    Nonsteroidal Anti-inflammatory Drugs Hypersensitivity in Pediatric Patients with Asthma

    Introdução: Os dados publicados sobre a frequência de hipersensibilidade (HS) a anti-inflamatórios não esteróides (AINEs) em doentes asmáticos são discrepantes, sendo escassos na população pediátrica. O objectivo deste estudo foi avaliar a frequência de HS a AINEs, reportada por inquérito telefónico em doentes asmáticos com idade pediátrica. Métodos: Incluíram -se os doentes com idades entre 6 e 17 anos com clínica de asma confirmada por prova de broncodilatação positiva, realizada no período entre 1 de Agosto de 2008 e 30 de Novembro de 2010. Aplicou-se um inquérito telefónico para questionar sobre alguma reacção adversa a fármacos, nomeadamente a AINEs. Perante o reportar de reacção adversa a AINEs, detalhava-se fármaco envolvido, idade na primeira reacção, manifestações clínicas, intervalo entre a toma e o início da reacção, reprodutibilidade, evicção do fármaco implicado e tolerância de fármacos alternativos. Resultados: Foram selecionados, por prova de broncodilatação positiva, 184 doentes. Foi possível aplicar o inquérito a 111/184 (60,3%). A maioria (59,4%) era do género masculino, com 11 ± 3,1 anos. Nove doentes (8,1%) reportaram reacção de HS a AINEs, reprodutível em três. A primeira reacção ocorreu com uma idade mediana de 2 anos (P25:1,8; P75:5,75), abaixo dos 10 anos em todos os doentes. O ibuprofeno foi o fármaco implicado em todos os casos, sendo o paracetamol usado em alternativa. Todos descreveram reacções imediatas, com as seguintes manifestações: respiratória (7), cutânea (3), gastrintestinal (1). A frequência reportada de sintomas respiratórios como manifestação de reacção de HS a AINEs nesta amostra de doentes asmáticos foi de 6,3% (7/111). Quatro doentes (3,6%) estavam sob evicção de AINEs apesar de negarem qualquer reacção de HS prévia. Conclusões: A frequência reportada de HS a AINEs contraria os dados que descrevem estas reacções como infrequentes abaixo dos 10 anos de idade. O paracetamol parece ser uma alternativa segura

    Spondylodiscitis associated with recurrent Serratia bacteremia due to a transjugular intrahepatic portosystemic shunt (TIPS): a case report

    We report a case of spondylodiscitis caused by multiresistant Serratia marcescens in a cirrhotic patient who had several Serratia bacteremias after the placement of a transjugular intrahepatic portosystemic shunt (TIPS) device. We concluded that an endovascular stent that can not be removed makes management of recurrent bacteremia difficult. Furthermore, back pain due to bacteremia is indicative of spondylodiscitis. Serratia marcescens can be an aggressive pathogen, causing spinal infection

    Seroepidemiological survey of transmissible infectious diseases in a portuguese prison establishment

    A cross-sectional study was conducted in 151 (71.6%) of 211 male inmates of a regional Portuguese prison in order to establish the seroprevalence for viral hepatitis (HAV, HBV, HCV), human immunodeficiency virus (HIV), syphilis and herpes simplex virus (HSV-1 and HSV-2) and to analyze some psychosocial and criminal characteristics. Mean age was 34 years. Anti-HAV was positive in 69.5% (n = 105) and in 34.4% (n = 52) for anti-HCV. One (0.7%) person had HBsAg and 29 (19.2%) had laboratory markers of past HBV infection. Non-immune inmates for HBV were 40.4% (n = 61). Syphilis was diagnosed in 6.0% (n = 9). The rate of HIV infection was 6.6% (n = 10; all HIV-1). The seropositivity of HSV-2 was 19.9% (n = 30) and of HSV-1 was 82.1% (n = 124). Alcohol dependence was reported by 26.5% (n = 40). Excluding tobacco and prescription medication, 73.5% (n = 111) reported drug use in prison. The most commonly used drugs were: cannabis (100%; n = 111) followed by heroin (56.7%; n = 63). Anti-HCV rate was noteworthy. The HIV infection rate (6.6%) in this regional prison is at least 13 to 22 times greater than in general population. As the inmate return to the community increases the risk of disease exposure for the general population, early detection and counseling is urgently needed for prisoners

