The perspectives of healthcare professionals and managers on patient involvement in care pathway development:A discourse analysis
BackgroundThe WHO advocates patient and public involvement as an ethical imperative, due to the value of the lived experience of patients. A deeper understanding of the shared meanings and underlying beliefs of healthcare professionals and managers for and against including patients in care pathway development.ObjectiveTo explore the considerations of healthcare professionals and managers on the involvement of patients and public in care pathway development.MethodsIn a medical rehabilitation centre we conducted a single case study that was part of a 2-year action research programme on blended care pathway development. Following 14 semistructured interviews with healthcare professionals and managers, we analysed their discourses on the value of patient involvement as well as the potential threats and opportunities.ResultsWe identified four discourses. Patient as expert frames involvement as relevant, as adding new perspectives and as required to fully understand the patient's needs. Skills and representation is based on the construct that obtaining valuable insights from patients requires certain skills and competences. Self-protection focusses on personal, interprofessional objections to patient involvement. Professional knows best reveals expertise-related reasons for avoiding or postponing involvement.ConclusionThese discourses explain why patient and public involvement in care pathway development is sometimes postponed, limited in scope and level of participation, and/or avoided. The following strategies might minimise the paralysing effect of these discourses: strengthen the capabilities of all stakeholders involved; use a mix of complementary techniques to gain involvement in distinct phases of care pathway development; and create/facilitate a safe environment. Put together, these strategies would foster ongoing, reciprocal learning that could enhance patient involvement.Patient or Public ContributionThis study belonged to an action research programme on blended care pathway development (developing an integrated, coordinated patient care plan that combines remote, digital telehealth applications, self-management tools and face-to-face care). Multidisciplinary teams took a quality collaborative approach to quality improvement (considering patients as stakeholders) to develop 11 blended care pathways. Although professionals and managers were instructed to invite patients onto their teams and to attend care pathway design workshops, few teams (3/11) actually did. Unravelling why this happened will help improve patient and public involvement in care pathway development

Adopting Patient Portals in Hospitals: Qualitative Study
BACKGROUND: Theoretical models help to explain or predict the adoption of electronic health (eHealth) technology and illustrate the complexity of the adoption process. These models provide insights into general factors that influence the use of eHealth technology. However, they do not give hospitals much actionable knowledge on how to facilitate the adoption process.
OBJECTIVE: Our study aims to provide insights into patient portal adoption processes among patients and hospital staff, including health care professionals (HCPs), managers, and administrative clerks. Studying the experiences and views of stakeholders answers the following question: How can hospitals encourage patients and HCPs to adopt a patient portal?
METHODS: We conducted 22 semistructured individual and group interviews (n=69) in 12 hospitals and four focus groups with members of national and seminational organizations and patient portal suppliers (n=53).
RESULTS: The effort hospitals put into adopting patient portals can be split into three themes. First, inform patients and HCPs about the portal. This communication strategy has four objectives: users should (1) know about the portal, (2) know how the portal works, (3) know that action on the portal is required, and (4) know where to find help with the portal. Second, embed the patient portal in the daily routine of HCPs and management. This involves three forms of support: (1) hospital policy, (2) management by monitoring the numbers, and (3) a structured implementation strategy that includes all staff of one department. Third, try to adjust the portal to meet patients' needs to optimize user-friendliness in two ways: (1) use patients' feedback and (2) focus on optimizing for patients with special needs (eg, low literacy and low digital skills).
CONCLUSIONS: Asking stakeholders what they have learned from their efforts to stimulate patient portal use in hospitals elicited rich insights into the adoption process. These insights are missing in the theoretical models. Therefore, our findings help to translate the relatively abstract factors one finds in theoretical models to the everyday pragmatics of eHealth projects in hospitals
Photon Energy-Dependent Ultrafast Exciton Transfer in Chlorosomes of Chlorobium tepidum and the Role of Supramolecular Dynamics
The antenna complex of green sulfur bacteria, the chlorosome, is one of the most efficient supramolecular systems for efficient long-range exciton transfer in nature. Femtosecond transient absorption experiments provide new insight into how vibrationally induced quantum overlap between exciton states supports highly efficient long-range exciton transfer in the chlorosome of Chlorobium tepidum. Our work shows that excitation energy is delocalized over the chlorosome in <1 ps at room temperature. The following exciton transfer to the baseplate occurs in ∼3 to 5 ps, in line with earlier work also performed at room temperature, but significantly faster than at the cryogenic temperatures used in previous studies. This difference can be attributed to the increased vibrational motion at room temperature. We observe a so far unknown impact of the excitation photon energy on the efficiency of this process. This dependency can be assigned to distinct optical domains due to structural disorder, combined with an exciton trapping channel competing with exciton transfer toward the baseplate. An oscillatory transient signal damped in <1 ps has the highest intensity in the case of the most efficient exciton transfer to the baseplate. These results agree well with an earlier computational finding of exciton transfer driven by low-frequency rotational motion of molecules in the chlorosome. Such an exciton transfer process belongs to the quantum coherent regime, for which the Förster theory for intermolecular exciton transfer does not apply. Our work hence strongly indicates that structural flexibility is important for efficient long-range exciton transfer in chlorosomes
A randomized controlled trial of three years growth hormone and gonadotropin-releasing hormone agonist treatment in children with idiopathic short stature and intrauterine growth retardation
We assessed the effectiveness and safety of 3 yr combined GH and GnRH
agonist (GnRHa) treatment in a randomized controlled study in children
with idiopathic short stature (ISS) or intrauterine growth retardation
(IUGR). Gonadal suppression, GH reserve, and adrenal development were
assessed by hormone measurements in both treated children and controls
during the study period. Thirty-six short children, 24 girls (16 ISS/8
IUGR) and 12 boys (8 ISS/4 IUGR), with a height SD score of -2 SD or less
in early puberty (girls, B2-3; boys, G2-3), were randomly assigned to
treatment (n = 18) with GH (genotropin 4 IU/m(2). day) and GnRHa
(triptorelin, 3.75 mg/28 days) or no treatment (n = 18). At the start of
the study mean (SD) age was 11.4 (0.56) or 12.2 (1.12) yr whereas bone age
was 10.7 (0.87) or 10.9 (0.63) yrs in girls and boys, respectively. During
3 yr of study height SD score for chronological age did not change in both
treated children and controls, whereas a decreased rate of bone maturation
after treatment was observed [mean (SD) 0.55 (0.21) 'yr'/yr vs. 1.15
(0.37) 'yr'/yr in controls, P < 0.001, girls and boys together]. Height SD
score for bone age and predicted adult height increased significantly
after 3 yr of treatment; compared with controls the predicted adult height
gain was 8.0 cm in girls and 10.4 cm in boys. Furthermore, the ratio
between sitting height/height SD score decreased significantly in treated
children, whereas body mass index was not influenced by treatment. Puberty
was effectively arrested in the treated children, as was confirmed by
physical examination and prepubertal testosterone and estradiol levels.
GH-dependent hormones including serum insulin-like growth factor I and II,
carboxy terminal propeptide of type I collagen, amino terminal propeptide
of type III collagen, alkaline phosphatase, and osteocalcin were not
different between treated children and controls during the study period.
Thus, a GH dose of 4 IU/m(2) seems adequate for stabilization of the GH
reserve and growth in these GnRHa-treated children. We conclude that 3 yr
treatment with GnRHa was effective in suppressing pubertal development and
skeletal maturation, whereas the addition of GH preserved growth velocity
during treatment. This resulted in a considerable gain in predicted adult
height, without demonstrable side effects. Final height results will
provide the definite answer on the effectiveness of this combined
treatment
