Fiatalkori hallásrehabilitáció Baha® Attract implantátumrendszerrel | Pediatric hearing rehabilitation with the Baha ® Attract implant system
Absztrakt:
Bevezetés: A Baha® Attract egy új, implantálható
transcutan csontvezetéses hallásjavító rendszer, amely fiatalkorban is
előnyösebb lehet a klasszikus percutan eszközökkel szemben.
Célkitűzés: Az eszköz alkalmazási lehetőségeinek bemutatása
a fiatalkori hallásrehabilitációs műtéteknél. Módszer:
Klinikánkon ez idáig nyolc esetben végeztünk fiatalkorban implantációt (átlag
13,2 ± 3,2 év) az általunk módosított „posterosuperior” feltárásos technikával,
5 mm-es implantátummal. Az életkori sajátságok indokolták, hogy kiegészítő
tanulmányként 72 fő, 1–8 éves gyermek koponya-CT-felvételét elemezve megmérjük a
koponyacsont vastagságát az implantátum beültetésének ideális lokalizációjában.
Eredmények: Az implantációk átlag 30 perces műtéti idővel
történtek. Intra- és posztoperatív szövődményt nem észleltünk. A
beszédprocesszorokat a negyedik héten illesztettük. Audiológiai eredményekben
51,58 ± 11,22 SD dBHL hallásteljesítmény, valamint 43,3 ± 16,02 SD dB
beszédhallásküszöb-javulást értünk el. A koponyacsont-vastagság átlagosan 3,39 ±
1,05 SDmm-nek adódott. Következtetés: A Baha®
Attract új lehetőség a fiatalkori hallásrehabilitációban. Az implantációt
megelőzően javasolt a koponyáról CT-vizsgálatot végezni, amellyel megállapítható
a csont vastagsága és megtervezhető az implantátum beültetésének optimális

Abstract:
Introduction: Baha® Attract is a new transcutaneous
bone-conduction hearing aid, which is more preferable in childhood than the
conventional percutaneous systems. Aim: Our aim was to
demonstrate the possibilities of application in childhood.
Method: Eight children have undergone surgeries (mean age
of 13.2 ± 3.2 years; “posterosuperior” incision technique, 5 mm implants). The
thickness of the skull bone was determined in 72 children (1-8 years old) at the
recommended implant site, based on CT scans. Results: The
average duration of surgeries was 30 minutes. There were no intra- and
postoperative complications observed. Sound processors were fitted at the
postoperative 4th week. Hearing measurements proved 51.58±11.22SD dBHL gain in
warble tone thresholds, and 43.3 ± 16.02 SD dB in speech discrimination
thresholds. The skull bone thickness was measured as 3.39 ± 1.05 SD mm.
Conclusion: The Baha Attract system is a new tool for
hearing rehabilitation in pediatric population. Preoperative CT provides
valuable knowledge about skull bone thickness. Orv. Hetil., 2017,

A maradványhallás megőrzésének lehetőségei cochlearis implantáció során Nucleus CI532 Slim Modiolar elektródasorral = Possibilities for residual hearing preservation with Nucleus CI532 Slim Modiolar electrode array. Case report
Absztrakt:
A hallássérült betegek rehabilitációjában alkalmazott cochlearis implantáció
során a residualis hallás posztoperatív megőrzése és a hallási teljesítmény
maximalizálása az elektródaprofil implantációjakor kialakult trauma
minimalizálásától függ. Ennek megvalósításához minimálinvazív módszerek, továbbá
vékonyabb, atraumatikus elektródasorok alkalmazására volt szükség. Célunk a
posztoperatív akusztikai hallásmaradvány-megőrzés lehetőségének audiológiai
nyomon követése volt. Betegünk veleszületett halláscsökkenése miatt gyermekkora
óta hagyományos, légvezetéses hallásjavító készüléket viselt mindkét fülén. A
cochlearis implantációt 6 hónappal megelőzően halláscsökkenésében mindkét
oldalon kifejezett progressziót mértünk, ezért cochlearis implantátum beültetése
mellett döntöttünk. A beteg a műtétet megelőzően mindkét fülén rendelkezett
residualis hallással, ezért Cochlear® Nucleus CI532 Slim Modiolar implantátumot
alkalmaztunk. A minimálisan invazív műtétet a beteg jobb fülén végeztük el
kerekablak-behatoláson keresztül. A preoperatív hallásküszöbhöz (átlag 85 dBHL)
viszonyítva a 4. posztoperatív héten 0,25–1,0 kHz között 5–10 dBHL, míg 2,0–4,0
kHz-en 20–25 dBHL mértékű iniciális hallásküszöbromlást tapasztaltunk. A 6.
hónapban mért hallásküszöb az 1 kHz feletti tartományban további kisfokú
progressziót mutatott, ugyanakkor a 12. hónapban a hallásküszöb javult, a 4.
héten kapott eredményekkel megegyezett. A cochlearis implantáció residualis
hallásra gyakorolt hatásait több tanulmány is vizsgálta, melyekben számos
sebészi és technikai tényező kulcsszerepét meghatározták. A CI532 Slim Modiolar
eletródaprofil modiolushoz közeli elhelyezkedése várhatóan kisebb endocochlearis
hidrodinamikai terhelést jelent, mindemellett lehetővé teszi, hogy a ganglion
spirale szomszédos idegelemeit alacsonyabb áramintenzitással, kisebb felületen
ingerelhessük, ami neuroprotektív hatású lehet. Az akusztikai hallásmaradvány
cochlearis implantáció kapcsán történő megőrzése javítja a beteg beszédértését
és hanglokalizációs képességét, különösen nehezített körülmények között. A
residualis hallás hosszú távú megőrzése kiemelkedő fontosságúnak bizonyulhat
továbbá a későbbi regeneratív eljárások, gyógyszeres kezelések

