Adherence to prophylaxis in adolescents and young adults with severe haemophilia, A qualitative study with patients

Published by Informa UK Limited, trading as Taylor & Francis Group.Abstract Introduction: Reported levels of adherence to prophylaxis among young people with haemophilia (YPH) vary widely and are predominately based on estimations made by healthcare professionals and parents. Reasons for (non)adherence among YPH in particular have not been evidenced. Aim: to examine experiences in relation to prophylaxis with YPH themselves, and barriers and facilitators to their adherence. Methods: 11 Participants were recruited in five haemophilia centres across England and Wales. All patients who met the inclusion criteria (aged 12-25, diagnosed with haemophilia, on prophylaxis) were approached during a routine check-up appointment, and all participants who agreed to take part were interviewed. Interviews were audio recorded, transcribed and analysed using Interpretative Phenomenological Analysis. Results: Self-reported adherence to prophylaxis was good. Few participants admitted to intentionally skipping injections although they reported sometimes forgetting. However, due to the increasingly personalised and flexible approach to prophylaxis, adherence is not straightforward to define. Barriers to adherence included a busy lifestyle, dislike of the intravenous injection, venous access issues, anxiety or stress and being out of one’s normal routine. Support was an important facilitator to adherence, including support from health professionals at the haemophilia centre as well as friends. Parents appear to be very involved with their sons’ haemophilia management, even after their sons leave home. Conclusion: What this study adds is that the increasingly flexible and personalized approach to managing prophylaxis in haemophilia may sometimes lead to confusion around treatment frequency and dosing. This may lead to accidental non-adherence, which is distinct from both skipping and forgetting. Advice from haemophilia teams may not always be consistent, and is likely to be interpreted differently by different individuals. Some additional training and education of patients and their families to increase their knowledge and skills around prophylaxis may reduce this confusion and therefore is likely to improve adherence further.Peer reviewedFinal Published versio
Performance of high-area-ratio annular dump diffuser using suction-stabilized-vortex flow control
A short annular dump diffuser having a geometry conductive to formation of suction stabilized toroidal vortices in the region of abrupt area change was tested. The overall diffuser area ratio was 4.0 and the length to inlet height ratio was 2.0. Performance data were obtained at near ambient temperature and pressure for inlet Mach numbers of 0.18 and 0.30 with suction rates ranging from 0 to 18 percent of total inlet mass flowrate. Results show that the exit velocity profile could be readily biased toward either wall by adjustment of inner and outer wall suction rates. Symmetric exit velocity profiles were inherently unstable with a tendency to revert to a hub or tip bias. Diffuser effectiveness was increased from about 38 percent without suction to over 85 percent at a total suction rate of 10 to 12 percent. At the same time diffuser total pressure loss was reduced from 3.1 percent to 1.1 percent at an inlet Mach number of 0.3
Performance characteristics of two annular dump diffusers using suction-stabilized vortex flow control
Test results are described for two abrupt area change annular diffusers with provisions for maintaining suction stabilized toroidal vortices at the area discontinuity. Both diffusers had an overall area ratio of 4.0 with the prediffuser area ratio being 1.18 for diffuser A and 1.4 for diffuser B. Performance was evaluated at near atmospheric pressure and temperature for a range of inlet Mach numbers from 0.18 to 0.41 and suction rates from 0 to 18%. Static pressure recovery improved significantly as the suction rate was increased to approximately 11%. Results obtained with diffuser A were superior to that obtained with diffuser B. Flat radial profiles of exit velocity were not obtained since the flow showed preferential hub or tip attachment at moderate suction rates. Performance of a short annular dump diffuser using suction-stabilized vortices at inlet Mach numbers to 0.41
A short, annular dump diffuser was designed to use suction to establish stabilized vortices on both walls for improved flow expansion in the region of an abrupt area change. The diffuser was tested at near ambient inlet pressure and temperature. The overall diffuser area ratio was 4.0. The inlet height was 2.54 cm and the exit pitot-static rakes were located at a distance from the vortex fence equal to two or six times the inlet height. Performance data were taken at near ambient temperature and pressure for nominal inlet Mach numbers of 0.18 to 0.41 with suction rates of 0 to 18 percent of the total inlet airflow. The exit velocity profile could be shifted toward either wall by adjusting the inner- or outer-wall suction rate. Symmetrical exit velocity profiles were unstable, with a tendency to shift back to hub- or tip-weighted profile. Diffuser effectiveness was increased from about 47 percent without suction to over 85 percent at a total suction rate of about 14 percent. The diffuser total pressure losses at inlet Mach numbers of 0.18 and 0.41 decreased from 1.1 and 5.6 percent without suction to 0.48 and 5.2 percent at total suction rates of 14.4 and 5.6 percent, respectively
Performance of semi-transportation-cooled liner in high-temperature-rise combustors
Results from tests with the Lamilloy combustor liner are compared with results obtained from a conventionally designed, film cooled, step-louver liner. Operation of the Lamilloy liner with counterrotating swirl combustor fuel modules with mixing venturis was possible to a fuel-air ratio of 0.065 without obtaining excessive liner metal temperatures. At the 0.065 fuel-air condition the average liner metal temperature was 140 K and the maximum local temperature 280 K above the inlet air temperature. Big Bang Nucleosynthesis with Independent Neutrino Distribution Functions
We have performed new Big Bang Nucleosynthesis calculations which employ
arbitrarily-specified, time-dependent neutrino and antineutrino distribution
functions for each of up to four neutrino flavors. We self-consistently couple
these distributions to the thermodynamics, the expansion rate and scale
factor-time/temperature relationship, as well as to all relevant weak,
electromagnetic, and strong nuclear reaction processes in the early universe.