    Towards a Semantic Gas Source Localization under Uncertainty.Communications in Computer and Information Science book series (CCIS, volume 855), doi:10.1007/978-3-319-91479-4_42This work addresses the problem of efficiently and coherently locating a gas source in a domestic environment with a mobile robot, meaning efficiently the coverage of the shortest distance as possible and coherently the consideration of different gas sources explaining the gas presence. The main contribution is the exploitation, for the first time, of semantic relationships between the gases detected and the objects present in the environment to face this challenging issue. Our proposal also takes into account both the uncertainty inherent in the gas classification and object recognition processes. These uncertainties are combined through a probabilistic Bayesian framework to provide a priority-ordered list of (previously observed) objects to check. Moreover the proximity of the different candidates to the current robot location is also considered by a cost function, which output is used for planning the robot inspection path. We have conducted an initial demonstration of the suitability of our gas source localization approach by simulating this task within domestic environments for a variable number of objects, and comparing it with an greedy approach.Universidad de Málaga.     Visceral leishmaniasis and HIV infection in the HAART era

    Visceral Leishmaniasis is an endemic infection in Portugal, as well as in other Mediterranean basin countries, where it has become a frequent complication of HIV infection. There are several studies published about Leishmania/HIV co-infection, however some particularities of its epidemiology, pathogenesis and especially of its treatment and prophylaxis remain unclear and undefined. The authors review some aspects of this co-infection, particularly epidemiology, clinical classic manifestations and laboratory features, diagnosis, treatment, prophylaxis and prevention and report the casuistic of the Infectious Diseases Department of the University Hospital of Coimbra during the last ten years (1996-2006) in the HAART (>) era. Visceral Leishmaniasis behaves as an opportunistic infection in HIV-infected patients and should be considered as an AIDS-defining disease. Nowadays and according to World Health Organization, VL is the second most important protozoan disease and one of the most neglected; therefore the establishment of treatment and prophylaxis guidelines is urgent

    Cervical abscess in an immunocompetent patient with Mycobacterium malmoense pulmonary disease

    BACKGROUND: Mycobacterium malmoense is a nontuberculous mycobacteria seen mainly in two age groups and with different clinical presentations. Most patients are male adults presenting clinical symptoms and signs similar to those of pulmonary tuberculosis. The second group is formed by immunocompetent children with localized cervical lymphadenitis. Although cervical adenitis is the main extrapulmonary manifestation of M. malmoense, virtually all cases of cervical disease were documented in children. Disseminated disease is rare and has been reported in patients with severely impaired immunity. CASE REPORT: We report a case of a 47-year-old immunocompetent man with a cervical abscess, in whom we identified a M. malmoense pulmonary disease with multiple cervical, thoracic and abdominal adenopathies. CONCLUSION: Extrapulmonary infection due to M. Malmoense needs to be considered on the differential diagnosis of cervical masses and adenopathies, not only in pediatric patients but also in adults with no impaired immunity. A high index of suspicion for nontuberculous mycobacteria is essential for the diagnosis and prognosis

    Microbial Interventions to Control and Reduce Blood Pressure in Australia (MICRoBIA): rationale and design of a double-blinded randomised cross-over placebo controlled trial

