    Cultural Policy in Leeds

    Bradford raised a few eyebrows with its bid to be City of Culture in 2008, but then demonstrated that its claim could not be so easily dismissed. In recent years, several of the region’s towns and cities have put cultural policies at the heart of their corporate visions and strategies in an effort to unite, develop and promote. This article reports on recent research on cultural policy making in Leeds, a city that clearly has a considerable 'cultural offer', but which has been making rather less of it than several of its northern rivals

    Background: To protect health workers (HCWs) from risky occupation exposure, CDC developed the universal precautions (Ups) including Personal Protective Equipment (PPEs). However compliance to it by HCWs has remained poor even in high-risk clinical situation. The objective of this study was to identify and describe the factors that influence a HCWs’ decision to wear PPEs and the barriers that exist in preventing their use Methods: A cross-sectional survey was carried out in the St. Mary’s Hospital Lacor in all the wards to collected quantitative information as well as qualitative and observational data on PPE use Results: Out of the total 59 respondents, 2% do not know the purpose of PPE, 23.7% do not know how to don and doff PPEs, 13.6% do not use PPE even when indicated and 10% are not using an appropriate PPE. The main barriers relates to poor fitting and weak domestic gloves, few of aprons, frequent stock out and inadequate PPE as well as lack of training in PPE Conclusion: This study provides a baseline for measuring the effectiveness of interventions to improve compliance

    Epic Human Failure on June 30, 2013

    Nineteen Prescott Fire Department, Granite Mountain Hot Shot (GMHS) wildland firefighters and supervisors (WFF), perished on the June 2013 Yarnell Hill Fire (YHF) in Arizona. The firefighters left their Safety Zone during forecast, outflow winds, triggering explosive fire behavior in drought-stressed chaparral. Why would an experienced WFF Crew, leave ‘good black’ and travel downslope through a brush-filled chimney, contrary to their training and experience? An organized Serious Accident Investigation Team (SAIT) found, “… no indication of negligence, reckless actions, or violations of policy or protocol.” Despite this, many WFF professionals deemed the catastrophe, “… the final, fatal link, in a long chain of bad decisions with good outcomes.” This paper is a theoretical and realistic examination of plausible, faulty, human decisions with prior good outcomes; internal and external impacts, influencing the GMHS; and two explanations for this catastrophe: Individual Blame Logic and Organizational Function Logic, and proposed preventive mitigations

    An Improved Approximate Consensus Algorithm in the Presence of Mobile Faults

    This paper explores the problem of reaching approximate consensus in synchronous point-to-point networks, where each pair of nodes is able to communicate with each other directly and reliably. We consider the mobile Byzantine fault model proposed by Garay '94 -- in the model, an omniscient adversary can corrupt up to ff nodes in each round, and at the beginning of each round, faults may "move" in the system (i.e., different sets of nodes may become faulty in different rounds). Recent work by Bonomi et al. '16 proposed a simple iterative approximate consensus algorithm which requires at least 4f+14f+1 nodes. This paper proposes a novel technique of using "confession" (a mechanism to allow others to ignore past behavior) and a variant of reliable broadcast to improve the fault-tolerance level. In particular, we present an approximate consensus algorithm that requires only 7f/2+1\lceil 7f/2\rceil + 1 nodes, an f/2\lfloor f/2 \rfloor improvement over the state-of-the-art algorithms. Moreover, we also show that the proposed algorithm is optimal within a family of round-based algorithms

    Missense mutations in the perforin (PRF1) gene as a cause of hereditary cancer predisposition

    Perforin, a pore-forming toxin released from secretory granules of NK cells and CTLs, is essential for their cytotoxic activity against infected or cancerous target cells. Bi-allelic loss-of-function mutations in the perforin gene are invariably associated with a fatal immunoregulatory disorder, familial haemophagocytic lymphohistiocytosis type 2 (FHL2), in infants. More recently, it has also been recognized that partial loss of perforin function can cause disease in later life, including delayed onset FHL2 and haematological malignancies. Herein we report a family in which a wide range of systemic inflammatory and neoplastic manifestations have occurred across three generations. We found that disease was linked to two missense perforin gene mutations (encoding A91V, R410W) that cause protein misfolding and partial loss of activity. These cases link the partial loss of perforin function with some solid tumours that are known to be controlled by the immune system, as well as haematological cancers. Our findings also demonstrate that perforin gene mutations can contribute to hereditary cancer predisposition

    Efficient UC Commitment Extension with Homomorphism for Free (and Applications)

