    Clinico-Pathological Measures and Management of Mucinous Ovarian Cancer: Single Institutional Study

    Background: Mucinous ovarian cancer is less common and less aggressive epithelial ovarian cancer, which represents 3% of epithelial ovarian cancer. Patient and methods: A retrospective descriptive analysis involving all patients with mucinous ovarian cancer who underwent surgical management or referred to National Cancer Institute (NCI), Cairo University (CU), from January 2010 to January 2020. Of 46 cases reviewed. 16 cases were excluded (10 cases had metastatic and 6 cases had incomplete data). Results: forty-six cases reviewed with median age of 48.3±18.1 years. Half of patients were in premenopausal status. Patients presented with distention with or without pain were 70% of cases, 10% of patients with abdominal mass and 6.7% of patients with bleeding. Most of cases 63.30% were diagnosed at late stages (1C and beyond). Patients had unilateral disease were 70% of cases and had elevated tumor markers were 53.3 % of cases. The most commonly elevated tumor marker was CA 125 (26.7%). Patients had comorbidities were 30% of cases. Surgery was the main line of management. Total abdominal hysterectomy and bilateral salpingo-oophorectomy plus infracolic omentectomy were done in 73.3% of patients, 6.7% underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy, 3.3% underwent cytoreductive surgery and 3.3% underwent cytoreductive surgery and 13% of patients underwent fertility sparing surgery. The role of pelvic lymphadenectomy was limited (13.3% underwent pelvic lymphadenectomy and only one patient had positive lymph node metastasis). Conclusion: The most important prognostic factors were disease stage, laterality, tumor markers and performance status

    Molecular characterization of Pseudomonas aeruginosa isolates from various clinical specimens in Khartoum/Sudan: Antimicrobial resistance and virulence genes

    Background: Pseudomonas aeruginosa is a pathogenic organism responsible for frequent wound and nosocomial infections worldwide. Its infections are difficult to control since the organism is known to rapidly develop antibiotic resistance and becomes multidrug-resistant (MDR) during treatment of patients. Aim of the study: This study was intended to investigate the occurrence of certain important types of (ESBL) and (MBL) enzymes in association with important specific virulence factors  associated with P. aeruginosa clinical isolates from Khartoum, Sudan. Methods: This study investigated 70 P. aeruginosa isolates which were collected from patients admitted to four major hospitals in Khartoum  (Fedail, Ribat, Ibn Sina and Soba hospitals). These isolates were recovered from 40 wound swabs (57.1%), 27 urine samples (38.6%), and 3 pleural fluid samples (4.3%) of patients. Higher numbers of isolates were recovered from males 42 (60%) than in females 28 (40%). All P. aeruginosa isolates were first confirmed by conventional biochemical and second using molecular PCR tests.   PCR methods were also used for detecting the presence of the virulence genes ToxA, AlgD, LasB, exoS, exoU, CTX, GES-1, and genes of VIM, IMP, KPC, CTX, VEB-1 and SHV-1. Results:   Antimicrobial susceptibility testing of P. aeruginosa isolates showed a high resistance to azetronam 49 (70%), followed by ceftazidime 32 (45.7%), 16 ciprofloxacin (22.9%), gentamicin 13 (18.6 %), piperacillin-tazobactam 11 (15.7%), amikacin 9 (12.9 %), and imipenem 6 (8.6%) showed the least resistance. All isolates were positive for algD and lasB (100%), followed by toxA (90%), exoS (34.3), exoU (24.3%), respectively. The rates of detected ESBL genes blaTEM, blaCTX-m, blaSHV-1,GES-1, were 3.3%, 6.6%, 10%, 3.3%,10%, respectively, but all isolates were negative for bla-KPC and bla- VIM and IMP . The percentages of pigment production were 61.4% for pyocyanin, 37.1% for pyoverdin and 1.4% for pyorubin. Conclusion: The study demonstrated high rates of antimicrobial resistance markers to most commonly used antibiotics in treatment of P. aeruginosa infections. The majority of the isolates from urine and wound samples carried at least three potential virulence factor genes of algD, lasB and toxA and without any significant relation to their antimicrobial resistance markers. &nbsp

    Metastatic rhabdomyosarcoma of the thyroid gland, a case report

    The thyroid gland is a known but an unusual site for metastatic tumors from various primary sites. Despite the fact that it is one of the largest vascular organs in the body, clinical and surgical cases have given an incidence of 3 % of secondary malignances of the organ. Nevertheless, thyroid metastases are not an exceptional finding at autopsy, they are encountered in 2 % to 24 % of the patients with malignant neoplasm. Soft tissue sarcomas metastatic to the thyroid are extremely rare as the majority of thyroid metastasis are caused by tumors of the kidneys, lungs, mammary glands, ovaries , and colon or by melanomas.     Novel packet scheduling algorithm based on cross component carrier in LTE-advanced network with carrier aggregation

    LTE-Advanced provides considerably higher data rates than the early releases of LTE. The carrier aggregation CA) technology allows scalable expansion of effective bandwidth provided to user equipment (UE) through simultaneous utilization of radio resources across multiple carriers. In this paper we propose a new packet scheduling (PS) criterion algorithm that charmingly satisfies the fairness among the different kinds of UEs by designing a weighting factor to proportional fair (PF) packet scheduling (PS) algorithms, while enhancing their throughput performance. The proposed PS algorithm is implemented and validated in a PS module for LTE\LTE-Advanced via system level simulations. Results show that PS-modified algorithms achieve higher throughput for both LTE and LTE-Advanced UEs
