.The article discusses the main types of managers' psychological readiness for teaching staff management in educational districts identified by factor and cluster analyses. These include βstrategicβ, βnormativeβ, βstableβ and βcomfortableβ types. The most optimal type of managers' psychological readiness for teaching staff management in educational districts was shown to be the strategic type, because it is best characterized by managers' innovative competence in teaching staff management in educational districts. The normative readiness was found to be the most effective type of readiness, because it is characterized by traditional approach to teaching staff management in educational districts. The authors give a detailed description of the components of and the quantitative distribution of each type of managers' psychological readiness for teaching staff management in educational districts

Various pathological factors accompanying any cardiac surgery can cause intraoperative systemic inο¬ammatory responses (SIR). As the number of cardiac surgical interventions grows worldwide, the issue of SIR prevention appears highly relevant.Aim of the study. To determine the eο¬ect of not using donor blood components in the priming of the cardiopulmonary bypass circuit in children with septal congenital heart defects, operated under cardiopulmonary bypass, on the severity of SIR.Material and methods. A prospective, randomized study included 40 children with a median age of 14 [12β22.5] months and weight of 8.8 [7.25β11] kg. All patients underwent radical correction of septal defect under cardiopulmonary bypass. The patients were divided into two groups depending on the use of donor blood components for priming the CPB. The severity of SIR was assessed using four speciο¬c serum biomarkers such as interleukin 1b (IL-1b), interleukin 6 (IL-6), interleukin 10 (IL-10), and tumor necrosis factor alpha (TNF-Ξ±), measured before the operation, after the CPB and 16 hours after the surgery. In addition, the intra- and postoperative periods were evaluated.Results. The safety of the proposed strategy of skipping the donor blood was confirmed by lack of any organ dysfunction in all patients, as well as a significant difference in the balance of oxygen delivery and consumption. In addition, the levels of systemic inflammation markers after CPB were significantly higher in patients who had transfusion: IL-1b was 3.3 [3.2β3.48] pg/mL vs 2.86 [2.7β3.11] pg/mL (P=0.003) and TNF-Ξ± reached 1.81 [1.37β3.3] pg/mL vs 1.33 [1.26β1.76] pg/mL (P=0.034). Meanwhile, 16 hours post surgery, IL-6 and IL-10 levels were significantly higher in the group using donor blood components with IL-6 being 48.91 [33.89β57.6] pg/mL vs 31.56 [26.83β48.89] pg/mL (P=0.087) and IL-10 reaching 0.8 [0.76β1.43] pg/mL vs 0.69 [0.6β0.83] pg/mL (P=0.005).Conclusion. HighlightsThis article is devoted to a review of studies on the mechanisms of cerebral protection and the effects of hypothermia on the patient.Β AbstractA large number of surgical corrections of congenital heart defects are performed around the world annually, most of them using cardiopulmonary bypass. This kind of surgery can be a pathological factor for the brain. In some patients, the kind of surgery involves the use of hypothermia. Supposedly, hypothermia should have a neuroprotective effect, however, as recent studies show, this is not always the case. This review is devoted to the mechanisms of the effect of hypothermia on the patientβs body, including not only a decrease in brain metabolism, but also a number of others. Aim. To establish the role of donor blood components which are used in the intraoperative period in children with septal congenital heart disease (CHD) who need surgical interventions with cardiopulmonary bypass (CPB) for the prevention of cerebral injury in the postoperative period.Methods. 