    Once a pond in time: employing palaeoecology to inform farmland pond restoration

    The restoration of highly terrestrialized farmland ponds that combines the removal of woody vegetation and pond sediment greatly enhances aquatic biodiversity. Nonetheless, questions remain regarding the historical precedent of pond restoration, and particularly if post-restoration aquatic macrophyte communities resemble pre-terrestrialization assemblages. We used a paleoecological approach to address these questions for a typical, recently restored farmland pond in Norfolk, eastern England. Plant and animal remains in pond sediment cores were used to infer decadal-centennial scale changes to pond communities and to identify past pond management events. We then evaluated the resemblance of restored and historical assemblages by comparisons with contemporary post-restoration vegetation data. Based on changes in the abundance of terrestrial leaf remains and other indicators (increases followed by declines of aquatic organisms), the study pond appears to have a long history (going back to the early-1800s) of canopy management (at least three inferred management events), but after the mid-1970s, steady and substantial increases in terrestrial indicators, suggest cessation of management resulting in uninterrupted terrestrialization. Aquatic macrophyte communities arising after restoration showed some similarities with historical assemblages, but also contained apparently new species. This study demonstrates how paleolimnological methods can improve understanding of pond ecological histories to better inform restoration targets and practices. Implications for Practice Paleolimnological methods can be successfully employed at small, human-made ponds to assess past biological communities and trajectories of ecological change. Restoration of heavily terrestrialized farmland ponds through major woody vegetation and sediment removal mimics periodic management activities undertaken over past centuries and is essential to the maintenance of open canopy conditions and biodiversity conservation. Caution must be taken when setting restoration targets for farmland ponds as rare macrophyte species indicative of high water quality may not necessarily return to restored pond habitats due to fragmentation effects associated with the loss of local populations and/or in-pond eutrophication development

    Improving the pollinator pantry: Restoration and management of open farmland ponds enhances the complexity of plant-pollinator networks

    In line with general biodiversity losses across agricultural landscapes, insect pollinators have experienced recent sharp declines. A range of conservation measures have been developed to address these declines, with plant-pollinator interaction networks providing key insights into the effectiveness of these measures. For the first time, we studied interactions between three diurnal pollinator groups (bees, hoverflies, and butterflies) and insect-pollinated plants to understand how they are affected by pond management and restoration. Major network contributors were identified, and important network-level parameters compared at nine farmland ponds under different management strategies to assess management effects on plant-pollinator interactions: three ‘overgrown’ tree-covered ponds, three ‘long-term managed ponds’ kept in an open-canopy, early- to mid-successional state by periodic interventions involving tree and sediment removal, and three ‘recently restored ponds’, initially heavily overgrown with woody vegetation, and subsequently rapidly transformed into an early succession state through major tree and sediment removal. Interaction complexity, as measured by the metrics ‘links per species’, ‘linkage density’, Fisher’s alpha and Shannon’s Diversity, was higher for both long-term managed and recently restored ponds compared to overgrown ponds. Several network-level parameters indicated that highest complexity levels were found at recently restored ponds due to their substantially higher plant diversity. Bipartite interaction analysis suggests major benefits of pond management and restoration for agricultural pollinator assemblages. We strongly advocate the inclusion of ponds in conservation strategies and policies aimed at pollinators - ponds should be part of the pollinator pantry

    Patients, health professionals, and the health system: Influencers on patients’ participation in ward rounds

    Background: The ward round is an opportunity to plan and deliver patient-centered care. Benefits include an effective and safer clinician-patient relationship, patient empowerment, reduced anxiety and increased trust in the health care system. Factors contributing to patient involvement in ward rounds is shaped by their preferences, ability, and opportunity. Aim: To investigate ward rounds and the patient experience with them, the relationship between the patient and clinicians, and how rounds facilitate collaboration between them. Patients and methods: A multimethod study was conducted in a single Australian facility in acute medicine and rehabilitation specialties. An observational study of ward rounds in each setting was conducted with 14 patients, aged between 55 and 89 years followed by semi-structured interviews conducted with the patients observed. Descriptive and thematic analysis was undertaken. Results: One third of participants had not heard of the term ward round or could describe their purpose. Three main influencers on the patient experience of rounds were: self; the health system; and medical officers. No meaningful difference was found between patients in acute medicine and rehabilitation although all wanted to receive information from the senior medical officers. Patients more familiar with the health system were more active participants and took greater responsibility for their involvement in rounds and described higher satisfaction. Conclusion: There is a level of acceptance within the health system that patients understand what a ward round is. However, their role on the round is complex and this may only be developed through experiencing them. High system users teach themselves to navigate rounding processes to ensure their needs are met. To ensure equity in participation patients should be educated on ward rounds, what to expect and how to they can participate

    How do interprofessional healthcare teams perceive the benefits and challenges of interdisciplinary ward rounds

