (vitamin C) in the plasma was, on average, 56.78 μM, while the mean concentrations of retinol (vitamin A) and α-tocopherol (vitamin E) were 1.24 μM and 25.74 μM, respectively. No correlations were found between individual 8-oxo-2'-deoxyguanosine levels in lymphocyte DNA and endogenous concentration of the vitamins
The GPR18 agonist PSB-KD-107 exerts endothelium-dependent vasorelaxant effects
GPR18 is an orphan GPCR that is activated by the cannabinoid tetrahydrocannabinol (THC). Emerging evidence indicates its involvement in the control of cardiovascular functions, including heart rate, contractility, vascular tone, as well as blood pressure. Therefore, we investigated the effects of selective GPR18 receptor ligands, namely PSB-KD-107 (agonist) and PSB-CB-92 (antagonist), on blood pressure, electrocardiogram (ECG), and vascular dilatation in vitro and in vivo, as well as their anti-oxidative potential in in vitro ferric reducing antioxidant power (FRAP) and 2,2-diphenyl-1-picryl-hydrazyl-hydrate free radical (DPPH) assays. Our results clearly show that PSB-KD-107 dilates blood vessels. This effect is related to its activation of GPR18 as it can be blocked by the GPR18 antagonist PSB-CB-92. Moreover, our finding confirms the presence of GPR18 in blood vessels. The mechanism of the vasorelaxant activity of PSB-KD-107 is mainly related to endothelial nitric oxide generation; however, we cannot exclude additional nitric oxide-independent mechanisms or a direct influence on K+ channels. PSB-KD-107 may affect blood pressure and heart function after a single administration; however, this effect was no longer observed after repeated administrations once daily for eight days. PSB-KD-107 does not affect platelet aggregation—an important feature considering the safety of its administration. PSB-KD-107 also shows a significant anti-oxidant effect and further studies of its antioxidant activity in vivo are justified
