    Investigating the function of the ventral visual reading pathway and its involvement in acquired reading disorders

    This thesis investigated the role of the left ventral occipitotemporal (vOT) cortex and how damage to this area causes peripheral reading disorders. Functional magnetic resonance imaging (fMRI) studies in volunteers demonstrated that the left vOT is activated by written words over numbers or perceptually-matched baselines, irrespective of the word’s location on the visual field. Mixed results were observed for the comparison of words versus false font stimuli. This response profile suggests that the left vOT is preferentially activated by words or word-like stimuli, due to either: (1) bottom-up specialisation for processing familiar word-forms; (2) top-down task-dependent modulation, or (3) a combination of the two. Further studies are proposed to discriminate between these possibilities. Thirteen patients with left occipitotemporal damage participated in the rehabilitation and fMRI studies. The patients were impaired on word, text and letter reading. A structural analysis showed that damage to the left occipitotemporal white matter, in the vicinity of the inferior longitudinal fasciculus, was associated with slow word reading speed. The fMRI study showed that the patients had reduced activation of the bilateral posterior superior temporal sulci relative to controls. Activity in this area correlated with reading speed. The efficacy of intensive whole-word recognition training was tested. Immediately after the training, trained words were read faster than untrained words, but the effects did not persist until the follow-up assessment. Hence, damage to the left vOT white matter impairs rapid whole-word recognition and is resistant to rehabilitation. The final study investigated the role of spatial frequency (SF) in the lateralisation of vOT function. Lateralisation of high and low SF processing was demonstrated, concordant with the lateralisation for words and faces to the left and right vOT respectively. A perceptual basis for the organisation of vOT cortex might explain why left vOT damage is resistant to treatment

    Mantle heterogeneity during the formation of the North Atlantic Igneous Province: Constraints from trace element and Sr-Nd-Os-O isotope systematics of Baffin Island picrites

    Sr-Nd-Os-O isotope and major and trace element data from ~62 Ma picrites from Baffin Island constrain the composition of mantle sources sampled at the inception of North Atlantic Igneous Province (NAIP) magmatism. We recognize two compositional types. Depleted (N-type) lavas have low 87Sr/86Sri (0.702990–0.703060) and 187Os/188Osi (0.1220–0.1247) and high 143Nd/144Ndi (0.512989–0.512999) and are depleted in incompatible elements relative to primitive mantle. Enriched (E-type) lavas have higher 87Sr/86Sri (0.703306–0.703851) and 187Os/188Osi (0.1261–0.1303), lower 143Nd/144Ndi (0.512825–0.512906), and incompatible element concentrations similar to, or more enriched than, primitive mantle. There is also a subtle difference in oxygen isotope composition; E-type lavas are marginally lower in δ18Oolivine value (5.16–4.84‰) than N-type lavas (5.15–5.22‰). Chemical and isotopic variations between E- and N-type lavas are inconsistent with assimilation of crust and/or subcontinental lithospheric mantle and appear to instead reflect mixing between melts derived from two distinct mantle sources. Strontium-Nd-O isotope compositions and incompatible trace element abundances of N-type lavas suggest these are largely derived from the depleted upper mantle. The 187Os/188Osi ratios of N-type lavas can also be explained by such a model but require that the depleted upper mantle had γOs of approximately −5 to −7 at 62 Ma. This range overlaps the lowest γOs values measured in abyssal peridotites. Baffin Island lava compositions are also permissive of a model involving recharging of depleted upper mantle with 3He-rich material from the lower mantle (Stuart et al., Nature, 424, 57–59, 2003), with the proviso that recharge had no recognizable effect on the lithophile trace element and Sr-Nd-Os-O isotope composition. The origin of the enriched mantle component sampled by Baffin Island lavas is less clear but may be metasomatized and high-temperature-altered recycled oceanic lithosphere transported within the proto Iceland plume. Differences between Baffin Island lavas and modern Icelandic basalts suggest that a range of enriched and depleted mantle sources have been tapped since the inception of magmatism in the province. Similarities between Baffin Island lavas erupted and those of similar age from East and West Greenland also suggest that the enriched component in Baffin Island lavas may have been sampled by lavas erupted over a wide geographic range

    Increasing Light Absorption and Collection Using Engineered Structures

    In recent years we have witnessed an explosion of interest in two dimensional (2D) materials, due to their unique physical properties. Excitement surrounds the promise of replacing conventional bulk photodetectors with devices based on 2D materials, allowing better integration, flexibility and potentially improving performance. However, the low inherent light absorption of 2D materials is an outstanding issue to be solved. In this chapter we review two independent approaches to tackling this problem, which have the potential to be combined to find a robust solution. The first approach involves patterning the substrate with a rod-type photonic crystal (PhC) cavity structure, which is shown to increase the light absorption into a 2D material flake coupled spatially to the cavity mode. Secondly, we review 2D–compatible solid immersion lenses (SILs) and their ability to increase both the optical magnification of the structures they encapsulate, and the longevity of the material. SILs have been shown to reduce the requirements for complex optics in the implementation of 2D materials in optoelectronic devices, and also in preserving the photodetector’s optical performance over long periods of time. Finally, we show how by combining rod-type PhC cavities with SILs, we can improve the performance of 2D material-based photodetectors

