    Patients with rheumatic conditions often suffer from related chronic pain. When first-line traditional medications such as acetaminophen and anti-inflammatory medications do not suffice, then other options are needed. The traditional medications may ultimately not provide sufficient pain relief, or alternatively, they can pose as a contraindication due to underlying hypertension, renal, and/or hepatic disease. Therefore, narcotics are an alluring alternative, which if used in a multidisciplinary and systematic approach to the patient, can prove to be quite beneficial in the lives of these patients

    The Sexual Impact of Infertility Among Women Seeking Fertility Care.

    IntroductionInfertility affects approximately 6.7 million women in the United States. Couples with infertility have significantly more anxiety, depression, and stress. This is compounded by the fact that almost 40% of couples undergoing assisted reproduction technology still cannot conceive, which can have an ongoing effect on quality of life, marital adjustment, and sexual impact.AimTo assess the sexual impact of infertility in women undergoing fertility treatment.MethodsThis study is a cross-sectional analysis of women in infertile couples seeking treatment at academic or private infertility clinics. Basic demographic information was collected. Respondents were surveyed regarding sexual impact and perception of their infertility etiology. Multivariate regression analyses were used to identify factors independently associated with increased sexual impact.Main outcome measureSexual impact of perceived fertility diagnosis.ResultsIn total, 809 women met the inclusion criteria, of whom 437 (54%) agreed to participate and 382 completed the sexual impact items. Most of the infertility was female factor only (58.8%), whereas 30.4% of infertility was a combination of male and female factors, 7.3% was male factor only, and 3.5% was unexplained infertility. In bivariate and multivariate analyses, women who perceived they had female factor only infertility reported greater sexual impact compared with woman with male factor infertility (P = .01). Respondents who were younger than 40 years experienced a significantly higher sexual impact than respondents older than 40 years (P < .01). When stratified by primary and secondary infertility, respondents with primary infertility overall reported higher sexual impact scores.ConclusionIn women seeking fertility treatment, younger age and female factor infertility were associated with increased sexual impact and thus these women are potentially at higher risk of sexual dysfunction. Providers should consider the role young age and an infertility diagnosis plays in a women's sexual well-being

    Investigating five key predictive text entry with combined distance and keystroke modelling

    This paper investigates text entry on mobile devices using only five-keys. Primarily to support text entry on smaller devices than mobile phones, this method can also be used to maximise screen space on mobile phones. Reported combined Fitt's law and keystroke modelling predicts similar performance with bigram prediction using a five-key keypad as is currently achieved on standard mobile phones using unigram prediction. User studies reported here show similar user performance on five-key pads as found elsewhere for novice nine-key pad users

    Dark Matter Annihilation Signatures from Electroweak Bremsstrahlung

    We examine observational signatures of dark matter annihilation in the Milky Way arising from electroweak bremsstrahlung contributions to the annihilation cross section. It has been known for some time that photon bremsstrahlung may significantly boost DM annihilation yields. Recently, we have shown that electroweak bremsstrahlung of W and Z gauge bosons can be the dominant annihilation channel in some popular models with helicity-suppressed 2 --> 2 annihilation. W/Z-bremsstrahlung is particularly interesting because the gauge bosons produced via annihilation subsequently decay to produce large correlated fluxes of electrons, positrons, neutrinos, hadrons (including antiprotons) and gamma rays, which are all of importance in indirect dark matter searches. Here we calculate the spectra of stable annihilation products produced via gamma/W/Z-bremsstrahlung. After modifying the fluxes to account for the propagation through the Galaxy, we set upper bounds on the annihilation cross section via a comparison with observational data. We show that stringent cosmic ray antiproton limits preclude a sizable dark matter contribution to observed cosmic ray positron fluxes in the class of models for which the bremsstrahlung processes dominate.Comment: 11 pages, 6 figures.     Review of hyperuricemia as new marker for metabolic syndrome

    Hyperuricemia has long been established as the major etiologic factor in gout. In recent years, a large body of evidence has accumulated that suggests that hyperuricemia may play a role in the development and pathogenesis of a number of metabolic, hemodynamic, and systemic pathologic diseases, including metabolic syndrome, hypertension, stroke, and atherosclerosis. A number of epidemiologic studies have linked hyperuricemia with each of these disorders. In some studies, therapies that lower uric acid may prevent or improve certain components of the metabolic syndrome. There is an association between uric acid and the development of systemic lupus erythematosus; the connection between other rheumatic diseases such as rheumatoid arthritis and osteoarthritis is less clear. The mechanism for the role of uric acid in disorders other than gout is not well established but recent investigations point towards systemic inflammation induced by urate, as the major pathophysiological event common to systemic diseases, including atherosclerosis

