Primary PEComa of the bladder treated with primary excision and adjuvant interferon-alpha immunotherapy: a case report
BACKGROUND: Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms of uncertain malignant potential, which have in common the co-expression of muscle and melanocytic immunohistochemical markers. CASE PRESENTATION: A 48-year-old man presented with dysuria, passage of urinary sediment and lower abdominal discomfort. A three centimeter mass was identified by cystoscopy in the posterior midline of the bladder. Computerized tomography suggested an enterovesical fistula. The patient underwent laparotomy, partial cystectomy and partial small bowel resection. Pathological examination revealed PEComa of the bladder. The patient underwent adjuvant interferon-α immunotherapy. Subsequent follow-up procedures, including cystoscopy and imaging, have not revealed evidence of recurrence. The patient is clinically free of disease 48 months after surgery. CONCLUSION: This case represents the second documented PEComa of bladder and demonstrates that adjuvant therapies, including anti-angiogenic and immunotherapy, may be considered for patients with locally advanced or metastatic genitourinary PEComa
Biochar: pyrogenic carbon for agricultural use: a critical review.
O biocarvĂŁo (biomassa carbonizada para uso agrĂcola) tem sido usado como condicionador do solo em todo o mundo, e essa tecnologia Ă© de especial interesse para o Brasil, uma vez que tanto a ?inspiração?, que veio das Terras Pretas de ĂŤndios da AmazĂ´nia, como o fato de o Brasil ser o maior produtor mundial de carvĂŁo vegetal, com a geração de importante quantidade de resĂduos na forma de finos de carvĂŁo e diversas biomassas residuais, principalmente da agroindĂşstria, como bagaço de cana, resĂduos das indĂşstrias de madeira, papel e celulose, biocombustĂveis, lodo de esgoto etc. Na Ăşltima dĂ©cada, diversos estudos com biocarvĂŁo tĂŞm sido realizados e atualmente uma vasta literatura e excelentes revisões estĂŁo disponĂveis. Objetivou-se aqui nĂŁo fazer uma revisĂŁo bibliográfica exaustiva, mas sim uma revisĂŁo crĂtica para apontar alguns destaques na pesquisa sobre biochar. Intracellular retention of ABL kinase inhibitors determines commitment to apoptosis in CML cells
Clinical development of imatinib in CML established continuous target inhibition as a paradigm for successful tyrosine kinase inhibitor (TKI) therapy. However, recent reports suggested that transient potent target inhibition of BCR-ABL by highdose TKI (HD-TKI) pulse-exposure is sufficient to irreversibly commit cells to apoptosis. Here, we report a novel mechanism of prolonged intracellular TKI activity upon HD-TKI pulse-exposure (imatinib, dasatinib) in BCR-ABL-positive cells. Comprehensive mechanistic exploration revealed dramatic intracellular accumulation of TKIs which closely correlated with induction of apoptosis. Cells were rescued from apoptosis upon HD-TKI pulse either by repetitive drug wash-out or by overexpression of ABC-family drug transporters. Inhibition of ABCB1 restored sensitivity to HD-TKI pulse-exposure. Thus, our data provide evidence that intracellular drug retention crucially determines biological activity of imatinib and dasatinib. These studies may refine our current thinking on critical requirements of TKI dose and duration of target inhibition for biological activity of TKIs.Daniel B.
