    Effect of TiO2 and Al2O3 Addition on the Performance of Chitosan/Phosphotungstic Composite Membranes for Direct Methanol Fuel Cells

    : Composite chitosan/phosphotungstic acid (CS/PTA) with the addition of TiO2 and Al2O3 particles were synthesized to be used as proton exchange membranes in direct methanol fuel cells (DMFCs). The influence of fillers was assessed through X-ray diffraction, scanning electron microscopy, thermogravimetric analysis, liquid uptake, ion exchange capacity and methanol permeability measurements. The addition of TiO2 particles into proton exchange membranes led to an increase in crystallinity and a decrease in liquid uptake and methanol permeability with respect to pristine CS/PTA membranes, whilst the effect of the introduction of Al2O3 particles on the characteristics of membranes is almost the opposite. Membranes were successfully tested as proton conductors in a single module DMFC of 1 cm2 as active area, operating at 50 °C fed with 2 M methanol aqueous solution at the anode and oxygen at the cathode. Highest performance was reached by using a membrane with TiO2 (5 wt.%) particles, i.e., a power density of 40 mW cm-2, almost doubling the performance reached by using pristine CS/PTA membrane (i.e., 24 mW cm-2)

    Non-Natural Linker Configuration in 2,6-Dipeptidyl-Anthraquinones Enhances the Inhibition of TAR RNA Binding/Annealing Activities by HIV-1 NC and Tat Proteins

    The HIV-1 nucleocapsid (NC) protein represents an excellent molecular target for the development of antiretrovirals by virtue of its well-characterized chaperone activities, which play pivotal roles in essential steps of the viral life cycle. Our ongoing search for candidates able to impair NC binding/annealing activities led to the identification of peptidylanthraquinones as a promising class of nucleic acid ligands. Seeking to elucidate the inhibition determinants and increase the potency of this class of compounds, we have now explored the effects of chirality in the linker connecting the planar nucleus to the basic side chains. We show here that the non-natural linker configuration imparted unexpected TAR RNA targeting properties to the 2,6-peptidyl-anthraquinones and significantly enhanced their potency. Even if the new compounds were able to interact directly with the NC protein, they manifested a consistently higher affinity for the TAR RNA substrate and their TARbinding properties mirrored their ability to interfere with NC-TAR interactions. Based on these findings, we propose that the viral Tat protein, sharing the same RNA substrate but acting in distinct phases of the viral life cycle, constitutes an additional druggable target for this class of peptidyl-anthraquinones. The inhibition of Tat-TAR interaction for the test compounds correlated again with their TAR-binding properties, while simultaneously failing to demonstrate any direct Tat-binding capabilities. These considerations highlighted the importance of TAR RNA in the elucidation of their inhibition mechanism, rather than direct protein inhibition. We have therefore identified anti-TAR compounds with dual in vitro inhibitory activity on different viral proteins, demonstrating that it is possible to develop multitarget compounds capable of interfering with processes mediated by the interactions of this essential RNA domain of HIV-1 genome with NC and Tat proteins

    The Italian Implementation of the EU Directives on Procedural Safeguards for Accused Persons in Criminal Proceedings

    Questo saggio è stato sviluppato nel corso di un progetto di ricerca di 30 mesi finanziato dalla Commissione europea - CrossJustice (https://site.unibo.it/cross-justice/en), condotto sotto la supervisione dell'UniversitĂ  di Bologna. L'obiettivo del progetto era verificare il livello di attuazione delle sei direttive sui diritti dell'imputato adottate dal 2009 nell'ambito del Programma di Stoccolma. La ricerca ha esaminato criticamente i diritti dell'imputato riconosciuti e tutelati dalla Direttiva 2010/64/UE del 20 ottobre 2010 sul diritto all'interpretazione e alla traduzione; dalla Direttiva 2012/13/UE del 22 maggio 2012 sul diritto all'informazione; la Direttiva 2013/48/UE sul diritto di accesso a un difensore e di informazione di terzi; la Direttiva 2016/343/UE del 9 marzo 2016 sulla presunzione di innocenza e il diritto di presenziare al processo; la Direttiva 2016/800/UE sulle garanzie procedurali per gli imputati minorenni; la Direttiva 2016/1919/UE del 26 ottobre 2016 sul patrocinio a spese dello Stato. I ricercatori coinvolti hanno combinato due diverse metodologie, esaminando la questione sia da una prospettiva tradizionale, condotta da studiosi specializzati in diritto dell'UE e diritto processuale penale nazionale, sia da una nuova analisi computazionale. Nell'ambito di quest'ultimo approccio, la ricerca ha sviluppato una piattaforma di intelligenza artificiale semi-automatizzata, per evidenziare meglio le lacune scoperte dei testi normativi e migliorare l'analisi comparativa tra i sistemi giuridici (https://www.crossjustice.eu/en/index.html#crossjustice-platform)   Il presente contributo si concentra – adottando un metodo tradizionale - sul modo in cui il legislatore italiano ha recepito e attuato le suddette direttive, sia con riferimento alle disposizioni normative sia nell'interpretazione giudiziaria dei vertici del sistema. Infatti, mentre l'acquis dell'UE stabilisce standard minimi comuni in materia di diritti processuali penali, la necessitĂ  di promuoverne un'applicazione efficace e coerente rimane particolarmente pressante a causa della forte frammentazione della legislazione nazionale e della relativa giurisprudenza. In termini generali, il quadro che emerge mostra alcuni punti di forza del sistema italiano, con particolare riferimento al diritto al difensore, al diritto all'informazione e alla disclosure (e, in misura meno uniforme, alle regole di esclusione probatoria quando si tratti di tutelare le violazioni delle garanzie difensive). Non mancano tuttavia alcune criticitĂ , spesso legate alla prassi (ad esempio, la necessaria formazione che i difensori degli imputati vulnerabili dovrebbero ricevere, il patrocinio a spese dello Stato, la qualitĂ  e l'efficacia del diritto all'interprete e la tradizione degli atti). La presente analisi del sistema italiano, insieme a quella sviluppata per gli altri 10 Stati Membri dell’UE coinvolti nel progetto (Bulgaria, Croazia, Francia, Germania, Paesi Bassi, Polonia, Portogallo, Romania, Spagna, Svezia) ed ai risultati dell’analisi semantica dei testi normativi, fondata su tecniche di Intelligenza Artificiale, ha consentito di sviluppare una ricerca innovativa nei metodi e nei contenuti, che, oltre alla piattaforma Crossjustice, ha trovato recente pubblicazione anche in un volume edito da Brill (Giuseppe Contissa, Giulia Lasagni, Michele Caianiello, Giovanni Sartor (eds.), Effective Protection of the Rights of the Accused in the EU Directives.     Critical COVID-19 Patients Through First, Second And Third Wave: Retrospective Observational Study Comparing Outcomes In ICU.

