    The coexistence of pressure waves in the operation of quartz-crystal shear-wave sensors

    It is demonstrated that an AT-cut quartz crystal driven in the thickness-shear-wave mode and typically used as a sensor to monitor the viscoelastic shear-wave properties of a fluid also produce longitudinal pressure waves. Unlike the shear wave, these waves are capable of long-range propagation through the fluid and of reflection at its boundaries, notably at an outer fluid–air interface. They introduce a component into the measured electrical impedance and resonance frequency shift of the crystal, which reflects the setting up of cyclic pressure-wave resonances in the fluid. This has important implications for the practical employment of these crystal as sensors. Under appropriate conditions, as demonstrated for water and n-octane, it is possible to determine the propagating properties of sound waves in a fluid simultaneously with the viscoelastic shear-wave properties. These experiments are supported by an analysis of the appropriate hydrodynamic equations for waves in the crystal–fluid system, which predicts electrical characteristics in close agreement with those found experimentally

    Control of the Geometrical Conformity of the LHC Installation with a Single Laser Source

    A large and complex accelerator like LHC machine needs to integrate several thousand different components in a relatively limited space. During the installation, those components are installed in successive phases, always aiming to leave the necessary space available for the equipment which will follow. To help ensure the correct conditions for the installation, the survey team uses a laser scanner to measure specific areas and provides this data, merged together in a known reference system, to the integration team who compare the results with the 3D CAD models. This paper describes the tools and software used to rebuild underground zones in the CATIA environment, to check interferences or geometrical non conformities, as well as the procedures defined to solve the identified problems

    Discrepancies in autologous bone marrow stem cell trials and enhancement of ejection fraction (DAMASCENE): weighted regression and meta-analysis

    Objective To investigate whether discrepancies in trials of use of bone marrow stem cells in patients with heart disease account for the variation in reported effect size in improvement of left ventricular function. Design Identification and counting of factual discrepancies in trial reports, and sample size weighted regression against therapeutic effect size. Meta-analysis of trials that provided sufficient information. Data sources PubMed and Embase from inception to April 2013. Eligibility for selecting studies Randomised controlled trials evaluating the effect of autologous bone marrow stem cells for heart disease on mean left ventricular ejection fraction. Results There were over 600 discrepancies in 133 reports from 49 trials. There was a significant association between the number of discrepancies and the reported increment in EF with bone marrow stem cell therapy (Spearman’s r=0.4, P=0.005). Trials with no discrepancies were a small minority (five trials) and showed a mean EF effect size of −0.4%. The 24 trials with 1-10 discrepancies showed a mean effect size of 2.1%. The 12 with 11-20 discrepancies showed a mean effect of size 3.0%. The three with 21-30 discrepancies showed a mean effect size of 5.7%. The high discrepancy group, comprising five trials with over 30 discrepancies each, showed a mean effect size of 7.7%. Conclusions Avoiding discrepancies is difficult but is important because discrepancy count is related to effect size. The mechanism is unknown but should be explored in the design of future trials because in the five trials without discrepancies the effect of bone marrow stem cell therapy on ejection fraction is zero

    A patient's experience of a new post-operative patient-controlled analgesic technique

    A patient underwent major spinal surgery, twice within a 3 week period. On the first occasion his post-operative pain was managed by conventional morphine patient-controlled analgesia (PCA). After the second procedure his pain was managed by a patient-controlled computer-assisted titration of alfentanil. This provided the opportunity to compare the efficacy of these two drug regimens in the same subject. The results showed comparable quality of analgesia and sedation and similar effects on respiration. However, the patient expressed a preference for morphine PCA.published_or_final_versio

    Glycoprotein IIb/IIIa Inhibitors Use and Outcome after Percutaneous Coronary Intervention for Non-ST Elevation Myocardial Infarction

