Is there a progress in treatment of dyslipidemia in diabetes? : what are the results of ACCORD study?
W pracy przedstawiono wyniki badania ACCORD Lipid, dotyczącego skuteczności klinicznej w prewencji
angiopatii i bezpieczeństwa leczenia skojarzonego
dyslipidemii cukrzycowej za pomocą statyny i fibratu w porównaniu z monoterapią statyną. Badaniem
objęto 5518 chorych na cukrzycę typu 2, leczonych
simwastatyną i fenofibratem, w porównaniu z simwastatyną i placebo. Po średnio 4,7 roku trwania
badania nie obserwowano istotnych różnic w zakresie punktów końcowych (zgonów sercowo-naczyniowych, zawałów serca i udarów mózgu) pomiędzy
porównywanymi grupami.
Korzystny trend terapii skojarzonej w zestawieniu
z monoterapią statyną w postaci 31-procentowej
redukcji incydentów sercowo-naczyniowych
obserwowano w podgrupie chorych z obniżonym
stężeniem cholesterolu frakcji HDL i podwyższonym
stężeniem triglicerydów - profilem typowym dla
aterogennej dyslipidemii. Wyniki badania ACCORD
Lipid sugerują stosowanie terapii skojarzonej
u pacjentów z odpowiednim profilem lipidowym.n this article ACCORD Study results, concerning
clinical efficacy in preventing cardiovascular disease
and safety of combined statin and fibrate treatment
in patients with type 2 diabetes in comparison to
statin monotherapy are presented. Study included
5518 patients with type 2 diabetes, treated with
simvastatin and fenofibrate in comparison to
simvastatin and placebo. After 4.7 years of treatment
no significant differences in primary end points
(cardiac death, myocardial infarction and stroke)
were observed between compared groups. However,
favourable trend of combined therapy in comparison
to statin monotherapy in the subgroup of patients
with low HDL-cholesterol and high triglycerides
- lipid profile typical for atherogenic dyslipidemia
- 31% decrease of cardiovascular end-points was

lipidowego. Modyfikacja stylu życia i ścisłe wyrównanie
glikemii, a także kontrola stężenia lipidów
i dostosowywanie dawek leków w celu uzyskania
stężeń docelowych mają istotne znaczenie w uzyskaniu

W pracy przedstawiono aktualne zasady leczeniadyslipidemii u chorych na cukrzycę na podstawiezaleceń ekspertów Polskiego Towarzystwa Diabetologicznego (PTD). Pierwszorzędowym celem leczenia jest obniżenie stężenia cholesterolu frakcji LDL. Docelowe stężenie cholesterolu frakcji LDL u chorych nacukrzycę wynosi < 2,6 mmol/l, a u chorych z cukrzycąi chorobami układu krążenia 1,8 mmol/l. Pożądanestężenie cholesterolu frakcji HDL wynosi > 1,3 mmol/lu kobiet i > 1,0 mmol/l u mężczyzn, a stężenie triglicerydów < 1,7 mmol/l. Lekiem pierwszego rzutu w leczeniu dyslipidemii cukrzycowej są statyny, a w przypadku braku osiągnięcia poziomu docelowego za pomocą statyn — terapia skojarzona, jednak brak wyników skuteczności i bezpieczeństwa takiej terapii.In this paper treatment recommendations of diabeticdyslipidemia are presented, based on PTD guidelines.The goal of treatment in patients with diabetesis to decrease LDL-cholesterol level below 2.6 mmol/land in patients with diabetes and cardiovasculardisease below 1.8 mmol/l. Desirable values of HDL-Cis > 1.3 mmol/l in women and > 1.0 mmol/l in men,and of triglycerides < 1.7 mmol/l. Statin is fi rst linetreatment; if targets are not reached combined therapywith other lipid lowering drugs may be considered, butthis therapy has not been evaluated in CVD and safetyoutcome studies

Serum pentraxin 3 concentration in patients with type 2 diabetes and nonalcoholic fatty liver disease
INTRODUCTION Nonalcoholic fatty liver disease (NAFLD) is common in patients with type 2 diabetes (T2D). Pentraxin 3 (PTX3), a marker of inflammation, is a cardiovascular risk factor.