    Histone H2AX Is Phosphorylated at Sites of Retroviral DNA Integration but Is Dispensable for Postintegration Repair

    The histone variant H2AX is rapidly phosphorylated (denoted {gamma}H2AX) in large chromatin domains (foci) flanking double strand DNA (dsDNA) breaks that are produced by ionizing radiation or genotoxic agents and during V(D)J recombination. H2AX-deficient cells and mice demonstrate increased sensitivity to dsDNA break damage, indicating an active role for {gamma}H2AX in DNA repair; however, {gamma}H2AX formation is not required for V(D)J recombination. The latter finding has suggested a greater dependence on {gamma}H2AX for anchoring free broken ends versus ends that are held together during programmed breakage-joining reactions. Retroviral DNA integration produces a unique intermediate in which a dsDNA break in host DNA is held together by the intervening viral DNA, and such a reaction provides a useful model to distinguish {gamma}H2AX functions. We found that integration promotes transient formation of {gamma}H2AX at retroviral integration sites as detected by both immunocytological and chromatin immunoprecipitation methods. These results provide the first direct evidence for the association of newly integrated viral DNA with a protein species that is an established marker for the onset of a DNA damage response. We also show that H2AX is not required for repair of the retroviral integration intermediate as determined by stable transduction. These observations provide independent support for an anchoring model for the function of {gamma}H2AX in chromatin repair

    Summary of the First Workshop on Sustainable Software for Science: Practice and Experiences (WSSSPE1)

    Challenges related to development, deployment, and maintenance of reusable software for science are becoming a growing concern. Many scientists’ research increasingly depends on the quality and availability of software upon which their works are built. To highlight some of these issues and share experiences, the First Workshop on Sustainable Software for Science: Practice and Experiences (WSSSPE1) was held in November 2013 in conjunction with the SC13 Conference. The workshop featured keynote presentations and a large number (54) of solicited extended abstracts that were grouped into three themes and presented via panels. A set of collaborative notes of the presentations and discussion was taken during the workshop. Unique perspectives were captured about issues such as comprehensive documentation, development and deployment practices, software licenses and career paths for developers. Attribution systems that account for evidence of software contribution and impact were also discussed. These include mechanisms such as Digital Object Identifiers, publication of “software papers”, and the use of online systems, for example source code repositories like GitHub. This paper summarizes the issues and shared experiences that were discussed, including cross-cutting issues and use cases. It joins a nascent literature seeking to understand what drives software work in science, and how it is impacted by the reward systems of science. These incentives can determine the extent to which developers are motivated to build software for the long-term, for the use of others, and whether to work collaboratively or separately. It also explores community building, leadership, and dynamics in relation to successful scientific software

    The Indo-U.S. Library of Coude Feed Stellar Spectra

    We have obtained spectra for 1273 stars using the 0.9m Coud\'e Feed telescope at Kitt Peak National Observatory. This telescope feeds the coud\'e spectrograph of the 2.1m telescope. The spectra have been obtained with the #5 camera of the coud\'e spectrograph and a Loral 3K X 1K CCD. Two gratings have been used to provide spectral coverage from 3460 \AA to 9464 \AA, at a resolution of \sim1\AA FWHM and at an original dispersion of 0.44 \AA/pixel. For 885 stars we have complete spectra over the entire 3460 \AA to 9464 \AA wavelength region (neglecting small gaps of << 50 \AA), and partial spectral coverage for the remaining stars. The 1273 stars have been selected to provide broad coverage of the atmospheric parameters Teff_{eff}, log g, and [Fe/H], as well as spectral type. The goal of the project is to provide a comprehensive library of stellar spectra for use in the automated classification of stellar and galaxy spectra and in galaxy population synthesis. In this paper we discuss the characteristics of the spectral library, viz., details of the observations, data reduction procedures, and selection of stars. We also present a few illustrations of the quality and information available in the spectra. The first version of the complete spectral library is now publicly available from the National Optical Astronomy Observatory (NOAO) via FTP and HTTP.Comment: 18 pages, 6 figures, 4 table
