    The minimal incubation period from the onset of Barrett's oesophagus to symptomatic adenocarcinoma

    Background:The interval between the onset of Barrett's oesophagus (BO) and oesophageal adenocarcinoma (OAC) can be termed the incubation period. However, the unrecorded onset of BO precludes its direct observation.Methods:Determining the range of intervals between BO diagnosis and OAC within the longest observational BO follow-up study. Exclusion criteria were presence of high-grade dysplasia (HGD) or OAC at baseline, death within <2 years of BO diagnosis, oesophagectomy without HGD/OAC and loss to follow-up. A total of 133 patients (M/F 73/60) were taken into account.Results:In 1967 person years of follow-up there were 13 cases of HGD/OAC, (0.66% p.a.; 95% CI 0.58-0.74), 96 patients died without HGD/OAC and 24 survived without HGD/OAC. The mean intervals between BO diagnosis and either HGD/OAC, death or end of follow-up were 10.8, 12.6 and 25.5 years, respectively, and the mean ages at endpoint were 72.5, 80.0 and 68.3 years, respectively. The survivors without HGD/OAC had a lower age at BO diagnosis (mean 42.8 vs 61.2 and 67.4 years, P=0.001). Baseline presence of low-grade dysplasia was associated with progression to HGD/OAC (log rank P=0.001).Conclusion:The Rotterdam BO follow-up cohort revealed a long incubation period between onset of BO and development of HGD/OAC, in patients without HGD/OAC at baseline as illustrated by 24 patients diagnosed with BO at a young age and followed for a mean period of 25.5 years. Their tumour-free survival established a minimum incubation period, suggesting a true incubation period of three decades or more

    Life-threatening hypokalaemia and quadriparesis in a patient with ureterosigmoidostomy

    We report quadriparesis as a result of severe hypokalaemia and acidosis in a 50-year-old man who had undergone ureterosigmoidostomy for bladder extrophy 48 years earlier. Aggressive suppletion with intravenous potassium and bicarbonate combined with potassium-sparing diuretics and ACE inhibitors resulted in complete restoration of the serum potassium and resolution of the neurological symptoms. The underlying mechanism as well as the treatment of hypokalaemia and hyperchloraemic metabolic acidosis after ureterosigmoidostomy are briefly discussed

    Hypertension is frequently present in patients with reflux esophagitis or Barrett's esophagus but not in those with non-ulcer dyspepsia

    Background: Elevated mortality due to cardiovascular disease has been reported for patients with Barrett's esophagus (BE). We compared the prevalence of risk factors for cardiovascular disease in patients with BE, reflux esophagitis (RE), and non-ulcer dyspepsia (NUD) with that of the general population. Methods: Patients with upper gastrointestinal complaints and BE, RE, or NUD were compared with a matched cohort from the general population using a questionnaire and blood pressure and cholesterol measurements. Results: Hypertension occurred more frequently in patients with BE (odds ratio 5.1, P<0.0001) and RE (odds ratio 3.8, P<0.001), but not in those with NUD. Serum total cholesterol was higher in BE (P=0.02) and borderline in RE (P=0.06) but not in NUD. Mean HDL cholesterol levels, body mass index, and smoking did not differ. Conclusions: This study suggests that BE and RE found at diagnostic endoscopy are associated with an increased prevalence of hypertension and a higher total cholesterol level than in the general population. If so, this would explain the increased mortality during the follow-up of BE patients, and it should be taken into account when designing or evaluating follow-up studies of BE

    Towards Automatic Modelling of Volleyball Players' Behavior for Analysis, Feedback and Hybrid Training

