Mega-sporting event legacies with reference to accommodation requirements of participants in the Two Oceans Marathon, Cape Town
All travelling sportspersons require accommodation while taking part in sporting events when away
from home. This was true of soccer players from the Professional Soccer League in South Africa,
who had to play the Black Leopards team in Thohoyandou, Black netball players in Johannesburg,
and generally South African competitors’ competing in the two areas identified by the Hospitality
students of the University of Venda in 2008, as part of their marketing modules for the Bachelor of
Commerce degree in Hospitality Management studies. Similar findings were obtained when
participants in the Old Mutual Two Oceans Marathon were surveyed in 2013 and 2014. Quality
service and friendliness of staff at commercial accommodation establishments, particularly with
regard to rooms, reception and restaurants, were identified by these participants, confirming the
2008 survey results
Ictal Spiking Patterns Recorded from Temporal Depth Electrodes Predict Good Outcome After Anterior Temporal Lobectomy
Purpose : Investigators have shown that the presence of ictal spiking (IS) recorded from temporal depth electrodes is associated with mesial temporal sclerosis (MTS). We investigated the relation of IS to seizure control and pathology after anterior temporal lobectomy (ATL). Methods : All patients undergoing intracranial ictal monitoring from a single institution since 1989 were identified. Those who did not undergo ATL or had postoperative follow-up of <1 year were excluded. All received at a minimum bilateral temporal depth electrodes. Ictal recordings were reviewed for the presence of IS, and the proportion of seizures with IS was determined for each patient. Outcome was determined by using Engel's classification. Surgical specimens were reviewed for pathology. Statistics used were X 2 , Fisher exact test, and Wilcoxon rank sum. Results : Forty patients with 571 seizures were reviewed. In 292 seizures from 32 patients, IS was seen. Outcomes were 24 class I (22 with IS), five class II (four with IS), three class III (one with IS), seven class IV (four with IS), and one lost to follow-up (with IS). Pathologic review revealed 25 with MTS, 22 of whom had IS. The presence of IS was associated with class I outcomes (p = 0.04), but not MTS (p = 0.06). Patients with class I outcomes had a significantly greater proportion of seizures with IS (mean, 0.58 ± 0.3) compared with other outcomes (mean, 0.30 ± 0.3, p = 0.02). Conclusions : The presence of IS and higher proportion of seizures with IS correlated with good seizure outcome after ATL. Health status in the TORCH study of COPD: treatment efficacy and other determinants of change
BACKGROUND: Little is known about factors that determine health status decline in clinical trials of COPD.
OBJECTIVES: To examine health status changes over 3 years in the TORCH study of salmeterol+fluticasone propionate (SFC) vs. salmeterol alone, fluticasone propionate alone or placebo.
METHODS: St George's Respiratory Questionnaire (SGRQ) was administered at baseline then every 6 months.
MEASUREMENTS AND MAIN RESULTS: Data from 4951 patients in 28 countries were available. SFC produced significant improvements over placebo in all three SGRQ domains during the study: (Symptoms -3.6 [95% CI -4.8, -2.4], Activity -2.8 [95% CI -3.9, -1.6], Impacts -3.2 [95% CI -4.3, -2.1]) but the pattern of change over time differed between domains. SGRQ deteriorated faster in patients with Global Initiative for Chronic Obstructive Lung Disease (GOLD) stages III & IV relative to GOLD stage II (p < 0.001). There was no difference in the relationship between deterioration in SGRQ Total score and forced expiratory volume in one second (FEV1) decline (as % predicted) in men and women. Significantly faster deterioration in Total score relative to FEV1 % predicted was seen in older patients (≥ 65 years) and there was an age-related change in Total score that was independent of change in FEV1. The relationship between deterioration in FEV1 and SGRQ did not differ in different world regions, but patients in Asia-Pacific showed a large improvement in score that was unrelated to FEV1 change.
CONCLUSIONS: In addition to treatment effects, health status changes in clinical trials may be influenced by demographic and disease-related factors. Deterioration in health status appears to be fastest in older persons and those with severe airflow limitation
Mapping HIV-related behavioural surveillance among injecting drug users in Europe, 2008.