OBJECTIVES We examined clinical and biochemical factors associated with serum PTX3 concentrations in patients with T2D with and without NAFLD.
PATIENTS AND METHODS Serum material was obtained from 116 patients with T2D (mean age, 59.1 years), including 79 patients with NAFLD.
RESULTS Median (interquartile range) PTX3 level was 4.264 (2.293) ng/ml in patients with and 3.773 (3.223) ng/ml in patients without NAFLD (P = 0.93). In the whole group, PTX3 level was associated with total cholesterol, low‑density lipoprotein cholesterol (LDL‑C), apolipoprotein (apo) B100, apo C3, triglyceride (TG) concentrations, and waist circumference after adjustment for age and gender. As indicated by partial regression coefficient b, increase of independent variable LDL‑C by 1 mmol/l was associated with the rise of PTX3 by 1.2017 ng/ml, increase of apo B100 by 1 mg/dl with the rise of PTX3 by 1.0051 ng/ml, and increase of apo C3 by 1 μg/dl with the rise of PTX3 by 1.0012 ng/ml. In patients with T2D with NAFLD, total cholesterol, LDL‑C, TG, apo C3, and apo B100 were associated with PTX3. Associations of PTX3 with apolipoproteins were observed only in the NAFLD group.
CONCLUSIONS Reported associations of PTX3 level add new insight into possible mechanisms of its atherogenic actions
Intima-media thickness correlates with features of metabolic syndrome in young people with a clinical diagnosis of familial hypercholesterolaemia
Background: Familial hypercholesterolaemia (FH) is a monogenic lipid metabolism disorder characterised by markedlyelevated serum low-density lipoprotein (LDL) cholesterol level due to a mutation in the LDL receptor gene. Clinical featuresof FH include premature atherosclerosis and coronary artery disease.Aim: To explore associations between noninvasive markers of atherosclerosis including intima–media thickness (IMT) andpulse wave velocity (PWV) and blood lipids, blood pressure (BP) and obesity in a group of young patients with FH.Methods: Study population included 36 patients aged < 35 years with the diagnosis of FH based on the Simon Broome Register criteria, and their 49 relatives who comprised the control group free of FH.Results: Mean IMT values were higher in FH patients than controls (0.60 ± 0.19 vs. 0.53 ± 0.07 mm, respectively, p < 0.05).Mean body mass index (BMI) and waist circumference were similar in patients and controls. The prevalence of carotid atheroscleroticplaques was significantly higher among FH patients (n = 6) than in controls (n = 1) (21.4% vs. 2.6%, p = 0.012). Arterial hypertension was present in 27.8% of patients with FH and 16.3% of subjects in the control group. Systolic blood pressure (SBP) in FH patients correlated significantly with age (r = 0.35), BMI (r = 0.48) and waist circumference (r = 0.47), and diastolic blood pressure (DBP) correlated with BMI (r = 0.42) and waist circumference (r = 0.41). PWV correlated significantly with age (r = 0.44), SBP (r = 0.63) and DBP (r = 0.52). We did not find any correlations between IMT and serumlipids, BP or obesity indices in FH patients.Conclusions: Our findings show a higher rate of arterial hypertension in young FH patients compared to their relatives freeof FH, with significant associations between BP and indices of obesity and arterial stiffness. Intensive lipid lowering and antihypertensive therapy along with a reduction in central fat may be considered a mandatory treatment strategy in young FH patients to prevent atherosclerosis and increased arterial stiffness.Wstęp: Rodzinna heterozygotyczna hipercholesterolemia (RHH) jest monogenowym zaburzeniem metabolizmu lipidów cechującymsię znacznym podwyższeniem stężenia cholesterolu LDL w wyniku mutacji w genie receptora LDL. RHH występuje z częstością ok.1/500 osób w populacji. W Polsce RHH dotyczy ok. 70 000 chorych, jednak znaczna ich część pozostaje niezdiagnozowana, a osobyz ustalonym rozpoznaniem często nie są leczone lub leczone nieadekwatnie. Charakterystyczne cechy kliniczne RHH obejmują podwyższenie stężenia cholesterolu całkowitego i cholesterolu LDL w surowicy, zmiany skórne o typie żółtaków ścięgien prostowników palców, ścięgna Achillesa, rąbek starczy