    Orthographic neighborhood effects during lateralized lexical decision are abolished with bilateral presentation

    Words presented to the right visual field (RVF) are processed more rapidly than those in the left visual field (LVF), presumably because of more direct links to the language dominant left cerebral hemisphere. This effect is moderated by a word’s orthographic neighborhood size (N), with LVF facilitation and RVF inhibition for words with a large N. Across two experiments, we sought to further examine lateralized N effects. Experiment 1 examined how hemispheric dominance for language influenced lateralized N effects, in 140 left-handers using a visual half-field task with bilateral presentation. Neither participants with a right ear advantage on a dichotic listening task nor participants with no right ear advantage showed the expected N effect, making the results ambiguous: it could be that left-handers fail to show N effects, or the effect could be abolished by some procedural aspect. Experiment 2 looked to test these options by testing 56 right-handers who responded to the same stimulus set under the original bilateral presentation condition and under unilateral presentation. N effects were found under unilateral but not bilateral presentation. We had adopted bilateral presentation because it had been recommended as better than unilateral presentation for controlling fixation and visual stimulation; our results indicate that this is not a minor methodological modification: it can dramatically affect lateralized N effects.     Review of Injection Therapy Clinics Performed by Physiotherapists Working in Primary Care.

    Objectives To determine painscores for injection therapy and to review the overall change in outcomes post injection for a variety of musculoskeletal injections compared with the previous audits of a NHS Primary care Physiotherapy injection clinic. Methods Subjects were assessed using a numerical rating scale between 0 to 10 before and 4 weeks post injection. Results All average painscale scores were reduced 4 weeks after injection for all conditions. There is an overall average reduction in pain on provoking activity of 45.7%. Trigger finger/thumb (65.0%) demonstrated the most significant reduction in pain. Contrary to a common patient perception, injection therapy is not an exceptionally painful experience, with an average score of 3.2. Discussion Results are comparable with previous scores for injections carried out within the same NHS Primary care Physiotherapy clinic

    An investigation of the laser-induced zircon 'matrix effect'

    This study aims to improve our understanding of the current limitations to high-precision U-Pb analysis of zircon by LA-ICP-MS by investigating the underlying causes of variation in ablation behaviour between different zircon matrices. Multiple factors such as: the degree of accumulated radiation damage, trace element composition; crystal colour; and crystallographic orientation are all systematically investigated. Due to the marked decrease in elastic moduli of natural zircon crystals with increasing radiation damage, the accumulation of this damage is the dominant factor controlling the rate of ablation for partially damaged to highly metamict zircon samples. There are slight differences, however, in ablation behaviour between highly crystalline matrices that cannot be attributed solely to differences in the degree of accumulated radiation damage. These differences are associated with structural weakening (i.e., decrease in elastic moduli and overall lower mechanical resistance) caused by an increasing degree of cation substitution in some of the zircon samples. Effects of crystallographic orientation and of crystal opacity (i.e., colour) on ablation behaviour are negligible compared to the combined influences of accumulated radiation damage and trace element substitution into the zircon structure. Experiments performed on natural and annealed zircon grains reveal that the reduction in ablation rates observed for the treated samples compared to the untreated grains is proportional to the degree of structural reconstitution achieved after annealing. Thermal annealing of natural zircon at temperatures >1000 °C results in much more uniform ablation characteristics. This 'homogenisation' of ablation behaviour between zircon matrices produces a decrease in the laser-induced matrix effects and subsequent improvement in the accuracy of 206Pb/238U ratio determinations by LA-ICP-MS

    Cervical and breast cancer screening uptake among women with serious mental illness: a data linkage study

    Background Breast and cancer screening uptake has been found to be lower among women with serious mental illness (SMI). This study aims to corroborate these findings in the UK and to identify variation in screening uptake by illness/treatment factors, and primary care consultation frequency. Methods Linked population-based primary and secondary care data from the London borough of Lambeth (UK) were used to compare breast and cervical screening receipt among linked eligible SMI patients (n = 625 and n = 1393), to those without SMI known only to primary care (n = 106,554 and n = 25,385) using logistic regression models adjusted first for socio-demographic factors and second, additionally for primary care consultation frequency. Results Eligible SMI patients were less likely to have received breast (adjusted odds ratio (OR) 0.69, 95 % confidence interval (CI), 0.57 - 0.84, p < 0.001) or cervical screening (adjusted OR 0.72, CI: 0.60 - 0.85, p < 0.001). Schizophrenia diagnosis, depot injectable antipsychotic prescription, and illness severity and risk were associated with the lowest odds of uptake of breast (adjusted ORs 0.46 to 0.59, all p < 0.001) and cervical screening (adjusted ORs 0.48 - 0.65, all p < 0.001). Adjustments for consultation frequency further reduced effect sizes for all subgroups of SMI patient, in particular for cervical screening. Conclusions Women with SMI are less likely to receive breast and cervical cancer screening than comparable women without SMI. Higher primary care consultation rates among SMI patients is likely a mediating factor between SMI status and uptake, particularly for cervical screening - a service organised in primary care. To tackle health disparities linked to SMI, efforts at increasing screening uptake are key and should be targeted at women with other markers of illness severity or risk, beyond SMI status alone
