    A novel push-and-pull hybrid data broadcast scheme for wireless information networks

    A new push-and-pull hybrid data broadcast scheme is proposed for providing wireless information services to three types of clients, general, pull and priority clients. Only pull and priority clients have the back channel for sending requests to the broadcast server. There is no scalability problem with the hybrid scheme because the amount of pull and priority clients is very small. Based on the requests collected from pull and priority clients, the server estimates the interest pattern changes of the whole client population. Then the broadcast schedule on the push channel for the next broadcast cycle is adjusted. Besides the push channel, a small amount of broadcast bandwidth is allocated to a pull channel. The data to be broadcast on the pull channel is decided by the server in real-time and priority is given to requests from priority clients. Simulations show that with a time-varying client interest pattern, the average data access time for all three types of clients can be minimized. Because of the priority in using the pull channel, priority clients can achieve the lowest access time and pull clients can achieve a lower access time than general clients.     Age-related differences in neural activities during risk taking as revealed by functional MRI

    Previous research has clearly documented that risky decision making is different in young and older adults. Yet, there has been a relative dearth of research that seeks to understand such age-related changes in the neural activities associated with risk taking. To address this research issue, 21 men (12 young men, mean age 29.9±6.2 years and 9 older men, mean age 65.2±4.2 years) performed a risky-gains task while their brain activities were monitored by an fMRI scanner. The older adults, relative to their younger peers, presented with contralateral prefrontal activity, particularly at the orbitofrontal cortex. Furthermore, stronger activation of the right insula was observed for the older-aged participants compared to the younger-aged adults. The findings of this study are consistent with the a priori speculations established in accordance with the HAROLD model as well as previous findings. Findings of this study suggest that when making risky decisions, there may be possible neuropsychological mechanisms underlying the change in impulsive and risk-taking behaviors during the course of natural ageing. © 2007 The Author(s).published_or_final_versio

    The encapsulation and controlled release of proteins from “meltable” chemically cross-linked hydrogels

Journal of Applied Polymer Science published by Wiley Periodicals LLC.Chemically crosslinked hydrogels can be used for the controlled delivery of therapeutic proteins, however, cargo loading can be challenging. A common approach involves performing the hydrogel crosslinking step in the presence of the cargo proteins, however, this can require the utilization of selective chemistries to ensure the cargos do not chemically react with the forming hydrogel network. We demonstrate that chemically crosslinked hydrogels based upon the protein polymer Capsular antigen fragment 1 (Caf1) can be used to encapsulate proteins within a chemically crosslinked hydrogel network. This encapsulation exploits the “meltable” feature of Caf1 hydrogels, where upon heating the Caf1 protein unfolds with concomitant depolymerization of the hydrogel network. Protein cargos can be loaded into the “molten” form of the hydrogel which cools with protein refolding and concomitant reformation of the hydrogel network to entrap the cargos, ensuring a more homogenous dispersion of the cargo. Analysis of release profiles indicated lower burst release in comparison to release from identical hydrogels loaded by an absorption method. This work is significant because it suggest that the “meltable” feature of Caf1 hydrogels may present an improved method of loading protein cargos into crosslinked hydrogel networks whilst avoiding the need for crosslinking chemistries

    Learning features for tissue classification with the classification restricted Boltzmann machine

    __Abstract__ Performance of automated tissue classification in medical imaging depends on the choice of descriptive features. In this paper, we show how restricted Boltzmann machines (RBMs) can be used to learn features that are especially suited for texture-based tissue classification. We introduce the convolutional classification RBM, a combination of the existing convolutional RBM and classification RBM, and use it for discriminative feature learning. We evaluate the classification accuracy of convolutional and non-convolutional classification RBMs on two lung CT problems. We find that RBM-learned features outperform conventional RBM-based feature learning, which is unsupervised and uses only a generative learning objective, as well as often-used filter banks. We show that a mixture of generative and discriminative learning can produce filters that give a higher classification accuracy

    Is pre-operative urodynamic bladder function the true predictor of outcome of male sling for post prostatectomy incontinence?

