    Bacterial contaminations of informally marketed raw milk in Ghana

    Background: Milk has an outstanding nutritional quality but is also an excellent medium for bacterial growth and an important source of bacterial infection when consumed without pasteurization. Objective: To estimate the bacterial health risk of milk consumption in Accra and Kumasi, the twomajor cities in Ghana. Method: A total of 96 raw milk samples collected in 2002 from the two sites were cultured and theisolated organisms identified by standard bacteriological methods. Results: Overall, the organisms identified and their prevalence rates were Yersinia spp. (19.8%), Klebsiella spp (16.7%), Proteus spp. (7.3%), Enterobacter spp. (6.3%), Escherichia coli (2.1%), andStaphylococcus spp (14.6%), Bacillus spp. (11.5%) and Mycobacterium spp. (1%). Most of the organisms identified were enterobacteria indicating probable faecal contamination of the milkas a result of poor hygiene. Most of the bacteria identified in the milk sampled are potential foodborne pathogens, and though some of them occurred in few samples, the practice of pooling milk from different sources by traders, and the absence of pasteurization generally observed among them could increase the risk posed by such organisms.Conclusion: The study has shown that informally marketed raw milk in the two cities could be an important source of infection with a wide range of organisms, particularly enterobacteria.     Barriers and facilitators to implementation of cancer treatment and palliative care strategies in low- and middle-income countries: systematic review

Objectives: To appraise improvement strategies adopted by low- and middle-income countries to increase access to cancer treatments and palliative care; and identify the facilitators and barriers to implementation. Methods: A systematic review was conducted and reported in accordance with PRISMA statement. MEDLINE, CINAHL, and the Cochrane Library databases were searched. Bias was assessed using the Standards for Quality Improvement Reporting Excellence, and evidence graded using the Australian National Health and Medical Research Council system. Results: Of 3069 articles identified, 18 studied were included. These studies involved less than a tenth (n = 12, 8.6%) of all low- and middle-income countries. Most were case reports (58%), and the majority focused on palliative care (n = 11, 61%). Facilitators included: stakeholder engagement, financial support, supportive learning environment, and community networks. Barriers included: lack of human resources, financial constraints, and limited infrastructure. Conclusions: There is limited evidence on sustainable strategies for increasing access to cancer treatments and palliative care in low- and middle-income countries. Future strategies should be externally evaluated and be tailored to address service delivery; workforce; information; medical products, vaccines, and technologies; financing; and leadership and governance

    Impurity and quaternions in nonrelativistic scattering from a quantum memory

    Models of quantum computing rely on transformations of the states of a quantum memory. We study mathematical aspects of a model proposed by Wu in which the memory state is changed via the scattering of incoming particles. This operation causes the memory content to deviate from a pure state, i.e. induces impurity. For nonrelativistic particles scattered from a two-state memory and sufficiently general interaction potentials in 1+1 dimensions, we express impurity in terms of quaternionic commutators. In this context, pure memory states correspond to null hyperbolic quaternions. In the case with point interactions, the scattering process amounts to appropriate rotations of quaternions in the frequency domain.     The perception and preference of consumers for local poultry meat in the Kumasi metropolis of Ghana

    The study examined the preference for and perception of consumers concerning the locally produced poultry meat in the Kumasi metropolis in the Ashanti Region of Ghana. Fifty consumers were randomly selected. The results demonstrated that most consumers preferred the local poultry meat to the imported meat. The logit estimates indicated that factors such as age, gender, cold store, cut parts, and healthiness significantly influence consumers’ preference for the locally produced poultry meat. Age, gender and healthiness positively influence consumer’s preference, while cold store and cut parts negatively associated with consumer preference for local poultry meat. On the perception, 68 per cent of the consumers strongly agreed that local poultry meat is tough, while 58 per cent alleged that imported poultry meat is rather tough. In terms of affordability, 46 per cent of the consumers disagreed that local poultry meat is affordable, while 52 per cent strongly objected to the idea that imported poultry meat is relatively cheaper. In terms of availability, 54 per cent of the consumers admitted that local poultry meat is not readily available, while 58 per cent indicated that imported poultry meat is readily available and can be obtained at anytime. Sixty per cent of the respondents were of the opinion that local poultry meat taste better, while 46 per cent of the respondents indicated that imported poultry meat do not taste better. This was probably due to long period of refrigeration, which reduced its freshness and taste and made it less healthy for consumption. It is suggested that the government should subsidize the cost of inputs for the local poultry industry, since the results indicated that high cost was the major factor that influenced consumers’ purchasing decision

    Seasonal Dynamics and Defoliation Impact on Herbage Yield in Aspen Boreal Habitats of Alberta

