    Auricular Oedema and Dyshidrotic Eczema in a Patient with Acute Myeloid Leukaemia Treated with Cytarabine

    Cytarabine is an effective drug in the treatment of haematological malignancies. The therapy is associated with various complications. Frequencies of dermatological side-effects range from 2–72% and occur most commonly after high-dose regimens. Although most cutaneous reactions are mild and resolve spontaneously within several days, they may result in an increased risk of infection and alterations in comfort. In some cases, severe life-threatening reactions have been reported. Here we describe the case of a patient with acute myeloid leukaemia, who developed severe exceptional skin toxicity in terms of auricular oedema and palmar dyshidrotic eczema after the application of low-dose cytarabine. Re-administration of the drug resulted in reduced skin toxicity during further cycles of chemotherapy. Negative epicutaneous patch-testing supported the existence of cytarabine-provoked toxicity

    Culture and management control interdependence: An analysis of control choices that complement the delegation of authority in Western cultural regions

    This study examines the influence of cultural regions on the interdependence between delegation of authority and other management control (MC) practices. In particular, we assess whether one of the central contentions of agency theory, that incentive contracting and delegation are jointly determined, holds in different cultural regions. Drawing on prior literature, we hypothesise that the MC practices that operate as a complement to delegation vary depending on societal values and preferences, and that MC practices other than incentive contracting will complement delegation in firms in non-Anglo cultural regions. Using data collected from 584 strategic business units across three Western cultural regions (Anglo, Germanic, Nordic), our results show that the interdependence between delegation and incentive contracting is confined to Anglo firms. In the Nordic and Germanic regions, we find that strategic and action planning participation operate as a complement to delegation, while delegation is also complemented by manager selection in Nordic firms. Overall, our study demonstrates that cultural values and preferences significantly influence MC interdependence, and suggests that caution needs to be taken in making cross-cultural generalisations about the complementarity of MC practices

    The impact of adjuvant therapy on contralateral breast cancer risk and the prognostic significance of contralateral breast cancer: a population based study in the Netherlands

    Background The impact of age and adjuvant therapy on contralateral breast cancer (CBC) risk and prognostic significance of CBC were evaluated. Patients and Methods In 45,229 surgically treated stage I–IIIA patients diagnosed in the Netherlands between 1989 and 2002 CBC risk was quantified using standardised incidence ratios (SIRs), cumulative incidence and Cox regression analysis, adjusted for competing risks. Results Median follow-up was 5.8 years, in which 624 CBC occurred <6 months after the index cancer (synchronous) and 1,477 thereafter (metachronous). Older age and lobular histology were associated with increased synchronous CBC risk. Standardised incidence ratio (SIR) of CBC was 2.5 (95% confidence interval (95% CI) 2.4–2.7). The SIR of metachronous CBC decreased with index cancer age, from 11.4 (95% CI 8.6–14.8) when <35 to 1.5 (95% CI 1.4–1.7) for ≄60 years. The absolute excess risk of metachronous CBC was 26.8/10,000 person-years. The cumulative incidence increased with 0.4% per year, reaching 5.9% after 15 years. Adjuvant hormonal (Hazard rate ratio (HR) 0.58; 95% CI 0.48–0.69) and chemotherapy (HR 0.73; 95% CI 0.60–0.90) were associated with a markedly decreased CBC risk. A metachronous CBC worsened survival (HR 1.44; 95% CI 1.33–1.56). Conclusion Young breast cancer patients experience high synchronous and metachronous CBC risk. Adjuvant hormonal or chemotherapy considerably reduced the risk of CBC. CBC occurrence adversely affects prognosis, emphasizing the necessity of long-term surveillance directed at early CBC-detection

    Concept for the fast modulation of light in amplitude and phase using analog tilt-mirror arrays

    The full complex, spatial modulation of light at high frame rates is essential for a variety of applications. In particular, emerging techniques applied to scattering media, such as Digital Optical Phase Conjugation and Wavefront Shaping, request challenging performance parameters. They refer to imaging tasks inside biological media, whose characteristics concerning the transmission and reflection of scattered light may change over time within milliseconds. Thus, these methods call for frame rates in the kilohertz range. Existing solutions typically over frame rate capabilities below 100 Hz, since they rely on liquid crystal spatial light modulators (SLMs). We propose a diffractive MEMS optical system for this application range. It relies on an analog, tilt-type micro mirror array (MMA) based on an established SLM technology, where the standard application is grayscale amplitude control. The new MMA system design allows the phase manipulation at high-speed as well. The article studies properties of the appropriate optical setup by simulating the propagation of the light. Relevant test patterns and sensitivity parameters of the system will be analyzed. Our results illustrate the main opportunities of the concept with particular focus on the tilt mirror technology. They indicate a promising path to realize the complex light modulation at frame rates above 1 kHz and resolutions well beyond 10,000 complex pixels

    The industrial world is undergoing many changes and often all these modifications are referred to as “industry 4.0”, a term introduced in Germany in 2011. There are many doubts about what is going to change within and without a company, in particular as regards the Cyber-Physical Systems (CPS) concept, and its use in this new industrial framework. Starting from the main concepts of industry 4.0 in this paper the meaning of CPS inside the framework of industry 4.0 is analyzed by defining its main features and comparing it to the business model of the fourth industrial revolution, which is based on a service oriented architecture, SOA, with the aim of a flexible, modular and customized production process
