DeepCare: A Deep Dynamic Memory Model for Predictive Medicine
Personalized predictive medicine necessitates the modeling of patient illness
and care processes, which inherently have long-term temporal dependencies.
Healthcare observations, recorded in electronic medical records, are episodic
and irregular in time. We introduce DeepCare, an end-to-end deep dynamic neural
network that reads medical records, stores previous illness history, infers
current illness states and predicts future medical outcomes. At the data level,
DeepCare represents care episodes as vectors in space, models patient health
state trajectories through explicit memory of historical records. Built on Long
Short-Term Memory (LSTM), DeepCare introduces time parameterizations to handle
irregular timed events by moderating the forgetting and consolidation of memory
cells. DeepCare also incorporates medical interventions that change the course
of illness and shape future medical risk. Moving up to the health state level,
historical and present health states are then aggregated through multiscale
temporal pooling, before passing through a neural network that estimates future
outcomes. We demonstrate the efficacy of DeepCare for disease progression
modeling, intervention recommendation, and future risk prediction. On two
important cohorts with heavy social and economic burden -- diabetes and mental
health -- the results show improved modeling and risk prediction accuracy.Comment: Accepted at JBI under the new name: "Predicting healthcare

Semi-autonomous wheelchair developed using a unique camera system configuration biologically inspired by equine vision
This paper is concerned with the design and development of a semi-autonomous wheelchair system using cameras in a system configuration modeled on the vision system of a horse. This new camera configuration utilizes stereoscopic vision for 3-Dimensional (3D) depth perception and mapping ahead of the wheelchair, combined with a spherical camera system for 360-degrees of monocular vision. This unique combination allows for static components of an unknown environment to be mapped and any surrounding dynamic obstacles to be detected, during real-time autonomous navigation, minimizing blind-spots and preventing accidental collisions with people or obstacles. This novel vision system combined with shared control strategies provides intelligent assistive guidance during wheelchair navigation and can accompany any hands-free wheelchair control technology. Leading up to experimental trials with patients at the Royal Rehabilitation Centre (RRC) in Ryde, results have displayed the effectiveness of this system to assist the user in navigating safely within the RRC whilst avoiding potential collisions. © 2011 IEEE
Evaluation of fast, high-detail projected light 3D sensing for robots in construction
© 2019 IEEE. Robots used on-site in construction need to perceive the surrounding environment to operate autonomously. This is challenging as the construction environment is often less than ideal due to changing lighting conditions, turbid air, and the need to detect fine details. In this work we evaluate a custom made projected light 3D sensor system for suitability and practicality in enabling autonomous robotics for construction. A series of tests are performed to evaluate the sensor based on ability to capture environmental details, operate robustly in challenging lighting conditions, and make accurate geometric measurements. Analysis shows that high fidelity measurements with accuracy in the order of millimeters can be obtained, making the technology a promising solution for robots operating in construction environments
Distant X-ray Galaxies: Insights from the Local Population
A full understanding of the origin of the hard X-ray background requires a
complete and accurate census of the distant galaxies that produce it.
Unfortunately, distant X-ray galaxies tend to be very faint at all wavelengths,
which hinders efforts to perform this census. This chapter discusses the
insights that can be obtained through comparison of the distant population to
local X-ray galaxies, whose properties are well characterized. Such comparisons
will ultimately aid investigations into the cosmic evolution of supermassive

New directions in cellular therapy of cancer: a summary of the summit on cellular therapy for cancer
A summit on cellular therapy for cancer discussed and presented advances related to the use of adoptive cellular therapy for melanoma and other cancers. The summit revealed that this field is advancing rapidly. Conventional cellular therapies, such as tumor infiltrating lymphocytes (TIL), are becoming more effective and more available. Gene therapy is becoming an important tool in adoptive cell therapy. Lymphocytes are being engineered to express high affinity T cell receptors (TCRs), chimeric antibody-T cell receptors (CARs) and cytokines. T cell subsets with more naïve and stem cell-like characteristics have been shown in pre-clinical models to be more effective than unselected populations and it is now possible to reprogram T cells and to produce T cells with stem cell characteristics. In the future, combinations of adoptive transfer of T cells and specific vaccination against the cognate antigen can be envisaged to further enhance the effectiveness of these therapies
MAXIPOL: a balloon-borne experiment for measuring the polarization anisotropy of the cosmic microwave background radiation
We discuss MAXIPOL, a bolometric balloon-borne experiment designed to measure the E-mode polarization anisotropy of the cosmic microwave background radiation (CMB) on angular scales of 10 arcmin to 2 degrees. MAXIPOL is the first CMB experiment to collect data with a polarimeter that utilizes a rotating half-wave plate and fixed wire-grid polarizer. We present the instrument design, elaborate on the polarimeter strategy and show the instrument performance during flight with some time domain data. Our primary data set was collected during a 26 hour turnaround flight that was launched from the National Scientific Ballooning Facility in Ft. Sumner, New Mexico in May 2003. During this flight five regions of the sky were mapped. Data analysis is in progress