40 children of 14 months old on average and 8.8 kg of weight were under the study. All patients underwent radical correction of the septal CHD with the use of CPB. The patients were divided into 2 groups according to the donated blood components used. Cerebral damage was assessed with the help of three specific markers: protein S-100-Γ, neuron-specific enolase (NSE) and glial fibrillar acidic protein (GFAP). The severity of the systemic inflammatory response was defined by the level of the following markers: interleukin 1, 6 and 10 in serum before operations, after the completion of IC and 16 hours after surgery. The clinical analysis of cerebral injury was performed via the Cornell Assessment for Pediatric Delirium.Results. Statistically the concentration of all three markers of cerebral injury was significantly higher in the transfusion group after the end of CPB. Similar dynamics was observed in the concentrations of interleukins. In addition, the values for GFAP were higher in the transfusion group and 16 hours after surgery.Conclusion. The safety and effectiveness of the strategy on refusal of intraoperative transfusion in order to prevent cerebral injury in children during the correction of septal CHD has been demonstrated.Β Π¦Π΅Π»Ρ. This analytical review presents data on brain injury in surgical management of congenital heart disease. A brief description of mechanisms of brain injury and methods of its prevention are given, moreover, a clinical case applying such techniques is given. The neurovascular unit in vitro model and its advantages are presented. Highlights. The article discusses the pathophysiological aspects of cardiopulmonary bypass and the mechanisms underlying the development of the systemic inflammatory response in children following congenital heart surgery. We summarize and report the most relevant preventive strategies aimed at reducing the systemic inflammatory response, including both, CPB-related methods and pharmacological ones.The growing number of children with congenital heart defects requires the development of more advanced technologies for their surgical treatment. However, cardiopulmonary bypass is required in almost all surgical techniques. Despite the tremendous progress and recent advances in cardiopulmonary bypass techniques, the systemic inflammatory response syndrome associated with these surgeries remains unresolved. The review summarizes the causes and mechanisms underlying its development. The most commonly used preventive strategies are reported, including standard and modified ultrafiltration, leukocyte filters, and pharmacological agents (systemic glucocorticoids, aprotinin, and antioxidants).The role of cardioplegia and hypothermia in the reduction of systemic inflammation is defined. Cardiac surgery centers around the world use a variety of techniques and pharmacological approaches, drawing on the results of randomized clinical studies. However, there are no clear and definite clinical guidelines aimed at reducing the systemic inflammatory response during cardiopulmonary bypass in children. Despite the availability of modern drugs, examination and treatment methods, the issue of cognitive dysfunction is still fairly important for intensive care and anaesthesiologic practice. The review presents statistic data on the frequency of cognitive dysfunction, discusses main tests used for detection of cognitive dysfunction, describes their specific features, advantages and deficiencies. The predictive value of various indicators of sperm for male fertility
Introduction. DNA fragmentation of sperm is one of the possible causes of reduced fertility potential of men. However, a significant correlation between conventional semen parameters and sperm DNA fragmentation was not found. This fact determines the relevance of the study of the influence of various parameters of sperm on male fertility.Materials and methods. The study included 60 men, aged 26β36 years (median β 30 years) with idiopathic infertility and the level of DNA fragmentation of sperm is higher than 15 %. These men were treated with hyperbaric oxygen therapy, after 3 months in vitro fertilization performed partners of these men. DNA fragmentation of sperm cells was determined by TUNEL (upper limit of normal β 15 %). The level of reactive oxygen species (ROS) of the ejaculate were determined by chemiluminescence (upper limit of normal β 0.64 mV/s).Results. The frequency of pregnancy in vitro fertilization was following: 62.8 and 64.7 % (p > 0.05) for the total number sperm of spermatozoa < 38 Γ 106 /ejaculate and β₯ 39 Γ 106 /ejaculate, respectively; 63.3 and 63.6 % (p > 0.05) for mobility (a + b) of spermatozoa < 40 and β₯ 40 %, respectively; 58.3 and 64.6 % (p > 0.05) for normal forms of spermatozoa < 4 and β₯ 4 %, respectively; 67.3 and 20.0 % (p < 0.05) for the level of DNA fragmentation of sperm β€ 15 and > 15 %, respectively; 64.9 and 33.3 % (p < 0.05) for the level of ROS in semen β€ 0.64 and > 0.64 mV/s, respectively.Conclusion.
