Association of osteoarthritis risk factors with knee and hip pain in a population-based sample of 29-59 year olds in Denmark: a cross-sectional analysis.
BACKGROUND: This study aimed to a) describe the prevalence of knee and hip osteoarthritis risk factors in a population of 29-59 year old individuals, b) estimate the association between persistent knee/hip pain and osteoarthritis risk factors, and c) describe the prevalence of osteoarthritis risk factors, including specific biomechanical risk factors, in individuals with prolonged persistent knee or hip pain. METHODS: Participants completed the "Early Detection and Prevention" pilot study questionnaire, including items on presence of knee/hip pain within the last month and osteoarthritis risk factors. Individuals reporting knee/hip problems completed a second questionnaire, including items about most problematic joint and specific biomechanical osteoarthritis risk factors. After describing the prevalence of persistent knee/hip pain and osteoarthritis risk factors among respondents stratified for sex and age, logistic regression was used to estimate the strength of associations between osteoarthritis risk factors and presence of knee/hip pain. The prevalence of prolonged persistent pain (i.e. knee/hip pain reported at both questionnaires) and osteoarthritis risk factors among respondents with prolonged persistent knee and hip pain, were described. RESULTS: Two thousand six hundred sixty-one respondents completed the first survey. The one-month prevalence of persistent knee/hip pain was 27%. Previous knee/hip injury was associated with persistent knee/hip pain for both sexes in all age groups, while a family history of osteoarthritis was associated with persistent knee/hip pain in all age groups except for 29-39 year old men. A higher BMI was associated with persistent knee/hip pain in 40-59 year old women, and 50-59 year old men. Eight hundred sixty seven respondents completed the second questionnaire. Knee/hip injuries and surgeries were more common in individuals with prolonged persistent knee than hip pain. CONCLUSIONS: Knee/hip pain within the last month was frequent among individuals aged 29-59 years. Multiple known osteoarthritis risk factors were associated with presence of knee/hip pain. Joint injury and previous surgery were more common in individuals with knee than hip pain. The results support the notion that joint injury and overweight during early adulthood are signs of a trajectory towards symptomatic osteoarthritis later in life and may help earlier identification of groups at high risk of future symptomatic osteoarthritis. Supergravity Solutions from Floating Branes
We solve the equations of motion of five-dimensional ungauged supergravity
coupled to three U(1) gauge fields using a floating-brane Ansatz in which the
electric potentials are directly related to the gravitational warp factors. We
find a new class of non-BPS solutions, that can be obtained linearly starting
from an Euclidean four-dimensional Einstein-Maxwell base. This class - the
largest known so far - reduces to the BPS and almost-BPS solutions in certain
limits. We solve the equations explicitly when the base space is given by the
Israel-Wilson metric, and obtain solutions describing non-BPS D6 and anti-D6
branes kept in equilibrium by flux. We also examine the action of spectral flow
on solutions with an Israel-Wilson base and show that it relates these
solutions to almost-BPS solutions with a Gibbons-Hawking base.Comment: 24 pages, 1 figur
Proximity to Sports Facilities and Sports Participation for Adolescents in Germany
Objectives - To assess the relationship between proximity to specific sports facilities and participation in the corresponding sports activities for adolescents in Germany.
Methods - A sample of 1,768 adolescents aged 11–17 years old and living in 161 German communities was examined. Distances to the nearest sports facilities were calculated as an indicator of proximity to sports facilities using Geographic Information Systems (GIS). Participation in specific leisure-time sports activities in sports clubs was assessed using a self-report questionnaire and individual-level socio-demographic variables were derived from a parent questionnaire. Community-level socio-demographics as covariates were selected from the INKAR database, in particular from indicators and maps on land development. Logistic regression analyses were conducted to examine associations between proximity to the nearest sports facilities and participation in the corresponding sports activities.
Results - The logisitic regression analyses showed that girls residing longer distances from the nearest gym were less likely to engage in indoor sports activities; a significant interaction between distances to gyms and level of urbanization was identified. Decomposition of the interaction term showed that for adolescent girls living in rural areas participation in indoor sports activities was positively associated with gym proximity. Proximity to tennis courts and indoor pools was not associated with participation in tennis or water sports, respectively.
