Unipolar temperature-guided radiofrequency ablation used concomitant to major cardiac surgery procedures for the treatment of atrial fibrillation

LiteraturverzeichnisVorhofflimmern (VHF) stellt angesichts der Symptomatologie und der
assoziierten Morbidität und Mortalität ein sehr großes gesundheitliches
Problem dar. Unipolare endokardiale temperaturgesteuerte Radiohochfrequenz-
Ablation (RFA) wird begleitend zu anderen kardiochirurgischen Eingriffen bei
Patienten mit chronischem VHF durchgefĂĽhrt. Eismatsch und eine Isoliermatte
werden im Sinus transversus eingebracht. Durch unipolare RF-Energie von dem
Osypka HAT 300 Gerät werden nach der Berliner Modifikation , die Läsionen
endokardial im linken Vorhof gesetzt. Die Temperatur retrokardial wird
kontinuierlich gemessen. Es erfolgt ebenfalls eine Occlusion des linken
Vorhofohres und bei Patienten mit sehr groĂźem linken Vorhof eine
Vorhofverkleinerungsplastik. Es wurden alle Patienten, die vom 01. April 2000
bis zum 31. März 2005 eine RFA im Sana Herzzentrum Cottbus erhielten,
retrospektiv untersucht (n=82). Nach einem für alle Patienten vollständigen
Follow-up (im Durchschnitt 41,5 +/- 14,4 Monate), sind 72% der Patienten im
Sinusrhythmus oder zeigen eine biatriale Kontraktion bei sequentiellem
Schrittmacher. Faktoren, die das Ergebnis positiv beeinflussen, sind das
Vorhandensein einer Mitralklappenstenose und ein paroxysmales VHF vor der
Operation. Negativ beeinflusst wird das Ergebnis durch das Vorhandensein einer
Hyperthyreose. Patienten mit schon lange bestehendem VHF und Patienten die
eine Ablation mit Zugang ĂĽber den rechten Vorhof und das interatriale Septum
erhielten, weisen deutlich schlechtere Erfolgsraten auf. Keinen Einfluss auf
die postoperativen Erfolgsraten haben: Durchmesser des linken Vorhofes,
Vorhandensein einer koronaren Herzerkrankung, Aortenklappenerkrankung,
Trikuspidalklappeninsuffizienz, Diabetes mellitus, pulmonale Hypertonie, Alter
und Geschlecht. Patienten mit normaler oder eingeschränkter LV-EF und
Patienten mit hohem, mittlerem oder niedrigem Risikoprofil (nach Euroscore)
profitieren gleichermaĂźen von dem Verfahren. Schwerwiegende Komplikationen,
die im Zusammenhang mit der Prozedur stehen, sind nicht aufgetreten und können
vermieden werden. Die direkte Sicht des Chirurgen und die in unserem Zentrum
angewendeten MaĂźnahmen sind eine ausreichende Prophylaxe zum Schutz der
benachbarten mediastinalen Strukturen. Es zeigt sich eine sehr hohe Inzidenz
an postoperativem VHF von ca. 70%. Meistens wird VHF am ersten und zweiten
postoperativen Tag registriert. Vorhofflimmern, welches am Operationstag
auftritt, ist mit einer deutlich schlechteren Prognose verbunden. Die Inzidenz
von Vorhofflattern postoperativ beträgt ca. 3,5%. Bis zum dritten
postoperativen Monat waren bei 98% der Patienten, die erfolgreich behandelt
wurden, die postoperativen Vorhofarrhythmien beendet. Die Ablationsdauer
betrug ca. fünf Minuten. Das HAT 300 Gerät zeigt Sicherheitsdefizite mit der
Tendenz zum Overshooting . Eine Zieltemperatur bis zu 65° C scheint genauso
effektiv zu sein, wie höher eingestellte Temperaturen. Das lange und starre
Instrument bereitet erhebliche Schwierigkeiten bei der DurchfĂĽhrung der RFA.Atrial fibrillation (AF) is an independent factor for mortality and morbidity.