    Purpose: Interdisciplinary bedside ward rounds have the capacity to facilitate coordinated interprofessional patient care. To be an effective means of care coordination, clinicians need an explicit understanding of how these rounds contribute to patient care. By identifying benefits and challenges to the effective use of interdisciplinary ward rounds, clinicians create an opportunity to improve interprofessional teamwork, care planning, and coordination of patient care. Methods: A survey was conducted with frontline professionals in two acute care and two rehabilitation wards from a metropolitan teaching hospital. There were 77 participants, representing medical officers, nurses, and allied health clinicians. Questions examined the perceived benefits and challenges of conducting interdisciplinary ward rounds in their units. Survey findings were coded for meaning and then grouped into themes. Results: Benefits revealed a desired care delivery model challenged by the complexities of organizational and professional cultures. The themes of “being on the same page”, “focusing on patients”, and “holistic care planning” underpinned the ideas of collaboration and improved patient-centred care, that is, benefits to patients. Challenges centred on health professionals' time constraints and the coordination of teams to enable participation in rounds. The themes were more distinct, logistical barriers of “time”, “workforce”, and “care planning”. Conclusion: Overall, clinicians recognise there are greater benefits to IBRs and have a willingness to participate. However, careful consideration is required to introduce and continually achieve the best from IBR as they require changes in organizational context and culture

    Seasonal variation in collective mood via Twitter content and medical purchases

    The analysis of sentiment contained in vast amounts of Twitter messages has reliably shown seasonal patterns of variation in multiple studies, a finding that can have great importance in the understanding of seasonal affective disorders, particularly if related with known seasonal variations in certain hormones. An important question, however, is that of directly linking the signals coming from Twitter with other sources of evidence about average mood changes. Specifically we compare Twitter signals relative to anxiety, sadness, anger, and fatigue with purchase of items related to anxiety, stress and fatigue at a major UK Health and Beauty retailer. Results show that all of these signals are highly correlated and strongly seasonal, being under-expressed in the summer and over-expressed in the other seasons, with interesting differences and similarities across them. Anxiety signals, extracted from both Twitter and from Health product purchases, peak in spring and autumn, and correlate also with the purchase of stress remedies, while Twitter sadness has a peak in the Winter, along with Twitter anger and remedies for fatigue. Surprisingly, purchase of remedies for fatigue do not match the Twitter fatigue, suggesting that perhaps the names we give to these indicators are only approximate indications of what they actually measure. This study contributes both to the clarification of the mood signals contained in social media, and more generally to our understanding of seasonal cycles in collective mood

    A short-term mouse model that reproduces the immunopathological features of rhinovirus-induced exacerbation of COPD

    © 2015 The Author(s). Viral exacerbations of chronic obstructive pulmonary disease (COPD), commonly caused by rhinovirus (RV) infections, are poorly controlled by current therapies. This is due to a lack of understanding of the underlying immunopathological mechanisms. Human studies have identified a number of key immune responses that are associated with RV-induced exacerbations including neutrophilic inflammation, expression of inflammatory cytokines and deficiencies in innate anti-viral interferon. Animal models of COPD exacerbation are required to determine the contribution of these responses to disease pathogenesis. We aimed to develop a short-term mouse model that reproduced the hallmark features of RV-induced exacerbation of COPD. Evaluation of complex protocols involving multiple dose elastase and lipopolysaccharide (LPS) administration combined with RV1B infection showed suppression rather than enhancement of inflammatory parameters compared with control mice infected with RV1B alone. Therefore, these approaches did not accurately model the enhanced inflammation associated with RV infection in patients with COPD compared with healthy subjects. In contrast, a single elastase treatment followed by RV infection led to heightened airway neutrophilic and lymphocytic inflammation, increased expression of tumour necrosis factor (TNF)-α, C-X-C motif chemokine 10 (CXCL10)/IP-10 (interferon γ-induced protein 10) and CCL5 [chemokine (C-C motif) ligand 5]/RANTES (regulated on activation, normal T-cell expressed and secreted), mucus hypersecretion and preliminary evidence for increased airway hyper-responsiveness compared with mice treated with elastase or RV infection alone. In summary, we have developed a new mouse model of RV-induced COPD exacerbation that mimics many of the inflammatory features of human disease. This model, in conjunction with human models of disease, will provide an essential tool for studying disease mechanisms and allow testing of novel therapies with potential to be translated into clinical practice

    The incidence of total hip arthroplasty after hip arthroscopy in osteoarthritic patients

    <p>Abstract</p> <p>Objective</p> <p>To assess the incidence of total hip arthroplasty (THA) in osteoarthritic patients who were treated by arthroscopic debridement and to evaluate factors that might influence the time interval from the first hip arthroscopy to THA.</p> <p>Design</p> <p>Retrospective clinical series</p> <p>Methods</p> <p>Follow-up data and surgical reports were retrieved from 564 records of osteoarthritic patients that have had hip arthroscopy between the years 2002 to 2009 with a mean follow-up time of 3.2 years (range, 1-6.4 years). The time interval between the first hip arthroscopy to THA was modelled as a function of patient age; level of cartilage damage; procedures performed and repeated arthroscopies with the use of multivariate regression analysis.</p> <p>Results</p> <p>Ninety (16%) of all participants eventually required THA. The awaiting time from the first arthroscopy to a hip replacement was found to be longer in patients younger than 55 years and in a milder osteoarthritic stage. Patients that experienced repeated hip scopes had a longer time to THA than those with only a single procedure. Procedures performed concomitant with debridement and lavage did not affect the time interval to THA.</p> <p>Conclusions</p> <p>In our series of arthroscopic treatment of hip osteoarthritis, 16% required THA over a period of 7 years. Factors that influence the time to arthroplasty were age, degree of osteoarthritis and recurrent procedures.</p
