    Prediction of the mechanical response of canine humerus to three-point bending using subject-specific finite element modelling

    Subject-specific finite element models could improve decision making in canine long-bone fracture repair. However, it preliminary requires that finite element models predicting the mechanical response of canine long bone are proposed and validated. We present here a combined experimental–numerical approach to test the ability of subject-specific finite element models to predict the bending response of seven pairs of canine humeri directly from medical images. Our results show that bending stiffness and yield load are predicted with a mean absolute error of 10.1% (±5.2%) for the 14 samples. This study constitutes a basis for the forthcoming optimization of canine long-bone fracture repair

    Interactive Proofs with Approximately Commuting Provers

    The class MIP∗ of promise problems that can be decided through an interactive proof system with multiple entangled provers provides a complexity-theoretic framework for the exploration of the nonlocal properties of entanglement. Very little is known in terms of the power of this class. The only proposed approach for establishing upper bounds is based on a hierarchy of semidefinite programs introduced independently by Pironio et al. and Doherty et al. in 2006. This hierarchy converges to a value, the field-theoretic value, that is only known to coincide with the provers’ maximum success probability in a given proof system under a plausible but difficult mathematical conjecture, Connes’ embedding conjecture. No bounds on the rate of convergence are known. We introduce a rounding scheme for the hierarchy, establishing that any solution to its N -th level can be mapped to a strategy for the provers in which measurement operators associated with distinct provers have pairwise commutator bounded by O(ℓ^2/√N) in operator norm, where ℓ is the number of possible answers per prover. Our rounding scheme motivates the introduction of a variant of quantum multiprover interactive proof systems, called MIP∗_δ in which the soundness property is required to hold against provers allowed to operate on the same Hilbert space as long as the commutator of operations performed by distinct provers has norm at most δ. Our rounding scheme implies the upper bound MIP∗_δ ⊆ DTIME(exp(exp(poly)/δ^2)). In terms of lower bounds we establish that MIP∗_(2−poly) contains NEXP with completeness 1 and soundness 1−2^(−poly). We discuss connections with the mathematical literature on approximate commutation and applications to device-independent cryptography

    Background This article aims to comprehensively describe indications, contraindications, technical aspects, diagnostic accuracy and complications of percutaneous lung biopsy. Methods Imaging-guided biopsy currently represents one of the predominant methods for obtaining tissue specimens in patients with lung nodules; in many cases treatment protocols are based on histological information; thus, biopsy is frequently performed, when technically feasible, or in case other techniques (such as bronchoscopy with lavage) are inconclusive. Results Although a coaxial system is suitable in any case, two categories of needles can be used: fine-needle aspiration biopsy (FNAB) and core-needle biopsy (CNB), with the latter demonstrated to have a slightly higher overall sensitivity, specificity and accuracy. Conclusion Percutaneous lung biopsy is a safe procedure even though a few complications are possible: pneumothorax, pulmonary haemorrhage and haemoptysis are common complications, while air embolism and seeding are rare, but potentially fatal complications

    <p><b>Purpose</b></p> <p>PERFORM is exploring the efficacy of terutroban versus aspirin for secondary prevention in patients with a history of ischemic stroke or transient ischemic attacks (TIAs). The PERFORM Vascular Project will evaluate the effect of terutroban on progression of atherosclerosis, as assessed by change in carotid intima-media thickness (CIMT) in a subgroup of patients.</p> <p><b>Methods and results</b></p> <p>The Vascular Project includes structural (CIMT, carotid plaques) and functional (carotid stiffness) vascular studies in all patients showing at least one carotid plaque at entry. Expected mean follow-up is 36 months. Primary endpoint is rate of change of CIMT. Secondary endpoints include emergent plaques and assessment of carotid stiffness. 1,100 patients are required for 90% statistical power to detect treatment-related CIMT difference of 0.025 mm. The first patient was randomized in April 2006.</p> <p><b>Conclusions</b></p> <p>The PERFORM Vascular Project will investigate terutroban’s effect on vascular structure and function in patients with a history of ischemic stroke or TIAs.</p&gt

    Airway and Esophageal Stenting in Patients with Advanced Esophageal Cancer and Pulmonary Involvement

    BACKGROUND: Most inoperable patients with esophageal-advanced cancer (EGC) have a poor prognosis. Esophageal stenting, as part of a palliative therapy management has dramatically improved the quality of live of EGC patients. Airway stenting is generally proposed in case of esophageal stent complication, with a high failure rate. The study was conducted to assess the efficacy and safety of scheduled and non-scheduled airway stenting in case of indicated esophageal stenting for EGC. METHODS AND FINDINGS: The study is an observational study conducted in pulmonary and gastroenterology endoscopy units. Consecutive patients with EGC were referred to endoscopy units. We analyzed the outcome of airway stenting in patients with esophageal stent indication admitted in emergency or with a scheduled intervention. Forty-four patients (58+/-\-8 years of age) with esophageal stenting indication were investigated. Seven patients (group 1) were admitted in emergency due to esophageal stent complication in the airway (4 fistulas, 3 cases with malignant infiltration and compression). Airway stenting failed for 5 patients. Thirty-seven remaining patients had a scheduled stenting procedure (group 2): stent was inserted for 13 patients with tracheal or bronchial malignant infiltration, 12 patients with fistulas, and 12 patients with airway extrinsic compression (preventive indication). Stenting the airway was well tolerated. Life-threatening complications were related to group 1. Overall mean survival was 26+/-10 weeks and was significantly shorter in group 1 (6+/-7.6 weeks) than in group 2 (28+/-11 weeks), p<0.001). Scheduled double stenting significantly improved symptoms (95% at day 7) with a low complication rate (13%), and achieved a specific cancer treatment (84%) in most cases. CONCLUSION: Stenting the airway should always be considered in case of esophageal stent indication. A multidisciplinary approach with initial airway evaluation improved prognosis and decreased airways complications related to esophageal stent. Emergency procedures were rarely efficient in our experience
