Dynamic Variation in Sexual Contact Rates in a Cohort of HIV-Negative Gay Men
Human immunodeficiency virus (HIV) transmission models that include variability in sexual behavior over time have shown increased incidence, prevalence, and acute-state transmission rates for a given population risk profile. This raises the question of whether dynamic variation in individual sexual behavior is a real phenomenon that can be observed and measured. To study this dynamic variation, we developed a model incorporating heterogeneity in both between-person and within-person sexual contact patterns. Using novel methodology that we call iterated filtering for longitudinal data, we fitted this model by maximum likelihood to longitudinal survey data from the Centers for Disease Control and Prevention's Collaborative HIV Seroincidence Study (1992–1995). We found evidence for individual heterogeneity in sexual behavior over time. We simulated an epidemic process and found that inclusion of empirically measured levels of dynamic variation in individual-level sexual behavior brought the theoretical predictions of HIV incidence into closer alignment with reality given the measured per-act probabilities of transmission. The methods developed here provide a framework for quantifying variation in sexual behaviors that helps in understanding the HIV epidemic among gay men
Measuring patient-perceived quality of care in US hospitals using Twitter
BACKGROUND: Patients routinely use Twitter to share feedback about their experience receiving healthcare. Identifying and analysing the content of posts sent to hospitals may provide a novel real-time measure of quality, supplementing traditional, survey-based approaches. OBJECTIVE: To assess the use of Twitter as a supplemental data stream for measuring patient-perceived quality of care in US hospitals and compare patient sentiments about hospitals with established quality measures. DESIGN: 404 065 tweets directed to 2349 US hospitals over a 1-year period were classified as having to do with patient experience using a machine learning approach. Sentiment was calculated for these tweets using natural language processing. 11 602 tweets were manually categorised into patient experience topics. Finally, hospitals with ≥50 patient experience tweets were surveyed to understand how they use Twitter to interact with patients. KEY RESULTS: Roughly half of the hospitals in the US have a presence on Twitter. Of the tweets directed toward these hospitals, 34 725 (9.4%) were related to patient experience and covered diverse topics. Analyses limited to hospitals with ≥50 patient experience tweets revealed that they were more active on Twitter, more likely to be below the national median of Medicare patients (p<0.001) and above the national median for nurse/patient ratio (p=0.006), and to be a non-profit hospital (p<0.001). After adjusting for hospital characteristics, we found that Twitter sentiment was not associated with Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) ratings (but having a Twitter account was), although there was a weak association with 30-day hospital readmission rates (p=0.003). CONCLUSIONS: Tweets describing patient experiences in hospitals cover a wide range of patient care aspects and can be identified using automated approaches. These tweets represent a potentially untapped indicator of quality and may be valuable to patients, researchers, policy makers and hospital administrators

An improved algorithm to evaluate the nonrelativistic three-electron
Hylleraas-Configuration Interaction (Hy-CI) kinetic energy integrals over
Slater orbitals and the Hamiltonian in Hylleraas coordinates is shown. The
resulting analytical expressions are general for all quantum numbers of the
orbitals. From there, the restriction of employing orbitals with quantum
numbers of the above algorithm presented in paper I of this series
has been removed. With the new algorithm it is possible, in the direct
integration method described in this series, to carry out Hy-CI atomic
structure calculations including -, -, , and higher
angular-momentum Slater orbitals and to determine , , , and

Automated smoother for the numerical decoupling of dynamics models
<p>Abstract</p> <p>Background</p> <p>Structure identification of dynamic models for complex biological systems is the cornerstone of their reverse engineering. Biochemical Systems Theory (BST) offers a particularly convenient solution because its parameters are kinetic-order coefficients which directly identify the topology of the underlying network of processes. We have previously proposed a numerical decoupling procedure that allows the identification of multivariate dynamic models of complex biological processes. While described here within the context of BST, this procedure has a general applicability to signal extraction. Our original implementation relied on artificial neural networks (ANN), which caused slight, undesirable bias during the smoothing of the time courses. As an alternative, we propose here an adaptation of the Whittaker's smoother and demonstrate its role within a robust, fully automated structure identification procedure.</p> <p>Results</p> <p>In this report we propose a robust, fully automated solution for signal extraction from time series, which is the prerequisite for the efficient reverse engineering of biological systems models. The Whittaker's smoother is reformulated within the context of information theory and extended by the development of adaptive signal segmentation to account for heterogeneous noise structures. The resulting procedure can be used on arbitrary time series with a nonstationary noise process; it is illustrated here with metabolic profiles obtained from <it>in-vivo </it>NMR experiments. The smoothed solution that is free of parametric bias permits differentiation, which is crucial for the numerical decoupling of systems of differential equations.</p> <p>Conclusion</p> <p>The method is applicable in signal extraction from time series with nonstationary noise structure and can be applied in the numerical decoupling of system of differential equations into algebraic equations, and thus constitutes a rather general tool for the reverse engineering of mechanistic model descriptions from multivariate experimental time series.</p