    Background: Hypertension is a prevalent chronic disease worldwide that remains poorly controlled. Recent studies support the concept that the gut microbiota is involved in the development of hypertension and that dietary fibre intake may act through the gut microbiota to lower blood pressure (BP). Resistant starch is a type of prebiotic fibre which is metabolised by commensal bacteria in the colon to produce short-chain fatty acids (SCFAs), including acetate, propionate, and butyrate. Previous work in pre-clinical models provides strong evidence that both prebiotic fibre as well as SCFAs (i.e. postbiotics) can prevent the development of hypertension. The aim of this clinical trial is to determine if acetylated and butyrylated modified resistant starch can decrease BP of hypertensive individuals via the modulation of the gut microbiota and release of high levels of SCFAs.Methods: This is a phase IIa double-blinded, randomised, cross-over, placebo controlled trial. Participants are randomly allocated to receive either a diet containing 40 g/day of the modified resistant starch or placebo (corn starch or regular flour) for 3 weeks on each diet, with a 3-week washout period between the two diets. BP is measured in the office, at home, and using a 24-h ambulatory device. Arterial stiffness is measured using carotid-to-femoral pulse wave velocity. Our primary endpoint is a reduction in ambulatory daytime systolic BP. Secondary endpoints include changes to circulating cytokines, immune markers, and modulation to the gut microbiome.Discussion: The findings of this study will provide the first evidence for the use of a combination of pre- and postbiotics to lower BP in humans. The results are expected at the end of 2021

    Photosynthesis by six portuguese maize cultivars during drought stress and recovery

    Photosynthesis, chlorophyll fluorescence and leaf water parameters were measured in six Portuguese maize (Zea mays L.) cultivars during and following a period of drought stress. The leaf relative water content (RWC) responded differently among cultivars but, except for cultivar PB369, recovered close to initial values after watering was restored. Photosynthetic rate and stomatal conductance decreased with drought but more slowly in cultivars PB269 and PB260 than in cultivars AD3R, PB64, PB304 and PB369. Water use efficiency (WUE) decreased during the water stress treatment although with cultivar PB260 the decrease was marked only when the RWC fell below 40%. Recovery of WUE was seen with all cultivars except PB369. The maximum quantum efficiency of photosystem II, the photochemical quenching coefficient, the electron transport rate in PSII and the estimated functional plastoquinone pool tended to decrease with drought, while the non -photochemical quenching coefficient increased. The parameters estimated from chlorophyll fluorescence did not recover in PB369, during re - watering. The results show that PB260 and PB269 were the most tolerant and PB369 was the least tolerant cultivars to water stress. The variation found amongst the cultivars tested suggests the existence of valuable genetic resources for crop improvement in relation to drought tolerance

    Identification and Behavioral Evaluation of Sex Pheromone Components of the Chinese Pine Caterpillar Moth, Dendrolimus tabulaeformis

    Background: The Chinese pine caterpillar moth, Dendrolimus tabulaeformis Tsai and Liu (Lepidoptera: Lasiocampidae) is the most important defoliator of coniferous trees in northern China. Outbreaks occur over enormous areas and often lead to the death of forests during 2–3 successive years of defoliation. The sex pheromone of D. tabulaeformis was investigated to define its chemistry and behavioral activity. Methodology/Principal Findings: Sex pheromone was collected from calling female D. tabulaeformis by headspace solid phase microextraction (SPME) and by solvent extraction of pheromone glands. Extracts were analyzed by coupled gas chromatography/mass spectrometry (GC-MS) and coupled GC-electroantennographic detection (GC-EAD), using antennae from male moths. Five components from the extracts elicited antennal responses. These compounds were identified by a combination of retention indices, electron impact mass spectral matches, and derivatization as (Z)-5-dodecenyl acetate (Z5-12:OAc), (Z)-5-dodecenyl alcohol (Z5-12:OH), (5Z,7E)-5,7-dodecadien-1-yl acetate (Z5,E7-12:OAc), (5Z,7E)-5,7-dodecadien-1-yl propionate (Z5,E7-12:OPr), and (5Z,7E)-5,7-dodecadien-1-ol (Z5,E7-12:OH). Behavioral assays showed that male D. tabulaeformis strongly discriminated against incomplete and aberrant blend ratios. The correct ratio of Z5,E7-12:OAc, Z5,E7-12:OH, and Z5,E7-12:OPr was essential for optimal upwind flight and source contact. The two monoenes, Z5-12:OAc and Z5-12:OH, alone or binary mixtures, had no effect on behavioral responses when added to the optimal threecomponen