oraz żółtaki powiek. Najgroźniejszym objawem klinicznym RHH jest przedwczesna miażdżyca naczyń wieńcowych i choroba niedokrwienna serca. Umieralność nieleczonych pacjentów z RHH jest znacznie większaniż osób zdrowych tej samej płci i wieku.Cel: W niniejszym badaniu oceniono związek między grubością błony intima–media (IMT) i prędkością fali tętna, nieinwazyjnymimarkerami miażdżycy naczyń a stężeniem lipidów, ciśnieniem tętniczym krwi i otyłością w grupie młodych pacjentów z RHH.Metody: Badaniem objęto 36 młodych pacjentów z rozpoznaniem RHH na podstawie kryteriów Simon Broome Register, w wieku < 35 lat i ich 49 krewnych bez RHH, stanowiących grupę kontrolną, dobraną pod względem wieku i płci. U wszystkich chorych przeprowadzono wywiad wg standardowego kwestionariusza, obejmujący dane na temat obecności choroby niedokrwiennej serca, chorób układu sercowo-naczyniowego, nałogu palenia tytoniu oraz sposobu leczenia.Wyniki: Średnie wartości IMT były wyższe u osób z RHH niż w grupie kontrolnej: 0,60 ± 0,19 vs. 0,53 ± 0,07 mm (p < 0,05).Średnie wartości wskaźnika masy ciała (BMI) i obwód pasa były podobne w obu grupach. Pacjenci z RHH cechowali się częstszym występowaniem blaszek miażdżycowych (n = 6) niż grupa kontrola (n = 1) (21,4% vs. 2,6%; p = 0,012). Nadciśnienie tętnicze występowało u 27,8% pacjentów z RHH i u 16,3% osób z grupy kontrolnej. Ciśnienie skurczowe u osób z RHH korelowało istotniez wiekiem (r = 0,35), BMI (r = 0,48) i obwodem pasa (r = 0,47), a ciśnienie rozkurczowe z BMI (r = 0,42) i obwodem pasa (r = 0,41).Prędkość fali tętna korelowała istotnie z wiekiem (r = 0,44), ciśnieniem skurczowym (r = 0,63) i rozkurczowym (r = 0,52). Nie zaobserwowano korelacji między IMT a stężeniem lipidów surowicy, ciśnieniem tętniczym lub wskaźnikami otyłości u pacjentów z RHH.Wnioski: Wyniki wskazują, że odsetek osób z nadciśnieniem tętniczym jest wyższy u młodych pacjentów z RHH niż u krewnych bez RHH oraz na obecność silnej korelacji między ciśnieniem tętniczym a wskaźnikami otyłości (takimi jak obwód pasa, BMI, współczynnik pas/biodra) oraz sztywnością naczyń. Intensywne leczenie hipolipemizujące i hipotensyjne, a także zmniejszenie otyłościbrzusznej wydają się ważnymi elementami leczenia młodych pacjentów z RHH w celu prewencji miażdżycy i sztywnienia naczyń
The genetic spectrum of familial hypercholesterolemia in South-Eastern Poland
BACKGROUND: Familial hypercholesterolemia (FH) is a common autosomal dominant disorder with a frequency of 1 in 200 to 500 in most European populations. Mutations in LDLR, APOB and PCSK9 genes are known to cause FH. In this study, we analyzed the genetic spectrum of the disease in the understudied Polish population. MATERIALS AND METHODS: 161 unrelated subjects with a clinical diagnosis of FH from the south-eastern region of Poland were recruited. High resolution melt and direct sequencing of PCR products were used to screen 18 exons of LDLR, a region of exon 26 in the APOB gene and exon 7 of PCSK9. Multiplex ligation-dependent probe amplification (MLPA) was performed to detect gross deletions and insertions in LDLR. Genotypes of six LDL-C raising SNPs were used for a polygenic gene score calculation. RESULTS: We found 39 different pathogenic mutations in the LDLR gene with 10 of them being novel. 13 (8%) individuals carried the p.Arg3527Gln mutation in APOB, and overall the detection rate was 43.4%. Of the patients where no mutation could be found, 53 (84.1%) had a gene score in the top three quartiles of the healthy comparison group suggesting that they have a polygenic cause for their high cholesterol. CONCLUSIONS: These results confirm the genetic heterogeneity of FH in Poland, which should be considered when designing a diagnostic strategy in the country. As in the UK, in the majority of patients where no mutation can be found, there is likely to be a polygenic cause of their high cholesterol level
Influence of growth and inflammatory factors on the development of nephropathy in patients with diabetes mellitus type 2
WSTĘP. W ostatnio opublikowanych badaniach wykazano istotną rolę czynników wzrostowych i cytokin prozapalnych w rozwoju nefropatii cukrzycowej.