Daratumumab plus bortezomib, cyclophosphamide, and dexamethasone in Asian patients with newly diagnosed AL amyloidosis: subgroup analysis of ANDROMEDA
Subcutaneous daratumumab plus bortezomib/cyclophosphamide/dexamethasone (VCd; D-VCd) improved outcomes versus VCd for patients with newly diagnosed immunoglobulin light-chain (AL) amyloidosis in the phase 3 ANDROMEDA study. We report a subgroup analysis of Asian patients (Japan; Korea; China) from ANDROMEDA. Among 388 randomized patients, 60 were Asian (D-VCd, n = 29; VCd, n = 31). At a median follow-up of 11.4 months, the overall hematologic complete response rate was higher for D-VCd versus VCd (58.6% vs. 9.7%; odds ratio, 13.2; 95% confidence interval [CI], 3.3–53.7; P < 0.0001). Six-month cardiac and renal response rates were higher with D-VCd versus VCd (cardiac, 46.7% vs. 4.8%; P = 0.0036; renal, 57.1% vs. 37.5%; P = 0.4684). Major organ deterioration progression-free survival (MOD-PFS) and major organ deterioration event-free survival (MOD-EFS) were improved with D-VCd versus VCd (MOD-PFS: hazard ratio [HR], 0.21; 95% CI, 0.06–0.75; P = 0.0079; MOD-EFS: HR, 0.16; 95% CI, 0.05–0.54; P = 0.0007). Twelve deaths occurred (D-VCd, n = 3; VCd, n = 9). Twenty-two patients had baseline serologies indicating prior hepatitis B virus (HBV) exposure; no patient experienced HBV reactivation. Although grade 3/4 cytopenia rates were higher than in the global safety population, the safety profile of D-VCd in Asian patients was generally consistent with the global study population, regardless of body weight. Lectin-like bacteriocins from pseudomonas spp. utilise D-rhamnose containing lipopolysaccharide as a cellular receptor
Lectin-like bacteriocins consist of tandem monocot mannose-binding domains and display a genus-specific killing activity. Here we show that pyocin L1, a novel member of this family from Pseudomonas aeruginosa, targets susceptible strains of this species through recognition of the common polysaccharide antigen (CPA) of P. aeruginosa lipopolysaccharide that is predominantly a homopolymer of d-rhamnose. Structural and biophysical analyses show that recognition of CPA occurs through the C-terminal carbohydrate-binding domain of pyocin L1 and that this interaction is a prerequisite for bactericidal activity. Further to this, we show that the previously described lectin-like bacteriocin putidacin L1 shows a similar carbohydrate-binding specificity, indicating that oligosaccharides containing d-rhamnose and not d-mannose, as was previously thought, are the physiologically relevant ligands for this group of bacteriocins. The widespread inclusion of d-rhamnose in the lipopolysaccharide of members of the genus Pseudomonas explains the unusual genus-specific activity of the lectin-like bacteriocins
Effects of Preoperative Aspirin on Cardiocerebral and Renal Complications in Non-Emergent Cardiac Surgery Patients: A Sub-Group and Cohort Study
BACKGROUND AND OBJECTIVE: Postoperative cardiocerebral and renal complications are a major threat for patients undergoing cardiac surgery. This study was aimed to examine the effect of preoperative aspirin use on patients undergoing cardiac surgery.
METHODS: An observational cohort study was performed on consecutive patients (n = 1879) receiving cardiac surgery at this institution. The patients excluded from the study were those with preoperative anticoagulants, unknown aspirin use, or underwent emergent cardiac surgery. Outcome events included were 30-day mortality, renal failure, readmission and a composite outcome - major adverse cardiocerebral events (MACE) that include permanent or transient stroke, coma, perioperative myocardial infarction (MI), heart block and cardiac arrest.
RESULTS: Of all patients, 1145 patients met the inclusion criteria and were divided into two groups: those taking (n = 858) or not taking (n = 287) aspirin within 5 days preceding surgery. Patients with aspirin presented significantly more with history of hypertension, diabetes, peripheral arterial disease, previous MI, angina and older age. With propensity scores adjusted and multivariate logistic regression, however, this study showed that preoperative aspirin therapy (vs. no aspirin) significantly reduced the risk of MACE (8.4% vs. 12.5%, odds ratio [OR] 0.585, 95% CI 0.355-0.964, P = 0.035), postoperative renal failure (2.6% vs. 5.2%, OR 0.438, CI 0.203-0.945, P = 0.035) and dialysis required (0.8% vs. 3.1%, OR 0.230, CI 0.071-0.742, P = 0.014), but did not significantly reduce 30-day mortality (4.1% vs. 5.8%, OR 0.744, CI 0.376-1.472, P = 0.396) nor it increased readmissions in the patients undergoing cardiac surgery.
CONCLUSIONS: Preoperative aspirin therapy is associated with a significant decrease in the risk of MACE and renal failure and did not increase readmissions in patients undergoing non-emergent cardiac surgery
Wet Granular Materials
Most studies on granular physics have focused on dry granular media, with no
liquids between the grains. However, in geology and many real world
applications (e.g., food processing, pharmaceuticals, ceramics, civil
engineering, constructions, and many industrial applications), liquid is
present between the grains. This produces inter-grain cohesion and drastically
modifies the mechanical properties of the granular media (e.g., the surface
angle can be larger than 90 degrees). Here we present a review of the
mechanical properties of wet granular media, with particular emphasis on the
effect of cohesion. We also list several open problems that might motivate
