    Exploring to Bangkok Chaay Rak Chaay (Gay Men) Accessing Health Services in Thailand

    Health services are central to the provision of services for those affected by HIV including, prevention, health promotion and treatment. Access to health services for gay men is significant in increasing their knowledge associated with issues relevant to HIV so that they can better protect and care for themselves. Aim: This ethnographic study sought to clarify and further understand the experience of Bangkok’s gay men and their access to health services relevant to HIV issues. Methods: The data collection was conducted between May and August, 2014. 45 to 60 minutes Face to face interview and digital audio recording were utilised for the data collection processes. Semi structured in-depth interviews undertaken at the Thai Red Cross Research Centre, Bangkok. All interview transcripts were translated from Thai to English. The NVivo program version 10 was utilised to organise the data coding and theming. Result: This study identified that there are few specific clinics for gay men with HIV in Thailand. Additionally, information associated with HIV and other related concerns are required to be more accurate and specific to men living with HIV. Many chaay rak chaay face difficulties accessing services, including a lack of information, privacy concerns and inadequate provision of services. Conclusion: the specific clinic and information relevant HIV issues need to be more readily available and widely circulated. Moreover, health care services should be aware of the privacy issues and personal confidentially issues. Such approaches toward providing services for gay me in Bangkok may assist in eliminating some of the many barriers faced by gay men when accessing health services in the city

    An original reaction from art : an analysis of the criticism of A.G. Stephens on the Red page of the Bulletin, 1894-1906

    Most of the criticism written by A.G. Stephens is contained in his writings for The Bulletin between 1894 and 1906, and these writings appeared on what is usually referred to as "The Red Page". In fact this description is not quite accurate in that the front covers of the issues of The Bulletin for that period, on the verso of which "The Red Page" appeared, have faded to dull pink and it seems unlikely that they were ever nearer to red than bright pink at the time they were issued. Furthermore "The Red Page" was not so-called until 29 August 1896 although, under other titles, Stephens' work began appearing on the verso of the front cover of The Bulletin on 1 September 1894. In later years several variations of title were introduced, sometimes representing changes in the function of the page, at other times reflecting no more than the editor's whim. For the purposes of this study I have used the Red Page as a generic term to cover the whole period of Stephens' editorship, while attempting to indicate clearly the periods in which other titles were in use. The title, "The Red Page" was certainly the most enduring once Stephens began to use it in August 1896

    Upregulation of arylsulfatase B in carotid atherosclerosis is associated with symptoms of cerebral embolization

    The aim of this study was to identify genes for which the expression within carotid atherosclerosis was reproducibly associated with the symptoms of cerebral embolization. Two publically available microarray datasets E-MEXP-2257 and GSE21545 were analysed using GeneSpring 11.5. The two datasets utilized a total of 22 and 126 carotid atherosclerosis samples, obtained from patients with and without symptoms of cerebral embolization, respectively. To assess whether the findings were reproducible we analysed carotid atherosclerosis samples from another 8 patients with and 7 patients without symptoms of cerebral embolization using real-time PCR. In vitro studies using VSMC were performed to assess the functional relevance of one of the validated genes. We identified 1624 and 135 differentially expressed genes within carotid atherosclerosis samples of symptomatic compared to asymptomatic patients using the E-MEXP-2257 and GSE21545 datasets, respectively (≥1.15-absolute fold-change, P < 0.05). Only 7 differentially expressed genes or 0.4% (7/1,752) were consistent between the datasets. We validated the differential expression of ARSB which was upregulated 1.15-fold (P = 0.029) in atherosclerosis from symptomatic patients. In vitro incubation of VSMCs with the ARSB inhibitor L-ascorbic acid resulted in marked upregulation of SIRT1 and AMPK. This study suggests that ARSB may represent a novel target to limit carotid embolization

    Sharing success - understanding barriers and enablers to secondary prophylaxis delivery for rheumatic fever and rheumatic heart disease

    Background: Rheumatic fever (RF) and rheumatic heart disease (RHD) cause considerable morbidity and mortality amongst Australian Aboriginal and Torres Strait Islander populations. Secondary antibiotic prophylaxis in the form of 4-weekly benzathine penicillin injections is the mainstay of control programs. Evidence suggests, however, that delivery rates of such prophylaxis are poor. Methods: This qualitative study used semi-structured interviews with patients, parents/care givers and health professionals, to explore the enablers of and barriers to the uptake of secondary prophylaxis. Data from participant interviews (with 11 patients/carers and 11 health practitioners) conducted in four far north Queensland sites were analyzed using the method of constant comparative analysis. Results: Deficits in registration and recall systems and pain attributed to injections were identified as barriers to secondary prophylaxis uptake. There were also varying perceptions regarding responsibility for ensuring injection delivery. Enablers of secondary prophylaxis uptake included positive patient-healthcare provider relationships, supporting patient autonomy, education of patients, care givers and healthcare providers, and community-based service delivery. Conclusion: The study findings provide insights that may facilitate enhancement of secondary prophylaxis delivery systems and thereby improve uptake of secondary prophylaxis for RF/RHD