    Introduction- The time-course of the COVID-19 pandemic was characterized by subsequent waves identified by peaks of Intensive Care Unit (ICU) admission rates. During these periods, progressive knowledge of the disease led to the development of specific therapeutic strategies. This retrospective study investigates whether this led to improvement in outcomes of COVID-19 patients admitted to ICU. Methods- Outcomes were evaluated in consecutive adult COVID19 patients admitted to our ICU, divided into three waves based on the admission period: the first wave from February 25th, 2020, to July 6th, 2020; the second wave from September 20th, 2020, to February 13th, 2021; the third wave from February 14th, 2021 to April 30th, 2021. Differences were assessed comparing outcomes and by using different multivariable Cox models adjusted for variables related to outcome. Further sensitivity analysis was performed in patients undergoing invasive mechanical ventilation. Results- Overall, 428 patients were included in the analysis: 102, 169 and 157 patients in the first, second and third wave. The ICU and in-hospital crude mortalities were lower by 7% and 10% in the third wave compared to the other 2 waves (p>0.05). A higher number of ICU and hospital free days at day 90 was found in the third wave when compared to the other 2 waves (p=0.001). Overall, 62.6% underwent invasive ventilation, with decreasing requirement during the waves (p=0.002). The adjusted Cox model showed no difference in the Hazard Ratio for mortality among the waves. In the propensity-matched analysis the hospital mortality rate was reduced by 11% in the third wave (p=0.044). Conclusions - With application of best practice as known by the time of the first three waves of the pandemic, our study failed to identify a significant improvement in mortality rate when comparing the different waves of the COVID-19 pandemic, notwithstanding, the sub-analyses showed a trend in mortality reduction in the third wave. Rather, our study identified a possible positive effect of dexamethasone on mortality rate reduction and the increased risk of death related to bacterial infections in the three waves

    Cytomegalovirus blood reactivation in COVID-19 critically ill patients: risk factors and impact on mortality.

    Purpose: Cytomegalovirus (CMV) reactivation in immunocompetent critically ill patients is common and relates to a worsening outcome. In this large observational study, we evaluated the incidence and the risk factors associated with CMV reactivation and its effects on mortality in a large cohort of COVID-19 patients admitted to the intensive care unit (ICU). Methods: Consecutive patients with confirmed SARS-CoV-2 infection and acute respiratory distress syndrome admitted to three ICUs from February 2020 to July 2021 were included. The patients were screened at ICU admission and once or twice per week for quantitative CMV-DNAemia in the blood. The risk factors associated with CMV blood reactivation and its association with mortality were estimated by adjusted Cox proportional hazards regression models. Results: CMV blood reactivation was observed in 88 patients (20,4%) of the 431 patients studied. SAPS II score (HR 1,031, 95% CI 1,010-1,053, p=0,006), platelet count (HR 0,0996, 95% CI 0,993-0,999, p=0,004), invasive mechanical ventilation (HR 2,611, 95% CI 1,223-5,571, p=0,013) and secondary bacterial infection (HR 5,041; 95% CI 2,852-8,911, p<0,0001) during ICU stay were related to CMV reactivation. Hospital mortality was higher in patients with (67,0%) than in patients without (24,5%) CMV reactivation but the adjusted analysis did not confirm this association (HR 1,141, 95% CI 0,757-1,721, p=0,528). Conclusion: The severity of illness and the occurrence of secondary bacterial infections were associated with an increased risk of CMV blood reactivation, which, however, does not seem to influence the outcome of COVID-19 ICU patients independently