    PURPOSE: To investigate pre-operative urodynamic parameters in male sling patients to ascertain whether this might better predict surgical outcomes and facilitate patient selection. METHODS: We performed a retrospective, case notes and video-urodynamics, review of men who underwent AdVanceXP male sling in three London hospitals between 2012 and 2019. Urodynamics were performed in all centres, while retrograde leak point pressure (RLPP) was performed in one centre. RESULTS: Successful outcome was seen in 99/130 (76%) of men who required one pad or less per day. The dry rate was 51%. Pad usage was linked to worse surgical outcomes, mean 2.6 (range 1-6.5) for success vs 3.6 (range 1-10) although the ranges were wide (p = 0.002). 24 h pad weight also reached statistical significance (p = 0.05), with a mean of 181 g for success group versus 475 g for the non-successful group. The incidence of DO in the non-successful group was significantly higher than in successful group (55% versus 29%, p = 0.0009). Bladder capacity less than 250 ml was also associated with worse outcomes (p = 0.003). Reduced compliance was not correlated with outcomes (31% for success groups vs 45% for non-successful group, p = 0.15). Preoperative RLPP was performed in 60/130 patients but did not independently reach statistical significance (p = 0.25). CONCLUSION: Urodynamic parameters related to bladder function-detrusor overactivity and reduced maximum cystometric capacity predict male sling outcomes and may help in patient selection for male sling (or sphincter) surgery; whereas urodynamic parameters of sphincter incompetency (RLPP) were not predictive. Further larger scale studies are required to confirm these findings

    Phylodynamics of HIV-1 Subtype B among the Men-Having-Sex-with-Men (MSM) Population in Hong Kong

    The men-having-sex-with-men (MSM) population has become one of the major risk groups for HIV-1 infection in the Asia Pacific countries. Hong Kong is located in the centre of Asia and the transmission history of HIV-1 subtype B transmission among MSM remained unclear. The aim of this study was to investigate the transmission dynamics of HIV-1 subtype B virus in the Hong Kong MSM population. Samples of 125 HIV-1 subtype B infected MSM patients were recruited in this study. Through this study, the subtype B epidemic in the Hong Kong MSM population was identified spreading mainly among local Chinese who caught infection locally. On the other hand, HIV-1 subtype B infected Caucasian MSM caught infection mainly outside Hong Kong. The Bayesian phylogenetic analysis also indicated that 3 separate subtype B epidemics with divergence dates in the 1990s had occurred. The first and latest epidemics were comparatively small-scaled; spreading among the local Chinese MSM while sauna-visiting was found to be the major sex partner sourcing reservoir for the first subtype B epidemic. However, the second epidemic was spread in a large-scale among local Chinese MSM with a number of them having sourced their sex partners through the internet. The epidemic virus was estimated to have a divergence date in 1987 and the infected population in Hong Kong had a logistic growth throughout the past 20 years. Our study elucidated the evolutionary and demographic history of HIV-1 subtype B virus in Hong Kong MSM population. The understanding of transmission and growth model of the subtype B epidemic provides more information on the HIV-1 transmission among MSM population in other Asia Pacific high-income countries

    Increased Genetic Diversity of HIV-1 Circulating in Hong Kong

    HIV-1 group M strains are characterized into 9 pure subtypes and 48 circulating recombinant forms (CRFs). Recent studies have identified the presence of new HIV-1 recombinants in Hong Kong and their complexity continues to increase. This study aims to characterize the HIV-1 genetic diversity in Hong Kong. Phylogenetic analyses were performed by using HIV-1 pol sequences including protease and partial reverse transcriptase isolated from 1045 local patients in Hong Kong from 2003 to 2008. For the pol sequences with unassigned genotype, the evidence of recombination was determined by using sliding-window based bootscan plots and their env C2V3 region were also sequenced. Epidemiological background of these patients was further collected. The pol phylogenetic analyses highlighted the extent of HIV-1 genetic diversity in Hong Kong. Subtype B (450/1045; 43.1%) and CRF01_AE (469/1045; 44.9%) variants were clearly predominant. Other genotypes (126/1045; 12.1%) including 3 defined subtypes, 10 CRFs, 1 unassigned subtype and 33 recombinants with 11 different mosaic patterns were observed. Recombinants of subtype B and CRF01_AE were mainly found among local Chinese MSM throughout 2004 to 2008, while the CRF02_AG and subtype G recombinants were circulating among non-Chinese Asian population in Hong Kong through heterosexual transmission starting from 2008. Our study demonstrated the complex recombination of HIV-1 in Hong Kong and the need in developing surveillance system for tracking the distribution of new HIV-1 genetic variants