    Development of a High Throughput (HT) Raman Spectroscopy Method for Rapid Screening of Liquid Blood Plasma From Prostate Cancer Patients

    Extensive research has been undertaken on the examination of tissue biopsies using vibrational spectroscopic techniques. However, fewer studies have focused on less invasive and commonly acquired blood samples. Recent studies have shown the ability of Raman and Fourier transform infrared (FTIR) spectroscopy to discriminate between non-cancer controls and cancer cases using blood serum or plasma. Even though many studies have proposed Raman spectroscopy as a potential diagnostic tool in various cancers, the Raman spectroscopic technique has not been introduced as a routine clinical technology. This is due to multiple drawbacks with the application of the technique, including sample preparation, the requirement for expensive substrates and long acquisition times. The current study aims to overcome these limitations and focuses on the translation of Raman spectroscopy into a high throughput clinical diagnostic tool for prostate cancer. In this study, the effect of different instrumental and sample preparation parameters were investigated, with the aim of identifying a combination that would reduce the overall acquisition time for spectra from peripheral blood plasma, reduce the complexity of sample preparation and retain the classification accuracy from Raman spectroscopic diagnostics. A high throughput (HT) system was developed and Raman spectroscopic measurements were performed on plasma samples from 10 prostate cancer patients and 10 healthy volunteers. The spectra were pre-processed and classified by principal component analysis – linear discriminant analysis (PCA-LDA) in the R environment. Statistically significant differences were observed between Raman spectra of prostate cancer patients and non-cancer controls. The (HT) classification resulted in a sensitivity and specificity of 96.5% and 95% respectively. Overall, this study has overcome some of the limitations associated with clinical translation of Raman spectroscopy. The HT-Raman spectroscopy method developed in this study can be used for rapid and accurate diagnosis of prostate cancer using liquid plasma samples

    Role of habit in treatment adherence among adults with cystic fibrosis

    Among adults with cystic fibrosis (CF), medication adherence is low and reasons for low adherence are poorly understood. Our previous exploratory study showed that stronger 'habit' (ie, automatically experiencing an urge to use a nebuliser) was associated with higher nebuliser adherence. We performed a secondary analysis of pilot trial data (n=61) to replicate the earlier study and determine whether habit-adherence association exists in other cohorts of adults with CF. In this study, high adherers also reported stronger habit compared with low adherers. Habit may be a promising target for self-management interventions. ACtiF pilot, ISRCTN13076797.     The acceptability, feasibility, and preliminary efficacy of a supported online self-help treatment program for binge-eating disorder

    IntroductionStudies in transdiagnostic eating disorder (ED) samples suggest supported online self-help programs (eTherapies) are effective and may improve access to treatment; however, their evaluation in those with binge-eating disorder (BED) is limited. Given BED’s high prevalence and low levels of treatment uptake, further eTherapy evaluation is needed to broaden access to effective, evidence-based treatment options. The aim of this study was to investigate the acceptability, feasibility, and preliminary efficacy of a supported eTherapy for those with BED or subthreshold BED, and to examine symptom change across the duration of therapy.MethodNineteen women with BED completed a supported, 10-session Cognitive Behavioural Therapy-based eTherapy in an uncontrolled, pre-post, and 3 months follow up intervention study. Key outcomes were assessed by the Eating Disorder Examination Questionnaire (EDE-Q): objective binge episode (OBE) frequency and ED psychopathology. Feasibility was evaluated via program adherence and dropout, whilst acceptability was assessed through participant feedback post-treatment. Weekly symptom change (ED psychopathology) during treatment was assessed by the Eating Disorder Examination - Questionnaire Short (EDE-QS).ResultsGeneralised estimating equations showed statistically and clinically significant reductions in OBEs and ED psychopathology (large effects) post-treatment, with these decreases maintained at follow up. Across weekly assessment, a marked slowing in the rate of change in ED psychopathology was observed after four sessions of the program. Program feasibility was high (i.e., 84% of content completed), as was program acceptability (i.e., 93% of participants expressed high levels of satisfaction).DiscussionThese results support the acceptability, feasibility, and preliminary efficacy of a supported eTherapy program for those with BED and suggest the variability of symptom change across the duration of therapy. Future research should further investigate findings in an adequately powered randomised controlled trial