Conclusions - Improved proximity to gyms is likely to be more important for female adolescents living in rural areas
Economic and other barriers to adopting recommendations to prevent childhood obesity: results of a focus group study with parents
Abstract Background Parents are integral to the implementation of obesity prevention and management recommendations for children. Exploration of barriers to and facilitators of parental decisions to adopt obesity prevention recommendations will inform future efforts to reduce childhood obesity. Methods We conducted 4 focus groups (2 English, 2 Spanish) among a total of 19 parents of overweight (BMI ≥ 85th percentile) children aged 5-17 years. The main discussion focused on 7 common obesity prevention recommendations: reducing television (TV) watching, removing TV from child's bedroom, increasing physically active games, participating in community or school-based athletics, walking to school, walking more in general, and eating less fast food. Parents were asked to discuss what factors would make each recommendation more difficult (barriers) or easier (facilitators) to follow. Participants were also asked about the relative importance of economic (time and dollar costs/savings) barriers and facilitators if these were not brought into the discussion unprompted. Results Parents identified many barriers but few facilitators to adopting obesity prevention recommendations for their children. Members of all groups identified economic barriers (time and dollar costs) among a variety of pertinent barriers, although the discussion of dollar costs often required prompting. Parents cited other barriers including child preference, difficulty with changing habits, lack of information, lack of transportation, difficulty with monitoring child behavior, need for assistance from family members, parity with other family members, and neighborhood walking safety. Facilitators identified included access to physical activity programs, availability of alternatives to fast food and TV which are acceptable to the child, enlisting outside support, dietary information, involving the child, setting limits, making behavior changes gradually, and parental change in shopping behaviors and own eating behaviors. Conclusions Parents identify numerous barriers to adopting obesity prevention recommendations, most notably child and family preferences and resistance to change, but also economic barriers. Toward optimal implementation of cancer prevention and control programs in public health: A study protocol on mis-implementation
Abstract Background Much of the cancer burden in the USA is preventable, through application of existing knowledge. State-level funders and public health practitioners are in ideal positions to affect programs and policies related to cancer control. Mis-implementation refers to ending effective programs and policies prematurely or continuing ineffective ones. Greater attention to mis-implementation should lead to use of effective interventions and more efficient expenditure of resources, which in the long term, will lead to more positive cancer outcomes. Methods This is a three-phase study that takes a comprehensive approach, leading to the elucidation of tactics for addressing mis-implementation. Phase 1: We assess the extent to which mis-implementation is occurring among state cancer control programs in public health. This initial phase will involve a survey of 800 practitioners representing all states. The programs represented will span the full continuum of cancer control, from primary prevention to survivorship. Phase 2: Using data from phase 1 to identify organizations in which mis-implementation is particularly high or low, the team will conduct eight comparative case studies to get a richer understanding of mis-implementation and to understand contextual differences. These case studies will highlight lessons learned about mis-implementation and identify hypothesized drivers. Phase 3: Agent-based modeling will be used to identify dynamic interactions between individual capacity, organizational capacity, use of evidence, funding, and external factors driving mis-implementation. The team will then translate and disseminate findings from phases 1 to 3 to practitioners and practice-related stakeholders to support the reduction of mis-implementation. Discussion This study is innovative and significant because it will (1) be the first to refine and further develop reliable and valid measures of mis-implementation of public health programs; (2) bring together a strong, transdisciplinary team with significant expertise in practice-based research; (3) use agent-based modeling to address cancer control implementation; and (4) use a participatory, evidence-based, stakeholder-driven approach that will identify key leverage points for addressing mis-implementation among state public health programs. This research is expected to provide replicable computational simulation models that can identify leverage points and public health system dynamics to reduce mis-implementation in cancer control and may be of interest to other health areas