Celem pracy była ocena wpływu wybranych czynników wzrostowych i markerów stanu zapalnego na
rozwój zmian w nerkach u pacjentów z nefropatią
cukrzycową.
MATERIAŁ I METODY. Badaniami objęto 50 chorych
z nefropatią w przebiegu cukrzycy typu 2, w tym 25
poddano obserwacji prospektywnej trwającej 36
miesięcy. U wszystkich pacjentów oznaczono stężenie czynników zapalnych i wzrostowych we krwi oraz
parametry wydolności nerek.
WYNIKI. Chorzy z nefropatią cukrzycową charakteryzowali się istotnie wyższym stężeniem fibrynogenu, hsCRP, IL-6 i sTNF-RII w porównaniu z wartościami referencyjnymi. W badaniu wyjściowym stwierdzono, że podwyższone stężenia IL-6 i sTNF-RII wiązały się ze spadkiem wartości GFR (r = –0,3; p = 0,01
oraz r = –0,7; p < 0,0001). Po 36 miesiącach obserwacji wykazano związek stężenia hsCRP, sTNF-RII
z albuminurią (r = 0,5; p = 0,008 i r = 0,5; p = 0,02)
oraz VEGF z wartością GFR (r = –0,4; p = 0,03).
WNIOSKI. W niniejszej pracy wykazano, że u chorych
z nefropatią cukrzycową, w stosunku do wartości
referencyjnych, występują istotnie wyższe stężenia
markerów zapalnych. W badaniu prospektywnym
wzrost wartości stężeń markerów zapalnych wiązał się
z progresją nefropatii - obniżeniem GFR i wzrostem
albuminurii.BACKGROUND. Presently published research points
to an essential role of growth and pro-inflammatory
cytokines in the development of diabetic nephropathy. The aim of the study was to evaluate influence
of certain growth factors and inflammatory markers
on the progression of kidney changes in patients with
diabetic nephropathy.
MATERIAL AND METHODS. Material included 50
patients with diabetic nephropathy, 25 out of them
re-evaluated after 36 months. Inflammatory and
growth factors were measured along with renal
parameters in all patients.
RESULTS. Patients with diabetic nephropathy were
characterized with significantly higher values of
fibrinogen, hsCRP, IL-6 and sTNF-RII in comparison to reference values. In the initial examination,
increased concentrations of IL-6 and sTNF-RII were
associated with decreased values of GFR (r= –0.3;
p = 0.01, as well as r = –0.7; p < 0.0001). After 36
months of observation - an association was found
between concentrations of hsCRP, sTNF-RII and
albuminuria (r = 0.5; p = 0.008 and r = 0.5; p =
0.02); as well as between VEGF and GFR values (r =
–0.4; p = 0.03).
CONCLUSIONS. Research showed that patients with
diabetic nephropathy in comparison to reference
values had significantly higher concentrations of
inflammatory markers. In the prospective examination - changes of inflammatory markers were
associated with progression of nephropathy (decrease of GFR and increase of albuminuria)