The systematic collection of behavioural information is an important component of second-generation HIV surveillance. The extent of behavioural surveillance among injecting drug users (IDUs) in Europe was examined using data collected through a questionnaire sent to all 31 countries of the European Union and European Free Trade Association as part of a European-wide behavioural surveillance mapping study on HIV and other sexually transmitted infections. The questionnaire was returned by 28 countries during August to September 2008: 16 reported behavioural surveillance studies (two provided no further details). A total of 12 countries used repeated surveys for behavioural surveillance and five used their Treatment Demand Indicator system (three used both approaches). The data collected focused on drug use, injecting practices, testing for HIV and hepatitis C virus and access to healthcare. Eight countries had set national indicators: three indicators were each reported by five countries: the sharing any injecting equipment, uptake of HIV testing and uptake of hepatitis C virus testing. The recall periods used varied. Seven countries reported conducting one-off behavioural surveys (in one country without a repeated survey, these resulted an informal surveillance structure). All countries used convenience sampling, with service-based recruitment being the most common approach. Four countries had used respondent-driven sampling. Three fifths of the countries responding (18/28) reported behavioural surveillance activities among IDUs; however, harmonisation of behavioural surveillance indicators is needed
Neurotrophic Effect of Citrus 5-Hydroxy-3,6,7,8,3′,4′-Hexamethoxyflavone: Promotion of Neurite Outgrowth via cAMP/PKA/CREB Pathway in PC12 Cells
5-Hydroxy-3,6,7,8,3′,4′-hexamethoxyflavone (5-OH-HxMF), a hydroxylated polymethoxyflavone, is found exclusively in the Citrus genus, particularly in the peels of sweet orange. In this research, we report the first investigation of the neurotrophic effects and mechanism of 5-OH-HxMF in PC12 pheochromocytoma cells. We found that 5-OH-HxMF can effectively induce PC12 neurite outgrowth accompanied with the expression of neuronal differentiation marker protein growth-associated protein-43(GAP-43). 5-OH-HxMF caused the enhancement of cyclic AMP response element binding protein (CREB) phosphorylation, c-fos gene expression and CRE-mediated transcription, which was inhibited by 2-naphthol AS-E phosphate (KG-501), a specific antagonist for the CREB-CBP complex formation. Moreover, 5-OH-HxMF-induced both CRE transcription activity and neurite outgrowth were inhibited by adenylate cyclase and protein kinase A (PKA) inhibitor, but not MEK1/2, protein kinase C (PKC), phosphatidylinositol 3-kinase (PI3K) or calcium/calmodulin-dependent protein kinase (CaMK) inhibitor. Consistently, 5-OH-HxMF treatment increased the intracellular cAMP level and downstream component, PKA activity. We also found that addition of K252a, a TrKA antagonist, significantly inhibited NGF- but not 5-OH-HxMF-induced neurite outgrowth. These results reveal for the first time that 5-OH-HxMF is an effective neurotrophic agent and its effect is mainly through a cAMP/PKA-dependent, but TrKA-independent, signaling pathway coupling with CRE-mediated gene transcription. A PKC-dependent and CREB-independent pathway was also involved in its neurotrophic action

Assessment of sub-Nyquist deterministic and random data sampling techniques for operational modal analysis
This paper assesses numerically the potential of two different spectral estimation approaches supporting non-uniform in time data sampling at sub-Nyquist average rates (i.e., below the Nyquist frequency) to reduce data transmission payloads in wireless sensor networks (WSNs) for operational modal analysis (OMA) of civil engineering structures. This consideration relaxes transmission bandwidth constraints in WSNs and prolongs sensor battery life since wireless transmission is the most energy-hungry on-sensor operation. Both the approaches assume acquisition of sub-Nyquist structural response acceleration measurements and transmission to a base station without on-sensor processing. The response acceleration power spectral density matrix is estimated directly from the sub-Nyquist measurements and structural mode shapes are extracted using the frequency domain decomposition algorithm. The first approach relies on the compressive sensing (CS) theory to treat sub-Nyquist randomly sampled data assuming that the acceleration signals are sparse/compressible in the frequency domain (i.e., have a small number of Fourier coefficients with significant magnitude). The second approach is based on a power spectrum blind sampling (PSBS) technique considering periodic deterministic sub-Nyquist “multi-coset” sampling and treating the acceleration signals as wide-sense stationary stochastic processes without posing any sparsity conditions. The modal assurance criterion (MAC) is adopted to quantify the quality of mode shapes derived by the two approaches at different sub-Nyquist compression rates (CRs) using computer-generated signals of different sparsity and field-recorded stationary data pertaining to an overpass in Zurich, Switzerland. It is shown that for a given CR, the performance of the CS-based approach is detrimentally affected by signal sparsity, while the PSBS-based approach achieves MAC>0.96 independently of signal sparsity for CRs as low as 11% the Nyquist rate. It is concluded that the PSBS-based approach reduces effectively data transmission requirements in WSNs for OMA, without being limited by signal sparsity and without requiring a priori assumptions or knowledge of signal sparsity
Insulin autoantibodies as determined by competitive radiobinding assay are positively correlated with impaired beta-cell function — The Ulm-Frankfurt population study
Out of a random population of 4208 non-diabetic pupils without a family history of Type I diabetes 44 (1.05%) individuals had islet cell antibody (ICA) levels greater or equal to 5 Juvenile Diabetes Foundation (JDF) units. 39 of these ICA-positives could be repeatedly tested for circulating insulin autoantibodies (CIAA) using a competitive radiobinding assay. The results were compared with the insulin responses in the intravenous glucose tolerance tests (IVGTT) and with HLA types. Six pupils were positive for CIAA. All of them had complement-fixing ICA, and 5 of them were HLA-DR4 positive. Three of the 6 showed a first-phase insulin response below the first percentile of normal controls. Our data indicate that in population-based studies CIAA can be considered as a high risk marker for impaired beta-cell function in non-diabetic ICA-positive individuals