Performed and preferred participation in science and technology across Europe: Exploring an alternative idea of "democratic deficit"
Republican ideals of active scientific citizenship and extensive use of deliberative, democratic decision making have come to dominate the public participation agenda, and academic analyses have focused on the deficit of public involvement vis-Ã -vis these normative ideals. In this paper we use latent class models to explore what Eurobarometer survey data can tell us about the ways in which people participate in tacit or in policy-active ways with developments in science and technology, but instead of focusing on the distance between observed participation and the dominant, normative ideal of participation, we examine the distance between what people do, and what they themselves think is appropriate in terms of involvement. The typology of citizens emerging from the analyses entails an entirely different diagnosis of democratic deficit, one that stresses imbalance between performed and preferred participation

Egocentric Social Network Structure, Health, and Pro-Social Behaviors in a National Panel Study of Americans
Using a population-based, panel survey, we study how egocentric social networks change over time, and the relationship between egocentric network properties and health and pro-social behaviors. We find that the number of prosocial activities is strongly positively associated with having more friends, or an increase in degree, with approximately 0.04 more prosocial behaviors expected for every friend added. Moreover, having more friends is associated with an improvement in health, while being healthy and prosocial is associated with closer relationships. Specifically, a unit increase in health is associated with an expected 0.45 percentage-point increase in average closeness, while adding a prosocial activity is associated with a 0.46 percentage-point increase in the closeness of one’s relationships. Furthermore, a tradeoff between degree and closeness of social contacts was observed. As the number of close social contacts increases by one, the estimated average closeness of each individual contact decreases by approximately three percentage-points. The increased awareness of the importance of spillover effects in health and health care makes the ascertainment of egocentric social networks a valuable complement to investigations of the relationship between socioeconomic factors and health
'Word from the street' : when non-electoral representative claims meet electoral representation in the United Kingdom
Taking the specific case of street protests in the UK – the ‘word from the street’– this article examines recent (re)conceptualizations of political representation, most particularly Saward’s notion of ‘representative claim’. The specific example of nonelectoral claims articulated by protestors and demonstrators in the UK is used to illustrate: the processes of making, constituting, evaluating and accepting claims for and by constituencies and audiences; and the continuing distinctiveness of claims based upon electoral representation. Two basic questions structure the analysis: first, why would the political representative claims of elected representatives trump the nonelectoral claims of mass demonstrators and, second, in what ways does the ‘perceived legitimacy’ of the former differ from the latter
Interactive Actor Analysis for Rural Water Management in The Netherlands
Recent developments in the policy sciences emphasize the social environment
in which decisions are made. The ‘network metaphor’ is often used to describe
the key role of interactions between interdependent actors involved in decision
making. These interactions take place in a policy arena drawn up by actors with an
interest in and control over decisions on the issues addressed. Interdependencies,
caused by the need for actors to increase their means of realizing objectives, are
the driving force behind these interactions. Dependency relations are of special
interest to water management and river basin management because of the fundamental
asymmetrical interdependencies that exist in river basins between upstream
and downstream stakeholders. Coleman’s linear system of action models decision
making process involving dependencies between multiple stakeholders as exchange
of control over issues, while interactions are required to negotiate exchanges of
control. We developed an interactive method for actor analysis based on Coleman’s
linear system of action and applied it to the national rural water management policy
domain in The Netherlands. The method is firmly rooted in mathematical sociology
and defies the criticism that methods for actor and stakeholder analysis do not specify
a theoretical basis explaining the causal relations between the variables analyzed and
policy change. With the application to the rural water management policy arena we
intended to increase our insight into the practical applicability of this analyticmethod
in an interactive workshop, the acceptability of the approach for the participating
actors, its contribution to the process of decision making and our understanding of
the rural water management policy arena in The Netherlands. We found that the
Association of Water Authorities, the Ministry of Public Works and the Ministry of
Agriculture are the most powerful actor in the policy domain, while governance and
cost and benefits of rural water management are the most salient issues. Progress
in policy development for rural water management is probably most promising for the issues governance, costs and benefits, safety and rural living conditions through
improved interaction between the Association of Water Authorities, the Ministry of
Agriculture and the Rural Credit Bank. Besides these analytic results the interactive
approach implemented increased the participants understanding of their dependency
on other actors in the rural water management policy domain and supported them
in developing a sound perspective on their dependency position. We concluded
that the method developed is acceptable to real-world policy decision makers, can
successfully be applied in an interactive setting, potentially contributes to the process
of decision making by increasing the participants understanding of their dependency
position, has the potential to delivers valuable advice for future decision-making and
increases our understanding of policy development for rural water management in
general
