able to find 11 previously reported in the literature consulted. We report a further case occurring
in a five-year-old girl because of its site and location treated surgically by means of
curettage and allograft. We believe that in bone cysts occupying more than 30-40% of the talus,
perforations and intracystic punctures should be avoided due to the risk of fracture and
necrosis
Almost commuting unitary matrices related to time reversal
The behavior of fermionic systems depends on the geometry of the system and
the symmetry class of the Hamiltonian and observables. Almost commuting
matrices arise from band-projected position observables in such systems. One
expects the mathematical behavior of almost commuting Hermitian matrices to
depend on two factors. One factor will be the approximate polynomial relations
satisfied by the matrices. The other factor is what algebra the matrices are
in, either the matrices over A for A the real numbers, A the complex numbers or
A the algebra of quaternions.
There are potential obstructions keeping k-tuples of almost commuting
operators from being close to a commuting k-tuple. We consider two-dimensional
geometries and so this obstruction lives in KO_{-2}(A). This obstruction
corresponds to either the Chern number or spin Chern number in physics. We show
that if this obstruction is the trivial element in K-theory then the

The assessment of water use and reuse through reported data: A US case study
Increasing demands for freshwater make it necessary to find innovative ways to extend the life of our water resources, and to manage them in a sustainable way. Indirect water reuse plays a role in meeting freshwater demands but there is limited documentation of it. There is a need to analyze its current status for water resources planning and conservation, and for understanding how it potentially impacts human health. However, the fact that data are archived in discrete uncoordinated databases by different state and federal entities, limits the capacity to complete holistic analysis of critical resources at large watershed scales. Humans alter the water cycle for food production, manufacturing, energy production, provision of potable water and recreation. Ecosystems services are affected at watershed scales but there are also global scale impacts from greenhouse gas emissions enabled by access to cooling, processing and irrigation water. To better document these issues and to demonstrate the utility of such an analysis, we studied the Wabash River Watershed located in the U.S. Midwest. Data for water extraction, use, discharge, and river flow were collected, curated and reorganized in order to characterize the water use and reuse within the basin. Indirect water reuse was estimated by comparing treated wastewater discharges with stream flows at selected points within the watershed. Results show that during the low flow months of July–October, wastewater discharges into the Wabash River basin contributed 82 to 121% of the stream flow, demonstrating that the level of water use and unplanned reuse is significant. These results suggest that intentional water reuse for consumptive purposes such as landscape or agricultural irrigation could have substantial ecological impacts by diminishing stream flow during vulnerable low flow periods

Linked CSF reduction of phosphorylated tau and IL-8 in HIV associated neurocognitive disorder.
HIV-associated neurocognitive disorder (HAND) is a common condition in both developed and developing nations, but its cause is largely unknown. Previous research has inconsistently linked Alzheimer's disease (AD), viral burden, and inflammation to the onset of HAND in HIV-infected individuals. Here we simultaneously measured cerebrospinal fluid (CSF) levels of established amyloid and tau biomarkers for AD, viral copy numbers, and six key cytokines in 41 HIV-infected individuals off combination anti-retroviral therapy (14 with HAND) who underwent detailed clinical and neuropsychological characterization, and compared their CSF patterns with those from young healthy subjects, older healthy subjects with normal cognition, and older people with AD. HAND was associated with the lowest CSF levels of phosphorylated tau (p-Tau181) after accounting for age and race. We also found very high CSF levels of the pro-inflammatory interferon gamma-induced protein 10 (IP-10/CXCL10) in HIV regardless of cognition, but elevated CSF interleukin 8 (IL-8/CXCL8) only in HIV-NC but not HAND. Eleven HIV-infected subjects underwent repeat CSF collection six months later and showed strongly correlated longitudinal changes in p-Tau181 and IL-8 levels (R = 0.841). These data suggest reduced IL-8 relative to IP-10 and reduced p-Tau181 to characterize HAND

Quality of life and mood in patients with medically intractable epilepsy treated with targeted responsive neurostimulation
AbstractPurposeThe primary efficacy and safety measures from a trial of responsive neurostimulation for focal epilepsy were previously published. In this report, the findings from the same study are presented for quality of life, which was a supportive analysis, and for mood, which was assessed as a secondary safety endpoint.MethodsThe study was a multicenter randomized controlled double-blinded trial of responsive neurostimulation in 191 patients with medically resistant focal epilepsy. During a 4-month postimplant blinded period, patients were randomized to receive responsive stimulation or sham stimulation, after which all patients received responsive neurostimulation in open label to complete 2years. Quality of life (QOL) and mood surveys were administered during the baseline period, at the end of the blinded period, and at year 1 and year 2 of the open label period.ResultsThe treatment and sham groups did not differ at baseline. Compared with baseline, QOL improved in both groups at the end of the blinded period and also at 1year and 2years, when all patients were treated. At 2years, 44% of patients reported meaningful improvements in QOL, and 16% reported declines. There were no overall adverse changes in mood or in suicidality across the study. Findings were not related to changes in seizures and antiepileptic drugs, and patients with mesial temporal seizure onsets and those with neocortical seizure onsets both experienced improvements in QOL.ConclusionsTreatment with targeted responsive neurostimulation does not adversely affect QOL or mood and may be associated with improvements in QOL in patients, including those with seizures of either mesial temporal origin or neocortical origin