Constructing near-horizon geometries in supergravities with hidden symmetry
We consider the classification of near-horizon geometries in a general
two-derivative theory of gravity coupled to abelian gauge fields and uncharged
scalars in four and five dimensions, with one and two commuting rotational
symmetries respectively. Assuming that the theory of gravity reduces to a 3d
non-linear sigma model (as is typically the case for ungauged supergravities),
we show that the functional form of any such near-horizon geometry may be
determined. As an example we apply this to five dimensional minimal
supergravity. We also construct an example of a five parameter near-horizon
geometry solution to this theory with S^1 X S^2 horizon topology. We discuss
its relation to the near-horizon geometries of the yet to be constructed

Black holes and black strings of N=2, d=5 supergravity in the H-FGK formalism
We study general classes and properties of extremal and non-extremal static
black-hole solutions of N=2, d=5 supergravity coupled to vector multiplets
using the recently proposed H-FGK formalism, which we also extend to static
black strings. We explain how to determine the integration constants and
physical parameters of the black-hole and black-string solutions. We derive
some model-independent statements, including the transformation of non-extremal
flow equations to the form of those for the extremal flow. We apply our methods
to the construction of example solutions (among others a new extremal string
solution of heterotic string theory on K_3 \times S^1). In the cases where we
have calculated it explicitly, the product of areas of the inner and outer
horizon of a non-extremal solution coincides with the square of the
moduli-independent area of the horizon of the extremal solution with the same

State-space Manifold and Rotating Black Holes
We study a class of fluctuating higher dimensional black hole configurations
obtained in string theory/ -theory compactifications. We explore the
intrinsic Riemannian geometric nature of Gaussian fluctuations arising from the
Hessian of the coarse graining entropy, defined over an ensemble of brane
microstates. It has been shown that the state-space geometry spanned by the set
of invariant parameters is non-degenerate, regular and has a negative scalar
curvature for the rotating Myers-Perry black holes, Kaluza-Klein black holes,
supersymmetric black holes, - configurations and the
associated BMPV black holes. Interestingly, these solutions demonstrate that
the principal components of the state-space metric tensor admit a positive
definite form, while the off diagonal components do not. Furthermore, the ratio
of diagonal components weakens relatively faster than the off diagonal
components, and thus they swiftly come into an equilibrium statistical
configuration. Novel aspects of the scaling property suggest that the
brane-brane statistical pair correlation functions divulge an asymmetric
nature, in comparison with the others. This approach indicates that all above
configurations are effectively attractive and stable, on an arbitrary
hyper-surface of the state-space manifolds. It is nevertheless noticed that
there exists an intriguing relationship between non-ideal inter-brane
statistical interactions and phase transitions. The ramifications thus
described are consistent with the existing picture of the microscopic CFTs. We
conclude with an extended discussion of the implications of this work for the

Stationary Black Holes: Uniqueness and Beyond
The spectrum of known black-hole solutions to the stationary Einstein
equations has been steadily increasing, sometimes in unexpected ways. In
particular, it has turned out that not all black-hole-equilibrium
configurations are characterized by their mass, angular momentum and global
charges. Moreover, the high degree of symmetry displayed by vacuum and
electro-vacuum black-hole spacetimes ceases to exist in self-gravitating
non-linear field theories. This text aims to review some developments in the
subject and to discuss them in light of the uniqueness theorem for the

A secondary RET mutation in the activation loop conferring resistance to vandetanib
Resistance to vandetanib, a type I RET kinase inhibitor, developed in a patient with metastatic lung adenocarcinoma harboring a CCDC6-RET fusion that initially exhibited a response to treatment. The resistant tumor acquired a secondary mutation resulting in a serine-to-phenylalanine substitution at codon 904 in the activation loop of the RET kinase domain. The S904F mutation confers resistance to vandetanib by increasing the ATP affinity and autophosphorylation activity of RET kinase. A reduced interaction with the drug is also observed in vitro for the S904F mutant by thermal shift assay. A crystal structure of the S904F mutant reveals a small hydrophobic core around F904 likely to enhance basal kinase activity by stabilizing an active conformer. Our findings indicate that missense mutations in the activation loop of the kinase domain are able to increase kinase activity and confer drug resistance through allosteric effects