    Aims. We investigate the effect of glycoprotein IIb/IIIa (GP IIb/IIIa) inhibitors on long-term outcomes following percutaneous coronary intervention (PCI) after non-ST elevation myocardial infarction (NSTEMI). Meta-analyses indicate that these agents are associated with improved short-term outcomes. However, many trials were undertaken before the routine use of P2Y12 inhibitors. Recent studies yield conflicting results and registry data have suggested that GP IIb/IIIa inhibitors may cause more bleeding than what trials indicate. Methods and Results. This retrospective observational study involves 3047 patients receiving dual-antiplatelet therapy who underwent PCI for NSTEMI. Primary outcome was all-cause mortality. Major adverse cardiac events (MACE) were a secondary outcome. Mean follow-up was 4.6 years. Patients treated with GP IIb/IIIa inhibitors were younger with fewer comorbidities. Although the unadjusted Kaplan-Meier analysis suggested that GP IIb/IIIa inhibitor use was associated with improved outcomes, multivariate analysis (including propensity scoring) showed no benefit for either survival (P=0.136) or MACE (P=0.614). GP IIb/IIIa inhibitor use was associated with an increased risk of major bleeding (P=0.021). Conclusion. Although GP IIb/IIIa inhibitor use appeared to improve outcomes after PCI for NSTEMI, patients who received GP IIb/IIIa inhibitors tended to be at lower risk. After multivariate adjustment we observed no improvement in MACE or survival and an increased risk of major bleeding

    An alternative approach to using LiDAR remote sensing data to predict stem diameter distributions across a temperate forest landscape

We apply a spatially-implicit, allometry-based modelling approach to predict stem diameter distributions (SDDs) from low density airborne LiDAR data in a heterogeneous, temperate forest in Ontario, Canada. Using a recently published algorithm that relates the density, size, and species of individual trees to the height distribution of first returns, we estimated parameters that succinctly describe SDDs that are most consistent with each 0.25-ha LiDAR tile across a 30,000 ha forest landscape. Tests with independent validation plots showed that the diameter distribution of stems was predicted with reasonable accuracy in most cases (half of validation plots had R2 ≥ 0.75, and another 23% had 0.5 ≤ R2 < 0.75). The predicted frequency of larger stems was much better than that of small stems (8 ≤ x < 11 cm diameter), particularly small conifers. We used the predicted SDDs to calculate aboveground carbon density (ACD; RMSE = 21.4 Mg C/ha), quadratic mean diameter (RMSE = 3.64 cm), basal area (RMSE = 6.99 m2/ha) and stem number (RMSE = 272 stems/ha). The accuracy of our predictions compared favorably with previous studies that have generally been undertaken in simpler conifer-dominated forest types. We demonstrate the utility of our results to spatial forest management planning by mapping SDDs, the proportion of broadleaves, and ACD at a 0.25 ha resolution

    MYOD-1 in normal colonic mucosa : role as a putative biomarker?

    Background DNA methylation of promoter-associated CpG islands of certain genes may play a role in the development of colorectal cancer. The MYOD-1 gene which is a muscle differentiation gene has been showed to be significantly methylated in colorectal cancer which, is an age related event. However the role of this gene in the colonic mucosa is not understood and whether methylation occurs in subjects without colon cancer. In this study, we have determined the frequency of methylation of the MYOD-1 gene in normal colonic mucosa and investigated to see if this is associated with established colorectal cancer risk factors primarily ageing. Results We analysed colonic mucosal biopsies in 218 normal individuals and demonstrated that in most individuals promoter hypermethylation was not quantified for MYOD-1. However, promoter hypermethylation increased significantly with age (p < 0.001 using regression analysis) and this was gender independent. We also showed that gene promoter methylation increased positively with an increase in waist to hip (WHR) ratio – the latter is also a known risk factor for colon cancer development. Conclusions Our study suggests that promoter gene hypermethylation of the MYOD-1 gene increases significantly with age in normal individuals and thus may offer potential as a putative biomarker for colorectal cancer