One of the most debilitating consequences of the disease is the accompanying
risk of stroke Unipolar temperature-guided (radio) high frequency ablation is
performed during cardiac arrest, concomitant to other heart surgery procedures
by patients with chronic AF. Saline ice slush and an isolation mat are placed
in the transverse sinus. Using unipolar RF-energy from the Osypka HAT 300,
linear lesions were created in the left atrium (LA) following the Berlin
modification lesion set. A probe registers the temperature during the
procedure. The LA appendage is occluded and by patients with very large LA a
reduction plastic is performed. All patients who became an ablation procedure,
between 01. April 2000 and 31. March 2005, were retrospectively examined.
After complete follow up for all patients (mean 41.5 +/- 14.4 months), 72% of
the patients are in sinus rhythm or show a biatrial contraction with
sequential pacemaker. Factors which positively affect the result are the
presence of a Mitral valve stenosis and paroxysmal AF before the operation.
The result is affected negatively by the presence of Hyperthyreosis. Patients
with long existing VHF and patients, who received ablation through a
transeptal approach show worse success rates. No influence on the success
rates have the diameter of the left atrium, coronary heart disease, Aortic
valve disease, Tricuspid valve insufficiency, Diabetes mellitus, pulmonary
hypertension, age and sex. Patients with normal or reduced LV-EF and patients
with high, middle or low risk profile (according to EuroSCORE) profit equally
from the procedure. Serious complications related with the procedure did not
occur and can be avoided without large expenditure. The direct view of the
surgeon and the safety measures used are a sufficient prophylaxis for the
protection of the neighbouring mediastinal structures. We registered a very
high incidence of postoperative AF of approx. 70%. Atrial fibrillation is
mostly registered on the first and second post operative day. However AF,
which arises on the operation day, is connected with a worse prognosis. The
incidence of postoperative atrial flutter is about 3.5%. Up to the third
postoperative month the postoperative atrial arrhythmias were terminated in
98% of the patients, who were successfully treated. The ablation duration was
approximately five minutes. The HAT 300 shows safety deficits with the
tendency to Overshooting . A temperature of 65° C seems to be just as
effective, as higher temperatures. The handling of the long and stare
instrument is difficult
Paediatric aortic valve replacement using decellularized allografts
OBJECTIVES: Options for paediatric aortic valve replacement (AVR) are limited if valve repair is not feasible. Results of paediatric Ross procedures are inferior to adult Ross results, and mechanical AVR imposes constant anticoagulation with the inherent risks. METHODS: The study design was a prospective, multicentre follow-up of all paediatric patients receiving decellularized aortic homografts (DAHs) for AVR in 8 European centres. RESULTS: A total of 106 children (77 boys) were operated (mean age 10.1 ± 4.8 years, DAH diameter 20.5 ± 3.8 mm). A total of 60 (57%) had undergone previous surgical interventions: 34 with 1, 15 with 2 and 11 with ≥3. There was one early death in a 12-year-old girl, who underwent her fourth aortic valve operation, due to intracerebral haemorrhage on extracorporeal membrane oxygenation after coronary reimplantation problems following 3-sinus reconstruction 1 year earlier. One 2-year-old patient died due to sepsis 2 months postoperatively with no evidence for endocarditis. In addition, a single pacemaker implantation was necessary and a 2.5-year-old girl underwent successful HTx due to chronic myocardial failure despite an intact DAH. After a mean follow-up of 3.30 ± 2.45 years, primary efficacy end points mean peak gradient (18.1 ± 20.9 mmHg) and regurgitation (mean 0.61 ± 0.63, grade 0-3) were very good. Freedom from death/explantation/endocarditis/bleeding/stroke at 5 years was 97.8 ± 1.6/85.0 ± 7.4/100/100/100% respectively. Calculated expected adverse events were lower for DAH compared to cryopreserved homograft patients (mean age 8.9 years), lower than in Ross patients (9.4 years) and in the same range as mechanical AVR (12.8 years). CONCLUSIONS: Even though the overall number of paediatric DAH patients and the follow-up time span are still limited, our data suggest that DAHs may present a promising additional option for paediatric AVR.status: publishe