Abstract:
During the rehabilitation of hearing-impaired patients, the preservation of
residual acoustic hearing following cochlear implantation by minimizing the
implantation trauma allows for improved hearing performance. To achieve this,
minimally invasive, soft surgery methods and thinner, atraumatic electrodes were
required. In our present study, we reported a case where Cochlear® Nucleus CI532
Slim Modiolar electrode was implanted in a patient with residual hearing. Our
aim was to study the possible preservation of postoperative acoustic residual
hearing by audiological monitoring. Since childhood, due to her congenital
hearing loss, she has been wearing a conventional, airborne hearing correction
device on both ears. Six months before cochlear implantation, we measured the
progression on both sides of the hearing loss, so we decided to perform cochlear
implantation. The patient had residual hearing on both ears prior to surgery
thus the Cochlear® Nucleus CI532 Slim Modiolar Implant was used. The minimally
invasive surgery was performed on the patient’s right ear through the round
window approach. Compared to the preoperative hearing threshold (average 85
dBHL) in the 4th postoperative week, an initial hearing threshold progression of
20–25 dBHL was observed between 0.25 and 1.0 kHz, while of 5–10 dBHL between
2.0–4.0 kHz. Hearing threshold measured in the 6th month showed a slight
progression in the range above 1 kHz, but improved by the 12th month, to the
results achieved at the 4th week. The effects of cochlear implantation on
residual hearing have been studied in numerous studies, in which several key
surgical and technical factors have been identified. Nucleus CI532 is a Slim
Modiolar electrode profile that is close to the modiolus, so it is expected to
have a lower endocochlear hydrodynamic load since it lies in the covering of the
osseus spiral lamina, thus less influencing the dynamics of the basilar
membrane. However, the perimodiolar location of the electrode array allows the
adjacent nerve elements of the spiral ganglion to be stimulated with a lower
electrical intensity and a reduced surface that may be neuroprotective.
Preservation of acoustic residual hearing following cochlear implantation
improves the patient’s speech perception and the sound localization skills,
particularly in difficult circumstances. Long-term residual hearing preservation
may also be of great importance in the subsequent feasibility for regenerative

Effects of 6-epi-ophiobolin A on mechanical and physiological parameters of cardiomyocytes and on the reorganization and amounts of their kv4.x ion channels
Ophiobolins are important members of the family of phytotoxic metabolites, and they possess antitumor, antibacterial, antifungal activities. At the same time, the knowledge on the effect of ophiobolins on ion channels is very limited. The voltage-dependent transient outward currents (I to) are determined by kv4.2 and kv4.3 ion channels. Increasing evidence points to a role of HMGB1 and synapse-associated protein 97 (Sap97) as underlying factors related to inflammation. The Sap97 associates with kv4.x type channels in the complex and modulating their kinetic properties. Our hypothesis was that Sap97 mainly localizes in the intercalated discs of the cardiac muscle, therefore the altered expression of ion channels is presumably involved in the inhibition of normal mechanical parameters and physiological function of transient outward current (I to). Our approach was to use purified 6-epi-ophiobolin A as effective molecular tools in the investigation of ion channels on cardiomyocytes. In this work, we have investigated the mechanical factors by atomic force microscope (AFM) technique and the surface expression of kv4 ion channels on the cardiomyocytes using confocal microscopy after immunofluorescence labeling. AFM study revealed that the elasticity of the cell surface, the Young’s modulus had been moderately changed, as well as the cell volume and the heights of the cells in the presence of low concentration of 6-epi-ophiobolin A. After this treatment the Sap97 binding to kv4.3 or kv4.2 channels and distribution of their complexes are also changed in the membrane accompanied by altered physiological behavior of cardiomyocytes as compared to control. A growing number of researches using these new reductionist models of inflammation on cardiomyocytes, demonstrate a role of Sap97 in specific ion channel stability important for cardiac functions. These results suggest that Sap97 deactivation or reduction can lead (directly or indirectly) to changes in the functional cell surface expression of kv4.x channels with mechanical parameters, with biophysical and biochemical properties of cardiac I to current
Laser-Doppler microvascular measurements in the peri-implant areas of different osseointegrated bone conductor implant systems
The objective of this study was to evaluate the impact of hydroxyapatite coating of newly designed osseointegrated fixtures' abutments on the postoperative complication rates. The integrity of peri-implant microcirculation was used as a marker to compare tissue viability after different surgical techniques. Laser-Doppler Flowmetry (LDF) measures alone, and coupled with heat provocation tests were applied to test the different microcircular patterns. Measures for 17 consecutively implanted patients (8 women, 9 men, ages ranged from 18 to 77 years) were recruited; seven with soft tissue reduction (STR); and 10 with soft tissue preservation (STP).Thirteen non-operated retro-auricular areas were examined as naive controls. In isotherm conditions the baseline blood flow remained stable in all groups. The naive control patients demonstrated significant changes of blood flux in the intact skin. The non-implanted yet previously operated contralateral sides of the patients demonstrated marginally lower (p = 0.09) blood flux index. The STR sides however, showed significantly lower (average 217 %) provoked blood flux compared to controls (p < 0.001). At the STP sides a maladaptation could be observed (average 316 %) compared to the contralateral sides (p = 0.53). STP sides demonstrated a significantly better blood flow improvement compared to the STR sides (p = 0.02). These results suggest a favorable postoperative condition of vascular microcirculation after STP, than after STR surgery. The possibly faster wound healing and lower potential complication rate may widen the inclusion criteria and maybe beneficial for the patient compliance with a better quality-of-life