    Homomorphic universally composable (UC) commitments allow for the sender to reveal the result of additions and multiplications of values contained in commitments without revealing the values themselves while assuring the receiver of the correctness of such computation on committed values. In this work, we construct essentially optimal additively homomorphic UC commitments from any (not necessarily UC or homomorphic) extractable commitment. We obtain amortized linear computational complexity in the length of the input messages and rate 1. Next, we show how to extend our scheme to also obtain multiplicative homomorphism at the cost of asymptotic optimality but retaining low concrete complexity for practical parameters. While the previously best constructions use UC oblivious transfer as the main building block, our constructions only require extractable commitments and PRGs, achieving better concrete efficiency and offering new insights into the sufficient conditions for obtaining homomorphic UC commitments. Moreover, our techniques yield public coin protocols, which are compatible with the Fiat-Shamir heuristic. These results come at the cost of realizing a restricted version of the homomorphic commitment functionality where the sender is allowed to perform any number of commitments and operations on committed messages but is only allowed to perform a single batch opening of a number of commitments. Although this functionality seems restrictive, we show that it can be used as a building block for more efficient instantiations of recent protocols for secure multiparty computation and zero knowledge non-interactive arguments of knowledge

    We located the polypeptide nascent chain as it leaves cytoplasmic ribosomes from the plant Lemna gibba by immune electron microscopy using antibodies against the small subunit of the enzyme ribulose-1,5-bisphosphate carboxylase. Similar studies with Escherichia coli ribosomes, using antibodies directed against the enzyme beta-galactosidase, show that the polypeptide nascent chain emerges in the same relative position in plants and bacteria. The eucaryotic ribosomal exit site is on the large subunit, approximately 75 A from the interface between subunits and nearly 160 A from the central protuberance, the presumed site for peptidyl transfer. This is the first functional site on both the eucaryotic and procaryotic ribosomes to be determined

    An antibody raised against a pathogenic serpin variant induces mutant-like behaviour in the wild-type protein.

    A monoclonal antibody (mAb) that binds to a transient intermediate may act as a catalyst for the corresponding reaction; here we show this principle can extend on a macro-molecular scale to the induction of mutant-like oligomerisation in a wild-type protein. Using the common, pathogenic Glu342Lys (Z) variant of α1-antitrypsin as antigen - whose native state is susceptible to the formation of a proto-oligomeric intermediate - we have produced a mAb (5E3) that increases the rate of oligomerisation of the wild-type (M) variant. Employing ELISA, gel shift, thermal stability and FRET time-course experiments, we show that mAb5E3 does not bind to the native state of α1-antitrypsin, but recognises a cryptic epitope in the vicinity of the post-helix A loop and strand 4C that is revealed upon transition to the polymerisation intermediate, and which persists in the ensuing oligomer. This epitope is not shared by loop-inserted monomeric conformations. We show the increased amenity to polymerisation by either the pathogenic Glu342Lys mutation or the binding of mAb5E3 occurs without affecting energetic barrier to polymerisation. As mAb5E3 also does not alter the relative stability of the monomer to intermediate, it acts in a manner similar to the Glu342Lys mutation, by facilitating the conformational interchange between these two states

    Prognostic indicators of early and late death in children admitted to district hospital in Kenya: cohort study

    OBJECTIVES: To identify clinical indicators of immediate, early, and late mortality in children at admission to a sub-Saharan district hospital and to develop prognostic scores. DESIGN: Prospective cohort study. SETTING: One district hospital in Kenya. PARTICIPANTS: Children aged over 90 days admitted to hospital from 1 July 1998 to 30 June 2001. MAIN OUTCOME MEASURES: Prognostic indicators of mortality. RESULTS: Of 8091 children admitted up to 1 June 2000, 436 (5%) died. Sixty (14%) died within four hours after admission (immediate), 193 (44%) after 4-48 hours (early), and 183 (42%) after 48 hours (late). There were marked differences in the clinical features associated with immediate, early, and late death. Seven indicators (neurological status, respiratory distress (subcostal indrawing or deep breathing), nutritional status (wasting or kwashiorkor), severe anaemia, jaundice, axillary temperature, and length of history) were included in simplified prognostic scores. Data from 4802 children admitted from 1 July 2000 to 30 June 2001 were used to validate the scores. For simplified prognostic scores the areas under the receiver operating characteristic curves were 0.93 (95% confidence interval 0.92 to 0.94), 0.82 (0.80 to 0.83), and 0.82 (0.81 to 0.84) for immediate, early, and late death, respectively.