    S-adenosylmethionine (SAM-e) for the treatment of depression in people living with HIV/AIDS

    BACKGROUND: This study reports on clinical data from an 8-week open-label study of 20 HIV-seropositive individuals, diagnosed with Major Depressive Disorder (DSM-IV), who were treated with SAM-e (S-Adenosylmethionine). SAM-e may be a treatment alternative for the management of depression in a population reluctant to add another "pill" or another set of related side effects to an already complex highly active antiretroviral therapy (HAART) regimen. METHODS: The Hamilton Rating Scale for Depression (HAM-D) and the Beck Depression Inventory (BDI) were used to assess depressive symptomatology from 1,2,4,6 and 8 weeks after initiation of treatment with SAM-e. RESULTS: Data show a significant acute reduction in depressive symptomatology, as measured by both the HAM-D and the BDI instruments. CONCLUSIONS: SAM-e has a rapid effect evident as soon as week 1 (p < .001), with progressive decreases in depression symptom rating scores throughout the 8 week study

    This collection of cases describes some unusual urological tumors and complications related to urological tumors and their treatment. Case 1: A case of uretero-arterial fistula in a patient with long-term ureteral stenting for ureteral oncological stricture and a second case associated to retroperitoneal fibrosis were described. Abdominal CT, pyelography, cystoscopy were useful to show the origin of the bleeding. Angiography is useful for confirming the diagnosis and for subsequent positioning of an endovascular prosthesis which represents a safe approach with reduced post-procedural complications. Case 2: A case of patient who suffered from interstitial pneumonitis during a cycle of intravesical BCG instillations for urothelial cancer. The patient was hospitalized for more than two weeks in a COVID ward for a suspected of COVID-19 pneumonia, but he did not show any evidence of SARS-CoV-2 infection during his hospital stay. Case 3: A case of a young man with a functional urinary bladder paraganglioma who was successfully managed with complete removal of the tumor, leaving the urinary bladder intact. Case 4: A case of a 61 year old male suffering from muscle invasive bladder cancer who was admitted for a radical cystectomy and on the eighth postoperative day developed microangiopathic hemolytic anemia and thrombocytopenia, which clinically defines thrombotic microangiopathy

    Clinical Features, Cardiovascular Risk Profile, and Therapeutic Trajectories of Patients with Type 2 Diabetes Candidate for Oral Semaglutide Therapy in the Italian Specialist Care

    Introduction: This study aimed to address therapeutic inertia in the management of type 2 diabetes (T2D) by investigating the potential of early treatment with oral semaglutide. Methods: A cross-sectional survey was conducted between October 2021 and April 2022 among specialists treating individuals with T2D. A scientific committee designed a data collection form covering demographics, cardiovascular risk, glucose control metrics, ongoing therapies, and physician judgments on treatment appropriateness. Participants completed anonymous patient questionnaires reflecting routine clinical encounters. The preferred therapeutic regimen for each patient was also identified. Results: The analysis was conducted on 4449 patients initiating oral semaglutide. The population had a relatively short disease duration (42%  60% of patients, and more often than sitagliptin or empagliflozin. Conclusion: The study supports the potential of early implementation of oral semaglutide as a strategy to overcome therapeutic inertia and enhance T2D management

    The Valuation of Environmental Damage Compensation: An Integrated Framework

    Environmental resources valuation is a critical issue, mainly when non-market value elements are relevant. The economics literature suggests the total economic value (TEV) approach as being in line with valuing environmental resources, but robust estimates are required of TEV change, when an environmental damage assessment is needed in order to ask for damage compensation in a court. Currently, for environmental damage claims the valuation methods usually proposed are subject to considerable scrutiny and therefore have to be used carefully. This paper aims to: a) analyse the pro and cons of the different valuation methods under the imputed-revealed-stated- preference economic approaches; b) present a matrix-based framework for environmental damage valuation; and c) establish a link between the proposed procedure and the Italian law on environmental damage claim

    Enhancement of Oxygen Reduction and Mitigation of Ionomer Dry-Out Using Insoluble Heteropoly Acids in Intermediate Temperature Polymer-Electrolyte Membrane Fuel Cells

    The use of Cs0.5H0.5PW12O40 insoluble salt as a superacid promoter in the catalyst layer of a polymer electrolyte membrane fuel cell (PEMFC) has been investigated. An increase of performance has been recorded at intermediate temperatures (110–130 °C) and under low relative humidity (R.H.). The promoter appears to mitigate the ionomer dry-out effects in the catalytic layer and produces an increase of the extent of the catalyst-electrolyte interface as demonstrated by cyclic voltammetry analysis. These effects are also corroborated by a significant decrease of polarization resistance at intermediate temperatures. Such characteristics have been demonstrated for a conventional membrane-electrode assembly based on a Pt-Co alloy and a Nafion 115 membrane