With this approach, we can treat any scenario in which neutrino or antineutrino
spectral distortion might arise. These scenarios might include, for example,
decaying particles, active-sterile neutrino oscillations, and active-active
neutrino oscillations in the presence of significant lepton numbers. Our
calculations allow lepton numbers and sterile neutrinos to be constrained with
observationally-determined primordial helium and deuterium abundances. We have
modified a standard BBN code to perform these calculations and have made it

Feeling good about being hungry: food-related thoughts in eating disorder
Objectives: This study explores the relationships to food and hunger in women living with anorexic type eating difficulties and asks how imagery-based elaborations of food and eating thoughts are involved in their eating restraint, and recovery. Design: The qualitative idiographic approach of Interpretative Phenomenological Analysis (IPA) was used. Four in-depth semi-structured interviews were conducted with women self-selected as having experienced anorexia or anorexic like behaviour. Methods: The data was analysed using IPA and an audit of the analysis was conducted to ensure that the process followed had been systematic and rigorous and appropriately considered reflexivity. Results: Hunger was perceived positively by participants as confirmation that they were achieving their goal of losing weight, or avoiding weight gain. Hunger conferred a sense of being in control for the participants. Intrusive thoughts about food were reported as being quickly followed by elaborative mental imagery of the positive aspects of weight loss, and the negative consequences of eating. Imagery appeared to serve to maintain anorexic behaviours rather than to motivate food seeking. However, negative imagery of the consequences of anorexia were also described as supporting recovery. Conclusions: The finding that physiological sensations of hunger were experienced as positive confirmation of maintaining control has potentially important clinical and theoretical implications. It suggests further attention needs to be focused upon how changes in cognitive elaboration, involving mental imagery, are components of the psychological changes in the development of, maintenance of, and recovery from, anorexia
XTE J1739-302 as a Supergiant Fast X-ray Transient
XTE J1739-302 is a transient X-ray source with unusually short outbursts,
lasting on the order of hours. Here we give a summary of X-ray observations we
have made of this object in outburst with the Rossi X-ray Timing Explorer
(RXTE) and at a low level of activity with the Chandra X-ray Observatory, as
well as observations made by other groups. Visible and infrared spectroscopy of
the mass donor of XTE J1739-302 are presented in a companion paper. The X-ray
spectrum is hard both at low levels and in outburst, but somewhat variable, and
there is strong variability in the absorption column from one outburst to
another. Although no pulsation has been observed, the outburst data from
multiple observatories show a characteristic timescale for variability on the
order of 1500-2000 s. The Chandra localization (right ascension 17h 39m 11.58s,
declination -30o 20' 37.6'', J2000) shows that despite being located less than
2 degrees from the Galactic Center and highly absorbed, XTE J1739-302 is
actually a foreground object with a bright optical counterpart. The combination
of a very short outburst timescale and a supergiant companion is shared with
several other recently-discovered systems, forming a class we designate as
Supergiant Fast X-ray Transients (SFXTs). Three persistently bright X-ray
binaries with similar supergiant companions have also produced extremely short,
bright outbursts: Cyg X-1, Vela X-1, and 1E 1145.1-6141.Comment: 16 pages, 7 figures, 2 tables, in press in The Astrophysical Journal;