    Automatic tagging of video recordings of sports matches and training sessions can be helpful to coaches and players and provide access to structured data at a scale that would be unfeasible if one were to rely on manual tagging. Recognition of different actions forms an essential part of sports video tagging. In this paper, the authors employ machine learning techniques to automatically recognize specific types of volleyball actions (i.e., underhand serve, overhead pass, serve, forearm pass, one hand pass, smash, and block which are manually annotated) during matches and training sessions (uncontrolled, in the wild data) based on motion data captured by inertial measurement unit sensors strapped on the wrists of eight female volleyball players. Analysis of the results suggests that all sensors in the inertial measurement unit (i.e., magnetometer, accelerometer, barometer, and gyroscope) contribute unique information in the classification of volleyball actions types. The authors demonstrate that while the accelerometer feature set provides better results than other sensors, overall (i.e., gyroscope, magnetometer, and barometer) feature fusion of the accelerometer, magnetometer, and gyroscope provides the bests results (unweighted average recall = 67.87%, unweighted average precision = 68.68%, and κ = .727), well above the chance level of 14.28%. Interestingly, it is also demonstrated that the dominant hand (unweighted average recall = 61.45%, unweighted average precision = 65.41%, and κ = .652) provides better results than the nondominant (unweighted average recall = 45.56%, unweighted average precision = 55.45, and κ = .553) hand. Apart from machine learning models, this paper also discusses a modular architecture for a system to automatically supplement video recording by detecting events of interests in volleyball matches and training sessions and to provide tailored and interactive multimodal feedback by utilizing an HTML5/JavaScript application. A proof of concept prototype developed based on this architecture is also described

    Reconocimiento y distribución del nemátodo quiste de la papa (Glodobera pallida Stone) en Colombia.

    Se informa sobre los resultados obtenidos en los muestreos efectuados en los Departamentos de Cauca, Valle del Cauca, Caldas, Tolima, Cundinamarca, Santander y Santander del norte. Solamente en el Departamento del Cauca se encontró este nemàtodo parasitando raíces de papa en condiciones de campo aunque el nivel de infestación fué relativamente bajo. De cultivos de papa de más de 3 meses de edad se arrancaron cuidadosamente algunas plantas y se observaron las raíces con el fin de determinar la presencia o ausencia de quistes. Se tomaron entre dos y tres muestras de 200-250 cm3 de suelos agrícolas donde se cultiva papa. A partir de 1976 el total de muestras tomadas fue de 5.512. En los Departamentos de Boyacá y Cundinamarca el 2.9 por ciento de las muestras de suelo analizadas presentó quistes que se identificaron morfométricamente como pertenecientes al género Globodera, posiblemente G. pallida Stone. Sin embargo, ante la imposibilidad de observar la población parasítica, entre dichos quistes y las plantas de papa y de lograr multiplicarlos bajo condiciones controladas aun no se puede aseverar que dicho nemátodo este presente en los anteriores Departamentos. Situación semejante se encontró en los suelos de los Departamentos de Caldas y Tolima cuyas muestras presentaban quistes vacios o con huevos y larvas muertas. En los suelos de fincas cultivadas con papa de los Departamentos del Valle del Cauca y Santander no se encontraron quistes, posiblemente por tratarse de areas aisladas de este cultivoPapa-Solanum tuberosu

    Weekly low-dose mitoxantrone plus doxorubicin as second-line chemotherapy for advanced breast cancer

    Weekly low dose mitoxantrone (3 mg/m2) plus doxorubicin (8 mg/m2) was administered as second-line chemotherapy to 33 patients with advanced breast cancer. Four out of 28 evaluable patients (14%) obtained a partial response with a median duration of 34 weeks (range 18-67+ weeks), while 8 patients (29%) showed stable disease with a median duration of 28 weeks (range 11+-60 weeks). Gastrointestinal toxicity and alopecia were mild. Grade II and III leukopenia occurred in 63% of the courses without serious infectious disease. Four patients experienced an asymptomatic drop of 16-20% in the left ventricular ejection fraction (LVEF) after relatively low cumulative doses of each drug, and one patient with a history of pericarditis carcinomatosa and mediastinal irradiation developed a heart failure. In conclusion, this second-line combination treatment had moderate activity in breast cancer and caused only few subjective side effects, especially with respect to gastrointestinal symptoms