    Within the aspen boreal ecosystems, little information exists on the seasonal dynamics of available herbage and the effects of varying defoliation regimes on accumulated herbage growth and associated opportunities for animal production. We examined seasonal changes in herbage phytomass in conjunction with defoliation treatments in Bromus inermis-Poa pratensis grasslands in central Alberta. Changes in herbage pools were examined by sampling at five monthly intervals from April to September 1997 and 1998, inclusive. Vegetation was also subjected to a factorial experiment with an initial defoliation in late-May, June or July, at heights of 2.5, 7.5, or 15 cm, and repeated at 3-, 6- or 9-week intervals until the end of September. Green herbage, standing dead and fallen litter increased from spring to summer and decreased from summer to fall. Average growing conditions resulted in a peak phytomass of 350 g m-2, and varied by year. Weathering losses of green herbage, standing dead and fallen litter over winter were 34%, 52% and 51%, respectively. Dry matter losses of total herbage (all three pools) over winter were 58% of 1997 summer green phytomass. Initial timing, height, and frequency of clipping all affected accumulated herbage yield (P < 0.001). The greatest accumulated herbage yield was from clipping initiated in May. Light clipping resulted in less phytomass accumulation relative to moderate and heavy clipping. Clipping frequencies of six and nine weeks resulted in similar phytomass removal, but were greater than herbage removal associated with three weeks frequency. The interaction between clipping date and frequency of clipping demonstrates the importance of temporal rest and sensitivity of forage plants to defoliation, and lends support to the use of rotational grazing systems

    Novel coronavirus mitigation measures implemented by radiotherapy centres in low and middle-income countries: a systematic review.

    Background: The aim of the study was to identify strategies adopted by radiotherapy centres in low- and middle-income countries (LMICs) to mitigate the effects of COVID-19. Studies summarising COVID-19 mitigation strategies designed and implemented by radiotherapy centres in LMICs to avoid delays, deferrments and interruptions of radiotherapy services are lacking. Materials and methods: A systematic review was conducted and reported in accordance with the preferred reporting items for systematic review and meta-analysis guideline. Ovid Embase, Ovid MEDLINE and CINAHL were searched for peer-reviewed articles that reported measures adopted by radiotherapy centres in LMICs to reduce the risk of COVID-19. Information on different strategies were extracted from the included studies and textual narrative synthesis was conducted. Results: Of 60 articles retrieved, eleven were included. Majority of the studies were conducted in China. Ten of the included studies employed a qualitative design. Four themes were identified: preparing and equipping staff; reinforcing infection prevention and control policies; strengthening coordination and communication; and maintaining physical distancing. Studies reported that radiotherapy centres had: formed COVID-19 response multidisciplinary team; maximised the use of telehealth; adjusted the layout of waiting areas; divided staff into teams; dedicated a room for isolating suspected cases; and adopted triage systems. Conclusions: Local adaptation of established global strategies coupled with timely development of guidelines, flexibility and innovation have allowed radiotherapy leaders to continue to deliver radiotherapy services to cancer patients in LMICs during the COVID-19 crisis. Robust data collection must be encouraged in LMICs to provide an evidence-based knowledge for use in the event of another pandemic

    Factors contributing to late presentation of breast cancer in Africa: a systematic literature review

    Background: Over 50% of people diagnosed with breast cancer in most African countries present late and report to the hospital with advanced stage III and IV disease, a major reason for the poor survival rate. This study reviewed studies focusing on patient-related factors or reasons contributing to the late presentation or delayed diagnosis of breast cancer in Africa. Method: A rigorous literature search was conducted with search terms “Breast Neoplasms” AND “Late Presentation” OR “Delayed Diagnosis” AND “Africa” OR “the name of any of the African countries” within CINAHL, African Index Medicus, MEDLINE, Web of Science and PsycINFO electronic databases. Additional hand searching of reference lists of included articles was conducted. A thematic synthesis was conducted. Result: Of the eighty-two studies identified, nine were eligible and included in the review. Studies included were conducted in Egypt, Nigeria, Ghana, Kenya and Libya. The factors identified as contributing to late presentation of breast cancer among most African women were negative symptom interpretation, fear, belief in alternative medicine, social relations and networks, lack of trust and confidence in orthodox medicine, and access to healthcare. Conclusion: A complex matrix of factors were identified that contribute to the late presentation or delayed diagnosis of breast cancer among most African women. The orthodox medical system in most African countries is gradually losing their relationship and credibility because of false reassurance, frequent misdiagnosis and strike actions, which is leading to late presentation of breast cancer

    Differential transcriptomic and metabolic profiles of M. africanum- and M. tuberculosis-infected patients after, but not before, drug treatment.

    The epidemiology of Mycobacterium tuberculosis (Mtb) and M. africanum (Maf) suggests differences in their virulence, but the host immune profile to better understand the pathogenesis of tuberculosis (TB) have not been studied. We compared the transcriptomic and metabolic profiles between Mtb- and Maf-infected TB cases to identify host biomarkers associated with lineages-specific pathogenesis and response to anti-TB chemotherapy. Venous blood samples from Mtb- and Maf-infected patients obtained before and after anti-TB treatment were analyzed for cell composition, gene expression and metabolic profiles. Prior to treatment, similar transcriptomic profiles were seen in Maf- and Mtb-infected patients. In contrast, post treatment, over 1600 genes related to immune responses and metabolic diseases were differentially expressed between the groups. Notably, the upstream regulator hepatocyte nuclear factor 4-alpha (HNF4α), which regulated 15% of these genes, was markedly enriched. Serum metabolic profiles were similar in both group pre-treatment, but the decline in pro-inflammatory metabolites post treatment were most pronounced in Mtb-infected patients. Together, the differences in both peripheral blood transcriptomic and serum metabolic profiles between Maf- and Mtb-infected patients observed over the treatment period, might be indicative of intrinsic host factors related to susceptibility to TB and/or differential efficacy of the standard anti-TB treatment on the two lineages
